Cytostatics for acromegaly. Marked improvement in a patient with an invasive pituitary tumour.

A 21-year-old woman suffering from acromegaly was treated with transsphenoidal subtotal hypophysectomy (microscopy: acidophilic adenoma), followed by x-ray and bromocriptine therapy. Seven years later she was re-operated because of a partial bitemporal loss of vision, intracranial hypertension, and regrowth of the pituitary tumour seen on CT-scan. A large part of the invasive suprasellar tumour was then removed by transcranial approach. The neurosurgery was followed by cobalt radiotherapy and bromocriptine administration. Two years later, symptoms and signs of tumour growth reappeared. Administration of cytostatics, such as doxorubicin (Adriamycin) and lomustine (Belustine), resulted in distinct clinical improvement associated with a seven-fold decrease in the serum growth hormone concentration. The visual field became normal and the intracranial mass on a CT scan decreased markedly. As a result the patient was able to resume work.

[Overlooked bronchial rupture after blunt thoracic injury].

A 72-year-old man developed a total atelectasis of the right lung six weeks after a blunt trauma in a motor vehicle accident. Bronchoscopy demonstrated a total fibrotic occlusion of the right main bronchus. By re-examination of earlier case sheets and chest x-rays it was suspected that the patient had suffered a partiel bronchial rupture from the accident. One and a half centimetres of the bronchus were resected with a good result. Findings and treatment of this rare complication are discussed.

Etiology of calcium oxalate nephrolithiasis in rats. II. The role of the papilla in stone formation.

In kidneys of healthy rats submitted to a crystal-inducing diet (CID) with ethylene glycol (EG) and NH4Cl, the fate of retained crystals in the papillar region is studied during a recovery period of one, five or ten days, as model system for human nephrolithiasis. Scanning electron microscopy (SEM) shows, at papillary tips bulging into the calycine space, crystal masses covered either by the epithelium or a thin fibrous veil, or by unidentified mobile cuboidal cells. After CID plus one or five days recovery, small sub-epithelial swellings are seen of large sub-epithelial crystals at or around the papillary tip. After CID plus ten days, massive sub-surface crystal-containing micrometer-sized stones are seen in which the presence of calcium is confirmed by X-ray microanalysis. The papillary tip of rats after a re-challenge with an oxalate load from 0.1 vol% EG for twelve or forty-two days shows minor lesions. But a re-challenge with 0.3 vol% EG for thirty-seven days induces large sub-epithelial papillary millimeter-sized stones. The Von Kossa section staining converts the crystals into a black precipitate, but large peri-tubular or peri-vascular calcium deposits are absent. A new hypothesis about the etiology of an inductive calcium oxalate monohydrate nephrolithiasis is formulated which differs from the one proposed by Randall based on his deductive human kidney studies.

Crystal structure of bacteriophage lambda cII and its DNA complex.

The tetrameric cII protein from bacteriophage lambda activates transcription from the phage promoters P(RE), P(I), and P(AQ) by binding to two direct repeats that flank the promoter -35 element. Here, we present the X-ray crystal structure of cII alone (2.8 A resolution) and in complex with its DNA operator from P(RE) (1.7 A resolution). The structures provide a basis for modeling of the activation complex with the RNA polymerase holoenzyme, and point to the key role for the RNA polymerase alpha subunit C-terminal domain (alphaCTD) in cII-dependent activation, which forms a bridge of protein/protein interactions between cII and the RNA polymerase sigma subunit. The model makes specific predictions for protein/protein interactions between cII and alphaCTD, and between alphaCTD and sigma, which are supported by previous genetic studies.

Tracking of a bronchoscope using epipolar geometry analysis and intensity-based image registration of real and virtual endoscopic images.

This paper describes a method for tracking the camera motion of a flexible endoscope, in particular a bronchoscope, using epipolar geometry analysis and intensity-based image registration. The method proposed here does not use a positional sensor attached to the endoscope. Instead, it tracks camera motion using real endoscopic (RE) video images obtained at the time of the procedure and X-ray CT images acquired before the endoscopic examination. A virtual endoscope system (VES) is used for generating virtual endoscopic (VE) images. The basic idea of this tracking method is to find the viewpoint and view direction of the VES that maximizes a similarity measure between the VE and RE images. To assist the parameter search process, camera motion is also computed directly from epipolar geometry analysis of the RE video images. The complete method consists of two steps: (a) rough estimation using epipolar geometry analysis and (b) precise estimation using intensity-based image registration. In the rough registration process, the method computes camera motion from optical flow patterns between two consecutive RE video image frames using epipolar geometry analysis. In the image registration stage, we search for the VES viewing parameters that generate the VE image that is most similar to the current RE image. The correlation coefficient and the mean square intensity difference are used for measuring image similarity. The result obtained in the rough estimation process is used for restricting the parameter search area. We applied the method to bronchoscopic video image data from three patients who had chest CT images. The method successfully tracked camera motion for about 600 consecutive frames in the best case. Visual inspection suggests that the tracking is sufficiently accurate for clinical use. Tracking results obtained by performing the method without the epipolar geometry analysis step were substantially worse. Although the method required about 20 s to process one frame, the results demonstrate the potential of image-based tracking for use in an endoscope navigation system.

Induction of chromosomal aberrations in the CHO mutant EM9 and its parental line AA8 by EcoRI restriction endonuclease: electroporation experiments.

EcoRI restriction endonuclease (RE), which produces cohesive-ended double-strand breaks (dsb) in DNA, was tested in the ethyl methanesulfonate- and X-ray-sensitive CHO mutant EM9 and its parental cell strain AA8 for its chromosomal aberration-inducing effect. The RE was efficiently introduced by electroporation into AA8 cells, while the mutant cells showed a very high sensitivity to electroporation, which consistently resulted in cell death. Nevertheless, the incubation of EM9 cells in the presence of EcoRI, without electroporation, was sufficient to induce about three times the chromosome damage observed in the electroporated parental cell line AA8 for any given dose of the RE.

[Postoperative invagination in early childhood].

Postoperative intussusception is a rare but serious complication which occurs usually during the early postoperative period with intermittent, but progressing intestinal obstruction with characteristic appearance of mucus-bloody stools. Two children with postoperative intussusception have been observed among 381 children who have had abdominal operations during the last three years (1985-1987) (0.5 per cent). Various postoperative complications developed in 45 children (11.8 per cent), in 2 of them (4.4 per cent) the complication being intussusception. Both children were operated for tumors. In one (1 1/2 years old) the postoperative intussusception was not diagnosed until it was re-operated; the child died. In the second child (3 months old) the postoperative intussusception was diagnosed by clinical and X-ray examination; the ileocolic portion of the intussusception was reduced by pneumocoloscopy, while the ileo-ileal required re-operation with manual reduction. The infant survived. Early diagnosis of this rare postoperative complication, especially when mucus-bloody stools appear and pneumocoloscopy is performed, is feasible. Appropriate treatment will help for a favourable outcome.

Aromatic vs aliphatic C-H cleavage of alkyl-substituted pyridines by (PNPiPr)Re compounds.

Both (PNP)Re(H)(4) and (PNP)ReH(cyclooctyne) (PNP(i)(Pr) = ((i)Pr(2)PCH(2)SiMe(2))(2)N) react with alkylpyridines NC(5)H(4)R to give first (PNP)ReH(2)(eta(2)-pyridyl) and cyclooctene and then, when not sterically blocked, (PNP)Re(eta(2)-pyridyl)(2) and cyclooctane. The latter are shown by NMR, X-ray diffraction, and DFT calculations to have several energetically competitive isomeric structures and pyridyl N donation in preference to PNP amide pi-donation. DFT studies support NMR solution evidence that the most stable bis pyridyl structure is one that is doubly eta(2)- with the pyridyl N donating to the metal center. When both ortho positions carry methyl substituents, cyclooctane and the carbyne complex (PNP)ReH(tbd1;C-pyridyl) are produced. Excess 2-vinyl pyridine reacts with (PNP)Re(H)(4) preferentially at the vinyl group, to give 2-ethyl pyridine and the sigma-vinyl complex (PNP)ReH[eta(2)-CH=CH(2-py)]. The DFT and X-ray structures show, by various comparisons, the ability of the PNP amide nitrogen to pi-donate to an otherwise unsaturated d(4) Re(III) center, showing short Re-N distances consistent with the presence of pi-donation.

[Rational surgical approaches to the treatment of small primary liver cancer, and the prevention of postoperative recurrence].

From Sep. 1985 to Dec. 1990, surgical treatment was performed in 27 patients with small primary liver cancer (SPLC, < or = 5cm in diameter). Of them, segmentectomy was done in 23 cases and radical local resection in 4 cases with recurrence rate of 66.77% (18/27). Non recurrent lesions were located in the incisal margin. In this group re-resection rate was 55.6% (10/18). (1) Early detection and treatment of recurrent lesions remain a mainstay of prolonging survival. (2) Serum Alpha-fetoprotein (AFP), ultrasonography and X-ray chest film were basic follow-up methods for subclinical recurrence of SPLC. For re-operation cases, digital subtract angiography (DSA) are useful in identifying subclinical lesions. (3) For recurrent liver cancer local hepatectomy was a reasonable approach. (4) For SPLC, radical segmentectomy or radical local resection with a safe margin of 1 to 2cm was the authors' choice.

Carbohydrate-appended 2,2'-dipicolylamine metal complexes as potential imaging agents.

Three discrete carbohydrate-appended 2,2'-dipicolylamine ligands were complexed to the {M(CO)(3)}(+) (M = (99m)Tc/Re) core: 2-(bis(2-pyridinylmethyl)amino)ethyl-beta-d-glucopyranoside (L(1)()), 2-(bis(2-pyridinylmethyl)amino)ethyl-beta-D-xylopyranoside (L(2)()), and 2-(bis(2-pyridinylmethyl)amino)ethyl-alpha-d-mannopyranoside (L(3)). An ethylene spacer is used to separate the carbohydrate moiety and the dipicolylamine (DPA) function in all three ligands. The Re complexes [Re(L(1-3))(CO)(3)]Br were characterized by (1)H and (13)C 1D/2D NMR spectroscopies, which confirmed the pendant nature of the carbohydrate moieties in solution. NMR measurements also established the long-range asymmetric effect of the carbohydrate functions on the chelating portion of the ligand. One analogue, [Re(L(1))(CO)(3)]Cl, was characterized in the solid state by X-ray crystallography. Further characterization was provided by IR spectroscopy, elemental analysis, conductivity, and mass spectrometry. Radiolabeling of L(1)-L(3) with [(99m)Tc(H(2)O)(3)(CO)(3)](+) afforded high yield compounds of identical character to the Re analogues. The radiolabeled compounds were found to be stable toward ligand exchange in the presence of a large excess of either cysteine or histidine over a 24-h period.

Stereoselectivity by enantiomeric inhibitors of matrix metalloproteinase-8: new insights from molecular dynamics simulations.

Molecular Dynamics simulations in aqueous solution were performed for the matrix metalloproteinase-8 (MMP-8) free catalytic domain and for its complexes with the (R)- and (S)-[1-(4'-methoxybiphenyl-4-sulfonylamino)-2-methylpropyl] phosphonate. The 144-155 loop of the enzyme undergoes a drastic decrease of mobility once complexed with both enantiomers. The two enantiomers induce a different decrease of conformational entropy upon complexation. The higher affinity of the R-enantiomer can be related to the lower loss of conformational entropy accompanying its binding. The differences in the dynamical behavior of the protein induced by the two enantiomers are discussed at molecular level and the mode of binding of the simulated complexes is compared with that previously determined by X-ray crystallography.

Outcome of residual mediastinal masses of thoracic lymphomas in children: impact on management and radiological follow-up strategy.

BACKGROUND: Following treatment with chemotherapy and radiotherapy, patients with thoracic lymphomas may demonstrate benign residual mediastinal masses, composed of inflammatory, fibrous or necrotic tissue. Because of the potential risk of viable tumour cells within the mass, histological verification of the nature of these masses may be requested. OBJECTIVE: To study the outcome of thoracic lymphomas in children in order to optimise the radiological follow-up strategy of residual mediastinal masses (RMM). MATERIALS AND METHODS: A retrospective study of 39 children [24 with Hodgkin's disease (HD), 10 with non-Hodgkin's lymphoma (NHL), and 5 with anaplastic lymphoma (AL)]. The results of chest X-rays (CXR) and thoracic CT performed at the time of re-assessment were compared with the histology of the residual masses (n = 11) or the clinical course (n = 28). RESULTS: At the time of re-evaluation, 16/39 patients had residual mediastinal enlargement (RME) on CXR, and 18/39 patients had RMM on CT. Good concordance was observed between the two imaging modalities (K = 0.69). Two children with a RMM died from extra-mediastinal progression. Two children with NHL had active residual mediastinal lesions but neither had RMM. Sixteen cases of RMM were observed in the remaining 35 children and 9 of these masses were histologically verified as benign. A favourable course was observed in these 35 cases. CONCLUSIONS: RMM are frequent and generally benign. They are well shown on CXR and have a non-specific appearance on CT. Except when required by a treatment protocol, they could be submitted to further radiological follow-up before contemplating surgical verification.

A Comparative Study of Two New Structure Types. Synthesis and Structural and Electronic Characterization of K(RE)P(2)Se(6) (RE = Y, La, Ce, Pr, Gd).

Two polytypes of potassium rare-earth-metal hexaselenodiphosphates(IV), K(RE)P(2)Se(6) (RE = Y, La, Ce, Pr, Gd), have been synthesized from the stoichiometric reaction of RE, P, Se, and K(2)Se(4) at 750 degrees C. Both single-crystal and powder X-ray diffraction analyses showed that the structures of these polytypes vary with the size of the rare earth metals. For the smaller rare-earth metals, Y and Gd, K(RE)P(2)Se(6) crystallized in the orthorhombic space group P2(1)2(1)2(1). The yttrium compound was studied by single-crystal X-ray diffraction with the cell parameters a = 6.7366(5) Å, b = 7.4286(6) Å, c = 21.603(2) Å, and Z = 4. This structure type comprises a layered, square network of yttrium atoms that are bound to four distinct [P(2)Se(6)](4)(-) units through selenium bonding. Each [P(2)Se(6)](4)(-) unit possesses a Se atom that is not bound to any Y atom but is pointing out into the interlayer spacing, into an environment of potassium cations. For larger rare-earth metals, La, Ce, and Pr, K(RE)P(2)Se(6) crystallized in a second, monoclinic polytype, the structure of which has been published. Both of these two different polytypes can be related to each other and several other isoelectronic chalcophosphate structures based on a Parthé valence electron concentration analysis. These structures include Ag(4)P(2)S(6), K(2)FeP(2)S(6), and the hexagonal M(II)PS(3) structure types. The magnetic susceptibilities of the title compounds have been studied, and the behavior can been explained based on a simple set of unpaired f-electrons. The diffuse reflectance spectroscopy also showed that these yellow plates are moderately wide band gap ( approximately 2.75 eV) semiconductors.

Rhenium chemistry of diazabutadienes and derived iminoacetamides spanning the valence domain II-VI. Synthesis, characterization, and metal-promoted regiospecific imine oxidation.

The reaction of diazabutadienes of type R'N=C(R)-C(R)=NR', L (R = H, Me; R' = cycloalkyl, aryl) with Re(V)OCl(3)(AsPh(3))(2) has furnished Re(V)OCl(3)(L), 1, from which Re(III)(OPPh(3))Cl(3)(L), 2, and Re(V)(NAr)Cl(3)(L), 3, have been synthesized. Chemical oxidation of 2(R = H) by aqueous H(2)O(2) and of 3(R = H) by dilute HNO(3) has yielded Re(IV)(OPPh(3))Cl(3)(L'), 5, and Re(VI)(NAr)Cl(3)(L'), 4, respectively, where L' is the monoionized iminoacetamide ligand R'N=C(H)-C(=O)-NR'(-). Finally, the reaction of Re(V)O(OEt)X(2)(PPh(3))(2) with L has furnished bivalent species of type Re(II)X(2)(L)(2), 6(X = Cl, Br). The X-ray structures of 1 (R = Me, R' = Ph), 3 (R = H, R' = Ph, Ar = Ph), and 4 (R = H, R' = cycloheptyl, Ar = C(6)H(4)Cl) are reported revealing meridional geometry for the ReCl(3) fragment and triple bonding in the ReO (in 1) and ReNAr (in 3 and 4 ) fragments. The cis geometry (two Re-X stretches) of ReX(2)(L)(2) is consistent with maximized Re(II)-L back-bonding. Both ReX(2)(L)(2) and Re(NAr)Cl(3)(L') are paramagnetic (S = (1)/(2)) and display sextet EPR spectra in solution. The g and A values of Re(NAr)Cl(3)(L') are, respectively, lower and higher than those of ReX(2)(L)(2). All the complexes are electroactive in acetonitrile solution. The Re(NAr)Cl(3)(L) species display the Re(VI)/Re(V) couple near 1.0 V versus SCE, and coulometric studies have revealed that, in the oxidative transformation of 3 to 4, the reactive intermediate is Re(VI)(NAr)Cl(3)(L)(+) which undergoes nucleophilic addition of water at an imine site followed by induced electron transfer finally affording 4. In the structure of 3 (R = H, R' = Ph, Ar = Ph), the Re-N bond lying trans to the chloride ligand is approximately 0.1 A shorter than that lying trans to NPh. It is thus logical that the imine function incorporating the former bond is more polarized and therefore subject to more facile nucleophilic attack by water. This is consistent with the regiospecificity of the imine oxidation as revealed by structure determination of 4 (R = H, R' = cycloheptyl, Ar = C(6)H(4)Cl).

New synthetic routes to cationic rhenium tricarbonyl bipyridine complexes with labile ligands.

Triflate abstraction from the complex [Re(OTf)(CO)(3)(bipy)] (1) using the salt NaBAr'(4) (Ar' = 3,5-bis(trifluoromethyl)phenyl) in dichloromethane solution in the presence of L = PPh(3), NCMe, NCPh, imines, ketones, Et(2)O, THF, MeOH, and MeI affords cationic complexes [Re(L)(CO)(3)(bipy)](+) as their BAr'(4)(-) salts. The new complexes have been characterized spectroscopically and, for [Re(eta(1)-O=C(Me)R)(CO)(3)(bipy)]BAr'(4) (R = CH(3), 6a; R = Ph, 6b), and [Re(THF)(CO)(3)(bipy)]BAr'(4) (9), also by single-crystal X-ray diffraction. Compared with conventional methodologies, the route reported here allows the coordination of a broader range of weakly coordinating ligands and requires considerably milder conditions. On the other hand, the reactions of lithium acetylides with [Re(THF)(CO)(3)(bipy)]BAr'(4) (9) can be used for the high-yield syntheses of rhenium alkynyls [Re(Ctbd1;CR)(CO)(3)(bipy)] (R = Ph, 12; R = SiMe(3), 13). Complex 9 was found to catalyze the aziridination of benzylideneaniline with ethyl diazoacetate.

[Instrumental diagnosis of reflux esophagitis].

One hundred and forty patients with clinical symptoms of gastroesophageal reflux disease were examined. Instrumental examination included esophagogastroduodenoscopy (140 patients) with target biopsy (44 patients), x-ray study of the esophagus and stomach (95), intragastric pH-metry (74), esophagomanometry (67). It is demonstrated that all the patients with clinical picture of gastroesophageal reflux disease suffer from reflux-esophagitis (RE) of various severity. Definite morphological equivalents correspond to each of endoscopic forms of RE. Complex instrumental diagnosis of RE must include intragastric pH-metry and esophagomanometry in line with esophagogastroduodenoscopy and morphological study. It is necessary to perform x-ray study in peptic strictures of the esophagus and in combination of RE with large axial hiatal hernias. Cardial or cardial-fundal hiatal hernia, increase of intragastric pressure and hyperacidity promote development of the most severe (destructive) forms of RE.

Carbonate induced dissolution of uranium containing precipitates under cement leachate conditions.

The effects of carbonate on uranium (VI) solubility under aerobic and cementitious conditions have been investigated. The information is of relevance to low level nuclear waste disposal. Aqueous NaOH, KOH, Ca(OH)2 and a cement leachate solution were added to uranyl nitrate solution. Afterwards, increasing amounts of ammonium carbonate were added to re-dissolve the precipitates. The precipitates were characterised by means of X-ray powder diffraction (XRPD) measurements and modelling studies. The model calculations were performed using the MINTEQA2 speciation code, with an expanded database incorporating uranium stability constants taken from the HATCHES database. The measured and predicted amounts of CO3(2-) needed to dissolve the precipitates were compared. The knowledge gained from the 'pure' systems was used to rationalise the precipitation and re-dissolution behaviour observed in the leachate system. The lack of uranium solubility at low carbonate levels brought into question literature formation constants for UO2(OH)3- and UO2(OH)4(2-). An approximate log K value of 26.8 for K2U2O7 formation was estimated from the KOH results at pH 12. Generally, uranium solubilities are expected to be insignificant at low level nuclear waste sites because anaerobic conditions should persist. However, this study has demonstrated that solubility in leachate could rise at high pH and high carbonate levels, if aerobic conditions were to develop. Soluble U(VI) species would be formed. The novel part of the study has been to reproduce the effects and to correlate enhanced solubilities with model predictions. Consequently, the study has re-emphasised the need for appropriate environmental monitoring of such sites.

[Patient's delay and doctor's delay in the pulmonary tuberculosis--the experience of the National Chiba Higashi Hospital].

Out of 1628 tuberculosis patient discharged from the to National Chiba Higashi Hospital, 537 previously untreated pulmonary tuberculosis patients were selected for the study to examine the delay in case-defection. Patient's delay were longer but doctor's delay were shorter in male than in female. The main reasons for doctor's delay were the delay in taking chest X-ray, and the error in the interpretation of initial chest X-ray findings. In 88 patients who were detected in the advanced stage, the total delay were longer than the others common cases and the doctor's delay was found in 13 cases. To avoid such an unnecessary delay, the importance of sputum examinations must be re-emphasized in all patients who show respiratory symptoms and/or abnormal chest X-ray findings.
