[Corneal perforated wounds treatment by "thermal contact": report of 25 cases].

Authors present a particular treatment of perforated corneal wounds. Patients presenting traumatic perforated corneal wound with iris hernia infected or not, have been treated by a weekly hernia's outside cauterization. Wound dressing, rifamycin eye drop until cicatrization, antibiotic and anti-inflammatory per os, are daily instituted for 10 days. Patients follow up runs from 5 months to 4 years. We have obtained in all cases ocular bulb conservation but one case of phthisis bulb. Visual acuity increase between 2/10 and 7/10 after eyes with peripheral perforated wounds cicatrization. Second eye is normal in all cases. This technique can be used to avoid mutilation surgery in our region when prosthesis is difficult to get.

[Filtering trepano-trabeculectomy with sponge-like scar].

Filtering trepanotrabeculectomy (TTE) is a modification of the operation after J. Fronimopoulos. A triangular scleral flap is created which is approximately one-half as thick as the sclera. Trepanation is performed with a 2 mm trephine. The scleral edge of the trepanation opening is heat-cauterized. Filtering TTE was performed on 36 eyes - 17 with glaucoma simplex, 16 with congestive glaucoma, and 3 with secondary glaucoma. Fifty percent of the eyes thus treated had narrow chamber angles. Regulation of IOP was achieved in 96% of the cases. Intraoperative and postoperative complications were rare, with the exception of choroidal detachment (13.8%). The fundamental requirement for regulation of IOP is a well-formed, active filtering bleb, developing as a result of the spongelike scar which is protected by the scleral flap. The scar creates a connection between the anterior chamber and the subconjunctival space.

[Congenital hand malformation--local disturbance in the limb bud and longitudinal deficiency: an experimental study on the pathogenesis].

Experimental congenital malformation produced in fetal chickens to investigate any correlation between a local disturbance in the limb buds and longitudinal deficiency, and to elucidate the pathogenesis of radial and ulnar ray deficiencies. Fertilized White Leghorn eggs were incubated at 38.0 degrees C and windowed. The right wing bud of the embryos was locally cauterized using a bipolar microcoagulator under a microscope. Cauterization was carried out from 2.5 days to 5.5 days of development, at 12-hour intervals. Each wing bud was divided into 6 areas: axial (distal and proximal), preaxial (distal and proximal) and postaxial (distal and proximal). After cauterization the eggs were sealed with micropore tape and further incubated until the 21st-23rd day. The chick wings were then removed and investigated macroscopically and roentgenographically. The wing buds had been examined microscopically after cauterization on the 3rd day until ray deficiencies were produced on the 5th day. Radial ray deficiencies were produced by cauterization in the distal preaxial area, and ulnar ray deficiencies by cauterization in the distal postaxial area. These results indicated a peculiarity of the longitudinal axis. However, the critical periods of the radial and the ulnar ray deficiencies were the same; from immediately after the formation of the wing bud to before the formation of the digital plate. The patterns of the combined defects in the digits and of other associated deformities were quite similar to those found in human malformations. Microscopically necrosis in the mesoderm and in the overlying ectoderm of the pre and postaxial areas of the tip of a wing bud caused defects in the cartilaginous primordium in the same ray.

Aluminum tattoo: a phenomenon that can resemble parasitized histiocytes.

We present a study of aluminum tattoos that resulted from aluminum chloride cauterization of biopsy sites. The stippled macrophages were studied by light and electron microscopy and by means of X-ray energy spectroscopy (XES) analysis. The light microscopic appearance may be striking, and may mimic the appearance of parasitized histiocytes, although the particles are larger and vary in size and shape. The basophilic particles are electron dense by electron microscopy. XES analysis demonstrated distinct emission peaks corresponding to aluminum, calcium, and phosphorus. Only a minor chloride emission peak was identified. The particles may represent an inorganic precipitate or an organic compound in which aluminum, calcium, and phosphate form a complex with protein.

The pharmacologic therapy of post-cauterization and post-laser vaporization with polydeoxyribonucleotide.

The most widely used ablative techniques in the therapy of benign cervical lesions are physical treatments with cauterization or laser vaporization; these are, however, usually used for the largest or symptomatic lesions. Many Authors suggest, after physical treatment, the use of topical chemotherapy in order to abolish any possible inflammatory reaction. The use of drugs such as polydeoxyribonucleotide (PDRN) 5 mg (POLIDES 5--Farmigea), provided with reepithelialization and anticomplement action, seems to promote a quicker recovery of the cauterized or vaporized zone, avoiding, at the same time, the secondary inflammatory reaction. The Authors have assessed the quality of reepithelialization by means of PDRN 5 mg ovules of the cervical zone which has been previously subjected to laser vaporization or cauterization for benign cervical lesions or CIN I. The trial was performed with two groups of patients: Group A: laser vaporization, 45 patients, 23 of whom treated with PDRN and 22 with placebo. Group B: cauterization, 46 patients, 24 treated with PDRN and 22 with placebo. The treatment with PDRN 5 mg ovules started on the day of physical treatment and continued for twelve days. The examination of the patients, performed before the treatment (TO) included the following tests: bacteriological test; PAP-smear, colposcopic examination with eventual direct biopsy. The first follow-up (T 1) was performed after 14 days and included a Pap-smear, colposcopic examination and microcolpohysteroscopy (MC) carried out in the zone where physical treatment had been performed, in order to obtain a map of the reepithelialization process process.(ABSTRACT TRUNCATED AT 250 WORDS)

Critical period for sensory experience-dependent survival of newly generated granule cells in the adult mouse olfactory bulb.

Granule cells in the olfactory bulb (OB) are continually produced and added into the neuronal circuit in the adult brain. Sensory input to the OB plays a key role in the survival of newly generated granule cells. Here, we examined in the adult mice whether there is a time window after the generation of new granule cells when their survival is strongly influenced by sensory input. New granule cells were labeled by BrdUrd injection, and the mice were deprived of sensory input unilaterally by naris cauterization. During the initial 14 days after BrdUrd labeling, the number of BrdUrd-positive granule cells was similar for deprived and undeprived OBs. At 28 days or later, the BrdUrd-positive cell number remarkably decreased in the deprived OB. Cauterization at days 14-28 effectively reduced the number of BrdUrd-positive granule cells, whereas 2-week cauterization before or after this period produced little effect. Administration of diazepam, a GABAA receptor modulator, decreased the number of BrdUrd-positive granule cells. The diazepam administration was most effective at days 14-28. Histochemical examination showed that activation of caspase-3 was accompanied by apoptotic cell death of granule cells that was induced by sensory deprivation or diazepam administration. Double labeling with activated caspase-3 and BrdUrd indicated that granule cells at days 14-20 were most susceptible to cell death. These results indicate that there is a critical period when the survival of new granule cells is determined in a sensory experience-dependent manner and that the pharmacological manipulation can mimic the effect of sensory deprivation.

Prospective, randomized evaluation of the efficacy of fibrin sealant as a topical hemostatic agent at the cannulation site in neonates undergoing extracorporeal membrane oxygenation.

BACKGROUND: The topical hemostatic effect of fibrin sealant that has been solvent/detergent treated and plasminogen depleted was evaluated in a multicenter prospective, randomized controlled study at the cannulation site wound of infants undergoing extracorporeal membrane oxygenation (ECMO). METHODS: The test group received standard cauterization and Fibrin sealant, while the control group was given cauterization alone to control hemostasis at this site. Efficacy data were available on 173 randomized study subjects of whom 149 met study entry criteria. All were managed according to standard ECMO practice. RESULTS: Fibrin sealant reduced the risk of bleeding, was associated with less shed blood, and was associated with shorter duration of hemorrhage. Further, control infants showed an increased bleeding risk with less depressed fibrinogen levels and prothrombin time elevations >18 seconds prior to ECMO. CONCLUSION: Fibrin sealant is useful as a topical hemostatic agent in patients with coagulopathy not responding to standard surgical techniques.

[A case report: anomaly of the fourth branchial pouch with recurring cervical abscesses. Cauterization with trichloroacetic acid closed the fistula opening and cured the patient].

A fourth branchial pouch sinus is a rare congenital anomaly, which in a 13-year-old girl presented clinically as recurrent deep cervical abscesses. The location of the majority of these anomalies is the left side of the neck (90%). Radiological and endoscopic investigations verified the diagnosis. The internal orifice located at the apex of the pyriform sinus could facilitate contamination by infectious pharyngeal secretions and lead to abscess recurrence. Traditionally, the recommended treatment is radical surgery. It can, however, be technically difficult to excise the whole fistula tract. In this patient we used a non-invasive treatment modality; chemocauterization with 40% trichloroacetic acid (TCA). After three treatments the fistula was closed. To date (month no. 15) there has been no abscess recurrence. TCA chemocauterization seems to be a safe first-line treatment for patients with pyriform sinus fistulas.

[Endothelium of the jugular vein of domestic chicken at the remote periods after injury].

Detailed analysis of changes in the endothelial layer at late terms after injury revealed that in one month after cauterization with a 0,25%solution of silver nitrate the recovered epithelium was not substatially different from the initial state by most features and retained its specific properties. From the normal state itdistinguished by the cell size, their orientation with respect to the longitudinal axis ofthe vessel, but the signs characterizing the lining as a whole, such as correct arrangement of the layer, the shape of the cells, gave evidence that the endothelium had recovered retainingits specific properties as a highly sensitive system with intrinsic signs of a tissue type. A significant amount of binuclear cells at late terms as well as greater amount of nuclei with centromeres as compared with the normal state pointed out that the traces of the reactive processretained for a very long time.

Office microlaparoscopy for female sterilization under local anesthesia. A cost and clinical analysis.

OBJECTIVE: To evaluate the clinical and cost effectiveness of permanent female sterilization using microlaparoscopy and bipolar cauterization. The authors also tested how well the procedure was tolerated when performed under local anesthesia and conscious sedation. STUDY DESIGN: Patients undergoing microlaparoscopic permanent sterilization were prospectively followed. The rate of procedure completion, patients' pain scores and complication rates were reviewed. RESULTS: The procedure was completed in 93% of patients (28 of 30). The pain scores decreased substantially within one day after the procedure. The total cost savings for the 29 cases were $16,211. Two patients experienced severe intraoperative discomfort and were rescheduled for endoscopic sterilization under general anesthesia. There were no complications. CONCLUSION: Permanent female sterilization using microlaparoscopy is cost effective and safe in selected patients, patient exclusion criteria need to be instituted to ensure the safety and success of the procedure.

Mouse model of sustained elevation in intraocular pressure produced by episcleral vein occlusion.

We have developed an inducible mouse model of glaucoma based on episcleral vein cauterization (EVC). Intraocular pressure (IOP) elevation in adult mice was produced by cauterizing three episcleral veins. Serial IOP measurements were done by induction-impact tonometry. IOP was significantly elevated by 104+/-20% in 20 out of 23 mice (87%) within the first day after EVC, and remained elevated for 4 weeks, with mean IOP 94% higher in EVC-treated vs. contralateral control eyes. Aqueous outflow blockade was verified from the IOP response to pulsed fluid infusions into the anterior chamber. Retinal ganglion cell (RGC) loss, determined by retrograde labelling using Fluoro-Gold applied to the superior colliculous, was approximately 20% at 2 weeks after EVC. We conclude that episcleral vein occlusion in mice produces significant and sustained elevation in IOP associated with increased outflow resistance and RGC loss, and thus may be useful to model glaucoma in genetically modified and drug-treated mice.

Four-dimensional real-time sonographically guided cauterization of the umbilical cord in a case of twin-twin transfusion syndrome.

In the past decade, three-dimensional (3D) sonographic technology has matured from a static imaging modality to near-real-time imaging. One of the more notable improvements in this technology has been the speed with which the imaged volume is acquired and displayed. This has enabled the birth of the near-real-time or four-dimensional (4D) sonographic concept. Using the 4D feature of the current 3D sonography machines allows us to follow moving structures, such as fetal motion, in almost real time. Shortly after the emergence of 3D and 4D technology as a clinical imaging tool, its use in guiding needles into structures was explored by other investigators. We present a case in which we used the 4D feature of our sonographic equipment to follow the course and motion of an instrument inserted into the uterus to occlude the umbilical cord of a fetus in a case of twin-twin transfusion syndrome.

Use of a cauterizing laparoscopic linear stapler in intestinal anastomosis.

The purpose of this study was to assess the usefulness of a cauterizing laparoscopic linear stapler for intestinal anastomosis. In a porcine model, intestinal anastomoses performed with a standard laparoscopic linear stapler, a cauterizing laparoscopic linear stapler (RF stapler), and a two-layer, hand-sewn technique were compared by measuring bursting pressures at 4 and 7 days after surgery. During surgery, the RF stapler provided better hemostasis than the regular stapler for mesenteric transection. At 4 days, one leak occurred in the RF stapler group, and the bursting pressure in the RF stapler group was significantly lower than the bursting pressures in the regular stapler group and the hand-sewn group. In addition, the bursting pressure was significantly greater in the hand-sewn group than in the regular stapler group at 4 days. By 7 days, there were no differences in bursting pressure among the groups. We recommend that the RF stapler not be used for intestinal anastomosis. However, the device may be beneficial for controlling vasculature.

Collagen metabolism during the healing process of the cauterized rat cornea. An experimental study.

Attempts were made (1) to develop an appropriate method for producing standardizable and reproducible experimental opacities of the cornea of rats and (2) to study quantitatively the course and intensity of the healing process of these corneal wounds. The right cornea of rats was cauterized by drops of 1.5 N HC1 over 30 s; the left one served as a control. The spontaneous course of wound healing, as well as the possible effect on this process of spray treatment with iodine brine or, for comparative purposes, with isotonic NaCl, was studied for 10 consecutive days by measuring the 24-h incorporation of 14C-labelled proline as a measure of de novo collagen synthesis. Spray treatment (1) abolishes the transient inhibition of proline incorporation observed in the unsprayed cornea on the first day after injury and (2) significantly advances the maximum collagen synthesis rate from day 3-4 to day 2 after the wound occurred. The position and magnitude of the 14C-proline incorporation peak are considered to be useful parameters for estimating corneal wound healing activity and its alteration by various agents. However, in this experimental design, it could not be proved that iodine has a specific effect.

Three experimental glaucoma models in rats: comparison of the effects of intraocular pressure elevation on retinal ganglion cell size and death.

Glaucoma is a chronic and progressive optic nerve neuropathy involving the death of retinal ganglion cells (RGCs). Elevated intraocular pressure (IOP) is considered to be the major risk factor associated with the development of this neuropathy. The objective of the present study was to compare the effects on RGC survival of three different experimental methods to induce chronic elevation of IOP in rats. These methods were: (i) injections of latex microspheres into the eye anterior chamber; (ii) injections into the anterior chamber of a mixture of microspheres plus hydroxypropylmethylcellulose (HPM) and (iii) cauterization of three episcleral veins. The IOP of right (control) and left (glaucomatous) eyes was measured with an applanation tonometer in awake animals. Thirteen to 30 weeks later, RGCs were retrogradely labeled with 3% fluorogold. Subsequently, we analyzed the density of RGCs, as well as the major axis length and area of RGC soma resulting from the application of each method. A significant increase in IOP was found following application of each of the three methods. Cell death was evident in the glaucomatous eyes as compared to controls. However, no statistical differences were found between the extent of cell death associated with each of the three methods. IOP increase also induced a significant increase in the size of the soma of the remaining RGCs. In conclusion, the three methods used to increase IOP induce a similar degree of RGC death. Moreover, the extent of cell death was similar when the retinas were maintained under conditions of elevated IOP for 24 weeks in comparison to 13 weeks.

Punctal occlusion in the management of chronic Stevens-Johnson syndrome.

PURPOSE: To evaluate the effect of lacrimal punctal occlusion in the management of the ocular surface disease in chronic Stevens-Johnson syndrome. DESIGN: Retrospective noncomparative case series. PARTICIPANTS: Thirty-one eyes of 18 patients with chronic Stevens-Johnson syndrome seen at the Department of Ophthalmology of the Tokyo Dental College were studied. INTERVENTION: Patients' lacrimal puncta were occluded by cauterization or with punctal plugs. MAIN OUTCOME MEASURES: The effect of lacrimal punctal occlusion was evaluated by changes in subjective symptoms, best-corrected visual acuity (VA), Schirmer test, tear clearance test, and ocular surface double vital staining before and after punctal occlusion. RESULTS: After lacrimal punctal occlusion, symptomatic improvement was observed in 19 eyes (61.3%). The mean logarithmic VA showed significant improvement from -0.64+/-0.87 to -0.52+/-0.86 (P<0.05). The Schirmer I test results before punctal occlusion yielded a mean score of 7.4+/-8.7 mm, which changed to 10.2+/-8.3 mm after punctal occlusion. The mean preocclusion and postocclusion tear clearance values were 7.5+/-6.6 times and 4.9+/-4.8 times, respectively. The Rose Bengal staining score decreased from 4.7+/-2.8 to 2.7+/-2.2 points (P<0.05), and the fluorescein staining score decreased from 5.0+/-2.3 to 2.2+/-2.5 points (P<0.05), respectively. CONCLUSIONS: Concurrent improvements in subjective symptoms, vital staining scores, and VA point to the favorable effects of lacrimal punctal occlusion for the ocular surface health in chronic Stevens-Johnson syndrome.

Inhibition of implantation of murine bladder tumor by thiotepa in cauterized bladder.

This study was designed to determine the role of immediate intravesical instillation of single dose thiotepa post transurethral resection of bladder tumor in the prevention of recurrence by tumor implantation, using murine bladder tumor line 2 and 201 C3H/He mice. Previous studies have suggested implantation may take place as early as the first hour and reach its maximum in 24 hours after resection of bladder tumor. An in vitro dose response curve of MBT2 to thiotepa was established by treatment with various concentrations of thiotepa of 0.00, 0.01, 0.21, 0.44, and 1.91 mg./ml. In a group of 201 mice, the bladder was catheterized with a 24G angiocatheter, and a fine copper wire was inserted through the lumen. The bladder was cauterized by touching the wire with a Bovie coagulator for four seconds at the lowest setting. All bladders were instilled with 1 x 10(6) cells of murine bladder tumor line 2, followed by instillation of 1.91 mg./ml. of thiotepa with various time delays per treatment group. The bladder implantation rates were 30.4% (17/56), 3.4% (2/59), 6.5% (2/31) and 26.9% (7/26) in the control, immediate, one-hour delay and 24-hour delay groups, respectively. The urethral implantation rates were 21.4% (12/56), 0% (0/59), 6.5% (2/31) and 0% (2/26), respectively. The overall implantation rates (bladder, urethra, or both) were 42.9% (24/56), 3.4% (2/59), 6.5% (2/31) and 25.9% (7/27), respectively. Implantation rates were significantly higher in the control and 24-hour delay groups than in the immediate and one-hour instillation groups (p less than 0.05, Fisher Exact Test). We conclude from this animal model that intravesical instillation of single dose thiotepa, to be effective, should be initiated within the first hour after tumor resection, since it dramatically decreased the incidence of bladder and urethral implantation.

Functional and radiographic evaluation of 214 aggressive benign bone lesions treated with curettage, cauterization, and cementation: 24 years of follow-up.

PURPOSE: Treatment with curettage, cauterization, and methylmethacrylate of aggressive benign bone lesions is a method now widely accepted in most orthopedic oncology centers. However, one of the controversies regarding this technique is the possible complications that may arise from the use of methylmethacrylate, which has caused some authors to remove it 2 years after the surgery and replace it with bone graft. The objective of this paper is to present a functional and radiographic evaluation of 214 patients presenting with aggressive benign bone lesions treated with curettage, cauterization, and methylmethacrylate from 1974 to 1998, with some of them having 24 years of follow-up. These patients were clinically and radiographically evaluated for the incidence of late osteoarthrosis, range of motion, and pain in the involved joint. METHOD: This study comprised 214 patients, with an average follow-up duration of 10.6 years (range: 2 to 24 years. All cases involved aggressive benign lesions. The patients were evaluated according to the Musculoskeletal Tumor Society Score (MSTS) functional evaluation system, and the complications are described. RESULTS: The MSTS functional evaluation was excellent in 166 cases (78%), good in 26 (12%), fair in 11 (5%), and poor in 11 (5%). The following complications were observed: late osteoarthrosis, 25 cases (12%); infection, 12 (6%); pathologic fracture, 11 (5%); and local recurrence, 19 (9%). CONCLUSION: Based on clinical assessment, no significant deleterious effects directly related to the use of methylmethacrylate were observed. The functional evaluation performed in 1998 (up to 24 years of follow-up) did not show significant change when compared to the evaluation performed in 1985.
