Evaluation of the BBL Crystal MRSA ID System for detection of oxacillin resistance in Staphylococcus aureus.

AIMS: To evaluate the BBL Crystal MRSA ID System for detection of oxacillin resistance in Staphylococcus aureus. METHODS: 52 methicillin resistant S aureus (MRSA) from five different countries and 85 methicillin susceptible S aureus (MSSA) were included. The species was confirmed by tube coagulation and detection of the clumping factor using the Staphaurex Plus. Clonal non-identity of the MRSA isolates was shown by pulsed field gel electrophoresis. MIC values (oxacillin) were determined using the Etest. Polymerase chain reaction was carried out to detect the mecA gene. The BBL Crystal MRSA ID System was carried out according to the manufacturer's instructions. RESULTS: The BBL Crystal MRSA ID System showed fluorescence in 45 of 52 MRSA (sensitivity 86.5%; negative predicitive value 92.2%), and the specificity was 97.6% (positive predicitive value 95.7%). Two of seven MRSA that failed to show fluorescence had MIC values (oxacillin) of 4 mg/litre. CONCLUSIONS: The BBL Crystal MRSA ID System is a valuable test for detecting oxacillin resistance in S aureus. Its major advantage is the short time (4-5 hours) required to perform the test when organisms are grown on tryptic soy agar or sheep blood agar. Difficulties may arise in borderline resistant isolates.

Id helix-loop-helix proteins negatively regulate TRANCE-mediated osteoclast differentiation.

Tumor necrosis factor (TNF)-related activation-induced cytokine (TRANCE) induces osteoclast formation from monocyte/macrophage lineage cells via various transcription factors, including the Mi transcription factor (Mitf). Here, we show that inhibitors of differentiation/DNA binding (Ids), helix-loop-helix (HLH) transcription factors, negatively regulate TRANCE-induced osteoclast differentiation. Expression levels of Id1, Id2, and Id3 genes are significantly reduced by TRANCE during osteoclastogenesis. Interestingly, overexpression of the 3 Id genes in bone marrow-derived monocyte/macrophage lineage cells (BMMs) inhibits the formation of tartrate-resistant acid phosphatase (TRAP)-positive multinuclear osteoclasts, but it does not alter the ability of BMMs to either phagocytose or differentiate into dendritic cells (DCs). Overexpression of Id2 in BMMs attenuates the gene induction of nuclear factor of activated T cells c1 (NFATc1) and osteoclast-associated receptor (OSCAR) during TRANCE-mediated osteoclastogenesis. Furthermore, Id proteins interact with Mitf, a basic HLH (bHLH) transcription factor, and inhibit its transactivation of OSCAR, which is a costimulatory receptor expressed by osteoclast precursors, by attenuating the DNA binding ability of Mitf to the E-box site of the OSCAR promoter. Taken together, our results reveal both a new facet of negative regulation, mediated by Id proteins, as well as the mechanism whereby TRANCE signaling overcomes it, allowing osteoclastogenesis to proceed.

The development of the items-easy (Ie) and items-difficult (Id) subscales for the MMPI-A.

This research involves the development, validation, and cross-validation of the Items-Easy (Ie) and Items-Difficult (Id) subscales for the Minnesota Multiphasic Personality Inventory-Adolescent (MMPI-A; Butcher et al., 1992). These subscales were designed to assess the degree to which reading comprehension deficits may be responsible for significant elevations of validity Scale F and the standard clinical scales on adolescents' MMPI-A profiles. A difference score, bared on the two 13-item subscales, was created in order to compare subjects' responses to subsets of the more comprehensible (Ie) and less comprehensible (Id) items within the test. Hit rate, sensitivity, specificity, positive predictive power, and negative predictive power were calculated on the basis of simple (Id - Ie) and weighted (3Id - 1Ie) difference scores for the validation (N = 495) and cross-validation (N = 264) samples, and for specific high-F profile subsamples. Although some of the indices reflected classification accuracy as high as 95%, none of the indices yielded consistently high results across the various samples and subsamples. It has been concluded that the Ie and Id subscales should be used only for research purposes at this time.

Use of false ID cards and other deceptive methods to purchase alcoholic beverages during high school.

Altered motor vehicle drivers's licenses or other falsified or counterfeit photo identification cards are widely and illegally used by teenagers to obtain beer and other alcohol beverages. We obtained information on the methods currently used by teenagers to purchase beer and wine by asking nine hundred teenagers, between 16-19 years old to complete a brief, confidential questionnaire. High school students most often obtained alcoholic beverages by requesting someone of legal age to purchase it for them. College students used borrowed, altered, or counterfeit identification (ID) more often than high school students. Photo IDs purchased through mail order from a magazine advertisement were used infrequently and when use was attempted, they were sometimes (25%) unsuccessful. Fifteen percent of high school students, 14% of college freshmen, and 24% of teenage drug abusers were able to purchase beer by the case with borrowed, altered, or fake ID. Suggestions to reduce sales of alcohol-containing beverages to minors include universal "carding" of prospective purchasers, use of two view or hologram photos on a drivers' license, requiring three different ID cards at the point of purchase, and penalties to stores that fail to make a good effort to identify underage customers.

Serodiagnosis of Trypanosoma cruzi infection using the new particle gel immunoassay--ID-PaGIA Chagas.

The ID-Chagas test is a particle gel immunoassay (PaGIA). Red coloured particles are sensitised with three different synthetic peptides representing antigen sequences of Trypanosoma cruzi: Ag2, TcD and TcE. When these particles are mixed with serum containing specific antibodies, they agglutinate. The reaction mixture is centrifuged through a gel filtration matrix allowing free agglutinated particles to remain trapped on the top or distributed within the gel. The result can be read visually. In order to investigate the ability of the ID-PaGIA to discriminate negative and positive sera, 111 negative and 119 positive, collected in four different Brazilian institutions, were tested by each of the participants. All sera were previously classified as positive or negative according to results obtained with three conventional tests (indirect immunofluorescence, indirect hemaglutination, and enzyme linked immunosorbent assay). Sensitivity rates of ID-PaGIA varied from 95.7% to 97.4% with mean sensitivity of 96.8% and specificity rates varied from 93.8 to 98.8% with mean specificity of 94.6%. The overall Kappa test was 0.94. The assay presents as advantages the simplicity of operation and the reaction time of 20 min. In this study, ID-PaGIA showed to be highly sensitive and specific.

How to save distressed IDS-physician marriages: a case study.

A hospital-driven IDS that encounters serious problems resulting from ownership of a physician practice should address those problems by focusing on three core areas: vision and leadership, effectiveness of operations, and physician compensation arrangements. If changes in these areas do not lead to improvements, the IDS may need to consider organizational restructuring. In one case study, a hospital-driven IDS faced the problem of owning a poorly performing MSO with a captive physician group. The IDS's governing board determined that the organization lacked effective communication with the physicians and that realization of the organization's vision would require greater physician involvement in organizational decision making. The organization is expected to undergo some corporate reorganization in which physicians will acquire an equity interest in the enterprise.

Two-part transfer pricing improves IDS financial control.

To help coordinate patient care services across its various provider entities, an integrated delivery system (IDS) must address two financial control matters: design of responsibility centers and selection of an appropriate transfer pricing methodology. Although designating an HMO as a profit center is appropriate, such a designation is inappropriate for an IDS's provider entities. Instead, these entities are more appropriately designated as standard expense centers. Further, an HMO owned and operated by an IDS should purchase patient care from the delivery entities by means of two-part transfer prices, that is, having fixed costs paid for in a lump sum and variable costs paid for on the basis of the actual volume and mix of services provided. The IDS's management must recognize, however, that both case mix and volume lie largely outside a hospital's control and that financial controls should focus on a combination of the hospital's fixed costs and variable costs associated with resources per case, efficiency of resource delivery, and factor prices.

[Murine model for experimental studies on attenuated mutants of a Pasteurella multocida strain with pathogenicity for the calf. 2. Primary immunization against lethal infection, using fully attenuated streptomycin-dependent (Sm-d) mutant and booster immunization with partially attenuated streptomycin-dependent (Sm-id) revertants].

An experimental study was conducted into the immunisation of mice to a strain of Pasteurella multocida with pathogenicity for calf. Here are the findings: (a) The attenuation of Sm-id revertants was measured with reference to the order of extinction and compared to the wild strain. (b) A selected range of Sm-id revertants with differentiated attenuation is presented. (c) Single immunisation, using fully attenuatted Sm-d mutant does prevent lethal infection with partially attenuated Sm-id revertants, although it is ineffective against wild strain infection. (d) Immunity against 100 times the lethal dose of the wild strain can be achieved by two immunisations, that is primary antigen application, using fully attenuated Sm-d mutant, and booster immunisation, using partially attenuated Sm-id revertant.

Functional antagonism between inhibitor of DNA binding (Id) and adipocyte determination and differentiation factor 1/sterol regulatory element-binding protein-1c (ADD1/SREBP-1c) trans-factors for the regulation of fatty acid synthase promoter in adipocytes.

We show that Id (inhibitor of DNA binding) 2 and Id3, dominant negative members of the helix-loop-helix (HLH) family, interact with the adipocyte determination and differentiation factor 1 (ADD1)/sterol regulatory element-binding protein (SREBP) 1c, a transcription factor of the basic HLH-leucine zipper family that controls the expression of several key genes of adipose metabolism. Gel mobility-shift assays performed with in vitro-translated ADD1, Id2 or Id3 proteins and a fatty acid synthase (FAS) promoter oligonucleotide showed evidence for a marked inhibition of the formation of DNA-ADD1 complexes by Id2 or Id3 proteins. Co-immunoprecipitation studies using in vitro-translated proteins demonstrated further the physical interaction of Id and ADD1/SREBP-1c proteins in the absence of DNA. Using the FAS gene as a model of an ADD1-regulated promoter in transiently transfected isolated rat adipocytes or mature 3T3-L1 adipocytes, a potent inhibition of the activity of the FAS-chloramphenicol acetyltransferase reporter gene was observed by overexpression of Id2 or Id3. Reciprocally, co-transfection of Id3 antisense and ADD1 expression vectors in preadipocytes potentiated the ADD1/SREBP-1c effect on the FAS promoter activity. Finally, in the non adipogenic NIH-3T3 cell line, most of the ADD1-mediated trans-activation of the FAS promoter was counteracted by co-transfection of Id2 or Id3 expression vectors. Previous studies have indicated Id gene expression to be down-regulated during adipogenesis [Moldes, Lasnier, Fève, Pairault and Djian (1997) Mol. Cell. Biol. 17, 1796-1804]. We here demonstrated that there was a dramatic rise of Id2 and Id3 mRNA levels when 3T3-L1 adipocytes or isolated rat fat cells were exposed to lipolytic and anti-lipogenic agents, forskolin and isoproterenol. Taken together, our data show that Id products are functionally involved in modulating ADD1/SREBP-1c transcriptional activity, and thus lipogenesis in adipocytes.

Doxorubicin inhibits differentiation and enhances expression of the helix-loop-helix genes Id and mTwi in mouse osteoblastic cells.

Treatment with doxorubicin is associated with decreased levels of expression of muscle-specific genes in myocytes. This may be related to an effect on expression of helix-loop-helix (h-l-h) regulatory molecules since in myoblastic cells, doxorubicin inhibits Myo D expression and enhances Id expression. We have reported that expression of Id and mouse Twist (mTwi), another h-l-h molecule, decline in association with differentiation in osteoblastic cells. We have sought, therefore, to determine the effect of doxorubicin on MC-3T3-E1 osteoblastic cells. Treatment with doxorubicin decreased total cellular protein content, reduced [3H]-leucine incorporation into protein, inhibited proliferation and diminished alkaline phosphatase activity. Glucose utilization and lactate production were not adversely affected. Id expression was increased by doxorubicin treatment under growth conditions but not differentiation conditions. Expression of mTwi was markedly increased under both growth and differentiation conditions. These data support the contention that Id and mTwi may regulate differentiation in osteoblastic cells.

The association of Id-1, MIF and GSTpi with acquired drug resistance in hormone independent prostate cancer cells.

Most patients with prostate cancer respond to androgen ablation therapy, but the tumor eventually progresses to an androgen-independent stage with multiple anti-cancer drug resistance. Novel therapeutic strategies for hormone independent prostate cancer need to be developed. The genes for Id-1, MIF and GSTpi were up-regulated in drug resistant cells of hormone independent prostate cancer cells using cDNA microarray gene expression analysis. In this study we aimed to investigate whether constitutive overexpression of these candidate genes in cells can affect cellular resistance to chemotherapeutic agents. The mRNA expression was determined by reverse transcriptase-polymerase chain reaction, and the Id-1, MIF and GSTpi protein contents in these cell lines were measured by Western blotting. Susceptibility of the cells to chemotherapeutic agents including doxorubicin, paclitaxel and cyclophosphamide was determined by microculture tetrazolium bromide assay. The analysis of apoptosis was assessed by annexin V assay. The cytotoxity assay results demonstrated that cells overexpressing the Id-1 gene increased in their resistance to doxorubicin, paclitaxel and cyclophosphamide. MIF expression can drive cells to increase in their resistance to paclitaxel, and GSTpi expression confers drug resistance to doxorubicin and cyclophosphamide. The induction of mdr1 gene expression was noted in up-regulation of MIF and GSTpi. Our findings suggest that overproduction of the Id-1, MIF and GSTpi proteins and coinduction of mdr-1 play an important role in the development of acquired drug resistance to various drugs of prostate cancer cells.

Molecular characterization of antibodies bearing Id-460. I. The structure of two highly homologous VH genes used to produce idiotype positive immunoglobulins.

The heavy chain immunoglobulin genes encoding a variety of antibodies specific for DNP or Pasteurella pneumotropica and bearing the dominant idiotype of MOPC 460, Id-460, were cloned and sequenced. The VH genes encoding the M460 and D35 (DNP binding) antibodies were found to be homologous but not identical to the VH gene encoding the LB8 (P. pneumotropica binding) monoclonal antibody. Two of at least eight genes in the VH460 cross-hybridizing gene family can encode Id-460 positive antibodies. The VH460 gene family overlaps with the gene family described by VH36-60 and more completely describes this germ-line VH gene family. We have previously demonstrated a genetic requirement for VK1 expression in order to observe the expression of Id-460 in anti-DNP antibody responses. Southern blot analysis of these monoclonal antibody-producing cells demonstrates, as with the cross-hybridizing VH, that two cross-hybridizing VK1 genes (VK1A and VK1C) can be used to encode Id-460-positive immunoglobulins. We demonstrate that immunoglobulin idiotype determinant expression can be the result of the expression of nonidentical but highly homologous genes in the VH460 cross-hybridizing VH gene family and also in the VK1 cross-hybridizing VL family.

IgA isotype-restricted idiotypes associated with T15 Id+ PC antibodies.

Idiotypes are believed to be due to the structural conformation of the variable region of immunoglobulins (Ig). We have found an idiotype (C3-24) that requires both variable and constant regions of the heavy chain to be expressed. C3-24 Id is associated with both the T15 variable region from anti-phosphorylcholine (PC) antibodies and the constant region for the alpha-heavy chain. High titer anti-PC serum from a variety of inbred strains of different Ig haplotypes failed to express C3-24 Id. However, when IgA but not IgG or IgM fractions were isolated from a pool of anti-PC serum from BALB/c mice, more than 70% of the molecules expressed C3-24 Id. The high frequency of the expression of C3-24 Id in IgA anti-PC hybridoma proteins from mice of different Ig haplotypes and in the IgA fraction of normal anti-PC antibodies from BALB/c and presumably other strains of mice suggests that idiotypic determinants produced by the three-dimensional product of VH and CH regions may not be unusual.

Site of anti-nociceptive action of a new benzomorphan derivative ID-1229.

The site of anti-nociceptive action of a new benzomorphan derivative ID-1229, 2-[3-(p-fluorobenzoyl)-1-propyl]-5 alpha, 9 alpha-dimethyl-2'-hydroxyl-6, 7-benzomorphan was investigated using electrophysiological methods. In rabbits, ID-1229 (0.1-0.5 mg/kg) caused slowing in the EEG, and depressed the evoked potentials recorded from the sensory cortex and the Nucleus ventralis posterolateralis of the thalamus elicited by sciatic stimulation. A small dose (1 mg/kg) of ID-1229 decreased the bradykinin-induced unitary discharges of lamina V neuron of the spinal dorsal horn in intact rabbits but not in spinal rabbits. These results suggest that ID-1229 inhibits the sensory transmission of bradykinin-induced pain at the dorsal horn of the spinal cord, through its facilitatory action on the descending inhibitory system from the supra-spinal structure.

Comparison of Micro-ID, a modified Minitek system, and conventional methods for four-hour identification of Enterobacteriacae directly from blood cultures.

The Micro-ID system and a four-hour modification of the Minitek system were compared to conventional tube media for identification of Enterobacteriacae directly from blood cultures. A total of 205 organisms representing 19 species were evaluated in the study. Of these, 151 were blood culture isolates and the remaining 54 were reference strains or other clinical isolates used in simulated cultures. Pellets of bacteria obtained by centrifuging aliquots of blood cultures were inoculated into Micro-ID and Minitek and incubated four hours at 36C. Compared to conventional methods, Micro-ID correctly named the genus and species of 195 (95.1%) of the isolates. Identification errors were related to the code manual and to some biochemical tests. The four-hour Minitek system agreed with conventional methods on the identification of 196 (95.6%) of the isolates. Although both methods were comparable in their ability to accurately identify enterics directly from blood cultures, Micro-ID had design features that made it more convenient for busy clinical laboratories.

[Comparison of Micro-ID, API 20 E and a conventional technique for the identification of Enterobacteriaceae].

A new system, Micro-ID, for the identification of Enterobacteriaceae is described. 100 strains of Enterobacteriaceae (Table I) are identified by Micro-ID and the identifications are compared with the identifications performed by conventional techniques and by the API 20 E system. There is a total agreement between Micro-ID and conventional techniques on 96.5% and between API 20 E and conventional techniques on 90% (Tables II and III). The differences in the reactions between the systems are discussed and explained. It is concluded, that the Micro-ID and the API 20 E both are very suitable for the identification of Enterobacteriaceae.

[Rapid identification of Enterobacteriaceae with the Micro-ID system (author's transl)].

Micro-ID, a new kit system for rapid identification of Enterobacteriaceae, was compared with a conventional media-system, the extended Enterotube system (completed with arabinose, rhamnose and the Voges-Proskauer reaction), and partially with the API-20E system. When the computer generated Identification Manual for Micro-ID was consulted the system showed complete agreement at the genus level, but differed in 5% at the species level. Problems arose from misidentifications by nonfermenting organisms. 15 turbid blood cultures with gramnegative rods were tested additionally. In 11 cases the Micro-ID provided correct identification of the species and in further 3 cases of the genus. In summary Micro-ID is easy to handle and an accurate, convenient kit for the short-term identification of Enterobacteriaceae.

Pharmacology of 9-gamma-methylaminopropyl-9,10-dihydro-9,10-methanoanthracene-HCl (ID-9206-HCl), a new potent antidepressant.

The pharmacological properties of 9-gamma-methylaminopropyl-9,10-dihydro-9,10-methanoanthracene-HCl (ID-9206-HCl) were examined in comparison with those of imipramine, amitriptyline and maprotiline in animals. ID-9206 showed the characteristic pharmacological profile of an antidepressent: anti-reserpine and anti-tetrabenazine activities, potentiation of central and peripheral actions of norepinephrine, and potentiation of methamphetamine and yohimbine actions. ID-9206 had weak peripheral anticholinergic activities and sedative effects. ID-9206 was less toxic in mice and rats than the other three antidepressants.
