[Unique case of caecum plasmablastic lymphoma CD138(+) in patient with late diagnosed colon neuroendocrine carcinoma].

Neuroendocrine tumors are frequently associated with other primary malignancies. Plasmablastic lymphoma is a rare, aggressive neoplasm, derived from large B-cell, associated with human immunodeficiency virus infection. Plasmablastic lymphoma cells share many cytomorphologic and immunophenotypic features with plasmablastic cells, causing some diagnostic problems. We present a unique case of coexisting two very uncommon neoplasms: plasmablastic lymphoma and neuroendocrine carcinoma in 54-years-old men. This is the first report of caecum localization of plasmablastic lymphoma. Presented case images diagnostic problems in rare neoplasms.

Cystic sebaceous neoplasms in Muir-Torre syndrome.

Cystic sebaceous neoplasms have been seen only in patients with Muir-Torre syndrome (MTS) and have recently been characterized as marker lesions of MTS. Histologically, these lesions form a spectrum of tumors ranging from benign cystic adenomas to proliferative cystic sebaceous tumors. We describe 2 proliferative cystic sebaceous tumors in a 53-year-old man whose workup revealed colonic adenocarcinoma and other sebaceous tumors consistent with MTS. Both the chest wall and the left thigh masses were grossly cystic, measuring 1.0 and 1.5 cm, respectively. Histologic sections demonstrated well-circumscribed cystic neoplasms located in the deep dermis and subcutaneous tissue. Each had a focally infolded cyst wall composed of immature basaloid cells with prominent nucleoli and mitoses, consistent with a proliferative cystic sebaceous tumor. Recognition of cystic sebaceous neoplasm by pathologists and communication to clinicians of its strong association with MTS is of diagnostic importance.

The immunologic approach to analysis of malignant lymphoma. Mantle zone lymphoma of the ileocecal region.

We studied an unusual type of lymphoma of the ileocecal region using an established protocol that combines morphologic, flow cytometric, and immunohistochemical analyses. This lymphoma grew as multiple submucosal nodules, some of which had coalesced into a mass in the terminal ileum. Histologically, the lesion resembled a lymphoma of follicular center-cell origin except for germinal centers that appeared reactive and greatly expanded follicular mantles. Flow cytometric studies showed that the lymphoma contained a monoclonal, kappa+ population of B-lymphocytes. Evaluation of sections of frozen tissue by an immunoperoxidase technique revealed that the germinal centers were polyclonal (nonneoplastic) and that the neoplastic cells were confined to the mantle zone. The additional information led to classification of the neoplasm as the mantle zone variant of intermediate lymphocytic lymphoma. This type of lymphoma may pursue a more aggressive course than a follicular lymphoma derived from the germinal center. We review our general approach to the analysis of hematolymphoid neoplasms and discuss how certain techniques may be useful adjuncts in the evaluation of these types of tumors.

Inflammatory myofibroblastic tumor with extensive involvement of the bowel in a 7-year-Old child.

We present a case of unusual localization of inflammatory fibroblastic tumor in the terminal ileum, cecum, and ascending colon in a 7-year-old child. Segmental resection of the terminal ileum, cecum, and ascending colon with a tumor mass up to 6 cm in diameter was performed. Pathohistological examination of biopsy specimen was performed on routine hematoxylin-eosin sections, as well as immunohistochemically with primary antibodies to CD3, CD20, CD68, factor VIII, vimentin, smooth muscle actin, desmin, cytokeratin and S-100 protein, and k and l light chains. The tumor was composed of highly vascularized tissue with interlacing fascicles of elongated spindle cells admixed with plasma cells, histiocytes, lymphocytes, and eosinophils. The diagnosis of inflammatory myofibroblastic tumor was confirmed by immunohistochemistry. Inflammatory myofibroblastic tumor cannot be distinguished clinically from highly malignant neoplasm or some other conditions. Surgical resection and careful pathohistological analysis are needed, and a long-term follow-up is recommended.

[Metastasis in metastasis].

Double metachron neoplasm was found in a 75 year-old man by the authors, myxoid chondrosarcoma as primer neoplasm of the right leg, and cecum adenocarcinoma. In the centre of myxoid chondrosarcoma's metastasis of the lung adenocarcinoma metastasis occurred. Metastasis in metastasis is a histological rarity.

[Plain abdominal film in neoplasms of the large bowel. - A comparative study with a population of normal subjects (author's transl)].

A comparative study was performed between plain abdominal films of 110 patients with neoplasms of the large bowel and 52 normal subjects for control. In neoplasms of the right colon the failed spontaneous visualization of caecum and ascendent colon or the direct recognization of lume's deformation allow us to suspect such a lesions in about 90% of the cases. Neoplasms of the other tracts of the large bowel may be suspected only when plain abdominal film demonstrates direct evidence of an obstruction: in about 25% of the cases.

Appendicovesical fistula associated with papillovillous adenoma of the appendix.

Appendicovesical fistulae represent 5% of all intestinal vesical fistulae. Usually a known cause is appendicitis, cecal diverticulitis, cystadenocarcinoma of the appendix and carcinoid tumors. We report the 2nd case, to our knowledge, where a benign tumor is a possible reason for the formation of an appendicovesical fistula.

[Hypertrichosis lanuginosa acquisita in ulcerative colitis with colon cancer].

Hypertrichosis lanuginosa acquisita is regarded as an obligatory cutaneous paraneoplasm and is defined as the sudden and excessive appearance of lanugo hairs on the entire integument associated with malignant neoplasm of internal organs. We observed such a case of hypertrichosis in a 30-year-old man with a long history of ulcerative colitis who developed carcinoma of the caecum with lymph node metastasis.

[Our experience with emergency surgery in cancer of the large intestine].

The experience concerning 44 cases of large intestine cancer operated in emergency is reported. For the right colon perforated cancers, the surgical solution effected is hemicolectomy and immediate anastomosis for the cases with smallest contamination, hemicolectomy with ileostomy and colonic mucous fistula for the cases with diffused peritonitis. As to left colon perforated cancers, the operations performed are rectocolonic resection according to Hartmann and resection with anastomosis of colonic ends to cutis, depending on the place of the perforated neoplasm. For right colon cancers in occlusive stage, hemicolectomy is performed, whereas, for left colon ones, rectocolectomy according to Hartmann is effected. The postoperative mortality was 16,6%, the percentage of complications was 25% for right colon cancers and 27,7% for left colon ones.

[Pronounced flush symptoms in carcinoid syndrome without liver metastases].

The case of a 78 years old patient with impressive flush symptoms is reported. 5-Hydroxyindoleacetic acid and serotonin were elevated. The responsible pathomorphological structure is a 8 x 4.5 x 6 cm ileocaecal neoplasm. Pre-, intra- and postoperatively liver metastases could not be detected during a 18 month period. After surgery of the carcinoid flush disappeared, so that the tumour seize resulting in enormous secretion of metabolites with endocrine activity should be claimed responsible for causing this symptom.

[Carcinoma of the cecum: is early diagnosis possible?].

Between 1982 and 1989, a consecutive series of 34 patients with cancer of the cecum were operated in our hospital. Overall, at 5 years the survival was 50%. We did not notice any correlation among symptom duration and survival. Patients admitted in the asymptomatic phase had the best survival rate (66.6%). Our study suggest that only neoplasm diagnosed in asymptomatic patients and treated promptly are related to a good survival rate. We, therefore, emphasize the importance and the necessity to investigate all patients with a suspected cecum cancer by means of colonoscopy.

The mechanism of plasma albumin loss in carcinoma of the cecum.

Investigation of hypoalbuminemia in an elderly woman with a large papillary adenocarcinoma of the cecum revealed that the hypoproteinemia was due to excessive loss of plasma albumin into the intestinal tract. Results of studies on this patient and analysis of published reports suggest that several factors were involved in the loss of albumin from the neoplasm, namely, the size and location of the tumour, absence of normal surface epithelium, presence of exudate on the surface of the tumour due to tumour necrosis and secondary infection, and bleeding from damaged vessels near the surface of the tumour.

[Gastrointestinal carcinoid tumor--an immunohistochemical and histochemical study].

Fourteen gastrointestinal carcinoid tumors were studied by immunohistochemical and histochemical methods. Neuro-specific enolase (NSE) was detected in all 14 cases with patterns of homogeneously moderate to strongly diffuse cytoplasmic stain. 12 of 14 neoplasms (85.7%) were positive for Grimelius technic. 5 (35.7%) were positive for Masson-Fontana stain. The results indicate that the immunohistochemical demonstration of NSE is a marker more sensitive than the silver stain for gastrointestinal carcinoid tumors. Distribution and significance of silver stain are discussed.

Bidirectional endoscopy in patients with fecal occult blood.

BACKGROUND: Current screening protocols for colorectal cancer depend primarily on fecal occult blood testing (FOBT). However, positive test results do not always indicate the presence of a colonic neoplasm. METHODS: We reviewed the results of 100 consecutive bidirectional (upper and lower) endoscopic procedures performed to evaluate positive FOBT results. Patients were excluded if they presented with gross bleeding, a history of bowel lesions, or previous intestinal operations. There were 31 women and 69 men whose mean age was 51 years. RESULTS: Major abnormalities were found on esophagogastroduodenoscopy (n = 24), colonoscopy (n = 13), or both studies (n = 2). Active bleeding was manifested in two patients, (Barrett's ulcer, duodenal arteriovenous malformation). Two other patients had malignancy: One had a cecal adenocarcinoma and the other a gastric adenocarcinoma. Various benign lesions also were identified in the stomach including esophagitis (n = 8), ulcers/erosions (n = 8) varices (n = 5), and arteriovenous malformations (n = 2). Colonic pathology included polyps (n = 8), arteriovenous malformations (n = 3), and rectal varices (n = 1). Diverticulosis and hemorrhoidal disease were present in 29 and 16 patients, respectively, but were not considered to be likely sources of a positive FOBT. CONCLUSION: Positive FOBT results may indicate the presence of either upper or lower intestinal pathology, and bidirectional endoscopy is an efficient and accurate technique for the comprehensive evaluation of occult bleeding.

Stercoraceous perforation of the cecum: report of two cases.

Two cases of stercoraceous perforation of the cecum with adjacent mass formation simulating a neoplasm are described. The predisposing factors for the formation of fecalomas and the mechanisms of perforation are discussed and a review of the literature on this subject is presented. There were no demonstrable predisposing colonic lesions to account for the fecaloma formation in these casesmboth patients recovered completely after surgical treatment of the lesions.

Canine intestinal carcinoids.

Intestinal carcinoids were diagnosed in three aged dogs. Two of the neoplasms occurred in the rectum and one at the cecocolic junction. Judged by histologic criteria, all three carcinoids were considered to be malignant. Amyloid was present in both rectal tumors. In each tumor the diagnosis was confirmed by the finding of electron-dense intracytoplasmic secretory granules in the neoplastic cells.
