[Qualitative determination of U-pregnanediol-3 alpha-glucuronide in 5 minutes. A clinical trial of a new matrix cushion test].

We have tested an easy and rapid "matrix cushion test" for qualitative determination of U-pregnandiol-3 alpha-glucuronide (U-PGD). The specificity of the test is high, the sensitivity is 4.0 microM, the inter-analysis variation is low as in 96% of the double determinations, agreement was present between the first and second analyses. A positive result on the 21st of the menstrual cycle indicates that ovulation has occurred while this cannot be exluded by a negative or doubtful result. If demonstration of a functioning corpus luteum is also required, the urine should be diluted 1:2 with tap water.

[A feminizing adrenal carcinoma in man: in vivo and in vitro study (author's transl)].

A 31 years old man with an adrenocortical carcinoma was studied. Clinically he had a bilateral and recidiving gynecomastia and showed high urinary oestrogens, 17 cetosteroids, tetra-hydro-desoxy-cortisol and pregnandiol excretion with normal cortisol production. A partial increase on ACTH, no suppression on dexamethasone and no variation on HCG administration were observed. The surgical resection of the tumor normalized this urinary excretion. The serum dehydro-epiandrosterone (DHEA) and sulfate (DHEAS), oestrone, oestradiol, androstenedione (A) levels were greatly elevated. No variations of the cortisol, A, DHEA and DHEAS was noted after ACTH injection. In vitro the lack of ACTH's action was related to an anomaly of ACTH receptor with normal protein kinase activity.

[Sequential test with clomiphene and LH-RH in cases of anovulatory sterility (author's transl)].

In sterile menstruating women with anovulatory cycle, the response of plasmatic gonadotropins and urinary total Estrogens and Pregnandiol to the administration of Clomifene and LH-RH in a sequential test is studied. Five different types of response, similar to those detected in previous studies using the LH-RH test, are identified; in them, uniformity in the response of gonadotropins exists to both LH-RH and Clomifene. The results obtained seem to confirm the existence of diverse etiopathogenic mechanisms in the alterations of the Hypothalamus-Pituitary-Ovarian axis resulting in anovulatory cycle.

Effects of sulpiride on levels of FSH, LH and steroid hormones.

In order to study the effects of prolactin upon the gonadotrophins and steroid hormones, hyperprolactinaemia was induced by the administration of sulpiride. 12 men between the ages of 18 and 20 were given 3 capsules of 50 mg of sulpiride daily for a period of 15 days, and the following parameters being measured before and after the treatment: (prolactine, FSH, LH, testosterone, estradiol, ACTH and DHEA-S) by RIA, (cortisol) by fluorimetry and (etiocholanone, androsterone, pregnandiol, pregnantriol, pregnantriolone, 11-keto etiocholanone and 11-OH androsterone) by gas chromatography. Our results show that on termination of the treatment there was a significant rise in the prolactin and DHEA-S serum levels and a drop in the FSH serum levels but not of LH. In addition there was a marked increase in all the androgen levels studied, (etiocholanone, androsterone and 11-keto etiocholanone) with the exception of testosterone.

[Cytodiagnosis and premature birth in light of the Bethesda system].

1. Evaluation of cytological cervico-vaginal smears by the Bethesda system enlisted cytodiagnostics among important laboratory methods which can be used also in risk pregnancies. 2. Vaginal cytology makes it possible to test at the same time the hormonal situation during pregnancy, which reflects the placental function, and to evaluate also the vaginal biocenosis. 3. The authors provided evidence that the large number of superficial cells on the cytological smear (more than 10%) is associated with low oestriol and pregnandiol levels which are warning signs of the approaching termination of pregnancy. 4. By the action of microorganisms on the vaginal epithelium typical morphological changes develop in the cell nucleus and in the cytoplasm. By polychromatic staining also the causal agents of inflammations and infections threatening the mother and foetus are apparent. 5. The authors assume that cytological examination and evaluation according to the Bethesda system should be included in the complex of antenatal examinations also in women without clinical symptoms of premature delivery or without signs of vaginal infection.

[Calorimetric study of thermoregulatory mechanisms at the onset of sweating and during the menstrual cycle (author's transl)].

1 At the onset of sweating, tympanic (Tty), vaginal (Tva) and mean skin (Ts) temperature as well as the sudoral response were studied by direct and indirect calorimetry. Eight women with a normal ovulatory cycle were consecutively exposedt wice to 37 degrees C/20% rh during the follicular phase and again during the luteal phase. 2 Under our conditions, Tty may be used as an index of deep body temperature without any notable influence from ambient temperature. A weighting factor of chi = 0,7 was applied to the internal part of the body for the calculation of mean body temperature Tb. chi was determined by calorimetry, before and after the onset of sweating. 3 The onset of thermal sweating depends on a characteristic Tb corresponding to a specific caloric content of the body. Neither Tty, Tva, nor Ts, alone constitutes the specific stimulus leading to thermolysis by sweating. 4 The set point value (Tb "set") of the thermoregulatory system increases in the luteal phase, compared with that of the follicular phase. 5 Thermal regulation of the proportional control system shows a gain which is not modified during the menstrual cycle. 6 There is no relation between the shift of the set point value (Tb "set") and the increase of urine pregnandiol in the luteal phase. 7 The increase in basal body temperature and its maintenance during the post-ovulatory phase do not seem to be related to an increase in metabolic heat production.

Study on physiopathology of the chronic psychoorganic syndrome. Investigation on the reactivity of the adrenal glands in patients with psychoorganic chronic.

Out of a great number of cases with chronic psychoorganic syndrome studied by us, we have selected, for investigation, a number of 100 cases which presented common symptomatology: a psychosyndrome showing, by a large number of manifestations such as asthenia, fatigability, adynamia with various degrees of intensity building up to reaction latency, diminution or even absence of initiative, basic-negativism, tendency to depression with feeling of futility, anxiety, lowered affective tonus. The intellectual activity is largely diminished, the stream of ideas is poor, and there is a limited domain of preoccupations. All these symptoms alongside with somatic, muscular, renal, respiratory, digestive and cardiovascular disorders have led us to the hypothesis of chronic deficiency of the hormones in the adrenal glands. The adrenal glands have been studied by indirect exploration of the hydroelectrolytic metabolism of the peripheral blood, the Thorn test and the Robinson-Power-Kepler test. For the exploration of the glucocorticoids, a basal test has been used such as the 17-hydroxycorticoid test, which measures cortisol elimination, or as the 11-desoxycorticosteroids and metabolites, or the colorimetric Porter-Silber method. Among the dynamic tests, we have used stimulating tests, the ACTH test (synthetic Synachtene), which measures cortisolemia and the 17-urinary corticosteroids, faster and easier than the Thorn test. In order to assess androgens, we have used the ACTH deposit dynamic test (Synachtene retard), which in the case of normal function of the adrenal glands, in 24 hours, doubles the elimination of 17-hydrocorticosteroids, 17-Ketosteroids, D. H. E. A., pregnandiol, pregnantriol.(ABSTRACT TRUNCATED AT 250 WORDS)

[Sex steroid profiles and ovarian activities of the female panda Yan Yan in the Berlin Zoo].

The nine years old giant panda YAN YAN was received in April 1995 on loan for 5 years to the Berlin Zoo. Urine samples were collected daily or every second day from April 1995 to June 1996 in order to follow up sex hormone secretions and ovarian activities. Conjugated steroids were hydrolysed, extracted and measured with two enzyme immuno assays (EIA) being specific for either total oestrogenes or pregnandiol. The evaluation of the hormone secretion pattern yielded the following results: There is a significant synchronous cross correlation between estrogen and progestin metabolites secretion indicating its simultaneous synthesis. In addition, we found a regular increase and decrease of both hormones with a 13 days interval. This secretory pattern indicates repeating development and atresia of follicle cohorts with a cycle length of about 13 days. Only a single period of slightly elevated oestrogen synthesis was monitored in Feb. 96 without any signs of oestrus. Obviously the stimulation of ovarian function was insufficient for complete ovulation and corpus luteum formation.

[The importance of hormone examinations in ovulation induction (author's transl)].

Those hormone-determination methods were reported by the authors which help to make the ovary response visible for the experts during the ovulation induction. They also dealt with the determination of basal temperature measuring, oestrogen, pregnandiol, progesterone, oestradiol-17beta, FSH, LH, epimestrol and clomiphen. After the routine determination of the above mentioned parameter a preprinted sheet was done, where every treated case was illustrated graphically. From these cases some of the typical ones were shown and analyzed. The determination of hormonal releasing curves characteristic of hyperstimulation, and the importance of these most dangerous complication was emphasized.

[Hormonal function of the ovaries in women with breast hyperplasia].

In females showing fibrous-cystic mastopathy and fibroadenomatosis of mammary glands a specific form of progesterone insufficiency- relative one, was revealed, a high persistantly maintained level of urine pregnandiol and blood progesterone in a luteal phase (indicating a high hormonal activity of the corpus luteum) being observed. But luteal transformations were insignificant both in endometrium and vaginal epithelium, a moderate hypoestrogenemia being noted in the first phase and increased estrogen excretion- in the second phase of the cycle.
