[Metabolic compensation and chronic obliterating arteriopathy in diabetes mellitus].

The following parameters were determined in 20 diabetics with chronic obliterating arteriopathy of the lower extremities: C-HDL, apo-A1, apo-B, triglyceridaemia, the glycaemic profile and HbA1c oscillometry, photoplethysmogram and claudication distance on a treadmill. Examination of the results obtained did not show any variation in the lipoproteic picture and only an improvement, in some subjects, in claudication distance. It is therefore hypothesised that in diabetic C-HDL and apo-A1 are not predictive of atherosclerotic risk although it cannot be excluded that changes in such constants may represent a synergetic factor in determining arteriosclerosis.

Self-measurement of blood pressure: assessment of equipment. Canadian Coalition for High Blood Pressure Prevention and Control.

Equipment for the self-measurement of blood pressure is readily available to consumers. These devices use one or more surrogate (indirect) measures of pressure to estimate systolic and diastolic blood pressure. Manual auscultatory devices using stethoscope and sphygmomanometer have been adapted for home use, but a variety of automated devices based on auscultation, oscillometry, and other techniques are available and may be more suitable for individuals who have limited vision, hearing or dexterity. Despite the existence of voluntary evaluation protocols and mandatory manufacturing standards, blood pressure readings from some automatic devices may not be accurate. Some devices are packaged with insufficient information to ensure proper use, and most individuals need some form of guidance in their use and calibration testing. If self-measurement of blood pressure is to be of benefit, the health care professional must recommend only those devices that are accurate and suitable to the patient or client. The Canadian Coalition for High Blood Pressure Prevention and Control will endeavour to develop a regular means by which health care professionals can keep informed of available devices for blood pressure self-measurement.

Effects of age and isolated systolic hypertension on cardiovascular reflexes.

OBJECTIVES: Given the reported relationship between systolic hypertension and orthostatic hypotension in the elderly, to test the hypothesis that systolic hypertension causes impairment of the cardiovascular reflex function additional to the effects of age alone. DESIGN: Responses were compared in normotensive healthy young (n = 12) and elderly (n = 15) participants and elderly participants with disproportionate supine systolic hypertension (n = 11) using a baroreceptor-mediated stress (head-up tilt) and two non-baroreceptor-mediated stimuli (cold pressor test and isometric exercise). METHODS: Blood pressure and heart rate were measured by oscillometry before and during the three stress tests. Forearm blood flow was measured by venous occlusion plethysmography and pulse wave velocity (PWV) by Doppler ultrasound. RESULTS: Percentage changes in systolic/diastolic (SBP/DBP) blood pressure with head-up tilt were 0/+11, -3/0 and -6/+1 mmHg in the young and elderly normotensives and elderly systolic hypertensives, respectively. Both elderly groups had reduced DBP responses to tilt compared with the young (P < 0.01). All three groups had similar percentage changes in blood pressure responses to non-baroreflex-mediated stresses (cold pressor test: +10/+23, +11/+11, +10/+15; sustained isometric exercise: +18/+33, +22/+24, +13/+17 in the young and elderly normotensives and elderly systolic hypertensives, respectively). Aorto-iliac PWV adjusted for blood pressure was significantly higher in both elderly groups compared with the young (P < 0.01) but there was no difference between elderly normotensives and hypertensives. Unadjusted PWV was higher in elderly hypertensives than in elderly normotensives (P < 0.05). CONCLUSIONS: Compared with healthy young participants, both elderly groups had similarly attenuated blood pressure responses to tilt and reduced arterial compliance. Systolic hypertension is not associated with additional impairment of cardiovascular reflex function over and above the effects of age. The reported association between supine systolic hypertension and orthostatic hypotension does not appear to be a causative one.

Determinants of blood pressure in infants admitted to neonatal intensive care units: a prospective multicenter study. Philadelphia Neonatal Blood Pressure Study Group.

There are few blood pressure (BP) data reported for premature and term newborn infants after 24 hours of age. To determine BP levels and BP trends in a representative population of infants admitted to neonatal intensive care units (NICUs), this study was conducted in 14 NICUs in the greater Philadelphia area. All infants admitted to the 14 NICUs during a 3-month period were entered into the study. BP data, along with data on clinical conditions and therapeutic interventions (independent variables), were prospectively collected by a uniform protocol. Systolic BP (SBP) and diastolic BP (DBP) were measured indirectly by oscillometry and recorded every 8 hours. Data from 608 infants followed up for 1 to 99 days after delivery generated 9911 infant-day records and 24,052 individual BP measurements. On day 1, birth weight and gestational age were strong correlates of SBP (r = 0.68, p < 0.0001 and r = 0.66, p < 0.0001, respectively) and DBP (r = 0.48, p < 0.0001 and r = 0.47, p < 0.001, respectively). During the first 5 days of life there was a progressive rise in SBP (2.23 to 2.67 mm Hg/day) and DBP (1.58 to 2.02 mm Hg/day) regardless of gestational age or weight at birth. After day 5 there was a more gradual increment in the daily SBP (0.24 to 0.27 mm Hg/day) and DBP (0 to 0.15 mm Hg/day). Stepwise linear multiple regressions were used to examine the multiple correlations among the independent variables and to build a regression model for BP. Gestational age and day of life emerged in the first two steps of the multiple regression analysis (multiple R = 0.463 and 0.655, respectively; p < 0.0001 for both). The multiple R values for day of life and gestational age were virtually identical to that for postconceptional age (day of life + gestational age at birth). Although other common diagnosis and treatment variables contributed a small amount to the total variance in BP, postconceptional age was the primary determinant of BP in this population of infants.

[Characteristics of disorders of body functions in post-thrombophlebitic disease].

397 patients with chronic diseases of the extremities veins were examined, 256 of them suffered postthrombophlebitic disease. Along with the general clinical examination special investigations were conducted: ECG under orthostatic stress conditions, test tube and instrumental auto-coagulograms, thromboelastograms, biomicroscopy of the anterior ocular segment, studies of the intraocular pressure, ocular fundus, rheovasography, skin thermometry, oscillometry (with nitroglycerin stress), the extremity volume under orthostatic stress conditions, myography, non-contrast roentgenography of the lower extremities, and also the phenomenon of intradermal diffusion and pheblography. It was found that with the progress of postthrombophlebitis trophic disorders are observed in all tissues of the involved extremity. Postthrombophlebitic disease is associated not only with local involvement of vessels, but also with functional disturbances of other organs.
