[Incident and accident reports in Tokyo Medical and Dental University Dental Hospital (FY 2001-2002)].

In 2002, the Dental Hospital of Tokyo Medical and Dental University set up a working group for risk management. This working group analyzed 225 incident and accident reports submitted to the hospital in 2001 and 2002. Each report was analyzed with regard to "type," "place," "reporter," "severity," and "cause" in order to diagnose hospital safety and prevent future incidents and accidents. The cause of incidents and accidents was analyzed using the SHEL model, where S stands for Software, H for Hardware, E for Environment, and L for Liveware. The severity of the consequence was classified into 6 levels, where level 0 = "error not applied," level 1 = "not affected," level 2 = "watch and see or additional test," level 3 = "treatment," level 4 = "aftereffect," and level 5 = "death." The incidents and accidents judged to have potentially high risk were given a score of "+H," irrespective of the level. The results of the analyses revealed that most of the incidents and accidents happened in "wards," "operation rooms," and "oral surgery clinics." This is probably because the incident and accident reporting system is well established by nurses working in these clinics. Additional analysis revealed that most of the reports were written and submitted by nurses. The frequencies of "treatment procedure," "misuse of dental instruments," "mis-prescription," "falling down" and "needlestick" related incidents and accidents were the highest and were caused mainly by L and S. There were only 3 accidents above level 4, however, less severe cases were given a score of +H due to the high potential risk involved.

[Career history and perceptions of dental hygiene education programs--questionnaire mail-in survey of alumni of the School of Dental Hygiene in Tokyo Medical and Dental University].

This study was conducted to investigate the career history and perceptions about dental hygiene education programs among the alumni (1952-1999 graduates) of the School of Dental Hygiene in Tokyo Medical and Dental University. A questionnaire containing demographic, practice characteristics and views on the programs was mailed to 997 alumni in 1999, and 576 alumni (57.8%) responded. Three hundred and forty-one respondents worked as dental hygienists. The majority who responded were in clinical practice. One hundred and thirty-one of the respondents worked in private clinics, 76 in public health centers, and 72 in clinics in companies. The rate of them who worked in public health centers was much higher than the national average cited in the Statistical Report on Public Health Administration and Services. Two hundred and ninety-one respondents reported inadequate programs. The rate of them who reported inadequate programs was significantly higher in alumni who were working than in alumni who were not. The items cited as insufficiently taught at the school were clinical practice, instrumentation, foreign language, psychology, counseling, and nursing related subjects. Thus, many alumni suggested the need for better programs and continuing education. From these results, it was suggested that dental hygienists need to change their education programs in order to meet the present and future needs of more diversified society. This view was particularly prevalent among alumni who were working. The result showed that dental hygiene educators and dental hygienists urgently need to reconsider the professionalism of their field.

Occurrence of Yersinia enterocolitica in the Tokyo Tama Zoo.

Yersinia enterocolitica serogroups O5A and O8 were isolated from fecal samples of one colony of Japanese macaques (Macaca fuscata) from the Tokyo Tama Zoo (Japan). Serogroup O5A was detected in brown bear (Ursus arctos) prior to the isolation from the macaques. Serogroup O5A organisms also were isolated from the Japanese macaques' breeding area. Serogroup O5A and O8 isolates were not pathogenic. Serogroup O8 isolates did not possess the O7 or O19 antigens.

A serological survey for human immunodeficiency virus types 1 and 2 of individuals who visited health centers in Tokyo.

The serum antibodies to human immunodeficiency virus types 1 (HIV-1) and 2 (HIV-2) were examined for among individuals who visited health centers in Tokyo. Of 8,198 sera screened, one was true-positive and 37 false-positive for HIV-1 antibodies. These 37 false-positives and 305 sera from the population groups at risk for HIV-2 (42 sojourners in Africa, 251 homo- and bisexuals, and 19 prostitutes) were further examined for HIV-2 antibodies by indirect immunofluorescence and Western blotting. No antibodies reactive with HIV-2 were detected in the sera examined. Serological cross-reactivity between HIV-1 and HIV-2 was examined by use of rabbit antisera.

Utilization of a computerized system at the pharmacy department of the University of Tokyo Hospital--impact of prescription order entry and computerized dispensing system.

(1) A medication order entry system, (2) a fully automatic tablet counting and packaging machine connected with a medication order entry system, and (3) a check system for one dose package developed and implemented in the University of Tokyo Hospital are described. The principal objectives of these systems are (1) to reduce medication errors and the clerical workload of staff, (2) to make more efficient use of staff, (3) to provide staff with sufficient drug information for patient drug therapy. We compared computerized systems (post-computerization) and traditional multi-dose dispensing systems (pre-computerization) by analyzing inquiry rate and dispensing time in the pharmacy for inpatients. Inquiry rate was 23.0% and 2.6% for pre-computerization and post-computerization, respectively. Total dispensing time per patient was 207 seconds and 147 seconds for pre-computerization and post-computerization, respectively. Implementation of the computerized systems helped significantly in decreasing inquiry rate and time needed to complete the dispensing process when compared with a traditional dispensing system. Implementation of the computerized systems was very useful for physicians, pharmacists, and nurses in conducting drug therapy.

[The survey of HIV infection among clients of foreign female prostitutes in Tokyo Metropolitan].

OBJECTIVES: It is important to elucidate the risks to foreign female prostitutes of HIV infection from their clients. We determined the prevalence of HIV among clients of foreign female prostitutes using anonymous testing of condom semen samples. METHODS: A total of 250 client condom semen samples were collected with client age and nationality by 15 Latin American and 37 Thai female prostitutes in Tokyo from March to May 1995. We screened by ELISA (GENELAVIA MIXT, Pasteur Sanofi Diagnostics) for antibody to HIV 1 and 2, and then confirmed by Western Blot (LAV Blot 1, LAV Blot 2, Sanofi Diagnostics Pasteur) and distinguished by Immuno Blot PEPTI-LAV 1, 2, (Sanofi Diagnostics Pasteur). RESULTS: The nationality of customers was reported as Japanese (74.8%), Iranian (7.6%), Pakistan (5.6%), Indian (2.8%), Taiwanese (2.0%), and Chinese (1.2%) with other nationalities less than 1.0%. Estimated age categories of the clients were < 20 (1.2%), 20-29 (41.2%), 30-39 (36.4%), 40-49 (12.4%), > or = 50 (6.4%) and undetermined (2.4%). Of the 250 samples collected, only 219 were sufficient for testing. Two samples (0.90%, 95% CI = 0.4-2.2%) were HIV 1 positive, both from Japanese clients (1.2% of Japanese clients, 95% CI = 0.5-2.9%). These data suggested that the prevalence of HIV in clients of female prostitutes was relatively high, and can be considered a risk group in Japan. The sentinel surveillance by anonymous of client condom semen samples is a very useful method to quantify HIV prevalence in this risk group.

Clinical and biological aspects of acute lymphoblastic leukemia in 62 infants: retrospective analysis of the Tokyo Children's Cancer Study Group.

BACKGROUND: As a first step to formulate a new treatment strategy for refractory acute lymphoblastic leukemia (ALL) in infants, clinical results and immunophenotypic and cytogenetic data were analyzed and compared with those from overseas. METHODS: There were 62 infants with ALL who were treated between 1977 and 1995 at 30 institutions affiliated with the Tokyo Children's Cancer Study Group. Clinical and laboratory data obtained from these infants (all under 1 year of age) were retrospectively studied. RESULTS: The morphological diagnoses were FAB-L1 for 51 patients (82.2%) and FAB-L2 for 11 patients (17.8%). Hepatomegaly and splenomegaly were found in 40 (70.0%) and 40 patients (68.3%), respectively. The mean (+/- SEM) leukocyte count at diagnosis was 205,900 +/- 35,700/microL. The involvement of the central nervous system was evident in nine of 36 patients who were subjected to lumbar puncture, while three of these nine patients were free of neurological symptoms at diagnosis. Thirty-one patients (55.4%) were CD10 negative and 14 (25.0%) were CD10 positive. Thirty-one of 47 patients (65.9%) exhibited chromosomal abnormalities, including 28 patients (59.6%) with 11q23 abnormalities. Rearrangements in the MLL gene were found in nine of 13 infants (69.2%) examined. Translocation of 11q23 and/or MLL gene rearrangement (11q23/MLL) was significantly associated with the absence of the CD10 antigen. Hyperleukocytosis of more than 50,000/microL and 11q23/MLL gene rearrangements were related to a poor prognosis. The probability of an event-free survival in 62 infants was 13.1 +/- 4.8% at 48 months. CONCLUSIONS: New therapeutic strategies and large-scale cooperative prospective trials are needed to improve the prognosis of ALL in infants.

Acute lower respiratory infections in hospitalized children over a 6 year period in Tokyo.

BACKGROUND: Acute lower respiratory infections are major causes of hospitalization in children and are mainly caused by respiratory viruses. In the present study, we investigated the etiologic agents responsible for acute lower respiratory infections from the period November 1986 to October 1992 in order to determine the seasonal pattern and different characteristics of age distribution of respiratory infectious agents, mainly virus infections. METHODS: A total of 1521 patients with lower respiratory infections was hospitalized in Saiseikai Central Hospital, Tokyo, Japan. Nasopharyngeal secretions were obtained for virus isolation and paired sera in the acute and convalescent phases were obtained for serological examination. RESULTS: Etiological agents were identified in 668 of 1521 patients (43.9%) by serological antibody responses, virus isolation and/or detection of virus antigen: 240 (15.8%) with respiratory syncytial (RS) virus; 62 (4.1%) with influenza virus type A; 26 (1.7%) with influenza virus type B; 86 (5.7%) with adenovirus; 81 (5.3%) with parainfluenza virus; 32 (2.1%) with measles virus; 20 (1.3%) with enteroviruses or Herpes virus other than respiratory viruses; 75 (4.9%) with Mycoplasma pneumoniae; 10 (0.7%) with pertussis; and 36 (2.4%) with mixed infections. In the remaining 853 patients (56.1%), etiologic agents were not identified. Respiratory syncytial (RS) virus was a main causative agent of respiratory infections in patients younger than 3 years of age. Influenza virus and M. pneumoniae were two main causative agents in patients with acute respiratory illness over 5 years of age. Parainfluenza virus type 3 was frequently observed in infants from 9 to 12 months of age. A distinct seasonal pattern of viral infections was consistently observed in each year during the study period; RS and influenza viruses were prevalent in winter, parainfluenza virus was prevalent in spring and M. pneumoniae was prevalent in summer and autumn. However, adenovirus infections were observed in all seasons. Serological responses were poor in patients younger than 1 year of age and they were mainly diagnosed by virus isolation or detection of virus antigen. CONCLUSIONS: Virological epidemiology provides useful information in daily clinical practice for the prediction of etiological agents based on patient age and the seasonal distribution of agents. We should examine virus isolation and the detection of virus antigen, along with serological examinations in patients with respiratory infections, especially in infants younger than 1 year of age because of poor serological responses.

[Lifestyle and health status of homeless people around Shibuya Station, Tokyo].

The number of homeless people in Tokyo is estimated to be 3,200-3,300. While studies on the health status of homeless people, including illness, injury and deaths have been previously reported, most of these reports concern the homeless who resided in housing facilities for the homeless or who admitted to hospitals. We undertook a comparison of lifestyle and health status between homeless people and people who live in houses (as a control group). Health status was also analyzed for differences among homeless people. Subjects were asked by questionnaire regarding their age, the length of being homeless, former and present employment, sleeping condition, food, whether they have friends or not, the amount of smoking per day, and Short-Form-36 Health Survey (SF36). As objective findings, measurement of blood pressure and blood testing were also performed. Fifty-three homeless people, 49 male, 4 female, average age 52, from the areas around Shibuya station and Yoyogi park, were enrolled. While 98% of the homeless people had previous employment, 73% were not working when the study was performed. Compared with control group, the homeless had fewer meals per day, fewer friends, excessive smoking, greater history of gastro-duodenal ulcer and injury, greater limitation due to physical problems, and higher general mental health as measured by SF36. The diastolic blood pressure of the homeless was higher than that of the control. The blood testing showed higher white blood cell counts and platelet counts. It was suggested that changes in the social structures were largely influential in causing life, and that access to health care was limited because of financial and social barriers. Further studies with more samples, survey of social volunteers involved in care of homeless and qualitative data would be necessary to find and develop better support system for the homeless.

Psycho-physiological effects of the terrorist sarin attack on the Tokyo subway system.

The investigation describes the follow-up of the victims of the 1995 Tokyo sarin attack who were followed by a team at the St. Luke's International Hospital. A symptom questionnaire ("St. Luke's Questionnaire") was developed and given 2, 3, and 5 years following the sarin attack. Somatic and psychological symptoms have continued for 5 years after the incident. New post-traumatic stress disorder diagnostic criteria, which include physical symptoms, were developed and applied for assessment of the outcomes. The St. Luke's cohort was also compared with other Japanese sarin attack cohorts. Interventions, including counseling, medical treatments, and support group activities, were associated with fewer symptoms among the victims.

[DOTS in Japan--Tokyo area].

The resurgence of tuberculosis has required a successful strategy to control TB in Japan. The World Health Organization has recommended the so-called DOTS (Directly Observed Therapy, Short-Course) strategy since 1995 and DOTS has been used with great success not only in many developing countries, but also in many developed countries. In Tokyo, especially in urban areas with a high prevalence of TB, DOT has been started. Using DOT, Sanya in Taito-ku (ward) and Arakawa-ku (ward), have shown high treatment completion--more than 90% since 1997. In 2000, other health care offices in Shinjuku-ku (ward), Yokohama city, Kawasaki city and Nagoya city started DOT for groups of homeless with TB. Presently, DOT is applied only to homeless people. However, the number of people who need DOT is much higher, due to poor adherence. It is also important to reconsider cost-efficiency for TB control in Japan.

Prevalence of Escherichia coli possessing the eaeA gene of enteropathogenic E. coli (EPEC) or the aggR gene of enteroaggregative E. coli (EAggEC) in traveler's diarrhea diagnosed in those returning to Tama, Tokyo from other Asian countries.

To elucidate the importance of enteropathogenic Escherichia coli (EPEC) and enteroaggregative E. coli (EAggEC) as etiological agents in traveler's diarrhea, the detection of the eaeA and aggR gene in E. coli strains isolated from overseas travelers with diarrhea in Tama, Tokyo was carried out using a PCR method. Of 192 travelers who were mostly adults and had visited Asian countries from April 1998 to March 1999, aggR-positive E. coli strains were detected in 26 (13.5%). These strains represent the second predominant enteropathogen following enterotoxigenic E. coli (ETEC), whereas eaeA-positive E. coli strains were confirmed in seven subjects (3.6%). In 13 cases with aggR and four cases with eaeA, the organisms were detected in stool samples of patients as the only potential enteric pathogen. The clinical symptoms of these patients were similar to those in patients with ETEC; however, the severity of illness was milder than that associated with ETEC alone. Three strains with eaeA and five strains with aggR were typed as six different kinds of O serogroups, of which four strains belonged to the classical EPEC serogroups (O55, O114, O119, and O127a). These findings suggest that aggR-positive E. coli (EAggEC) is a significant causative agent in traveler's diarrhea. In addition, it was demonstrated that eaeA-positive E. coli (EPEC) is markedly correlated with diarrhea in adults.

[Actual situation and social prognosis of women seeking psychiatric care at the emergency hostel of Tokyo Metropolitan Women's Counseling Center].

The emergency hostel of Tokyo Metropolitan Women's Counseling Center, established in 1957, provides protection and care for about 600 Japanese or foreign women per year. The women housed there need social support for various reasons such as prostitution, poverty, somatic or mental diseases, or domestic violence (DV). We investigated the sociodemographic characteristics, psychiatric diagnoses and social prognoses of 2667 women who consulted the psychiatric clinic in the emergency hostel between 1961 and 1997. Seventy-four women consulted the psychiatric clinic per year, on average. Most were aged between 20 and 49. During the study period, there was a gradual decline in the number referred through the prostitution prevention law. Psychiatric diagnoses at the first visit varied widely. Annual comparison showed a gradual decrease in schizophrenia and manic-depressive illness, but an increase in substance abuse, psychogenic reaction, and personality disorder. Sociodemographically, most subjects appeared to have been children from underprivileged backgrounds. The social prognoses of 930 cases judged in March 1998 were good in 25%, moderate in 23% and poor in 48%. The poor prognosis group contained significantly more women with schizophrenia and personality disorder than the other two groups. The poor prognosis group tended to include more cases of substance abuse, while the good prognosis group contained more cases of depression and mental retardation. Women in the DV group tended to have more children than those in the non-DV group, and to have a higher prevalence of psychogenic reaction and a lower prevalence of schizophrenia. The DV group also tended to include more subjects with a moderate social prognosis and fewer subjects with a poor social prognosis. Specialized treatment should be provided for women after emergency admission to the hostel and this treatment needs to be aimed at improving social adaptation of the hostel residents, especially those with schizophrenia, personality disorders and substance abuse. Victims of DV should receive more conscious attention. In particular, prevention of mental disorders should be aimed not only at the residents but also their children.

Eight-year trend of new patients at Clinic for Disabled Patients, University Hospital, Faculty of Dentistry, Tokyo Medical and Dental University.

The trend of new patients was investigated at the Clinic for Disabled Patients, University Hospital, Faculty of Dentistry, Tokyo Medical and Dental University for eight years from 1989 to 1996. The purpose of this study was to explore the future role of this specialized clinic. The study revealed the following information: 1. A total of 1,388 new patients, corresponding to 173.5 new patients per year on average, visited the clinic. 2. The percentage of patients aged 30 or more increased. As a result, the mean patient age rose from 40.4 in 1989 to 47.6 in 1996. 3. Referrals accounted for more than 80% of new patients. Medical doctors referred more patients to our clinic than dentists did. 4. While the percentage of disabled patients tended to decrease, the percentage of medically compromised patients increased. 5. The predominant complaint was dental caries. Patients with cardiovascular diseases also complained of gingival bleeding and ill-fitting dentures. The growing percentage of medically compromised patients and the increasing mean age of patients over the eight-year period seems to reflect advances in medical science and the aging society. These tendencies could be more remarkable in the future. Therefore, much closer cooperation between dentists and medical doctors will be required.

[A survey of infants requiring long-term neonatal intensive care in Tokyo: 1989-1998].

In order to grasp the characteristics and outcomes with infants hospitalized long-term in NICUs, we reviewed all summary charts of 18 perinatal medical centers in Tokyo for the period from January 1989 to December 1998. We sampled 3,000 infants who required neonatal intensive care over 90 consecutive days out of 46,309 registered cases during the decade. The duration of hospital stay, making a comparative analysis of the number of days for the 50 percentile, was as follows. As a whole the infants required 125 days until discharge. Infants with 29-30 weeks gestation and infants with birth weights 1,000-1,499 g required shorter stays (106 days in both cases). The "discharge with complications" group required 136 days, and the "discharge on remission" group 119 days. Within the 31-32 weeks gestation group, those with "discharge with complications" required 107 days. Within the 29-30 weeks gestation group, those with "discharge on remission" required 104 days. Infants with 1,000-1,499 g birth weights for the "discharge with complications" and "discharge on remission" groups required 116 and 104 days respectively. Focusing on birthplace, the group of "inside-born" (born at perinatal medical centers) infants required 124 days, and the "outside-born" (born at non-perinatal medical centers) required 127 days. Respiratory distress syndrome (RDS), bronchopulmonary dysplasia (BPD) and chronic lung disease (CLD) were often seen in patients under 29 weeks gestation and under 1,000 g birth weight. Hypoxic ischemic encephalopathy (HIE), convulsions, congenital malformations and chromosomal abnormalities were frequent in the groups over 31 weeks and over 1,500 g. Apnoea and transient tachypnoea of newborn (TTN) often occurred in these at 29-30 weeks and 1,000-1,499 g. Also, apnoea and TTN were often seen in the "discharge on remission" group. RDS, apnoea and TTN occurRed frequently in the "inside-born" infants with over 31 weeks of gestation and over 1,500 g birth weight. There were many cases of HIE and convulsions in the "outside-born" infants of these groups. We found infants who required long-term intensive care to comprise three main groups. The first group consisted of infants of 29-30 weeks gestation and 1,000-1,499 g birth weight and demonstrated mild or few complications. The second consisted of under 29 weeks and under 1,000 g and exhibited complications of chronic lung diseases caused by immaturity of respiratory organs. The third was the group of over 31 weeks and over 1,500 g who had complications due to central nervous system disease, congenital malformations and chromosomal abnormalities.

[Mortality patterns of non-institutionalized severely retarded children in Tokyo--the third report 1999-2001].

We investigated mortality patterns of severely retarded children who were cared at their homes in Tokyo ten years after our first report. During the past two and a half years (from April 1999 to September 2001), we identified 41 fatal cases. The annual case fatality rate (the mortality rate among the investigated children) was 8.2@1000 in all the school-attending children and was 10.1@1000 in children with severe motor and intellectual disabilities. The rate was less than half of that in our past report and seemed to the lowest among those in several studies. Aggravation of basal disorders, severe respiratory infections and/or sepsis, and unexplained sudden death accounted for most of the death. We conclude that advance of medical service and the propagated medical education of teachers working for special schools play an important role to reduce the mortality in severely retarded children.

Dietary patterns and stomach cancer among middle-aged male workers in Tokyo.

Many epidemiological studies have examined the associations of nutrients and foods with risk of stomach cancer. Few studies have addressed the dietary patterns that reflect various components of food consumption and their interactions. We identified major four dietary patterns; "vegetable and fruit", "Western breakfast", "meat", and "rice/snack" with principal component analysis using food consumption questionnaire in a prospective study of 5,765 middle-aged male workers in Tokyo. After 10 years of follow-up between September 1988 and August 1998, 84 incident cases of stomach cancer were documented. Using proportional hazards regression to estimate risk ratios, we found no clear association between each of dietary pattern and stomach cancer risk. After adjustment for age and other potential confounding factors and after exclusion of the cases diagnosed in first follow-up year, the risk ratio (RR) associated with high tertile compared to low tertile was 0.78(95%CI 0.42-1.44) for "vegetable and fruit" pattern and 0.71(95%CI 0.40-1.24) for "Western breakfast" pattern. The V-shaped associations between dietary patterns and stomach cancer risk were appeared in the "meat" (RR=1.00, 0.55, and 1.10) and the "rice/snack" (RR=1.00, 0.52, and 1.19) patterns, while the linear trend of these associations was statistically on borderline. The roll of overall dietary patterns in predicting stomach cancer risk requires further investigation.

[Treatment of psychiatric patients with serious physical co-morbidity: administrative model in Tokyo].

In Japan, treatment of physical co-morbidity among psychiatric patients has been difficult in many aspects. Conflicts frequently arise when referrals are made from mental hospitals to general hospitals for many reasons, such as prejudice, lack of psychiatrists, and lack of psychiatric beds in general hospitals. Tokyo Metropolitan District, with over 12 million people in population, has suffered this problem, consequently leading to an establishment of a government-involved referral system in 1981. The government mediates between mental hospitals and designated treatment centers so that smooth transfers are made. After two decades of operation, the number of transferred patients via the system is on the increase. While this system still has many problems to be solved, it may become a model administrative system for other governments to apply and provide hints for effective treatment of med/psych cases.
