AIM: This paper reports a study evaluating the sensitivity of a semi-structured interview schedule and card sort methods in assessing postpartum concerns of women. BACKGROUND: Several methods have been used to assess postpartum maternal concerns and the process of becoming a mother, but few studies have evaluated the methods with respect to their sensitivity for obtaining information. METHOD: A cohort of mothers was followed-up at one (n = 110) and 6 weeks (n = 83) after childbirth in Dar es Salaam, Tanzania. Women with a minimum of 7 years of primary education were interviewed and they also sorted cards. Those with less fewer than 7 years of primary education were interviewed only. The methods were used in alternate order to assess method interaction. RESULTS: In the interviews at 1 week, mothers more often expressed worry and interest related to the baby or themselves when they had sorted cards first. The extent to which women expressed worry and interest about specific baby- and mother-related topics was generally higher for women who had sorted cards before the interview at both 1 and 6 weeks. Independent of whether they were interviewed only, interviewed after sorting cards or before, mothers more often expressed a higher degree of interest than of worry about the baby and self at both 1 and 6 weeks. The order of the data collection methods did not influence the way women sorted cards as being worries and interests. CONCLUSION: Compared to interview using a semi-structured interview schedule, our findings suggest that the card sort is more sensitive in obtaining information about women's concerns. Although the interview method has the advantage of reaching less educated people, the card sort is a technique that is associated with fewer barriers and is a more participatory method for those who can use it.
The serial trends of the whole blood folate level in two groups of patients have been followed throughout pregnancy and up to six weeks postpartum. In those receiving iron alone the whole blood folate remained normal until the test at six weeks after delivery, at which time over half were in the deficient range. There appears to be a delay before this test reflects the current folate status when this changes rapidly. In those receiving iron plus 330 mug. of folic acid a day the results at this time were close to those at the beginning of pregnancy. Subnormal whole blood folate, red cell folate, and serum folate values occurred close to term in patients receiving iron alone, but were not found in those also receiving folic acid. Megaloblastic changes occurred at term in three patients receiving iron alone in whom the whole blood folate had repeatedly been low in early pregnancy. The observations are consistent with the previous suggestion that 300 mug. of folic acid daily is a suitable supplement to prevent deficiency in late pregnancy and the puerperium.

We have compared the pharmacokinetics of a bolus dose of propofol 2 mg kg-1 in eight patients undergoing Caesarean section with those in eight postpartum patients undergoing sterilization by mini-laparotomy. The Caesarean section group had a total body clearance of (median) 31.5 (range 24.4-53.3) ml min-1 kg-1, apparent volume of distribution at steady state 5.10 (2.46-6.61) litre kg-1 and mean residence time 161 (52.3-251) min; values for the post-partum group were 33.8 (21.5-47.2) ml min-1 kg-1, 5.17 (3.47-8.09) litre kg-1 and 163 (92.3-238) min, respectively. The 95% confidence interval for the umbilical venous to maternal venous ratio of propofol at delivery was 0.62-0.86. Plasma protein binding studies showed there was less unbound propofol in maternal plasma (1.28-2.29%) compared with umbilical plasma (2.08-3.88%) (P less than 0.01). Neonatal concentrations of propofol were greater than maternal concentrations at 2 h and were in the range 0.05-0.11 micrograms ml-1 at 4 h.

This study investigated the relationship between plasma concentrations of beta-carotene and the alpha-tocopherol/lipid ratio, and smoking in 233 postpartum women, from July to November 2002, in the city of Joinville, Brazil. Thirty-seven percent (37%) of the women included in this study had low concentrations of alpha-tocopherol (< 11.63 micromol/L) and 61% of the women presented low concentrations of beta-carotene (< 0.39 micromol/L). Multivariate regression analysis investigated the relationship between smoking and the concentration of the vitamins, adjusting by confounding variables. The final regression model showed inverse associations between beta-carotene concentrations and smoking (p = 0.036), and beta-carotene concentrations and triglyceride levels (p = 0.021). There was an association between the plasma alpha-tocopherol/lipid ratio and per capita income (p = 0.003), but no association with smoking. We advise further studies to evaluate the impact of smoking and oxidative stress on the concentrations of these vitamins.

The objective of this research was to investigate sequential serum levels and level/dose ratios of the tricyclic antidepressant nortriptyline (NTP) through the first 17 postpartum weeks. The initial NTP dose was given immediately postpartum to 16 mothers and increased gradually to 70 mg over the first week. A dose of 75 mg was prescribed until adjustment according to serum levels. Serum levels of NTP and its metabolites Z- and E-OH-NTP were determined. At postpartum Week 2, the women developed a mean level/dose (L/D) ratio for NTP of 1.11 (range 0.37 to 3.23), and subsequently experienced an increase in the L/D ratios which continued through Week 6. At Week 8, the NTP L/D ratios declined, and became relatively stable at Week 11 and beyond. For postpartum women treated with NTP, side effect profiles should be carefully followed during the first 6 weeks after delivery as a clinical marker for elevation of serum levels. Since our highest L/D ratios for NTP occurred at Week 6, a serum level is recommended at this time. If the dose needs to be lowered to maintain a nontoxic level, a repeat serum level should be obtained at Week 11, at which time an increase in dose may be required.
Cows were prepared for surgery, either within 12 h after parturition (Day 0, n = 3), on Day 1 (n = 1) or on Day 2 (n = 1) postpartum. The previously gravid uterine horn was exposed through a high lumbar laparotomy, and catheters inserted into an uterine branch of the ovarian vein (UBOV) and in the main uterine artery (UA) via a branch of the main uterine artery. A blood flow transducer (8-13 mm, internal diameter) was placed around the UA. Starting 12-24 h following surgery, simultaneous blood samples were collected from UBOV and UA at 15-min intervals for 1 or 2 h daily from Days 1 to 10 postpartum (mean = 5.3 days). Concurrently, uterine blood flow (UBF) was recorded daily for 6 h. Mean concentrations of prostaglandin F2 alpha (PGF2 alpha) and 15-keto-13, 14-dihydro-PGF2 alpha (PGFM) were higher (P less than 0.01) in UBOV than UA (4.64 +/- 0.6, n = 96 vs. 0.08 +/- 0.02 ng/ml, n = 92 and 5.67 +/- 0.41, n = 98 vs. 1.02 +/- 0.09, n = 94, ng/ml, respectively). Concentrations of PGF2 alpha and PGFM in the UBOV increased (P less than 0.1) on Days 2 and 3 and decreased (P less than 0.01) thereafter. Mean UBF (ml/min) decreased (P less than 0.01) from 3079 +/- 683 on Day 1 to 602 +/- 56 on Day 2 and was 413 +/- 55 on Day 5.(ABSTRACT TRUNCATED AT 250 WORDS)
