Determination of beta2-microglobulin in biological samples using an immunoenzymometric assay (chemiluminescence detection) or an immunoturbidimetric assay: comparison with a radioimmunoassay.

Monitoring beta2-microglobulin (beta2M) in biological fluids has gained considerable interest in pathologies such as haematologic malignancies, renal diseases, and chronic inflammatory diseases. Due to limitations of the RIA in the routine laboratory, we measure beta2M with non-isotopic methods. 189 patients suffering from myeloma (n=66), end stage renal failure (n=54) or inflammation (n=69) were included in this study. beta2M was determined in serum, urine and dialysate using an immunoenzymometric assay with chemiluminescence detection [Immulite Diagnostic Products Corporation (DPC), La Garenne Colombes, France] and an immunoturbidimetric assay (Olympus, Rungis, France). The data were compared with a radioimmunoassay (Immunotech, Marseille, France) taken as a reference. Using serum samples, the immunoenzymometric assay with chemiluminescence detection and the immunoturbidimetric assay have reliable analytical performances. Values obtained with serum samples are highly correlated with the radioimmunoassay (DPC/RIA r2=0.84; Olympus/RIA r2=0.94) whatever the type of pathology; however an over-estimation which could be related to cross reactivity with beta2M fragments was observed with the RIA method as suggested by crossover calibration and recovery studies. Values obtained with urinary samples (n=96) are closely related to those obtained with the RIA (DPC/RIA r2 = 0.98; Olympus/RIA: r2=0.99). Despite the low levels observed in dialysate (n=57) good correlations were observed between Olympus vs DPC (r2=0.85). By contrast, the two non-isotope methods are poorly related with the RIA method (DPC vs RIA r2=0.47 and Olympus vs RIA r2=0.54). In conclusion, the immunoenzymometric assay with chemiluminescence detection or the immunoturbidimetric assay could be used in the routine laboratory in order to determine beta2M in plasma, urine and dialysate.

[The pathogenetic and prognostic significance of the level of class-E immunoglobulins in erysipelas].

The authors determined the content of total immunoglobulins of the E class in the blood plasma: native (IgE) and exhausted by a streptococcal allergen in the dynamics of the disease in 145 patients with erysipelas. The level of antistreptococcal reagins was evaluated. It was established that at the onset of erysipelas the concentration of IgE and a-IgE showed a short-term increase in 52 +/- 4% and 87 +/- 3% of patients correspondingly. During convalescence the level of these antibodies did not differ from that of the normal excluding high risk patients for recurrence. The participation of IgE-mediated type of allergic reactions in the pathogenesis of erysipelas is suggested. It is recommended to use the content of IgE and a-IgE for the prognosis of immediate and remote recurrences of erysipelas.

[Blood IgE level in children with chronic viral diseases of the liver].

PRIST was used to examine the time course of changes in the content of total blood IgE in 128 children aged 3 to 15 years suffering from chronic virus hepatitis and liver cirrhosis. A well-defined relationship was discovered between the above parameter and allergic background as well as the process activity in the liver, pointing to the capacity of that class immunoglobulins to reflect the intensity of not only allergic reactions but also of immunopathological processes lying at the basis of chronic active hepatic diseases.

[Role of the NS gene in regulating the synthesis of RNA-segments of the influenza A virus].

To find the role of any influenza virus gene in regulation of the RNA-segments replication the transfer of ts-mutants to nonpermissive temperature on the late step of infection has been used (shift-up). The mutants having impaired the NS or NP-genes have been obtained and studied. The transfer of mutants to partially nonpermissive conditions (when the amount of replication is decreased, but it still continues) results in the distinct return to the early mode of replication in ts-mutant with the mutation in NS-gene. This suggests the NS-gene role in replication of viral RNA-segments, in particular, in the switch from the "early" stage of replication to the "late" one. In NP-gene mutant only the decrease in general replication takes place without the shift to "early" replication mode.

[IgE concentration in healthy children of various age groups].

Serum IgE was estimated in 181 healthy children aged 2 months-17 years. The concentration of IgE was measured by radioimmunosorbent technique (RIST). To eliminate serum interference the IgE levels were calculated using the correction factors: 0,96 and 0,91 (for the sera diluted respectively 1:10 and 1:5). Statistical calculations were made after logarithmic transformation of the particular results. On the base of the mean geometric values linear and parabolic regression lines have been made. On the grounds of the received regression equations it has been found that parabolic variant of the changes of the IgE concentration with respect to age, shows more accurately the real state. The index of the parabolic correlation (r = 0,98) shows a close and positive relations between IgE concentration and the age of children. The lowest concentrations of IgE were found in the youngest group of infants and then the level increased during childhood. The parabolic regression line and 95% confidence limits for the geometric mean values ar inserted in the paper.

[Immunologic studies of secretory otitis].

Evaluation of total serum proteins and RIST immunoelectrophoresis of immunoglobulins IgG, IgA and IgM were carried out on a group of 35 children of both sexes suffering from secretory otitis. The transudate aspirated from the middle ear was examined only by immunoelectrophoresis. The values for albumin and gammaglobulins were decreased for all the patients. The values for RIST were significantly lower than those where an allergic cause was present. The values for immunoglobulins in transudate were also significantly lower than normal.

[Simple and rapid heterologous method of radioimmunochemical determination of luteinizing hormone in the blood serum of rats. 1. Basic principles of the method].

Possibilities of creation of a radioimmunochemical method (RICM) of determination of the luteinising hormone (LH) were studied. The method is based on the application in the binding system of a water-insoluble immunosorbent obtained by copolymerization of antisera with isobutyl ester of chlorocarbonic acid. The antisera were obtained by immunization of rabbits with a highly-purified LH isolated from sheep hypophyses. The method is rapid and precise, and is carried out in a single stage. Significant results of RICM in the LH determination can be obtained with the minimal two-hour incubation time. Using different amounts of the immunosorbent in the reaction mixture with the same indicator dose of I125-LH it is possible to vary the method sensitivity (from 0.05 to 5 ng/test).

Partial characterization of a rapidly released antigenic/allergenic component (Ag 5) of Aspergillus fumigatus.

An antigenic component of Aspergillus fumigatus, which was responsible for inducing an initial early antibody response in rabbits immunized with A. fumigatus germlings, has been identified and partially characterized. By immunofluorescent antibody and ELISA techniques, this antigen was demonstrated to be present on the germling surface, although it was also detected in supernatants of conidia/germlings within 1 hour and was present in all shake and stationary culture-filtrate extracts. By incorporation of a monospecific antiserum to this component in an intermediate gel of the reference self-crossed radioimmunoelectrophoresis pattern for allergic bronchopulmonary aspergillosis sera, it was recognized as Ag 5 and was also demonstrated to bind specific IgE. Further immunochemical analyses have revealed that Ag 5 is relatively heat labile, does not bind to concanavalin A, and has a molecular weight of approximately 35 kd. This rapidly released antigenic/allergenic component may play an important role in the initiation of immunologic responses in patients with allergic bronchopulmonary aspergillosis.

An efficient one-step method for isolating immune complexes from whole serum using a monoclonal anti-C3g affinity immunosorbent.

A simple and efficient method of extracting complement-fixing immune complexes (IC) from whole serum and recovering them has been developed. 125I-labelled, in vitro prepared IC in whole serum were incubated with Sepharose 4B covalently linked to monoclonal anti-C3g or anti-C1q and the binding and recovery of IC was monitored by radioactivity. The anti-C3g immunosorbent bound 45% and 76% of HAGG and BSA-anti-BSA IC respectively, all of which were recovered by elution with 4M MgCl2. The anti-C1q immunosorbent only bound 14% and 12% of the same IC and only 64% were recovered by eluting with 4M MgCl2. The IC extracted by the anti-C3g immunosorbent included those capable of extraction by the anti-C1q. The anti-C3g monoclonal recognizes not only the iC3b fragment of C3 and will therefore bind IC with affinity for bovine conglutinin but also subsequent degradation products containing the C3g antigen. Its wide range of reactivity for IC plus its excellent recovery properties make it the immunosorbent of choice for isolating complement-fixing IC.

Allergy to egg yolk possibly induced by sensitization to bird serum antigens.

Hypersensitivity to ingested hen's egg yolk is described in a 65-year-old female patient. The patient's symptoms started in adult life and coincided with the acquisition of a parrot as a household pet. The bird's presence caused complaints of allergic rhinitis, while the ingestion of eggs caused urticaria and angioedema. Analysis of sequential blood serum samples revealed high levels of IgE antibodies to egg yolk and to various bird sera, but no precipitins. RAST-inhibition experiments indicated that the primary sensitization may have been to antigens from the parrot.

[The determination of IgE in human aqueous humor in a case of intraocular parasitosis (ophthalmomviasis interna) (author's transl)].

A case of intraocular myasis is here reported, as well as the difficulties of diagnosis--mostly too late--of this disease, which is rare in Europe and the northern hemisphere and which leads, almost invariably, to functional or even anatomical loss of the eye.--The determination of IgE in human aqueous humor (ah) and corresponding serum (s) by means of the ultrasensitive and quite simple PRIST method could represent a possibility of timely diagnosis, and this theoretically too in the case of other, much more frequent oculoparasitosis cases.--A retrospective study of the preoperative aqueous humor of the ophthalmomyiasis case presented here revealed an IgE concentration of 113 U/ml in the aqueous humor, with only 3 U/ml (1 U congruent to 2 ng) in the corresponding serum. A simple serum-IgES-determination would not have permitted the diagnosis--at least not in the case examined--just as little as numeration of eosinophilic granulocytes in blood and/or aqueous humor.
