Genetic influences on variation in female orgasmic function: a twin study.

Orgasmic dysfunction in females is commonly reported in the general population with little consensus on its aetiology. We performed a classical twin study to explore whether there were observable genetic influences on female orgasmic dysfunction. Adult females from the TwinsUK register were sent a confidential survey including questions on sexual problems. Complete responses to the questions on orgasmic dysfunction were obtained from 4037 women consisting of 683 monozygotic and 714 dizygotic pairs of female twins aged between 19 and 83 years. One in three women (32%) reported never or infrequently achieving orgasm during intercourse, with a corresponding figure of 21% during masturbation. A significant genetic influence was seen with an estimated heritability for difficulty reaching orgasm during intercourse of 34% (95% confidence interval 27-40%) and 45% (95% confidence interval 38-52%) for orgasm during masturbation. These results show that the wide variation in orgasmic dysfunction in females has a genetic basis and cannot be attributed solely to cultural influences. These results should stimulate further research into the biological and perhaps evolutionary processes governing female sexual function.

Attitude change in a human sexuality course that de-emphasizes small group activities.

The attitudes of second-year medical students were measured to determine if positive attitude changes could be obtained in a human sexuality course that de-emphasized small group activities. Students were given a semantic differential instrument and were asked to rate four concepts related to human sexuality: (a) my sexuality, (b) masturbation, (c) homosexuality, and (d) my role in understanding sexual problems. Significant changes in student attitudes were reported at the 0x05 level for the concept, my sexuality; at the 0x01 level for the concept, my role in understanding sexual problems; and at the 0x001 level for the concepts, homosexuality and masturbation. Females had more positive attitudes (P < 0x001), pre and post, towards homosexuality than did males. No significant differences were found between married and single students.

Klismaphilia--a physiological perspective.

Dr. Joanne Denko coined the work klismaphilia to describe the practices of some of her patients who enjoyed the use of enemas as a sexual stimulant. Since then questions occasionally appear in the professional literature asking about the relationship between enemas and sexual pleasure. This paper considers some of the physiological aspects of the human sexual apparatus that relate to anal sensitivity and explores why klismaphilia can be sexually grafifying. The paper starts with a discussion of the physiological basis for anal sensitivity and anal masturbation in both the human male and the human female. The paper then goes on to relate all this to the sexual sensations received from an enema, and discusses the similarities and differences between all these types of stimulation. Some of the psychological aspects of klismaphilia are also considered in relationship to the physiology involved. The paper concludes with a brief discussion of masked anal masturbation among the population at large. A comprehensive list of references from the literature is given to support these findings.

The incidental orgasm: the presence of clitoral knowledge and the absence of orgasm for women.

Women report anorgasmia and other difficulties achieving orgasm. One approach to alleviating this problem is to teach women about the clitoris. This assumes that women lack information about the clitoris and that knowledge about the clitoris is correlated with orgasm. Using a non-random sample of 833 undergraduate students, our study investigates both assumptions. First, we test the amount of knowledge about the clitoris, the reported sources of this knowledge, and the correlation between citing a source and actual knowledge. Second, we measure the correlation between clitoral knowledge and orgasm in both masturbation and partnered sex. Among a sample of undergraduate students, the most frequently cited sources of clitoral knowledge (school and friends) were associated with the least amount of tested knowledge. The source most likely to correlate with clitoral knowledge (self-exploration) was among the most rarely cited. Despite this, respondents correctly answered, on average, three of the five clitoral knowledge measures. Knowledge correlated significantly with the frequency of women's orgasm in masturbation but not partnered sex. Our results are discussed in light of gender inequality and a social construction of sexuality, endorsed by both men and women, that privileges men's sexual pleasure over women's, such that orgasm for women is pleasing, but ultimately incidental.

The sexual practices of adolescent virgins: genital sexual activities of high school students who have never had vaginal intercourse.

OBJECTIVES: The purpose of this study was to determine whether high school-aged virgins engage in sexual practices that can transmit sexually transmitted diseases, including the human immunodeficiency virus (HIV). METHODS: Data were collected from an anonymous self-administered survey of 2026 urban students in 9th through 12th grades. RESULTS: Forty-seven percent of adolescents were virgins (42% of male adolescents and 53% of female adolescents). Of those who were virgins, 29% and 31% reported that, during the prior year, they had engaged in heterosexual masturbation of a partner and masturbation by a partner, respectively. The corresponding rates for heterosexual fellatio with ejaculation, cunnilingus, and anal intercourse were 9%, 10%, and 1%. Homosexual sexual activities were rare. Condom use for fellatio was also rare. Level of risk of virgins' sexual activities was associated with illicit substance use and other non-sexual risk behaviors, even after demographic variables had been controlled. CONCLUSIONS: Few high school-aged virgins engaged in anal intercourse, but many engaged in other genital sexual activities. Some of these activities can transmit disease, and all can indicate a need for counseling about sexual decision making, risk, and prevention.

Traumatic purpuric penile ulcer.

A 33-year-old man presented with a history of a penile ulcer of four days' duration. He was immediately treated with 2.4 million IU of benzathine penicillin IM and tested for syphilis, but subsequently proved non-reactive. He was already receiving doxycycline BID from another physician for this nonspecific genital ulcer and minocycline for acne. Past medical history revealed periodic flares of hidradenitis suppurativa, limited to the buttocks and inguinal region; acne, involving the face and back; and angiokeratoma of the scrotum. He had no known drug allergies, nor had he admitted to taking any other medicaments. Examination revealed an 8 x 15 mm irregularly shaped, shallow, tender ulcer over a larger purpuric base and involving part of the corona and the distal portion of the shaft. No inguinal adenopathy or buboes could be found on palpation. The patient was placed on a cream containing 3% iodochlorhydroxyquin 1% hydrocortisone. By the end of two weeks of therapy, the lesion had healed with no residual discoloration. Subsequently, the patient revealed that he masturbated daily, rubbing his penis back and forth on the bed sheet while lying prone on the bed. Eleven months later, he consulted us again for a new purpuric penile ulcer, because he had begun to masturbate in his usual fashion. The ulcer healed within several days, and there were no sequellae.

Pregnancy by insemination of cryopreserved spermatozoa from a man with retrograde ejaculation: a case report.

BACKGROUND: Owing to the prevalence of diabetes mellitus, spinal injuries and aggressive surgical treatment of cancer, the number of younger patients with retrograde ejaculation is increasing. Since medical treatment to restore antegrade ejaculation often fails, several options for accomplishing insemination by these patients, including the use of sperm-rich urine obtained after masturbation and in vitro fertilization with sperm retrieved from the seminal tract, have been reported. We used the least invasive and most inexpensive procedure in a patient/couple with this condition. CASE: A 23-year-old man suffered from retrograde ejaculation after a spinal injury. He could achieve erection and engage in sexual intercourse but seldom had an orgasm or the sensation of ejaculation. We obtained spermatozoa from urine produced after masturbation at home and froze them. We used these frozen-thawed spermatozoa for intrauterine insemination, leading to the term birth of a healthy infant. CONCLUSION: In selected patient/couples, frozen spermatozoa obtained from postmasturbation urine can be used successfully for intrauterine insemination. This minimally invasive and most inexpensive procedure should be tried before planning in vitro fertilization.

[Opinions on the subject of sex life among students of the Medical University in Gdańsk].

AIM: The aim of this study was to illustrate opinions on the subject of sex life among students of the Faculty of Medicine, Medical University of Gdańsk. METHOD: A group of 340 medical students aged 18 to 28 (195 females and 118 males) were examined with the study questionnaire from October till December 2003. The examined group was representative for the investigated population (chi squared test: p> 0.005 for the entire group and both sexes). 313 questionnaires were qualified for further analysis. The questionnaire comprised three groups of questions: questions concerning relationship and associated sexuality, questions concerning attitudes towards various aspects of sexuality and a set of questions dedicated to the role of sexology in the medical curriculum and general medical practice. This paper focuses on the question sets concerning the relationships between sex and emotions, factors influencing sex life, opinions on various issues of human sexuality i.e. masturbation, adultery, prostitution and common stereotypes of male and female sexuality. RESULTS: The examined group has a well defined and mature attitude towards their sex life. The emotional bond to the partner is essential for finding pleasure in sex. Love and faithfulness are the two most important factors that influence sex in a positive way. Successful sex life is thought to be crucial for a happy and lasting relationship. Adultery is viewed negatively, as harm done to the partner. Students perceive masturbation as something pleasant and as an alternative form of sexual activity, however there is a wide spectrum of negative attitudes towards it.

[A little-known epileptic phenomenon].

The epileptic patients were investigated in different age groups displaying hypersexual paroxysms resulting in masturbation. In adults the EEG monitoring was performed. Both in adults and children, epileptic foci were recorded in the temporal lobes. Apparently similar masturbation attacks had peculiarities typical of children vs. adults. In children, the hypersexuality developed exclusively through the automatisms; in adults the motivated brain operated in the hypersexual paroxysms.

Sexual function and practice in elderly men of lower socioeconomic status.

Normal aging plus certain prevalent diseases are believed to render many elderly men impotent. Recent studies have suggested that educated middle-class and upper-class elderly men continue sexual activity, despite erectile dysfunction, by employing alternative practices such as mutual masturbation and oral sex. Few elderly men of lower socioeconomic background have been included in these studies, however. Using physician-administered interviews, 87 men attending an urban Veterans Administration geriatric clinic were studied to determine (1) the prevalence of erectile dysfunction, and (2) the sexual practices and attitudes of this group. Of the 87 men, 28% reported complete loss of erectile function, while 31% had frequent difficulties achieving vaginal intromission. Unlike economically advantaged groups, only 29% used mutual masturbation and 16% used oral sex. Attitudes toward these practices were negative. With one exception, men unable to perform coitus ceased all heterosexual activities.

Self-perceived differences in the female orgasmic response.

This investigation was concerned with perceived differences between orgasms experienced via masturbation, petting, and sexual intercourse and the relationship of such differences, if any, to sexual satisfaction. An anonymous questionnaire was distributed to registered nurses in fifteen states concerning sexual attitudes, sexual behavior and the female sexual response. Although 76% of these respondents reported perceived differences between orgasms experienced via masturbation, petting, and sexual intercourse, no differences were found between respondent groups for either physiological or psychological sexual satisfaction. Several other variables were identified which affected perceived levels of sexual satisfaction. A clear understanding of these findings should be helpful in counseling patients with sexual problems.

Patterns of stallion sexual behaviour in the absence of mares.

In the first experiment the response of 93 adult stallions to stimulated and natural sexual stimuli was observed just before and 2 weeks after the breeding season (December and July respectively). About 72% of the stallions reacted the same before and after the breeding season (P less than 0.01) while 9% behaved similarly and 19% differently. Also, 72% of the stallions reacted with sexual arousal (mounting) to one of 3 successive simulated sexual stimuli; 25% to a dummy, an additional 44% to a gentle stallion and another 3% to a dummy smeared with mucus and urine from a mare in oestrus. In another experiment, the sexual behaviour of 4 young stallions and 3 older stallions was observed while they were in the stable. Erection was observed an average of 7.4 times in 24 h, a full erection 3.8 times and attempts to masturbate 4.1 times. Complete masturbation with ejaculation was only sporadically noted. The total time of erection averaged 38 min/24 h. The peak of erection time occurred in the morning and the low point during the night and at 05:00-08:00 h. No significant differences were found between the young and older stallions; differences occurred only between individual stallions.

Attitudes of caretakers toward the sexual behavior of mentally retarded persons.

A multidimensional questionnaire was administered to staff members at three residential facilities for retarded persons to determine their attitudes toward the actual and potential sexual behavior of retarded persons. The questionnaire covered the areas of masturbation and heterosexual and homosexual behavior. Dimensions were scaled to reflect progressively more intimate behavior so that acceptability of each response along the dimensions could be assessed. A mean of 31.2 percent of those questioned felt that no sexual behavior, not even simple physical contact, was acceptable for retarded persons. This indicates that sex-education programs for retarded persons may be met with resistance by a substantial percentage of staff. Among those staff members who found it acceptable for retarded people to engage in sexual behavior, peak acceptability occurred for heterosexual behavior. Sexual behavior in public, especially public masturbation, was considered a significant problem. More specific effects were identified, and the implications of these results for educational programs and the development of intervention procedures were discussed.

Freud's views on early female sexuality in the light of direct child observation.

Freud's writings on early female sexuality are reviewed in order to demonstrate which of his central assumptions are supported and which have been corrected by the direct observation of young children. The study of the emergence of core gender identity in little girls is a key to the modification of Freud's statements on the onset of and crucial factors in the development of femininity. Cognitive functions, learning experiences, and language are believed to be more important than Freud stressed, and penis envy and feelings of inferiority are relegated to a less universal and less necessary place in the onset of femininity. The role of the father is given different emphasis. Direct observation clarifies many aspects of masturbation or early genital self-stimulation in the young female: its onset; its feminine rather than masculine character; its early vicissitudes; its importance relative to other behavior; the impact of the discovery of anatomical difference; one special way it is affected by parental attitude; and how it contrasts with comparable behavior in the young male. Observation refutes Freud's often quoted statement that masturbation is further removed from the nature of women than of men.

Problems in female development. Comments on the analysis of an early latency-age girl.

A 6 1/2-year-old girl's clinical material relevant to the emergence of clitoral and vaginal masturbation and masturbation fantasies during analytic sessions is presented. The intense genital excitation, which she called "jeebies," often overwhelmed her ego functioning. She assumed the jeebies and the fantasies that precipitated and accompanied them would cause bad things to happen to her. External events and the nature of her object relations reinforced and confirmed her belief that being only female was unacceptable. Her penis envy, a wish to be a boy-girl, had multiple determinants. A detailed description of her evaluation and subsequent treatment reveals how her unconscious conflicts were resolved in analysis through the evolution and resolution of her transference neurosis, validating the psychoanalytic approach.

[Practical and psychological aspects of sperm collection].

The present study was made on 480 men who consulted in our laboratory for a sperm exam. Semen were collected by masturbation. Only in very few cases (3 p. 100) ejaculation could not be obtained. In most cases ejaculation was obtained after a few minutes. Earlier or usual practice of masturbation does not seem to be of any influence on the result of semen collection or on the time necessary until ejaculation. 75 p. 100 of the patients attached much importance to the exterior aspect of the collection room. Actually, apprehension or eventual difficulties seem to be linked with a lack of sufficient information on the physician's part before semen collection. This study gives evidence that semen collection could be systematically made in the laboratory itself. It allows better sperm exam conditions.

Fracture of the penis--a rare injury on the Jos Plateau, Nigeria.

A case of a 37 years old veterinary surgeon who sustained right sided fracture of the base of the penis during masturbation is presented. He was seen more than 48 hours after his injury with the typical painful swelling, angulation and tenderness at the fracture site. The tear was promptly repaired using the Laubscher's subcoronal sleeve technique. He regained his ability to achieve erection on the third post operative day. This is a rare injury on the Jos Plateau--being the only documented case in 11 years in this tertiary health facility and so we consider it necessary to highlight salient points in the diagnosis and treatment. Psychotherapy was found very useful in this patient in order to wean his off masturbation especially during the immediate post operative period. His short term follow up has been uneventful; he is now able to achieve normal and straight erection and sexual intercourse without pain--a view corroborated by his wife.

Body dissatisfaction and sexuality among women with bulimia nervosa.

OBJECTIVE: Past research and clinical observations have implicated sexuality as a factor involved in eating disorders. Yet little research has been conducted on possible links between body image and sexuality. We investigated such relationships within a large sample of women with bulimia nervosa. METHOD: A sample of adult women (N = 221) diagnosed with bulimia nervosa, purging type, completed a widely used measure of body dissatisfaction and answered questions about basic sexual experiences (coital and masturbation experience, age of onset of these activities, and self-rated satisfaction with current sexual activity). RESULTS: After controlling for age, age of onset of menses, and current body size, body dissatisfaction was unrelated to coital experience. However, even after controlling for these relevant covariates, current body dissatisfaction was predictive of lower incidence, and later onset, of masturbation. Current body dissatisfaction was marginally related (p < .10) to self-rated satisfaction with one's current sex life. DISCUSSION: Results are discussed with regard to past research, directions for future research, and clinical implications.
