Acceptability ratings of tracheoesophageal speech.

Thirty-one naive listeners rated the intelligibility and acceptability of tracheoesophageal, esophageal, and normal laryngeal speech of 15 adult speakers. Intelligibility was measured using a multiple-choice word test while acceptability was rated on a seven-point equal-appearing interval scale. Judgments were made on the features of fluency, pitch/quality, speech rate, inflection, and overall acceptability. Tracheoesophageal speech was found not to differ significantly from normal speech in intelligibility, rate, and inflection, but was less acceptable for fluency, pitch/quality and overall acceptability. Tracheoesophageal speech was more acceptable than esophageal speech for all features measured.

The acceptability of the female and male condom: a randomized crossover trial.

CONTEXT: Although studies have assessed the acceptability of male and female condoms, comparative trial data are lacking. METHODS: A sample of 108 women in stable relationships recruited from an urban, reproductive health clinic were randomly assigned to use 10 male or female condoms, followed by use of 10 of the other type. A nurse provided instruction in correct method use. Demographic information was collected in a baseline questionnaire; acceptability data were collected in follow-up and exit questionnaires and coital logs. Nonparametric and chi-square statistics were used to analyze measures of the methods' relative acceptability. Bowker's test of symmetry was adapted to test the null hypothesis of no difference in acceptability between condom types. RESULTS: Participants used 678 female and 700 male condoms. Although neither method scored high on user satisfaction measures, the 63 women completing the study protocol preferred the male condom to the female condom for ease of application or insertion, ease of removal, general fit, feel of the condom during intercourse and ease of penetration. Participants reported that their partner also favored the male condom, although women generally appeared to like this method more than their partner did. In a direct comparison between the methods at the end of the study, women generally judged male condoms superior on specified preference criteria. CONCLUSIONS: Across a range of criteria, the female condom was less acceptable than the male condom to most women and their partners. Although both types had low acceptability, they are needed and valid methods of pregnancy and disease prevention. That neither rated high on user satisfaction measures underscores the need for more barrier methods that women and men can use.

Acceptance of moderate drinking by alcoholism treatment services in Canada.

OBJECTIVE: The present study was conducted to provide a nationwide survey of acceptance of nonabstinence goals and related alcoholism treatment practices by Canadian alcoholism treatment services. METHOD: A random sample of 335 Canadian alcoholism treatment service agencies were mailed a 4-page questionnaire designed to assess acceptance of moderate drinking as a drinking goal and related alcoholism treatment practices. RESULTS: Acceptance varied by type of service, with considerably more acceptance by outpatient programs (62%) and mixed inpatient/outpatient programs (43%) than inpatient/detoxification/ correctional facilities (27%) and halfway houses (16%). Two-thirds of the respondents who reported moderate drinking as unacceptable in their own agencies categorically rejected moderation for all alcoholism clients. CONCLUSIONS: Individuals seeking services in Canadian alcoholism treatment agencies are more likely to have a choice of drinking goals if they present to an outpatient program than a residential institution, and Canadian agencies appear more accepting of moderation goals than American programs, but less accepting than British and Norwegian service agencies.

Infant acceptance of breast milk after maternal exercise.

OBJECTIVE: Previous research reported that breast milk lactic acid (LA) levels increase after lactating women complete a bout of exhaustive exercise, resulting in poor infant acceptance of the postexercise breast milk. This highly publicized finding may not apply to more practical, everyday exercise conditions of lactating women. The purpose of the present study was to reexamine the composition and infant acceptance of postexercise breast milk while controlling maternal diet, exercise intensity, and the method, timing, and assessment of infant feeding. RESEARCH DESIGN AND METHODS: Twenty-four women, 2 to 4 months' postpartum, completed 3 test sessions: a maximal oxygen uptake test, a 30-minute bout of moderate exercise, and a resting control session. One hour before and 1 hour after each session, participants fully expressed their milk, placed it in a bottle familiar to the infant, fed their infant, and rated their infant's acceptance of the milk. Each feeding was videotaped and viewed individually by 3 lactation consultants who rated infant acceptance; consultants were blinded to the test sessions. Milk was analyzed for LA and infant milk consumption was measured. RESULTS: There were no differences in presession versus postsession values for maternal skin temperature, breast milk temperature, and infant milk acceptance as judged by either the mothers or lactation consultants. These results prevailed despite a small but significant increase in breast milk LA premaximal versus postmaximal exercise (0.09 vs 0.21 mM, respectively); there was no difference in milk LA premoderate versus postmoderate exercise, or prerest versus postrest. CONCLUSION: These data support the hypothesis that moderate or even high-intensity exercise during lactation does not impede infant acceptance of breast milk consumed 1 hour postexercise.

Reliability of retrospective reports of perceived maternal acceptance-rejection in childhood.

Research reported here addresses the issue of the reliability of retrospective reports of children's perceptions of maternal acceptance-rejection as measured by the Parental Acceptance-Rejection Questionnaire. A sample of 49 middle-class Caucasian 7- to 11-yr.-old children were asked to respond to the questionnaire reflecting on their mothers' current accepting-rejecting behaviors. Seven years later the same children--now adolescents--responded to the same questionnaire with the instruction to reflect back on their mothers' behavior when the youth were about 7 to 11 years of age. None of these youth recalled having been tested seven years earlier. A simple zero-order correlation between scores in childhood and adolescence was 62, indicating that adolescents' retrospective recollections were in moderate agreement with their reports during childhood. Thus, it seems clear that, at least with respect to perceptions of maternal acceptance-rejection as measured by the Parental Acceptance-Rejection Questionnaire, researchers can have reasonable confidence that adolescents' current recollections about their experiences of maternal acceptance-rejection are likely to be in moderate agreement with what they would have reported had they been tested during childhood.

A systematic literature review of the effectiveness of non-pharmacological interventions to prevent wandering in dementia and evaluation of the ethical implications and acceptability of their use.

OBJECTIVES: To determine the effectiveness and cost-effectiveness of non-pharmacological interventions (excluding subjective barriers) in the prevention of wandering in people with dementia, in comparison with usual care, and to evaluate through the review and a qualitative study the acceptability to stakeholders of such interventions and identify ethical issues associated with their use. DATA SOURCES: Major electronic databases were searched up until 31 March 2005. Specialists in the field. REVIEW METHODS: Selected studies were assessed and analysed. The results of two of the efficacy studies that used similar interventions, designs and outcome measures were pooled in a meta-analysis; results for other studies which reported standard deviations were presented in a forest plot. Owing to a lack of cost-effectiveness data, a modelling exercise could not be performed. Four focus groups were carried out with relevant stakeholders (n = 19) including people with dementia and formal and lay carers to explore ethical and acceptability issues in greater depth. Transcripts were coded independently by two reviewers to develop a coding frame. Analysis was via a thematic framework approach. RESULTS: Ten studies met the inclusion criteria (multi-sensory environment, three; music therapy, one; exercise, one; special care units, two; aromatherapy, two; behavioural intervention, one). There was no robust evidence to recommend any non-pharmacological intervention to reduce wandering in dementia. There was some evidence, albeit of poor quality, for the effectiveness of exercise and multi-sensory environment. There were no relevant studies to determine the cost-effectiveness of the interventions. Findings from the narrative review and focus groups on acceptability and ethical issues were comparable. Exercise and distraction therapies were the most acceptable interventions and raised no ethical concerns. All other interventions were considered acceptable except for physical restraints, which were considered unacceptable. Considerable ethical concerns exist with the use of electronic tagging and tracking devices and physical barriers. Existing literature ignores the perspectives of people with dementia. The small number of participants with dementia expressed caution regarding the use of unfamiliar technology. Balancing risk and risk assessment was an important theme for all carers in the management of wandering. CONCLUSIONS: There is no robust evidence so far to recommend the use of any non-pharmacological intervention to reduce or prevent wandering in people with dementia. High-quality studies, preferably randomised controlled trials, are needed to determine the clinical and cost-effectiveness of non-pharmacological interventions that allow safe wandering and are considered practically and ethically acceptable by carers and people with dementia. Large-scale, long-term cohort studies are needed to evaluate the morbidity and mortality associated with wandering in dementia for people both in the community and in residential care. Such data would inform future long-term cost-effectiveness studies.

Home-style beef jerky: effect of four preparation methods on consumer acceptability and pathogen inactivation.

The safety of homemade jerky continues to be questioned. Producing a safe product that retains acceptable quality attributes is important. Lethality of Salmonella, Escherichia coli O157:H7, and Listeria monocytogenes as well as consumer acceptability and sensory attributes of jerky prepared by four methods were examined. Preparation methods were drying marinated strips at 60 degrees C (representing a traditional method), boiling strips in marinade or heating in an oven to 71 degrees C prior to drying, and heating strips in an oven after drying to 71 degrees C. A 60-member consumer panel rated overall acceptability. A 10-member descriptive panel evaluated quality attributes. Samples heated after drying and samples boiled in marinade prior to drying had slightly higher acceptability scores but were not statistically different from traditional samples. Although the four treatments were significantly different in color (P = 0.0001), saltiness (P = 0.0001), and texture (P = 0.0324), only texture appeared to influence overall consumer acceptability. Microbial challenge studies subjecting the pathogens to the four treatments showed a 5.8-, 3.9-, and 4.6-log reduction of E. coli O157:H7, L. monocytogenes, and Salmonella, respectively, even with traditional drying. Oven treatment of strips after drying was shown to have the potential to reduce pathogen populations further by approximately 2 logs. In conclusion, a safer, yet acceptable home-dried beef jerky product can be produced by oven-heating jerky strips after drying.

[Self-acceptance as adaptively resigning the self to low self-evaluation].

In past studies, the concept of self-acceptance has often been confused with self-evaluation or self-esteem. The purpose of this study was to distinguish these concepts, and operationally define self-acceptance as Carl Rogers proposed: feeling all right toward the self when self-evaluation was low. Self-acceptance as adaptive resignation, a moderating variable, therefore should raise self-esteem of only those people with low self-evaluation. Self-acceptance was measured in the study as affirmative evaluation of own self-evaluation. Two hundred and forty college students, 120 each for men and women, completed a questionnaire of self-evaluative consciousness and self-esteem scales. Results of statistical analyses showed that among subjects with low self-evaluation, the higher self-acceptance, the higher the person's self-esteem. The same relation was not observed among those with high self-evaluation. Thus, it may be concluded that self-acceptance was adaptive resignation, and therefore meaningful to only those with low self-evaluation.

The acceptance and satisfaction of Saudi males to vasoactive autoinjection intracavernous therapy, external negative pressure device and penile prosthesis in the treatment of erectile dysfunction.

BACKGROUND: The aim of the study was to determine the acceptance and satisfaction of Saudi impotent male patients to intracavernous vasoactive autoinjection therapy, external negative pressure device and penile prosthesis insertion in the treatment of their impotence. SUBJECTS AND METHODS: The medical records of 210 Saudi patients who presented with erectile dysfunction and were offered intracavernous vasoactive autoinjection therapy, external negative pressure device or penile prostheses for the treatment of their erectile dysfunction were retrospectively analyzed to determine the acceptance and satisfaction of their selected treatment modalities. RESULTS: Intracavernous vasoactive autoinjection therapy was the most acceptable treatment option, followed by negative pressure device, and then insertion of penile prosthesis. About 11.9% of patients refused all treatment options. Only 40% and 12% of patients who were treated with intracavernous vasoactive autoinjection therapy and external negative pressure device, respectively, were satisfied with their treatment modality. After one year of follow-up, insertion of penile prosthesis had the highest satisfaction rate among the treatment modalities, with 92% and 87% of patients being satisfied at 3 and 12 months' follow-up, respectively. CONCLUSION: Intracorporeal vasoactive autoinjection therapy had the highest acceptance rate compared with external negative pressure devices and penile prosthesis, however, it had low satisfaction rate and the dropout rate was high. External negative pressure devices are generally poorly accepted and are rarely satisfactory in our patient population. Penile prosthesis insertion is poorly accepted, however, it has the highest satisfaction rate. Such information will allow patients under treatment for impotence and their treating physicians to make better, educated choices on the mode of treatment.

Acceptability of maxillary major connectors in removable partial dentures.

BACKGROUND: The choice of connector lies between plate, a bar or a combination of bars, which may cross the palate in various positions. Many opinions have been expressed concerning the acceptability to the patients of the various forms of palatal connectors but there have been few investigations. OBJECTIVES: The aim of this study is to compare subjective patients' reactions to three maxillary major connectors: metal bar, metal plate, and acrylic connectors and to establish the most acceptable and the least acceptable maxillary major connectors. METHODS: Fifteen consecutive patients attending the prosthetic out-patient clinic of Dental Center, University College Hospital, Ibadan who had never worn dentures were recruited into the study. Three dentures each with a different maxillary major connector design were fabricated for each patient. A questionnaire was administered to ascertain their reactions to each of the three denture designs. RESULTS: Fourteen patients (93.3%) preferred the denture with metal palatal bar major connector whereas only one patient preferred the denture with acrylic plate major connector design. No patient preferred the palatal metal plate connector design. CONCLUSIONS: The metal bar major connector was the most acceptable maxillary major connector while acrylic resin plate was more acceptable than metal plate maxillary major connector. It also confirmed the influence of connector design on patient acceptance of removable partial denture.

Measuring the significance of participant evaluation of acceptability of cases in the College of American Pathologists Interlaboratory Comparison Program in cervicovaginal cytology.

CONTEXT: The quality of gynecologic cytology slides within educational and proficiency testing programs may deteriorate during use. Participant evaluation of the acceptability of these slides subsequent to possible deterioration is not known. OBJECTIVE: To assess participants' evaluation of the acceptability of slides circulating within an educational gynecologic cytology glass slide program. DESIGN: The College of American Pathologists Interlaboratory Comparison Program in Cervicovaginal Cytology is a peer comparison and educational program that evaluates the ability of the participants to correctly classify gynecologic cytology preparations. The program uses both expert review and field validation to select slides for the graded portion of the program. Participants were asked to assess the acceptability of slides within the College of American Pathologists Interlaboratory Comparison Program in Cervicovaginal Cytology, and their responses were assessed with respect to type of slide preparation, validation status, and reference diagnosis. In addition, we compared the cytodiagnostic discordancy rates of slides that were deemed acceptable by participants with those that were deemed unacceptable. SETTING: Participant assessments were derived from pathologists and cytotechnologists from cytology laboratories of all types. RESULTS: A total of 17,210 slide interpretations were reviewed, and 2.91% of the cases were labeled unacceptable by participants. For all slides, the percentage of cases called unacceptable varied from 1.65% for cases with a reference interpretation of herpes to 45% for cases with a reference interpretation of unsatisfactory. The percentage of slides deemed unacceptable was higher for validated slides than for educational slides (3.27% vs 2.55%, P = .006). The discordancy rate (to reference diagnosis series) for cases deemed unacceptable was significantly higher than the discordancy rate for cases deemed acceptable for both validated (10.39% vs 1.76%) and educational slides (21.72% vs 3.53%, P < .001). CONCLUSION: Greater than 97% of all slides in the College of American Pathologists Interlaboratory Comparison Program in Cervicovaginal Cytology were judged acceptable by participants. Despite expert review and field validation, a small percentage of slides (almost 3%) in this program were deemed unacceptable by participants. These results support the use of participant evaluation of cases to continually improve the quality of cases in this program.

Acceptance of pharmacists' suggestions by prescribers: a literature review.

Acceptance of pharmacists' suggestions by prescribers is a necessary component of the evaluation of clinical pharmacy services that is often overlooked. A literature search was performed to review the information on this topic. Acceptance was defined as a measure of a prescriber's compliance with a pharmacist's recommendation regarding drug therapy. In order for the pharmacist to impact upon the quality of patient care or drug costs, the prescriber must accept the suggestions. A total of 23 studies was found that addressed the topic. The average rate of acceptance was 85.5%. Factors affecting acceptance included time, communication, solicited versus unsolicited recommendations, type of prescriber, and type of pharmacist. Factors leading to non-acceptance included lack of physician awareness of pharmacokinetic parameters, quality of suggestions, prescribers' exercise of caution with respect to patient safety and well-being, and negative attitude toward clinical pharmacy. More research needs to be done in order to evaluate the importance of these factors.

Acceptance of the xylitol chewing gum regimen by preschool children and teachers in a Head Start program: a pilot study.

PURPOSE: This pilot study evaluated the acceptance and compliance of a xylitol chewing gum regimen by both children and classroom teachers in a Head Start program. METHODS: Thirty-five children chewed 100% xylitol gum (XyliFresh 100%, Leaf, Inc., 500 Field Dr., Lake Forest, IL 60045 U.S.A.) three times a day over a three week period. Children's acceptance was evaluated using a picture selection test. Teachers' acceptance was evaluated using a questionnaire. The children's and teachers' compliance was evaluated using a daily checklist that was completed by each teacher. RESULTS: Positive ratings were given for xylitol gum chewing (94%) and for taste (86%). Children's acceptance and compliance for chewing was excellent. Children chewed the gum at designated times and none of the children swallowed the gum. Teachers' acceptance of the chewing program was low. Three out of five participating teachers thought the gum chewing disturbed the classroom routine and four were not willing to participate in the program next year. Teachers' compliance was good and they followed by instructions during a three-week period. CONCLUSIONS: This study supports the suggestion that chewing xylitol gum is well accepted by children. Collaboration and education is essential to motivate teachers to adopt and supervise school-based prevention programs.

[Disclosure of a diagnosis of childhood autism and parents' acceptance of the disability].

Mothers of 14 children (10 boys, 4 girls) with childhood autism answered the self-report questionnaire regarding the disclosure of a diagnosis of autism and their acceptance of the disability. The children's mean age at the study was 8.6 years, and their ages at the time of the disclosure and acceptance were 4.0 and 5.2 years, respectively. The age at acceptance exhibited a statistically significant positive correlation with the age of disclosure indicating that early disclosure was related to early acceptance. The results obtained in the questionnaire suggested that the increased understanding of autistic behavior, the accumulated experience as to how to communicate with their own child and the existence of a self-care group to support the parents were important factors for the acceptance of the disability. The significance of the professionals' sympathetic manner at the time of disclosure, full explanation of the disability, instruction as to how to cope the disability and providing the information on social resources available in the local district was also discussed in relation to the acceptance of the disability.

[Acceptance of social security by the German population].

Against the background of persistent problems of financing and monitoring the system of social security as practised in the Federal Republic of Germany, empirical research into preferences and acceptance has gained increasing importance. The acceptance of social security by the population has been checked empirically by means of data of the Socio-Economic Panel. The following questions have been studied: Which degree of acceptance applies generally speaking to the existing system? Are there differences in respect of acceptance regarding the various subdivisions of social insurance and how are they to be explained? What are the conclusions that can be arrived on the basis of these results? Overall assessment of the indicators employed shows that on the whole a high degree of acceptance is accorded to the existing security system. It cannot be concluded from the results that a fundamental reorganisation of the entire system towards security mainly on the individual's own responsibility would be in accordance with the preferences of the great majority of the population. Reorganisation would have to reckon with a low acceptance level. However, the monthly contributions--especially in respect of sickness insurance--are considered to be too high by part of the population.

Acceptability of and adherence to dispersible zinc tablet in the treatment of acute childhood diarrhoea.

Zinc treatment is now recommended by the World Health Organization as part of the routine management of acute childhood diarrhoea. A dispersible zinc tablet formulation was developed taking into account the taste, cost, and feasibility to distribute and store. Only limited information is available on the acceptability of and adherence to dispersible zinc tablet. No study has formally assessed whether the formulation is acceptable to children and if caretakers can adhere to the instructions regarding preparation, dosage, and duration of treatment. This community-based study aimed at determining the acceptability of and adherence to a dispersible zinc tablet formulation in a cohort of children (n=320) aged less than five years. Caretakers of children with acute childhood diarrhoea were prescribed zinc tablet treatment and followed up after 2-3 weeks. The formulation was acceptable to children; 90.1% of 303 caretakers perceived that the tablets were equally or even more acceptable to their children compared to other medicines. Ninety-eight percent of the children received the standard dose of one tablet per day, and 55.8% completed the full 10-day course of zinc treatment. Adherence rates did not vary by age or gender of the child. These findings indicate that the tablet formulation is acceptable, but further efforts are required to enhance adherence.

Codes and colleagues: Is there support for universal patient acceptance?

This article is a refinement of verbal reactions to O'Toole's and Corsino's remarks at a national conference on Access to Oral Health Care held at the headquarters of the American Dental Association in August 2005. The article consists of two parts, each part an answer to specific questions. The first is a reaction to Corsino's explanation of Patthoff's concept of Universal Patient Acceptance. Acceptance is supported and endorsed, and a case is made for the importance of a clear and accurate explanation of Universal Patient Acceptance, as it has a much greater likelihood of being embraced by dentists than "access" seems to have. A review of relevant codes of ethics in dentistry reveals mixed and uneven support for Universal Patient Acceptance. The second part of this article compares the way that the profession of psychology views access and acceptance with the way that dentistry seems to view them and concludes that, if dentistry is to remain a caring profession rather than a commercial enterprise, acceptance must be embraced.

Maximum acceptable weight of lift for side and back lifting.

A series of laboratory experiments were conducted to find maximum acceptable weights in front, side, and back lifting. Fifteen college students participated in the experiment. Experimental trials for each type of lifting were conducted for 10 min for each subject at a rate of 4 lifts/min. Psychophysical methodology was used to find the acceptable weight based upon their perceived feeling of stress in the lower back. It was found that subjects are willing to lift the heaviest load using back lifting (average maximum acceptable weight: 41.5 lbs). Front lifting was the close second with 39.4 lbs. Also, there was a significant difference in maximum acceptable weight of lift between side lifting (average maximum acceptable weight: 25.5 lbs) and the other two types of lifting. It was also found that leg strength was a limiting variable for maximum acceptable weight in front lifting. Composite strength and shoulder strength were found to be limiting variables in side lifting. Composite strength was the limiting variable in the back lifting.
