[Telecommunication and telepathology in orthopedic oncology. Possibilities in diagnosis and therapy of primary malignant bone tumors].

The rareness of primary malignant bone tumors and the difficulty of differential diagnosis make this field a part of oncology in which the consulting physician must rely on consistent histopathological monitoring of highest quality throughout the diagnostic and therapeutic process. In a pilot project of the Department of Bone Pathology of the University of Hamburg and the Orthopedic Department of the University of Heidelberg, 190 cases of suspected primary bone tumor were the subject of interdisciplinary discussion between July 2001 and March 2003 using a telepathology system established for this purpose. Diligent differential diagnoses were made prior to the biopsy; optimal location and further strategy for the biopsy were discussed and determined according to the guidelines of the international bone tumor centers. This also included the decision if intraoperative pathological examination of frozen sections should be performed. In 51 cases such examinations were performed. Those results immediately influenced the surgical procedure. Telepathology performed as described in this article led to a close interdisciplinary cooperation over a large distance and helped significantly to optimize therapy of patients with malignant bone tumors.

[Telecommunication and telepathology. New diagnostic routes for orthopaedics and orthopaedic pathology].

In a pilot project of the Department of Bone Pathology of the University of Hamburg and the Orthopaedic Department of the University of Heidelberg, the cases of 121 patients with suspicion of a primary bone tumour have been discussed at weekly interdisciplinary conferences during the period from July 2001 to May 2002., The consequent differential diagnoses were made prior to the biopsy, the optimal location of the biopsy and the further strategy was determined according to the guidelines of the international bone tumour centres. The latter includes the decision if a conventional biopsy or a intraoperative pathology examination on frozen sections should be performed. In 27 cases an intraoperative pathology examination was performed and then assessed in Hamburg. In 24 cases this diagnosis was identical with the final diagnosis. In three cases no definitive diagnosis could be made from the frozen sections. Additionally the pathohistological diagnoses of the cases of the previous week have been discussed in the video-conferences. Through this a unusually close interdisciplinary cooperation over a large distance has evolved, that is highly appreciated especially by the young and less experienced colleagues at the department of bone pathology and the orthopaedic department in Heidelberg. The awareness of the potential and limitations of a medical subject leads to an improved safety in the diagnostic process for bone tumours. The interdisciplinary discussion of all aspects of the diseases may also optimise the therapy of bone tumours.

Health and medical software related to computer user satisfaction: an interactive online expert system using diagnostic audit trails through telecommunication networks.

The selection of a health and medical software (HMS) system is a complicated process. The overall satisfaction derived from a system depends on many variables. This study analyzes the influence of HMS predictor variables on overall satisfaction as determined by multiple regression. The null hypothesis, that multiple correlation coefficient is zero, is rejected. Th alternative hypothesis is accepted. This study confirms the theories that suggest that HMS ease of operation, independent variables, technical support troubleshooting, and reliability of computer are the major determinations of overall computer user satisfaction. The factors identified were used in the design of an HMS expert system based on artificial intelligence (ESAI).

A telecommunications system for monitoring and counseling patients with hypertension. Impact on medication adherence and blood pressure control.

This study was conducted to evaluate the effect of automated telephone patient monitoring and counseling on patient adherence to antihypertensive medications and on blood pressure control. A randomized controlled trial was conducted in 29 greater Boston communities. The study subjects were 267 patients recruited from community sites who were >or= 60 years of age, on antihypertensive medication, with a systolic blood pressure (SBP) of >or= 160 mm Hg and/or a diastolic blood pressure (DBP) of >or= 90 mm Hg. The study compared subjects who received usual medical care with those who used a computer-controlled telephone system in addition to their usual medical care during a period of 6 months. Weekly, subjects in the telephone group reported self-measured blood pressures, knowledge and adherence to antihypertensive medication regimens, and medication side-effects. This information was sent to their physicians regularly. The main study outcome measures were change in antihypertensive medication adherence, SBP and DBP during 6 months, satisfaction of patient users, perceived utility for physicians, and cost-effectiveness. The mean age of the study population was 76.0 years; 77% were women; 11% were black. Mean antihypertensive medication adherence improved 17.7% for telephone system users and 11.7% for controls (P = .03). Mean DBP decreased 5.2 mm Hg in users compared to 0.8 mm Hg in controls (P = .02). Among nonadherent subjects, mean DBP decreased 6.0 mm Hg for telephone users, but increased 2.8 mm Hg for controls (P = .01). For telephone system users, mean DBP decreased more if their medication adherence improved (P = .03). The majority of telephone system users were satisfied with the system. Most physicians integrated it into their practices. The system was cost-effective, especially for nonadherent patient users. Therefore, weekly use of an automated telephone system improved medication adherence and blood pressure control in hypertension patients. This system can be used to monitor patients with hypertension or with other chronic diseases, and is likely to improve health outcomes and reduce health services utilization and costs.

[Telecommunications and networks: perspectives and issues].

The transmission of images, audio, video and computed data provides new facilities in medicine. The uses of telemedicine include teleeducation, training, consulting, diagnostic or therapeutic assistance between remote medical teams, or consultation for remote patients without the need of displacement. Network, telemedicine technologies, perspectives for teaching, medical teams and patients, national legislation, current and new ethical problems raising from telemedicine applications are successively reviewed.

[60]Fullerene-containing polyurethane films with large ultrafast nonresonant third-order nonlinearity at telecommunication wavelengths.

A significantly enhanced, ultrafast third-order optical nonlinearity at the wavelengths of 1150-1600 nm was demonstrated with cross-linked C60-containing polyurethane films using the Z-scan technique. Good-quality polymer films with a high loading of C60 derivative were obtained by cross-linking of the hydroxyl-containing C60 derivative and a triisocyanate. The positive Kerr coefficient with nonresonant nonlinear refractive index n2 falls in the range of (3.7 +/- 0.80) x 10-4 to (2.0 +/- 0.6) x 10-3 cm2/GW, and the calculated chi(3) and gamma values are up to 9.7 x 10-11 and 9.6 x 10-32 esu at 1550 nm, which are several orders of enhancement in third-order optical nonlinearity over pristine C60 in solution and 1-2 orders of enhancement over recently reported C60 derivatives and conjugated polymers.
