Reversal of left-right asymmetry: a situs inversus mutation.

A recessive mutation was identified in a family of transgenic mice that resulted in a reversal of left-right polarity (situs inversus) in 100 percent of the homozygous transgenic mice tested. Sequences that flanked the transgenic integration site were cloned and mapped to mouse chromosome 4, between the Tsha and Hxb loci. During early embryonic development, the direction of postimplantation turning, one of the earliest manifestations of left-right asymmetry, was reversed in homozygous transgenic embryos. This insertional mutation identifies a gene that controls embryonic turning and visceral left-right polarity.

Discordant atrioventricular connection and concordant ventriculoarterial connection in situs inversus: isolated ventricular noninversion.

A case of discordant atrioventricular (AV) connection and concordant ventriculoarterial (VA) connection (with aortomitral fibrous continuity and tricuspid pulmonary discontinuity) in situs inversus (I, D, I) is reported because of its rarity. This segmental combination with abnormally related great arteries in the absence of anomalies of venous connections and/or splenic anomalies is extremely rare.

Electrophysiologic delineation of the specialized atrioventricular conduction system in two patients with corrected transposition of the great arteries in situs inversus (I,D,D).

Electrophysiologic delineation of the atrioventricular conduction system at surgery is described in two patients with corrected transpostion of the great arteries in situs inversus. Intra-atrial electrograms were recorded in one patient from sites immediately adjacent to the coronary sinus located in the left-sided right atrium. The intraventricular portion of the atrioventricular conduction system was identified in both patients along the posterior and inferior margin of the ventricular septal defect, in contrast to the superior and anterior location found in corrected transposition of the great arteries in situs solitus. In contrast to the superior and anterior location found in corrected transposition of the great arteries in situs solitus. The course of the conduction system in the hearts of these two patients and a possible relationship to the cardiac loop and dual origin of the atrioventricular node is discussed. These cases illustrate the usefulness of segmental diagnosis of congenital heart disease and of electrophysiologic identification of the specialized atrioventricular conduction system at surgery.

Double-inlet and double-outlet left ventricle in situs inversus.

Double-outlet left ventricle is a rare congenital cardiac malformation that has been traditionally difficult to diagnose accurately. We report a unique case of situs inversus totalis, L-loop, double-inlet left ventricle and double-outlet left ventricle with pulmonary stenosis, diagnosed mainly by transesophageal echocardiography and magnetic resonance imaging.

Laparoscopic cholecystectomy for acute cholecystitis in patient with situs viscerum inversus.

Situs viscerum inversus is a rare condition with perfect mirror image organs position. Despite the well-described existence of this condition, the implication for the treating of emergency pathology can be important following to the difficult interpretation of signs and symptoms. We report a case of acute cholecystitis in patients with known dextrocardia who was successfully treated by laparoscopic cholecystectomy. The diagnostic problems and surgical technique pitfalls are discussed.

Situs inversus donor heart successfully transplanted.

Orthotopic heart transplantation was successfully performed in a 62-year-old man by use of a heart harvested from a donor with situs inversus. This procedure is a reasonable alternative when no other donor is available. The technique of this operation is described here. A month later the patient was discharged in good condition and is still doing well 1 year after the transplantation.

[Anatomo-surgical aspects of the liver in situs inversus].

The hepatic veins and the intra-parenchymal branches of the portal vein and of the hepatic artery were dissected in two isolated livers form subjects with situs inversus. As a general observation, the intra-hepatic distribution of the portal vein in these cases represented a "looking-glass image" of the normal liver. The intra-hepatic branches differ from the normal by the presence of a large interarterial anastomotic canal in one of the cases. In the other case branches were found, of the left hepatic artery, for the upper sub-segments of the right lobe, that were irrigated from the venous standpoint, by the right branch of the portal vein.

Bilateral renal dysplasia, pancreatic fibrosis, intrahepatic biliary dysgenesis, and situs inversus totalis in a boy.

This is a case report of a Japanese newborn infant showing bilateral renal dysplasia, severe pancreatic fibrosis simulating cystic fibrosis, intrahepatic biliary dysgenesis, and situs inversus totalis. The chromosomal analysis was normal. A review of the literature showed that there was another case exactly the same. These two rare cases had no difinite evidence of blood relationship, but occurred in the same town in a remote area.

Situs inversus abdominis in association with duodenal obstruction and internal hernia.

Persistent bile-stained vomiting in a neonate led to a radiologic workup that revealed the existence of a situs inversus and a partial duodenal obstruction. A duodenal diaphragm with a central aperture was diagnosed. Surgery confirmed the preoperative diagnosis. In addition, an annular pancreas with an anterior gap and a right parietocolic internal hernia containing all the small bowel were discovered. Embryologic aspects, therapeutic options, and the literature were discussed.

Catheter ablation of atrioventricular nodal reentrant tachycardia in a patient with complete situs inversus, atrial septal defect and "inverse" persistent left superior vena cava.

We report a case of successful radiofrequency ablation for atypical atrioventricular nodal reentrant tachycardia in a patient with complete situs inversus, secundum atrial septal defect and an "inverse" persistent left superior vena cava. The procedure was performed under the monoplane fluoroscopic guidance using the inferior approach.

Familial situs inversus and congenital heart defects.

The midline is a major determinant in the lateral symmetry of the embryo and represents a developmental field which, if defective, may lead to an abnormal situs determination. A family in which four out of seven children had situs inversus and/or congenital heart defects is presented. This report confirms the autosomal recessive inheritance of this developmental field defect.

[Abdominal situs inversus: report of case].

Changes in the normal anatomy of the biliary tract cause intra-operative injuries of the bile duct. Cholecystectomy, in those cases, must be done by an experienced group. In situs inversus Abdominalis we must consider this pathology as an abnormal presentation of the biliary tract.
