[Diagnosis of "cataphasia" based on a drawing].

Drawings done by a patient before examination led to the suspicion of cataphasia, in view of previous experience of similar pictures drawn by verbigerate patients, and this proved to be so. We assume that the verbigerate affection of the thinking process is pictorially reflected in spontaneous casual drawing. In a less spontaneous situation, on other hand, the patient will not be so completely disorganised, and thus the author of an utterly confused drawing can still retain some measure of mental control in the course of tests.

[People taking care of people: the art of caring through radio waves].

The nurse as a primary care provider professional participates actively in health education activities. Within the spoken language, the radio, because of its power of penetration, assumes an important role in the dissemination of health information. Thus, the School of Nursing at Santo Amaro University, in São Paulo, Brazil, initiated a series of community health programs using the services of the university radio. The purpose of this paper is to report on the experience of elaborating and implementing the weekly programs "Healthy Life" and "People taking care of People", in first one implemented in 1996. Due to the positive results obtained, this activity became part of the University Extension Program with the participation of teachers and students.

Functional cerebral asymmetries and cognitive abilities in musicians, painters, and controls.

The relative participation of left- and right-hemisphere functions in verbal and spatial processing with musical composers, instrumentalists, painters, and non-musicians from student and junior high school populations was investigated. Hemispheric lateralization was related to the outcome of tests measuring spatial orientation, spatial visualization, tactile-visual discrimination, and verbal fluency. The relationship between lateral dominance and cognitive variables was influenced by sex and musical talents and the ability to paint. Males, irrespective of talents, were lateralized stronger than females. These sex differences were due to nonmusicians, only. Male and female composers, instrumentalists, and painters did not differ in language lateralization. Female left-handers showed a marked tendency for reversed language lateralization; left-handed males did not.

Long-term outcomes in multiple gestations.

Children born from a multiple gestation are at increased risk for cerebral palsy, learning disability, and language and neurobehavioral deficits. With the increased incidence of multiple pregnancies and use of assisted reproductive technology (ART), these issues are more commonly affecting parents. Long-term outcomes are a critical part of preconceptual and early pregnancy counseling for parents faced with a multiple gestation or considering ART, and the provider should be well versed on issues surrounding zygosity, gestational age, higher-order multiples, and the effects of options such as multifetal pregnancy reduction.

[Guidelines Clearing House Statement "Hypertension". Summary and recommendations for a rational hypertension guideline in Germany].

BACKGROUND: In order to promote quality of hypertension management in Germany, a national hypertension guidelines clearing project was initiated in 1999 by the German Guidelines Clearinghouse. OBJECTIVES: To identify and review published German- and English language hypertension guidelines. To establish criteria for future guideline development and implementation. To familiarize stakeholders in Germany with state-of-the-art hypertension guidelines. To identify key topics for a future national evidence-based guideline. METHODS: Search procedure, formal appraisal: Systematic search using literature databases and English-/German-language databases, published between 1990 and 1999. Abstract screening of the search results according to the inclusion criteria (n = 132 of a total of 548 hits). Systematic guideline evaluation using checklist with predefined criteria. APPRAISAL OF GUIDELINES' CONTENTS: Peer review of guidelines with the following inclusion criteria: hypertension--general, German and English language, published later than 1994, original or primary guideline or update, issued for nationwide use. Peer review was performed by a multidisciplinary focus group of EBM experts (primary and secondary care physicians, clinical pharmacologist, clinical epidemiologist). None of these was involved in hypertension guideline development during the review period. DOCUMENTATION OF CRITICAL APPRAISAL RESULTS: Systematic documentation of methodological appraisal and peer review results using a structured abstract form. The focus group wrote a final report (clearing report) including methodological abstracts for each guideline, essential topics for a future German hypertension guideline based on examples from the appraised guidelines, comments and recommendations for health care policy markers in Germany. RESULTS: 11 out of 132 guidelines were in accordance with the formal minimal standard with a wide range range within the following domains: "description of the development process", "declaration of authors' independence", "explicit link between recommendations and the supporting evidence", "management options", "tools for implementation". None of the guidelines identified all the key identified by the focus group, such as: (1) definition of hypertension--epidemiology--health care problems--intended guideline users/goals, (2) blood pressure measurement, (3) medical history and physical examination, (4) case-finding/screening, (5) indications for referral, (6) risk-stratification, (7) diagnostic procedures, (8) therapeutic goals/indications for therapy, (9) nonpharmacological measures, (10), pharmacotherapy, (11) follow-up/patient education/motivation/compliance, (12) comorbidity, hypertension in childhood/elderly, pregnancy, (13) primary prevention, (14) quality assurance/quality management, (15) dissemination/implementation, (16) open questions/challenges for the future. SUMMARY POINTS: To improve the quality of hypertension management in Germany, the expert panel suggested to develop a national evidence-based guideline. This should follow internationally agreed criteria and procedures. The experts identified and reviewed 11 out of 132 hypertension, which might make useful contributions for a future German Hypertension guideline. The expert group identified 16 key topics for a national hypertension guideline.

Symbolization levels in communicative behaviors of children showing pervasive developmental disorders.

A total of 48 children with severe pervasive developmental disturbance were observed in their communicative behaviors across modalities of language, gesture, play, and drawing. As predicted, levels of symbolization in gesture, play, and drawing were significantly intercorrelated and most strongly correlated with the criterion variable of expressive language. Children who were echolalic showed levels of symbolization intermediate between those of mute children and those with nonecholalic language. The correlation between the expressive language measure and a standardized measure of receptive language for 22 testable children also was significant. The results are discussed in the context of earlier studies of symbol production in verbal samples of disturbed and nonhandicapped children.

SIMPSON: a general simulation program for solid-state NMR spectroscopy.

A computer program for fast and accurate numerical simulation of solid-state NMR experiments is described. The program is designed to emulate a NMR spectrometer by letting the user specify high-level NMR concepts such as spin systems, nuclear spin interactions, RF irradiation, free precession, phase cycling, coherence-order filtering, and implicit/explicit acquisition. These elements are implemented using the Tcl scripting language to ensure a minimum of programming overhead and direct interpretation without the need for compilation, while maintaining the flexibility of a full-featured programming language. Basically, there are no intrinsic limitations to the number of spins, types of interactions, sample conditions (static or spinning, powders, uniaxially oriented molecules, single crystals, or solutions), and the complexity or number of spectral dimensions for the pulse sequence. The applicability ranges from simple 1D experiments to advanced multiple-pulse and multiple-dimensional experiments, series of simulations, parameter scans, complex data manipulation/visualization, and iterative fitting of simulated to experimental spectra. A major effort has been devoted to optimizing the computation speed using state-of-the-art algorithms for the time-consuming parts of the calculations implemented in the core of the program using the C programming language. Modification and maintenance of the program are facilitated by releasing the program as open source software (General Public License) currently at http://nmr.imsb.au.dk. The general features of the program are demonstrated by numerical simulations of various aspects for REDOR, rotational resonance, DRAMA, DRAWS, HORROR, C7, TEDOR, POST-C7, CW decoupling, TPPM, F-SLG, SLF, SEMA-CP, PISEMA, RFDR, QCPMG-MAS, and MQ-MAS experiments.

Automatic speech recognition--can it improve the man-machine interface in medical expert systems?

Computer-aided medical diagnosis has existed for two decades, but has not yet attained widespread acceptance among physicians. It is proposed that automatic speech recognition may be a significant factor in the eventual acceptance of the technology by the medical profession. The current state-of-the-art of automatic speech recognition is briefly surveyed, and problems with the technology are discussed. A potential natural language interface with DIAG, an expert system for aiding in dermatologic diagnosis, is described. A system that has been developed for accepting input of body parts in freestyle format is presented as a prototype for a natural language interface with an automatic speech recognition device.

Relating deaf students' reading and language scores at college entry to their degree completion rates.

Graduation patterns were examined for 905 deaf students (1990-1998) at the National Technical Institute for the Deaf. Students with higher reading and language skills had the best overall graduation percentage. Comparison of recipients of different degrees--bachelor of science (BS) versus fine arts (BFA); associate of applied science (AAS) versus occupational studies (AOS)--showed 92% of BS and 82% of AAS graduates reading at the 9th-grade level or above, versus 65% of BFA and 47% of AOS graduates. Interestingly, 80% of non-degree-earning students read at the 9th-12th grade levels; in absolute terms, they outnumbered graduates with similar reading skills in the AAS and BFA programs combined, and in the BS program. This indicates a need for improved counseling, placement, and retention strategies. Students performed similarly across degree categories, regardless of curriculum requirements and difficulty. Only non-degree-earning students had significantly lower grade averages.

The surgery for epilepsy with speech arrest.

The problems confronting patients with epilepsy, their families, and the surgeons wishing to help such patients, are discussed. It is important for physicians in other specialties to realize that epilepsy surgery is not nowadays complex, difficult, painful or uncertain; furthermore such operations are based on finding and removing focal lesions rather than "epileptogenic cortex" and the result in terms of integration of the patient into society is much improved if such intervention is performed while the patient is young, with time to gain academic and social skills after the operation. The selection of patients suitable for operation is discussed as well as methods of determining which hemisphere is dominant for speech and whether or not the focal lesion involves language centres. The majority of patients with drug resistant epilepsy suitable for operation have abnormalities in one temporal lobe. The pathological lesion is described and the advantages and disadvantages of various operations for temporal lobe epilepsy discussed. Extra-temporal cortical resection in the dominant hemisphere is also considered, particularly with reference to the preservation of language function. It is important that neurosurgeons realise that MRI and CT scanning have transformed epilepsy surgery from being a rather nebulous, time consuming art, to being for the majority of patients, a clear cut, straight forward procedure firmly based on "Oslerian" pathological principles. Far too few patients are being offered an operation (which renders 60-70% seizure free); neurosurgeons should respond to this challenge.

Reading, writing and drawing disorder due to right brain damage.

The author studies the written language in a series of right-handed patients with a right side cerebral lesion. Reduplications and omission of strokes and of letters are noted on writing. The cause of these disorders is attributed by the author to an impairment of visual and kinesthetic control of the act of writing, and not to a truly aphasic alteration. This impairment is part of a more general deficit, namely a distorted interpretation of spatial data. The author proposes to call this writing disorder afferent dysgraphia.

Mining free-text medical records.

Text mining projects can be characterized along four parameters: 1) the demands of the market in terms of target domain and specificity and depth of queries; 2) the volume and quality of text in the target domain; 3) the text mining process requirements; and 4) the quality assurance process that validates the extracted data. In this paper, we provide lessons learned and results from a large-scale commercial project using Natural Language Processing (NLP) for mining the transcriptions of dictated clinical records in a variety of medical specialties. We conclude that the current state-of-the-art in NLP is suitable for mining information of moderate content depth across a diverse collection of medical settings and specialties.

Laparoscopic radical prostatectomy.

PURPOSE: After the pioneering period when only few teams were performing the procedure, the laparoscopic approach to radical prostatectomy has become widespread with several technical variations. A comprehensive review of the published literature on laparoscopic radical prostatectomy was performed to determine the current state of the art of this surgical innovation in terms of perioperative parameters, functional results and cancer control. MATERIALS AND METHODS: English language, peer reviewed articles published before June 2004 concerning laparoscopic radical prostatectomy were found by MEDLINE query. All articles were analyzed and none were a priori excluded. Conclusions were drawn from series of 50 or more patients. RESULTS: Laparoscopic radical prostatectomy is being performed at multiple centers worldwide using various surgical approaches and technologies. Analysis of perioperative parameters, including surgical blood loss, operative time, complications and convalescence, demonstrated low morbidity and showed a clear trend toward improvement with increased experience. The reported positive surgical margin rates were lower in more recent series. As measured by prostate specific antigen recurrence and disease-free intervals, oncological results and cancer control rates are difficult to ascertain in the immature series published to date. Functional results in terms of postoperative urinary and sexual function appear encouraging. CONCLUSIONS: Overall the current operative, oncological and functional results of laparoscopic radical prostatectomy appear to approximate those of open radical retropubic prostatectomy. These results justify the considerable interest of the urological community in laparoscopy, as evidenced by its widespread application. Nevertheless, longer followup and more mature data are needed definitively to establish laparoscopic radical prostatectomy as an alternative to the retropubic approach.
