[Quantitative aspects of the percutaneous uptake of wool wax alcohols (cetyl alcohol) and paraffins (octadecane) from different ointment bases].

Since topical vehicles are partly responsible for the effects of active agents--both the wanted and the unwanted effects--attempt was made to obtain quantitative data on the possible penetration of cetyl alcohol contained in an aqueous hydrophilie cream (DAB 8) and in a wool fat alcohol ointment (lanoline) (DAB 8). Also the behaviour of the long-chained hydrocarbon, paraffin (octadecan), in these vehicles and in petrolatum (DAB 8) was studied. We found that the emulsifying agent in a W/O emulsion was detectable in the epidermis in low concentrations after 100 min, but the same agent in O/W emulsions only after 1000 min. It is, therefore, unlikely that cetyl alcohol penetrates the intact skin in allergicologically relevant concentrations. However, with diseased skin, a marked percutaneous absorption of cetyl alcohol, but not with paraffin, must be expected.

Stability of chlortetracycline hydrochloride and chloramphenicol in some ophthalmic ointment bases.

The stability of chlortetracycline hydrochloride at 25 and 35 degrees C in absorption base with and without antioxidant decreased during storage for 12 months, while, in water-in-oil emulsion base the medicament was more stable than in oil-in-water emulsion ointment bases. Chloramphenicol was more stable in the absorption base that contained lanolin than in the base which contained cetyl alcohol. Chloramphenicol was more stable in oil-in-water than in water-in-oil emulsion bases. The decomposition rate constant of chlortetracycline hydrochloride and chloramphenicol followed first order kinetics, when the stability study was carried out at 25 and 35 degrees C.

[Effect of silicoorganic compounds on protein biosynthesis in granulation-fibrous tissue].

The authors studied the effect of propoxysilatran (POS) on the biosynthesis of the main connective tissue biopolymeres. The use of POS in the form of 0.5 and 2.0% ointments on lanolin-vaseline base caused stimulation of cellular proliferation in the granulation fibrous tissue developing in the open skin defects of albino rats. Stimulation of cellular proliferation in these animals was accompanied by increase of collagen biosynthesis and of noncollagen proteins. In concentrations of 10(-3)--10(-4) M POS caused intensification of collagen biosynthesis (formation of peptide-bound nondialyzed 14C-oxyproline) in vitro in the cartilage tissue of chick embryos. Thus, silatrans are biologically-active substances producing a regulating effect on the course of the reparative-proliferative processes in the connective tissue.

Comparison of patch test results in two adjacent areas of England. II. Medicaments.

A 3-year study of patch test reactions to the Standard European Battery (S. E. B.) has been carried out in two adjacent areas of South-east England with the same climatic and cultural background. The source of materials and method of testing were identical; all observers have worked together in the same Contact Dermatitis Clinic and the interpretation of reactions can therefore be considered to be consistent. The main differences between the two areas lay in the pattern of employment and in local prescribing habits. Only the second aspect is considered here. Seven allergens--lanolin, neomycin, parabens, iodochlorhydroxyquinoline, ethylene diamine, caine 'mix' and chlorocresol--accounted for approximately 30% of all positive patch test reactions. 'Medicament sensitivity' correlated well with the known prescribing habits of both regions. Certain sites were shown to be especially important in the pathogenesis of medicament contact sensitivity.

[Trial of the safety, reactivity and therapeutic effectiveness of an ointment containing swine leukocytic interferon].

An ointment containing porcine leucocytic interferon (PLI) with the use of lanolin as the base and addition of human serum albumin for stabilization of the antiviral activity and Tris-or Hepes-buffer solutions for stabilization of the medium pH value was studied. The trials on volunteers with herpetic affections of the face and genitalia showed absolute safety of the drug, its areactivity and sufficiently high therapeutic efficacy as compared to an ointment containing human leucocytic interferon. The positive effect was observed in 52 out of 78 patients. On the basis of these data the PLI may be recommended for further clinical trials.

[Frequency of contact allergy in housewives (author's transl)].

A survey was taken of 616 housewives with a total of 1405 positive reactions in the patch tests. These women belong to a group of 3300 patients who showed contact allergic reactions in the patch test during the period between 1970 and 1974. The highest rate of sensitivity was found in the group of housewives in the age group over 60. The most common allergens of the housewives have been: p-aminodiphenylamin. Peruvian balsam, lanolin alcohol. Specific differences between men and women in the allergen spectrum are the following: in men there was a strong tendency to be allergic to potassium dichromate, and in women the allergic tendency was to nickel sulphate. Among the housewives no special preference toward allergies to nickel sulphate and potassium dichromate was observed.

[Allergy in cosmetology].

The computer analysis of a sample collecting 2,028 patients suffering from an eczematous dermatitis and subordinated to epicutaneous tests allowed us to analyze the rather difficult question of cosmetic allergy. This allergy is observed only in 2 p. 100 of the cases, if one considers the cosmetic allergy isolated; it reaches 5 p. 100 if it is associated with allergens coming from other origins (drugs of professional). However, in a more selected population of 91 patients suffering from a face dermatitis, these levels reach respectively 25 and 43 p. 100. The respective role of topic drugs and cosmetics is studied as well as main allergens associated with cosmetic allergy. The good tolerance of cosmetics encountered in patients allergic to one of their presumed components seems paradoxical. A prevention model of cosmetic allergy is presented, with an hypoallergenic variety of lanolin.

[The dynamics of the drug release from ointment bases. Part 5: The effects of tensides on the liberation of atropine from eye ointments (author's transl)].

The liberation of atropine sulphate from 12 eye ointments added with lipophil emulsifiers (cholesterol and glycerol monostearate) and propylene glycol was determined in vitro in the apparatus of Olszewski and Kubis, using the reaction of atropine with bromothymol blue for spectrometric estimation. The best release was observed with vaseline added with propylene glycol. In contrast, glycerol monostearate and cholesterol produced no considerable increase in liberation. Glycerol monostearate exerted the greatest effect on the liberation of atropine sulphate from the base described in the Polish Pharmacopoeia IV. Glycerol monostearate was the most suited emulsifier for a paraffin-lanolin-water base. The maximum of release is delayed by the addition of the emulsifier to the ointment base.

The comparative effect of two teat dips on udder health and teat skin quality.

The effect of two teat dips on udder health and teat skin appearance was examined. Ewodip N contained urea and a methylglycoside with a final iodine concentration of 0.25%. Ewodip with lanolin as emolient had a final iodine concentration of 0.4%. Ewodip N had a more favourable effect on teat skin quality than Ewodip. The percentage of cows with elevated cell count and without udder pathogenic microorganism (non-specific mastitis), was significantly lower in the group dipped with Ewodip N. The level of new infections of Staphylococcus aureus was lowest in the Ewodip group, but the difference was not statistically significant.

In vitro keratinocytes responses to chemical allergens.

An early event during the induction of skin sensitization is the production by keratinocytes (kc) of cytokines. Considering the role of kc in skin inflammatory and immune responses, we investigated the effect of three skin allergens (oxazolone, nickel sulfate and mixture of methyisothiazolinone and methylcholoisothiazolone) on IL-1 alpha production by a murine kc cell line (HEL30). Confluent cells were treated for different times with various concentrations of oxazolone, NiSO4 and mixture of methyisothiazolinone and methylcholoisothiazolone. IL-1 alpha was measured by ELISA both in conditioned media and cell-associated. All chemicals were able to induce a dose-response production of IL-1 alpha. This increase was related to an increase in protein synthesis as measured by 3H-leucine incorporation. At high concentrations all chemicals were cytotoxic as measured by LDH release. In the same cell system we also measured the effect of lanolin, a non irritant compound, on IL-1 alpha release. Even at the concentration of 300 micrograms/ml no IL-1 alpha release could be detected in the conditioned media. We have previously found a good rank correlation between in vitro surfactants induced IL-1 alpha release and in vivo Draize test. We now extend the possibility to use IL-1 alpha production for preliminary screening of chemicals for their irritative/allergenic potential, reducing the need for in vivo models.

Evaluation of controlled-release mosquito repellent formulations.

Eight polymer and 9 microcapsule formulations of deet were tested on laboratory rabbits against Aedes aegypti and Anopheles albimanus. Several formulations were significantly more effective than simple (unformulated) deet at the same strength for periods up to 24 h. Best results were obtained with a polymer formulation containing a high molecular weight fatty acid and 3 microcapsule formulations containing lanolin, gum arabic, gelatin, tannic acid, stearic acid, polypropylene glycol, water, and a commercial lotion in the microcapsule and carrier fractions.

[Allergologic survey in 251 patients with moderate or severe dermatitis. Incidence and value of the detection of contact eczema, food allergy or sensitization to air-borne allergens].

INTRODUCTION: Because of the increased recruitment of uncontrolled atopic dermatitis (AD) necessitating chronic use of dermocorticosteroids, we developed a prospective allergologic survey in a serie of 251 patients presenting with moderate or severe AD. PATIENTS AND METHOD: 251 patients were refered for allergologic assessment and followup. The clinical severity was assessed by use of standardized scores. Patients were grouped by age: group 1 (70 children younger than 2 years), group 2 (93 children between 2 and 7 years), group 3 (23 children between 7 and 15 years), group 4 (65 children over 15 years and adults). All the patients were systematically screened for contact dermatitis and IgE mediated sensitization (inhallant and food allergens) with blood tests for IgE, prior to evaluation of clinical relevance. RESULTS: Aero-allergen sensitization was demonstrated in 51 p. 100 of children and 89 p. 100 of adults. It was present earlier in severe AD with main clinical involvement for nose and throat and respiratory symptoms. Clinical responsibility for dermatitis was documented in only 6 p. 100 of AD. Food allergy was early incriminated as flare factors in most of severe AD (96 p. 100 of children and 81 p. 100 of adults) with major and persistant improvement under eviction diet. Main allergens were eggs (46 p. 100), pea-nuts (29 p. 100), shellfish (24 p. 100), milk (20 p. 100), flour (14 p. 100), fish (14 p. 100), soybeans (8.9 p. 100). Food allergy to yeasts (7.2 p. 100) was important in adults. Food allergy is the earliest allergy in the course of severe AD and the number of involved trophallergens increases in older patients. Patch tests were positive in 40 p. 100 of patients (i. e. 31 p. 100 of children and 66 p. 100 of adults) with a greater incidence in moderate AD. Main allergens were metals (54 p. 100), fragrances (19 p. 100), balsam of Peru (10 p. 100), parabens (8 p. 100) and lanoline (4 p. 100). CONCLUSION: When AD is not efficiently controlled by dermocorticosteroids, allergologic screening and treatment of children and adults proves to be very interesting. Specific measures regarding food allergy and contact dermatitis reduce or vanish cutaneous flares. As for inhallant sensitizations, Dermatologists should be awared that they may play a role regarding assessment of sensitization and prevention of respiratory symptoms in moderate and severe AD since the risk of complications is important in both groups.
