Sipple's syndrome with liver tumors examined by iodine-131 MIBG and technetium-99m(V)-DMSA.

This case report describes the localization and categorization of tumors using 99mTc(V)-dimercaptosuccinic acid and [131I]metaiodobenzylguanidine scans in a very uncommon case of medullary thyroid carcinoma associated with pheochromocytoma (Sipple's syndrome) and hepatocellular carcinoma. Technetium-99m(V)-dimercaptosuccinic acid showed accumulation only in medullary thyroid carcinoma, but [131I]metaiodobenzylguanidine scans were positive in both medullary thyroid carcinoma and pheochromocytoma. In advanced Sipple's syndrome, combined use of [99mTc(V)]dimercaptosuccinic acid and [131I]metaiodobenzylguanidine may be useful for the categorization of tumor mass lesions and planning appropriate therapy.

False-positive technetium-99m DMSA renal imaging in two cases of malrotated kidney.

Technetium-99m dimercaptosuccinic acid (Tc-99m DMSA) has been used as a renal imaging agent of choice. A few false-positive cases of Tc-99m DMSA imaging have been reported. The authors have experienced false-positive Tc-99m DMSA imagings in two cases of a malrotated kidney, and this finding previously has not been reported in the literature. The possibility of its nuclear imaging diagnosis with Tc-99m DTPA is also discussed.

Technetium-99m sestamibi in chronic coronary artery disease: the European experience.

Since the introduction of technetium-99m methoxy-isobutyl isonitrile (Tc-99m sestamibi) in Europe, there has been a growing interest in its use. Several European multicenter trials have been conducted to evaluate this new agent in relation to the traditional perfusion marker thallium-201, and other studies are in progress to understand the use of this perfusion marker for the diagnosis of coronary disease, for use in conjunction with pharmacologic vasodilation, for use in the assessment of ventricular function and wall motion and for the assessment of interventions.

Failure to visualize acutely injured kidneys with technetium-99m DMSA does not preclude recoverable function.

A 35-yr-old patient developed severe acute tubular necrosis requiring hemodialysis. A [99mTc]dimercaptosuccinic acid scan of the kidneys showed no renal uptake at 4 or 24 hr, but the patient subsequently recovered normal renal function as judged by a normal serum creatinine. Based on this case report and a review of the literature, one cannot assume irreversible loss of function in patients with acute renal failure, based on the absence of radiopharmaceutical uptake by the kidneys.

[X-ray and radionuclide studies of bones and joints in psoriasis].

Radiography of the bones and joints was performed in 133 psoriatic patients; osteoscintigraphy with 99mTc-phosphate compounds was also performed in 42 of them. On the basis of x-ray findings 3 types of osteoarticular psoriasis were singled out: psoriatic polyarthritis, psoriatic polyarthrosis and psoriatic arthropathy. The most ample information on the type, activity and spreading of a pathological process in the bones and joints in psoriasis can be obtained from the combination of the x-ray and radionuclide methods.

Synthesis and biological studies of neutral technetium (V) complexes containing NNOS donor sets.

Complexation of ligands containing an N3S donor set has been affected with [99mTc]. These are part of a ligand series of analogous structures which exhibit similar chemistry and potentially interesting biology. The complexes which have been characterized with [99Tc] as [TcOL] are neutral and lipophilic and their biological behaviour has been assessed in rats. After HPLC purification of the no-carrier added preparation, brain uptake of the tracers was greater than 1% at 15 min p.i. Muscle activity was significant with slow blood clearance.

Radiolabeling with technetium-99m to study high-capacity and low-capacity biochemical systems.

After a brief review of the history of the development of technetium-99m-labeled radiopharmaceuticals, the use of technetium chelates in high-capacity systems is discussed. The latter are used in the study of five organ systems, the kidneys, liver, bone, brain, and heart. The chemical characterization of 99mTc complexes is also reviewed, followed by discussion of the various approaches to the labeling of proteins with direct labeling, the preformed chelate approach, and the antibody chelator conjugate approach. Thereafter, the labeling of biochemicals with 99mTc for use with easily saturated sites, e.g., receptors and enzymes, is considered. Finally, attention is given to factors that affect the preparation of high specific activity, high affinity 99mTc-labeled biochemicals.

Measurement of myocardium at risk and salvage in myocardial infarction with ST-segment depression.

A patient with symptoms of acute myocardial infarction but ST-segment depression rather than elevation constitutes a clinical dilemma for which few guidelines exist. Herein we describe such a patient, in whom serial tomographic imaging with a new radiopharmaceutical agent, technetium-99m sestamibi, was useful in demonstrating a large area of myocardium at risk and subsequent substantial benefit from acute reperfusion therapy. Because this perfusion agent washes out slowly from the myocardium, imaging can be delayed for several hours; thus, acute reperfusion therapy can be performed without delay. Subsequent imaging, however, will reflect myocardial perfusion at the time of administration of the radionuclide. Additional studies with this agent may be valuable in identifying those patients with ST-segment depression who will benefit from acute reperfusion therapy.

Disappearance of soft tissue 99mTc diphosphonate activity following radiation therapy.

The appearance of soft tissue concentrations of technitium labeled phosphate compounds has been observed in a variety of pathological conditions. The mechanism and pathophysiology of this phenomenon remain unclear.Seven new patients diagnosed with lung cancer received (99m)Tc diphosphonate bone scans during the period of January 1975 to November 1975. Three of the seven subjects showed a significant accumulation of isotope in the region of the chest infiltrate at the time of diagnosis. Upon repeat technitium bone scans six months post radiation, two patients showed a sharp reduction in the soft tissue activity previously observed. The material is presented and a possible explanation of this phenomenon is offered.

Dynamic hepatobiliary SPECT: a method for tomography of a changing radioactivity distribution.

Dynamic single photon emission computed tomography (SPECT) of a changing radioactivity distribution can be performed with a rotating scintillation camera by acquiring several 360 degree studies and generating a new 360 degree study for which the acquisition time for every image is shifted to a single selected time. Following suppression of gallbladder filling with a fatty meal, dynamic SPECT of the hepatobiliary system was carried out by acquiring two successive studies with each head of a dual-headed camera within a 35-min period following injection of technetium-99m DISIDA. Each of the four acquired studies consisted of 60 images acquired at 6 degree intervals, spanning 360 degrees. Time-shifted studies, each consisting of 60 images over 360 degrees, were generated for times 9, 17, and 26 min postinjection. Transverse, sagittal, and coronal images were generated for each study. These images were artifact-free and demonstrated physiologic shifting of the radioactivity distribution over time. When gallbladder filling was not suppressed gross artifacts were obtained. This procedure permits examination of regional liver function and provides improved visualization of the biliary tree.

Congenital renal anomaly: evaluation with 99mTc-dimercaptosuccinic acid renal scintigraphy.

Technetium 99m-2,3, dimercaptosuccinic acid (99mTc-DMSA) preferentially accumulates in the renal cortex, demonstrating functioning cortical mass. We used 99mTc-DMSA renal scintigraphy in ten patients with horseshoe kidneys and five patients with unilateral fused kidneys. The results show that 99mTc-DMSA renal scintigraphy reliably establishes the diagnosis of horseshoe kidney and clearly shows the isthmus, which is very essential for proper management. The technique also aids in the definitive assessment of separate kidney function and of total radionuclide uptake is possible using 99mTc-DMSA scintigraphy.

Synthesis and biodistribution of six novel 99mTc complexes of 2-hydroxybenzaldehyde-amino acid Schiff bases.

For the purpose of developing novel diagnostic pharmaceuticals of 99mTc-labeled small-size complexes, six novel complexes of 99mTc-2-hydroxybenzaldehyde-amino acid Schiff bases were designed and synthesized, and their biodistributions in mice were investigated. All the compounds were obtained in radiochemical yields higher than 90% at optimal conditions, and poor uptakes in muscle, brain, heart and tumor were commonly observed with rapid blood clearance. Potentiality was revealed of good kidney imaging by 2-hydroxybenzaldehyde-alanine (L2) complex within 40 min post-injection. Good bone uptake of 2-hydroxybenzaldehyde-histidine (L4) and 2-hydroxybenzaldehyde-aspartic acid (L5) complexes, high spleen accumulation of 2-hydroxybenzaldehyde-glycine (L1) and 2-hydroxybenzaldehyde-cysteine (L3) complexes, and non-specific biodistribution of 2-hydroxybenzaldehyde-glutamic acid (L6) complex were demonstrated.

The role of technetium-99m pentavalent DMSA in the management of patients with medullary carcinoma of the thyroid.

In order to assess the role of 99Tcm pentavalent dimercaptosuccinic acid (99Tcm (V)DMSA) scanning in the management of patients with medullary carcinoma of the thyroid, we imaged 10 patients with histologically proven disease. Nine of the 10 patients were scanned after removal of the primary tumour, but with symptomatic or biochemical evidence of recurrence. One patient was imaged prior to thyroidectomy. In eight of the 10 patients 99Tcm(V)DMSA successfully identified tumour deposits, and it has been shown in this study to be a cheap, convenient radiopharmaceutical for studying this group of patients, producing high-quality images with low radiation doses, and contributing significantly to patient management.

Preparation of technetium-99-DMS renal complex in solution and its chemical and biological characterization.

99Tc-DMS solution was prepared by double isotope labelling, purified by molecular sieving through Sepharose 2B column, and checked by chromatographic and spectrophotometric methods, as well as by assessment of its biodistribution in rats. The preparations, obtained in a series of nine experiments, showed high renal uptake (30-40% of injected dose per organ) and could be used for further investigations of Tc-DMS biochemical behaviour in kidney tissue at subcellular level. Radioactive concentrations of purified 99Tc-DNS preparations were 37-74 kBq/ml and absorption maxima at 412-425 nm (yellow complex). By application of 99mTc, 99Tc-DMS preparation in rats, good renal scans have been obtained.

Imaging of infection and inflammation with an improved 99mTc-labeled LTB4 antagonist.

UNLABELLED: Studies have demonstrated that the bivalent (111)In-labeled leukotriene B4 (LTB4) antagonist DPC11870 reveals infectious and inflammatory lesions in various rabbit models. The radioactive tracer accumulates quickly at the site of infection and clears rapidly from the circulation, resulting in high-quality images. In this study, 2 new hydrazinonicotinamide (HYNIC)-conjugated compounds that are structurally related to DPC11870 were studied to further improve image quality. METHODS: A bivalent HYNIC-conjugated LTB4 antagonist (MB81) and a monovalent one (MB88) were labeled with (99m)Tc. The radiolabeled compounds were intravenously injected into New Zealand White rabbits with E. coli infection in the left thigh muscle. The imaging characteristics of both compounds were compared with those of the bivalent (111)In-labeled LTB4 antagonist. RESULTS: Both (99m)Tc-labeled LTB4 antagonists revealed the abscess from 2 h after injection onward. Abscess uptake at 8 h after injection was similar for both compounds (0.22 +/- 0.08 percentage injected dose per gram [%ID/g] and 0.36 +/- 0.13%ID/g for the bivalent and monovalent compounds, respectively). However, visualization of the abscess and the quality of the images were better after injection of MB88 than after injection of either of the bivalent LTB4 antagonists. The excellent delineation of the abscess by MB88 was mainly due to the more rapid clearance of this compound from nontarget organs. CONCLUSION: The (99m)Tc-labeled HYNIC conjugated LTB4 antagonists MB88 and MB81 revealed infectious foci in rabbits within a few hours after injection. Imaging characteristics of monovalent (99m)Tc-MB88 were superior to those of the bivalent LTB4 antagonists DPC11870 and MB81. Therefore, of the 3 LTB4 antagonists, the monovalent LTB4 antagonist MB88 is the most potent and promising agent for visualizing and evaluating infection and inflammation in patients.

Utility of technetium-99m-sestamibi to assess osseous tumor spread.

We report the staging results and the surgical outcome of a male patient with squamous-cell carcinoma in the floor of the mouth and a bone SPECT scan suggestive of local tumor infiltration of the mandible. Additional 99mTc-sestamibi SPECT imaging of the primary tumor and superimposing of both studies excluded osseous tumor spread and less extensive surgery was performed. Pathohistological examination confirmed the scintigraphic results and indicated a nonspecific periostal reaction as the cause of the positive bone scan. Nevertheless, a high-resolution camera and careful superimposition of both studies is mandatory if the differential diagnosis of an osseous tumor spread of a malignant tumor in the floor of the mouth and possibly less extensive surgery is at stake.

Ester-modified 99mTcO[SN(R)S/S] mixed ligand complexes: synthesis and preliminary evaluation.

Two novel 99mTc-(SNS/S) complexes: a mono-ester compound carrying an ethyl ester group on the tridentate ligand, 99mTcO[C(2)H(5)OOCCH(2)N(CH(2)CH(2)S)(2)][SC(6)H(4)CH(3)], 3, and a diester compound, carrying a second ethyl ester group on the monodentate ligand, 99mTcO[C(2)H(5)OOCCH(2)N(CH(2)CH(2)S)(2)][SC(6)H(4)COOC(2)H(5)], 4, were synthesized. The corresponding oxorhenium(V) complexes, 1 and 2 were also synthesized. Enzymatic hydrolysis demonstrated that 3 remains intact after 10 min incubation while 4 is totally converted to an unidentified hydrophilic complex. Tissue distribution data in mice revealed that both complexes, 3 and 4, exhibit significant initial brain uptake (1.42 and 1.01% of injected dose at 5 minutes post injection respectively) and fast blood clearance.

Synthesis and characterization of thiol containing furoxan derivatives as coligands for the preparation of potential bioreductive radiopharmaceuticals.

The synthesis and characterization of thiol-containing 1,2,5-oxadiazole N-oxide (TONO) derivatives and their use as monodentate coligands for the preparation of (99m)Tc complexes is presented. 3-Mercaptomethyl-4-phenyl-1,2,5-oxadiazol N(2)-oxide and 3-(4-mercaptophenylmethylidenhydrazinocarbonyloxymethyl)-4-phenyl-1,2,5-oxadiazol N(2)-oxide were successfully synthesized and combined with the tridentate ligand N,N-bis(2-mercaptoethyl)-N',N'-diethylethylenediamine (BMEDA) to prepare "3+1 mixed ligand" technetium complexes. The( 99m)Tc complexes were obtained in high yield and radiochemical purity using low concentration of ligand and coligand. An alternative procedure using a xantate and a disulphide precursor of 3-mercaptomethyl-4-phenyl-1,2,5-oxadiazol N(2)-oxide yielded the same complex. Biological evaluation of the potentiality of the( 99m)Tc complexes as bioreductive radiopharmaceuticals was performed in normal CD1 mice and in mice bearing induced sarcoma. Tumour uptake was moderate but tumour/soft tissue ratio was favourable. Although these results are encouraging, further development is still necessary in order to achieve higher tumour uptake and lower gastrointestinal activity.
