Otitis media/interna and suppurative meningoencephalomyelitis associated with Listeria monocytogenes infection in a llama.

Listeria monocytogenes was found to be the cause of fatal suppurative meningoencephalomyelitis in a 3.5-month-old cria. The cria initially had clinical signs of unilateral peripheral vestibular disease, but on the following day, the cria developed progressive signs of encephalitis. Treatment with antibiotics, flunixin meglumine, and anticonvulsant drugs failed to stop progression of the disease, and the cria was euthanatized. Post-mortem examination revealed otitis media-interna and diffuse suppurative meningoencephalomyelitis. Listeria monocytogenes was isolated from CSF and brain tissue.

Fatal necrotizing mediastinitis secondary to acute suppurative parotitis.

A unique case of fatal necrotizing mediastinitis secondary to acute suppurative parotitis is reported. The infection was the result of synergistic necrotizing cellulitis caused by mixed aerobic and anaerobic bacteria. To the best of the authors' knowledge, this is the first reported case of necrotizing mediastinitis resulting from a descending acute suppurative parotitis. Acute parotitis should be included in the broad spectrum of infections resulting from synergism between aerobic and anaerobic bacteria in which anaerobes indigenous to the oral cavity predominate.

Chronic suppurative otitis media in Sudanese patients.

A retrospective study was carried out to show the pattern of chronic suppurative otitis media in Sudanese. Patients with chronic suppurative otitis media were 43% of all patients with ear problem. The majority of patients affected by the disease were between 11-30 years with even distribution. Males were affected more than females. The duration of symptoms varied between 0-20 years but most of the patients had the disease for over six years. Pseudomonas aeruginosa, pseudomonas pyocyaneous and B. Proteus were the commonest organism isolated in cultures. The predominant presenting symptoms were otorrhoea followed by deafness. The rate of complications was relatively high. The majority of patients were improved after conservative medical treatment, by repeated aural toilet and the use of the appropriate antibiotic.

[Experimental bases of directed transport of antibiotics in suppurative-inflammatory diseases of the liver and biliary tract].

The possibility of use and effectiveness of directed transport of antibiotics in erythrocyte carriers in the treatment of suppurative-inflammatory diseases of biliary ducts has been studied on dogs. It has been found that erythrocyte carriers were resistant to desorption of antibiotics in the blood circulation. Pharmacokinetic investigation has shown the more prolonged collection of antibiotics in hepatic tissue in comparison with traditional intravenous route. The results of clinical, pharmacokinetic and morphological investigation have shown more effectiveness of directed transport of antibiotics to the liver in autologous erythrocyte carriers in treatment of suppurative diseases of biliary ducts in comparison with traditional methods of infusion of antibiotics. For reasons given it is recommended to use this method in clinical practice.

[Conservative treatment of suppurative hematogenic arthritis of the hip joint].

Authors collected from the 82 suppurations of the hip, during the 12 years activity of their Department, the cases treated from some reason conservatively. They had 21 such cases, 17 of them males. The average age of the patients was 38 years and the haematogenous way of the infection could be proven in 15 cases. The scheme of the treatment: very long.lasting giving of broad spectra antibiotics, temporary immobilization with extension, long-lasting relief of weight bearing, if necessary hip spica. The results are assessed critically. In this the tendency of recurrence of the disease, and that the longest follow-up was 10 years, in average however only 3.6 years, play a role. At closing of the material 12 patients stated to be free of complaints, the hip movements were completely retained in 4.9 cases are demonstrated in details and the "strategic" questions of the conservative treatment of the suppurations of the hip are discussed.

Bacteriology and drug sensitivity of chronic suppurative otitis media at a central hospital in Zimbabwe.

A retrospective study was done on 173 cases of chronic suppurative otitis media at Harare Central Hospital. Using conventional pus swabs from the discharging ears only aerobic organisms were isolated in 155 cases. These were, in order of frequency, Staphylococcus aureus, Pseudomonas aeruginosa, Lactose fermenting, Coliforms and Proteus. There were no anaerobes reported, unlike in other studies. Mixed infections were present in 48 percent of the cases. Penicillin G, Ampicillin, Tetracycline Sulphamethoxazole and trimethoprin which are commonly used when culture and sensitivity results are not available, were found to be less useful. Gentamyoin, Chloramphenical, Clindamyoin and erythromycin were more useful. More research needs to be done on the role of topical drugs in chronic suppurative otitis media.

[The CO2 laser in the surgical treatment of suppurative processes in the lungs].

Operations were carried out on 58 patients with suppurative processes in the lungs. The authors worked out a method of thoracotomy by means of a laser with layer-by-layer division of the muscles during compression with special clamps, as well as a method of economical resection consisting in cutting the pulmonary tissue with a laser scalpel, electrocoagulation of small vessels, and stitching through the large vessels and bronchi with a suturing instrument. The use of a CO2 laser in operations for suppurative processes in the lungs makes it possible to conduct economical operations and reduce the number of complications.

[Non-spore-forming anaerobes in suppurative-inflammatory diseases of the ORL organs].

Nonspore-forming anaerobic microorganisms were isolated in 83.3% of patients with suppurative-inflammatory diseases (SID) of ENT organs. It was found that so-called sterile inoculations of the pus from the inflammation focus were produced by obligate anaerobes and primarily by bacteroids and gram-positive anaerobic cocci present. The application of anaerobic bacteriological procedures for examining the pathological samples from these patients enlarged significantly the spectrum of isolated microorganisms at the expense of obligate anaerobes. They formed predominant microorganisms of the suppurative-inflammatory focus causing SID due to microbial synergism. It was demonstrated that gas-fluid chromatography can be successfully used to rapidly identify anaerobic infection. Effective drug therapy of SID of ENT organs should address anaerobic microflora and should include agents with antianaerobic action.

A case report of acute obstructive suppurative cholangitis in a non-0-1 Vibrio cholerae biliary carrier.

A case report is presented of a 73-year-old male who was seen with fever, jaundice, abdominal pain and central nervous system depression. He failed to respond to intensive antibiotic therapy, and subsequently acute obstructive suppurative cholangitis fully developed. Upon laparotomy, the patient's gallbladder was found to be enlarged with the bile from the gallbladder and bile duct itself containing a high pus content. Its cultured organism revealed non-0-1 Vibrio cholerae. To our knowledge, no prior case of acute obstructive suppurative cholangitis in a non-0-1 Vibrio Cholerae biliary carrier has been reported in Japan.

[Interstitial electrophoresis in acute pleural and lung suppuration].

Different variants of interstitial electrophoresis elaborated in the hospital were used with the aim of raising the effectiveness of treatment of acute abscess of the lungs, of abscessing pneumonia and acute empyema of the chest. The principle of electroelimination of medicinal preparations from the vessels or directly from the suppurative cavity into the surrounding tissue is the basis of interstitial electrophoresis. As the result of experimental and clinical observations the following variants of interstitial electrophoresis are proposed: intrapulmonary, endobronchial, cavitary, intracavitary and combined. The use of the proposed method of treatment in acute pleuropulmonary suppuration permitted to achieve substantial raising of the effectiveness of the used complex therapy. The method of interstitial electrophoresis is recommended for wide use in practical pulmonary and thoracic surgery.

[A parallel comparative double blind study of cefixime with cefaclor in the treatment of acute suppurative otitis media in children].

A double blind study was carried out to compare the efficacy and safety of cefixime (CFIX), a new oral cephem with cefaclor (CCL) in the treatment of 245 children weighing 10 approximately 30 kg, with acute suppurative otitis media. The daily dosages of CFIX and CCL were 3 approximately 6 mg/kg in 2 divided portions, and 20 approximately 40 mg/kg in 3 divided portions, respectively, and the drugs were administered for 7 days. The results obtained in this study are summarized as follows. Analyzed subjects were 211 patients (CFIX group: 108 patients, CCL group: 103 patients) for clinical efficacy. Efficacy rates judged by the doctor in charge were 88.9% and 83.5% in CFIX and CCL group, respectively, without significant difference between the 2 groups. Similar results were also obtained by the committee. When clinical effects were classified by clinical isolates, the efficacy rates against monomicrobial infections with Gram-negative bacteria were judged by the doctor in charge to be 100% in the CFIX group and 84.6% in the CCL group. Thus CFIX was judged to be significantly superior to CCL (P less than 0.05). The overall eradication rates of bacteria were 97.1% in the CFIX group and 90.3% in the CCL group. The eradication rate of CFIX was significantly superior to that of CCL (P less than 0.05). When improvements of individual symptoms were evaluated, regarding redness of the tympanic membrane and the tympanic cavity on the 3rd day of dosing, CFIX group (improvement in 84.1% of the cases) was significantly superior (P less than 0.05) to the CCL group (67.6%). Regarding purulent secretion on the 7th day of dosing, the CFIX group (improvement in 98.1% of the cases) was also significantly superior (P less than 0.05) to the CCL group (91.3%). Two hundred thirty eight patients were analyzed for side effects (CFIX group: 120 patients, CCL group: 118 patients). The incidence rates of side effects were 0.8% (1/120) in the CFIX group and 1.7% (2/118) in the CCL group, and there was no significant difference between the 2 drugs. From the above results, it is concluded that CFIX is a useful oral antibiotic in the treatment of acute suppurative otitis media in children. Furthermore, CFIX is expected to be equal or superior to CCL in clinical effects.

Suppurative chondritis: its incidence, prevention, and treatment in burn patients.

A retrospective study of 317 cases of suppurative otochondritis occurring in a population of 4794 burned patients successively admitted to one institution between 1967 and 1984 is presented. During the study interval, the incidence of the disease decreased from 20 percent to less than 3 percent in patients with burns of one or both ears. The bacterial species associated with chondritis, principally Pseudomonas aeruginosa and Staphylococcus species, did not change. Patients admitted later than the second postburn day exhibited a significantly greater incidence of chondritis. The crucial factors in prevention of this complication appear to be avoidance of pressure on burned ears and topical chemotherapeutic control of local Pseudomonas infection. Systemic antibiotic prophylaxis did not appear to influence the incidence in the studied population. Several approaches to clinical management of suppurative chondritis are reviewed.

[Focalized irrigation-lavage and sequential use of a new antiseptic by local and general route. Preliminary report apropos of 2 cases of suppurative pancreatic necrosis].

In view of the severe course seen in the presence of any suppurated pancreatic necrosis, it was felt to be of value to treat two patients by the adjuvant use of a new antiseptic tauroline, administered locally and, where appropriate, systemically. After surgical excision of pancreatic and peripancreatic necrotic tissue, the authors prefer to avoid active suction drainage of the residual pocket, replacing it by continuous and prolonged post-operative irrigation-lavage, together with the sequential, twice-daily use of a solution of 2 p. 100 tauroline. In view of the presence of a Gram negative septicemia (organisms identical to those of the suppurated area), the same substance was given by intravenous infusion in a 0.5 p. 100 solution, this being the only systemic, anti-infectious therapy. The effectiveness, good tolerance and originality of this new substance, active against Gram negative, Gram positive, aerobic and anaerobic organisms as well as yeast, have led the authors to immediately report their initial experience with its use.

Isolation of Vibrio fluvialis, and unusual pathogen in acute suppurative cholangitis.

We report a case of acute suppurative cholangitis, from which were isolated Enterobacter aerogenes and Vibrio fluvialis. There have been no previous reports of the isolation of V. fluvialis in this disease. It is usually found in seawater and various seafoods and may cause acute diarrhea, but this is the first report in which it has been associated with acute suppurative cholangitis.

[Clinical evaluation of the TMS-19-Q.GC tablet on superficial suppurative disease. A comparative double blind study with midecamycin].

Clinical efficacy and safety of TMS-19-Q.GC tablet (TMS), a new macrolide preparation, were compared with those of midecamycin (MDM) in superficial suppurative skin and soft tissue infections. The study was made by the double-blind controlled trial at the dosage of daily 600 mg in TMS group and 1,200 mg in MDM group. Total 218 cases (106 in TMS, 112 in MDM) were analyzed and the final global improvement rating were 82.1% in TMS and 83.9% in MDM. The clinical effectiveness of TMS was favorable and significantly different from MDM in the aged patients (greater than or equal to 60 years old) and the patients infected with susceptible strains (MIC less than or equal to 3.13) of Staphylococcus aureus. TMS is prepared with a specific formulation to make the absorption easier in the patients with lower acidity of gastric juice, and the favorable effect of TMS is considered to be a contribution of the devise in older patients. Slight adverse reactions were observed at 5.0% (6 cases) in TMS and 2.4% (3 cases) in MDM. In conclusion, TMS at the daily half dose of MDM is as effective as MDM in superficial suppurative skin and soft tissue infections.

Recognition and management of acute suppurative parotitis.

Acute suppurative parotitis (ASP) is a rare infectious process occurring after surgery or in debilitated patients. This case describes a 41-year-old man who was hospitalized initially for acute bacterial endocarditis. Late in the hospital course his recovery was complicated by the development of ASP. Subsequent cultures of blood and parotid-fluid samples grew Pseudomonas aeruginosa. Despite early empiric antibiotic therapy followed by more specific therapy with an aminoglycoside, the patient continued to experience severe pain, swelling, and facial-nerve palsy. Surgical incision and drainage of the parotid gland was accomplished on the ninth day after onset of ASP, and the patient experienced a rapid resolution of the disease process. Acute suppurative parotitis was originally attributed to infection with gram-positive organisms, primarily Staphylococcus aureus and Streptococcus species. More recently, reports of ASP caused by gram-negative and anaerobic bacteria have been published. This review describes the etiology, pathogenesis, microbiology, and clinical features of ASP. Nonpharmacologic, pharmacologic, and surgical management of the disease is discussed. Although ASP is a rare occurrence in the hospital setting, early recognition and appropriate management are important in avoiding serious morbidity and mortality.

[Surgical indications in treatment of chronic bronchial suppurations].

The Authors report their experience (22 operations) concerning chronic bronchial suppurations of surgical interest. During the period considered (1964 - April 1982), among many pathological processes capable of conditioning a chronic bronchial suppuration, it has been observed a progressive decrease of "primitive" bronchiectasis and a percentage increase of "secondary" ones. Surgical treatment, unquestionably required in cases of cystic malformation or secondary obstruction, is now restricted to "primitive" cases with well-limited bronchiectasis or with complications (hemoptysis). Surgical therapy performed in these cases appears to grant good results both immediately (O% surgical mortality) and in the future.

[Suppurative thyroiditis and epithelioma of the piriform sinus].

The authors report a case of suppurative thyroiditis revealing cancer of the piriform sinus. The course ran two distinct phases. Initially, symptoms resolved under methylprednisolone and ampicillin given with a diagnosis of acute thyroiditis. After corticosteroids were discontinued, a tumefaction in the thyroid area recurred, with clinical features indicating centesis that removed 30 cc of pus ORL examination connected this suppurative thyroiditis to an epithelioma of a piriform sinus.
