[Quantitative thermography of the knee joint].

Computer-assisted evaluation of thermography of the knee joints allows diagnosis and quantitation of inflammatory processes. This needs an adequate thermography camera, registration standards and a thermographic index. With the aid of this index normal knee joints and those with inflammatory changes can clearly be differentiated: an index of less than 3.5 is normal, values above 5.0 indicate an inflammation. The author proposes to correct the thermographic index with reference to actual rectal temperature in order to improve inter- and intra-individual comparability of thermograms. Apart from the thermographic index, the formal analysis of the line-scan over the joint space and the maximal temperature of the joint can be used as further diagnostic criteria.

[Thermography in the follow-up of bone and soft tissue tumors].

Thermography was used for the follow-up of 16 cases of bone tumors and of 16 cases of soft tissue tumors, after conservative treatments (surgery and/or radio- and chemio-therapy). The hyperthermia, which corresponds to the primary tumor, underwent a marked reduction during the first six months after the treatment. The thermographic picture appeared normal at the first follow-up of 12/16 bone tumors and of 9/16 soft tissue tumors. The presence of local recurrencies, at 12 months distance or more, corresponded to a new hyperthermic spot and to the increase of local vascularity. Thermography is proposed as a simple and harmless procedure for the follow-up of bone and soft tissue tumors after conservative treatments.

Thermography in artificially induced coronary circulatory disorders.

In the open chest the heart loses more heat and its epicardiac temperature will fundamentally depend upon the temperature of the body and its environment, upon the flow in the coronaries and upon the actual state of myocardial metabolism. The experiments were carried out with the AGA Thermovision Type 750 equipment. The epicardial temperature changes induced by reactive hyperaemia due to the temporary coronary ligation in 24 dogs, and by permanent coronary ligation in 70 dogs, as well as by the gradually reduced coronary flow of another 8 dogs were recorded by means of thermography. The results indicated a relationship between the changes in epicardial flow and epicardial temperature, consequently thermography is considered a method suitable for the study of myocardial flow.

[Thermography in detecting breast diseases in mass prophylactic examinations].

The article presents the results of a prolonged time course thermographic observation over the mammary glands' heat flow in sound females and in those with nodal and diffuse forms of mastopathy, fibroadenoma and large common cysts. Thermography efficiency was shown in searching the foci with more active heat production, associated with proliferative activity of cellular elements that belong to different malformations within the mammary gland. Comparison of thermographic and mammographic data during mass prophylactic screening allowed to define the possibilities and limitation of thermography as a screening method for further mammographic examining and early detection of breast cancer.

Thermography for the clinical assessment of inferior alveolar nerve deficit: a pilot study.

Neurosensory deficit is a major complication encountered in maxillofacial surgery. This study assessed the ability of electronic thermography to identify inferior alveolar nerve deficits in a pilot clinical study. The study population comprised six patients with inferior alveolar nerve deficit and 12 normal subjects. Frontally projected facial thermograms were taken on 18 subjects and measured using an Agema 870 unit and thermal image computer. Mathematical analysis of thermal measurements included temperature and delta T calculations of the anatomic zone over the mental region of the face. Results included (1) high levels of thermal symmetry of the chin in normal subjects (delta T = 0.1 degree C, standard deviation = 0.1 degree C); (2) low levels of thermal symmetry in patients with inferior alveolar nerve deficits (delta T = +0.5 degree C, standard deviation = 0.2 degree C); (3) statistically significant differences in delta T values (t = 4.82, P > .001) in patients with inferior alveolar nerve deficit; and (4) absolute temperature variations of the mental region in both groups. This pilot study demonstrated thermal asymmetry in patients with inferior alveolar nerve deficit and suggests that electronic thermography has promise as a simple, objective, noninvasive method for evaluating nerve deficits. However, more extensive studies are needed before thermographic procedures are accepted clinically.

The use of thermography in sympathetically maintained pain.

This paper reviews the symptomatology, pathophysiology, and treatment of reflex sympathetic dystrophy and sympathetically maintained pain. It is the author's experience that there exists a group of patients who present with chronic, unexplained pain following trauma, but lack the physical findings and positive investigative tests to confirm the diagnosis of reflex sympathetic dystrophy. For these patients, thermography serves as a useful and sensitive test to diagnosis sympathetically maintained pain. This paper presents six case reports in which thermography was used to diagnosis sympathetic dysfunction as the cause of chronic pain.

Ethyl alcohol induced skin temperature changes evaluated by thermography. Preliminary results.

The authors have investigated the validity of thermography in the study of ethyl alcohol induced hand blood flow changes in normal subjects before and after a standard meal. The superficial temperature changes of the hand of ten healthy subjects, 7 males and 3 females, 25-40 years old, were recorded in 4 conditions: a) on an empty stomach, b) after ingestion of a standard dose of ethyl alcohol (25 cc of whisky at 40%, corresponding to 10 g of alcohol), c) after a standard meal (1200 Kcal), d) after a standard meal and a standard dose of alcohol. Three male subjects were not drinkers. Arterial blood pressure and ECG were monitored during the different phases of the experiment. Results show a typical time course of the hand temperature in response to intake of alcohol, stronger in fasting non drinkers subjects. Our results confirm the valuable role of thermography in the study of alcohol-induced calorigenic effects.

Contact thermography in the diagnosis of childhood migraine.

The objective of our prospective study was to assess the role of contact thermography in children with migraine. Contact thermograms were done in 54 children aged 4.2-16.5 years (median 10.5 years), who were seen for headache and on 10 age-matched controls, between July and December 1991. Thermograms were interpreted as definitely normal, equivocally normal, equivocally abnormal, and definitely abnormal by a radiologist who was blinded to clinical information. Forty-eight children had the test between headaches; of these, four out of 26 patients (15%) who had migraine without aura and 3 out of 14 children (21%) who had migraine with aura had definitely abnormal thermograms. Nine out of 10 normal controls had definitely normal thermograms. The sensitivity of contact thermography in the diagnosis of childhood migraine, when done between headaches, was low in our study.

[Skin-thermography with fluid crystals in orthopedics and sport-medicine (author's transl)].

Thermography with fluid crystals is an effective diagnostic aid for the differentiation between bacterial and non-bacterial inflammation i.e. for the differential diagnosis between joint damage due to overloading and irritative arthrosis. Thermography enables us to assess severity and extent of the condition and to prove the success of treatment.

[Usefulness of contact thermography for the evaluation of chemotherapeutic effectiveness in breast cancer].

We examined temperature differences between a cancerous breast and its counterpart normal one by contact thermography before and after preoperative chemotherapy, and evaluated the relationship between the changes in the thermograms and response to chemotherapy in six patients with breast cancer. We used the following definitions: 1) delta Tmean: temperature differences between a mean temperature of a cancerous breast and that of the contralateral healthy breast; 2) delta Tmax: temperature differences between a cancer-related hyperthermic area in a breast and the mirror area of contralateral breast; 3) and the thermal patterns in thermogram were estimated by the criteria of Tada et al. In responders the thermograms after chemotherapy indicated an improvement in the hyperthermic vascular pattern (HVP) or hyperthermic area and a decrease of delta Tmean and delta Tmax. In contrast, little or no changes were observed in the thermograms of non-responders. Degrees of changes in thermograms reflected the effectiveness of chemotherapy. Our study showed that chemotherapeutic effectiveness may be better evaluated by combining contact thermography with the present method measuring tumor sizes than by only the present one.

Correlation study on infrared thermography and nerve root signs in lumbar intervertebral disk herniation patient: a short report.

OBJECTIVE: To determine whether infrared skin thermography is an objective measurement reflecting the seriousness of nerve root irritation in lumbar disk herniation patients. DESIGN: Quantified nerve root signs by physical examination were collected from the patients along with the infrared skin temperature measurement on the lumbosacral region and posterior part of thighs. A correlation study was applied to observe the relation between the nerve root signs and the skin temperature before a successful conservative treatment (mainly spine manipulation), and between the alteration of nerve root signs and that of skin temperature after the treatment. SETTING: Hospitalized care. PATIENTS: Twenty-seven hospitalized samples with computed tomography or magnetic resonance approval were consecutively selected during the latter half of 1990. MAIN OUTCOME MEASURE: Changes in nerve root signs. RESULTS: The temperature difference between a troubled thigh and healthy one is significantly correlated to the score of the nerve root signs before the treatment; and the reduction of temperature difference between two thighs is also significantly correlated with decreasing score of nerve root signs after the treatment. The correlation between the temperature difference on the left and right sides of the lumbosacral region and the nerve root signs before the treatment is insignificant; and the variation of the temperature difference of the same region after the treatment is not correlated with the decreasing score of the nerve root signs. CONCLUSION: Infrared skin thermography of lower extremities might be an objective sign in signaling the soothing process of the nerve root irritation in lumbar disk herniation patients, which may help a doctor in checking the responses of the patient to treatment.

[Serial computed thermography for Raynaud's phenomenon of patient with progressive systemic sclerosis (PSS)].

We reported a case with progressive systemic sclerosis, a relatively rare disease in childhood. Raynaud's phenomenon (RP) of the patient was serially monitored by computed thermography for 1 year. Although clinically apparent RP manifested only in winter season, thermographical changes were documented through all season, suggesting that RP is a constitutional sign which may emerge in winter by provocation of atmosphere. Thus, we recommend to use the words, "Raynaud's phenomenon" and " Raynaud's symptom", in an appropriate meaning. RP of the patient, however, varied depending on the time determined, which indicated that the cause of RP is not due to the irreversible tissue degenerations, but due to the reversible functional factors. Computed thermography was proved useful for detecting RP objectively, and will be valuable for estimating the effects of medications for RP.

[Changes in symptom and thermography images after lumbar sympathetic ganglion block].

In 35 lower limbs of 22 patients with peripheral circulation disturbance, we evaluated the effects of lumbar sympathetic ganglion block (LSGB) by skin temperature difference (delta T) between the two measured by thermography before and after LSGB. We divided the patients into two groups; ASO group (n = 15): patients with chronic arterial occulusive diseases, and nonASO group (n = 7): those with peripheral circulation disturbance partly due to decrease nervous function. After LSGB, we recognized a significant increase in delta T and remission of ischemic symptoms in all the patients of nonASO group and those in early stage of ASO group. This suggests that in ASO group LSGB should be applied as soon as possible to obtain good outcome, and thermography is useful to evaluate the effects of LSGB in patients with peripheral circulation disturbance of the lower limbs.

Electronic thermography for the assessment of acute temporomandibular joint pain.

Electronic thermography (ET) has the potential to be a nonionizing, noninvasive, low-cost diagnostic alternative for evaluating temporomandibular joint (TMJ) disorders. This study was designed to evaluate the use of ET as a diagnostic aid in the assessment of patients with acute TMJ pain. Computer measurements made using facial thermography were able to distinguish normal patient populations from symptomatic patients with acute TMJ pain. Additional studies are needed before thermographic diagnosis of TMJ disorders will be clinically accepted.

[Infra-red thermography testicular studies of patients with short spermatic cords and quality of sperm morphology].

116 patients with too short spermatic cords were examined by infra-red thermography and semen analysis. Examinations of semen included number, mobility, viability and pathological forms of spermatozoa. The values of the thermographic index of scrotum were compared with the control parts of the body. The incidence of hyperthermia of scrotum in observed men was 79%. In analysis of semen there were 79% teratozo-ospermia, 73% oligozoospermia, 59% astenozoospermia and good viability in 70%. We suppose that hyperthermia of scrotum in infra-red thermography, oligoteratozoospermia in semen analysis and no progress in pharmacological treatment may be an excellent qualification method for surgical treatment of too short spermatic cords.

[The value of liquid crystal thermography in the detection of breast cancer by the gynaecologist (author's transl)].

A report is present of 1300 unselected patients investigated for breast cancer by liquid crystal thermography. The value of this method is comparable to the value of mammography, since the number of carcinomas which remained undetected by each diagnostic procedure was equal. The high degree of diagnostic accuracy achieved by the use of liquid crystal thermography indicates that this simple dignostic procedure is recommendable as a screening method for the detection of breast cancer in gynaecological practice.

Relation between Locoregional Hyperthermic Area Detected by Contact Thermography and the Maximum Density of Tumor Stain Obtained by IV-DSA in Breast Cancer Patients.

We studied the significance of the locoregional hyperthermic area observed by contact thermography in relation to the maximum density of tumor enhancement obtained by IV-DSA. The subjects were 34 patients with primary breast cancer. The thermal difference between the locoregional hyperthermic area and the surrounding mammary tissues was obtained, and the maximum density of tumor stain on IV-DSA was calculated from the time-density curve. Twenty-three (67.6%) patients had a locoregional hyperthermic area corresponding to the tumor. The higher the degree of thermal difference between the hyperthermic area and the surrounding tissues, the higher the maximum density of tumor enhancement. As the maximum density of tumor stain is considered to reflect the vascularity of the tumor, the degee of the locoregional hyperthermic area obtained by thermography may also reflect tumor vascularity.
