The conservative treatment of ameloblastoma, using diathermy or cryosurgery. A 29-year review.

At present, both the treatment, and the results of treatment, for ameloblastoma are variable, with high recurrence rates being reported. Since 1951, seven cases of extensive mandibular tumours have been treated by local excision, combined with diathermy and/or cryosurgery. After long-term follow-up there is no evidence of recurrence. In this review some cases have involved more radical treatment, viz. maxillectomy and radiotherapy, but it is now proposed that the majority of cases of mandibular ameloblastoma can be treated conservatively. Cryosurgery is advocated as an adjunct in the operative treatment of ameloblastoma, and its use is supported by recent research and clinical publications, although the time-span for review of these cases is, at yet, limited.

Cervical function following treatment of intraepithelial neoplasia by electrocoagulation diathermy.

At the Royal Women's Hospital Melbourne, treatment of cervical intraepithelial neoplasia has consisted mainly of electrocoagulation diathermy. A random group of 420 consecutive patients attending the follow-up clinic has been studied to assess subsequent cervical function. Long-term follow-up has revealed minimal adverse effects on fertility, parturition, and menstrual function.

[Surgical diathermy treatment of buccopharyngeal tumors].

Surgical diathermy excision was employed to treatment 138 cases of buccopharyngeal cancer, either as initial followed by irradiation therapy, or as complementary treatment to radiotherapy in unsuccessful cases. Results were favorable in 34 patients with tonsillar carcinoma and 38 cases of soft palate cancer, but the method was only moderately effective for recurrences in these regions. In 36 cases of tongue cancer, and 30 with cancer of the floor of the mouth, this therapeutic procedure was particularly effective for treating patients not responding to radium needle implantation.

[Outpatient excision of low grade cervical intraepithelial lesions using loop diathermy].

Twenty five years after the introduction of outpatient treatment for cervical precancers, several cases of invasive cancer after laser, cryotherapy or electrocautery have been described. As a consequence, the diathermy loop excision has recently been proposed for the treatment of cervical intraepithelial neoplasia. The new generators seem to assure a high quality of cut and allow the histological analysis of all the lesional tissue excised as deep as the laser vaporisation usually goes (7 mm). We have tested this treatment on a series of CIN I on an outpatient basis and our results confirm that loop excision can replace destructive treatments for CIN I. As for high grade lesions, we still think that laser conisation under general anesthesia remains the best choice.

[Ligature and excision method with diathermy for haemorrhoidectomy: in comparison with conventional method].

The usefulness of ligature and excision method with diathermy (group D) for haemorrhoidectomy was evaluated in comparison with conventional ligature and excision method with scalpel and scissors (group S) from several parameters. Intraoperative bloodloss and hospitalization days in group D were significantly smaller than those in group S. Moreover, post-operative woundal bleeding and edema in group D were significantly smaller than those in group S. As regards post-operative pain, although there were no significant differences between the two groups, the values in group D were slightly improved as compared with group S. These results indicate the usefulness of diathermic method for haemorrhoidectomy because of its better cost-efficacy as compared with conventional method with scalpel and scissors.

[The role of outpatient conization with a diathermy loop in the diagnosis and treatment of CIN].

The directed punch biopsy has a relevant role in the diagnosis of the CIN, mainly because of its simplicity. It is however advisable to take into account that it doesn't always reveal the exact histological degree of the cervical lesion, to the point of underestimating the invasive carcinoma. It is thus recommended, when atypical colposcopy and/or CIN2-CIN3 are diagnosed by directed punch biopsy, to define the diagnosis by preliminary large loop diathermy, also in those cases where a simple hysterectomy, because of a combined pathology, is being planned.
