HLA antigens in Jews with pemphigus vulgaris.

The association of HLA antigens with pemphigus in Jewish patients of various ethnic origins was investigated. Of 45 Ashkenazi patients, 31 (68.6%) had A26 (p less than 10(-5) and 32 (7175) had BW38 ( less than 10(-5)0. Among the 11 non-Ashkenazi patients, 7 (63.6%) had A26 and only 1 had BW38. The question whether the antigen A26 or BW38 is linked with the disease is discussed. Our findings may suggest that the primary association of pemphigus is with A26. This association may be stronger in Jewish male patients than in females. In view of the recent literature and our results it may be presumed that pemphigus in Jews is associated with antigens of Loci A and DR. No differences between pemphigus vulgaris and pemphigus erythematosus were observed.

Cystic fibrosis heterozygote screening in the Orthodox Community of Ashkenazi Jews: the Dor Yesharim approach and heterozygote frequency.

In the community of the Orthodox Jews most of the marriages are arranged a screening program that is aimed at preventing the marriage of two carriers of autosomal recessive disorders is conducted by the Dor Yesharim organization. A random sample of 6,076 individuals of the Orthodox Jewish Ashkenazi community, were screened for the five mutations common in Ashkenazi patients (delta F508, W1282X, G542X, N1303K, 3849 + 10Kb C-->T). Two hundred thirty-two carriers were identified, giving a heterozygote frequency of 1:26. The relative frequencies of the individual mutations in the general population were comparable to those in the patients.

Lung cancer histology in Jews and Arabs in Israel, 1962-1982.

Lung cancer rates in Israel are lower than in Western countries, not explainable by smoking habits. Because of the different relations of squamous cell carcinoma and adenocarcinoma to smoking it was of interest to study the histologic distribution in Israel. A total of 7,871 histologically confirmed lung cancer cases were studied in the period 1962-1982. Squamous cell carcinoma was the leading tumor type in Jewish men and adenocarcinoma in Jewish women. Rates of both adenocarcinoma and squamous cell carcinoma increased throughout the period in both Jewish men and women, but the increase in adenocarcinoma was more pronounced in the last study period than that in squamous cell carcinoma. In 1977-1982 the rate ratio of squamous cell carcinoma to adenocarcinoma among Jewish men was 1.7. In Arab men it was 2.9, and in Jewish women 0.57. The Kreyberg I/II ratio among Jewish men was about 2.7 with no clear trend throughout the study period, and among Arab men this gradually decreased from 8.1 to 3.5. Jewish women had a constant Kreyberg I/II ratio of about 1 through the whole study period, but the ratio in Arab women was significantly higher than 1, with a mean overall ratio of 3.2. Jews and Arabs in Israel are different from each other in their patterns of lung cancer histology and are different to some extent from other populations in the Western world.

Religious delusions among Jews.

Though most previous publications described religious delusions only among Christians, a review of cases at Friends Hospital revealed that they are equally common among Jews. Five cases are given, four illustrating psychotic reactions to stress, one demonstrating that a religious experience does not necessarily result in psychosis. Content may come from either the Old or New Testaments. The Messiah delusion is discussed in more detail, with a historical example from the 17th century. Since persecution and other social stress leads to fantasies of a savior, Jewish people are now particularly vulnerable to Messianic delusions.

Internal body perceptions of Ethiopian Jews who emigrated to Israel.

Previous research regarding knowledge about internal organs among school-age children tied this knowledge to the development of cognitive capabilities. Studies have rarely considered the impact of culture on this knowledge. The purpose of this study was to examine internal body perceptions among Ethiopian Jews who emigrated to Israel. A total of 65 children (stratified according to age) and 19 adults were interviewed. Findings indicated that Ethiopian children reported fewer body organs than did Western children. Further, Ethiopian children and adults, unlike people from Western societies, did not think about the body in terms of different biophysical functions. Rather, they perceived the body as a holistic system in which a divine providence was responsible for body organs working together harmoniously. Implications of the differing perceptions about internal body organs, health, illness, and treatment are discussed.

Mitochondrial myopathy, sideroblastic anemia, and lactic acidosis: an autosomal recessive syndrome in Persian Jews caused by a mutation in the PUS1 gene.

We report the seventh case of autosomal recessive inherited mitochondrial myopathy, lactic acidosis, and sideroblastic anemia The patient, a product of consanguineous Persian Jews, had the association of mental retardation, dysmorphic features, lactic acidosis, myopathy, and sideroblastic anemia. Muscle biopsy demonstrated low activity of complexes 1 and 4 of the respiratory chain. Electron microscopy revealed paracrystalline inclusions in most mitochondria. Southern blot of the mitochondrial DNA did not show any large-scale rearrangements. The patient was found to be homozygous for the 656C-->T mutation in the pseudouridine synthase 1 gene (PUS1). Mitochondrial myopathy, lactic acidosis, and sideroblastic anemia is an oxidative phosphorylation disorder causing sideroblastic anemia, myopathy, and, in some cases, mental retardation that is due to mutations in the nuclear-encoded PUS1 gene. This finding provides additional evidence that mitochondrial ribonucleic acid modification impacts the phenotypic expression of oxidative phosphorylation disorders.

Obesity, the Jews and psychoanalysis: on shaping the category of obesity.

Hilde Bruch was one of the most important researchers into the question of weight during the 20th century. Best known for her popularization of anorexia nervosa, she was equally important in articulating a psychological aetiology for obesity. This work was rooted in her historical experiences in Germany and in the USA, and specifically the claim made at that time for the predisposition of the Jews to obesity.

Mutations in CYP11B1 and congenital adrenal hyperplasia in Moroccan Jews.

CONTEXT: In Jews of Moroccan descent (MJ), the prevalence of steroid 11beta-hydroxylase deficiency (11-OHD) is relatively high, with a carrier rate estimated as approximately one in 40. A single mutation in the CYP11B1 gene (encoding 11beta-hydroxylase), R448H, was suggested to account for the disease alleles in this population. STUDY SUBJECTS: We screened 236 healthy MJ for R448H. RESULTS: Only two of the subjects screened were found to be carriers, suggesting that the R448H allele frequency is lower than was assumed previously. An R448H/R448C compound heterozygote patient, diagnosed with 11-OHD, was identified. However, a subsequent screen of MJ subjects for R448C failed to detect any carriers, suggesting that this was a private mutation of this family. CONCLUSION: The high incidence of 11-OHD in MJ, therefore, is only partially explained by the presence of R448H as a founder mutation.

The nature of the defect in a salt-wasting disorder in Jews of Iran.

Studies in 8 families of Iranian Jews revealed 12 patients with selective aldosterone deficiency due to a biosynthetic defect. There was a marked range in clinical severity which varied from acute salt-wasting crisis in infancy to an asymptomatic state in adults detectable only by biochemical screening. Manifestations of intermediate degrees of severity included unexplained short stature and postural hypotension. This clinical variability in the manifestations of aldosterone deficiency was not due entirely to quantitative differences in hormone secretion but also to a changing pattern of requirement throughout life in which deficiency during the first year of life had grave consequences while a similar degree of deficiency in the adult was well-tolerated, suggesting that the hormone was no longer essential. Most families came from a relatively isolated community in Isfahan with a high incidence of consanguinity and three were related. Aldosterone deficiency was due to an inborn error involving the terminal portion of the biosynthetic pathway and characterized by marked overproduction of glomerulosa zone 18-hydroxycorticosterone relative to aldosterone. The best diagnostic index was the excretory ratio of the major urinary metabolites of these steroids. This ratio, normally less than 3.0, was frequently greater than 100 in untreated patients with this defect. Plasma aldosterone was not a reliable index of the disorder since some patients achieved normal levels but at the expense of marked elevation in plasma renin activity and overproduction of precursors.

Presence of prion protein in peripheral tissues of Libyan Jews with Creutzfeldt-Jakob disease.

The prion protein (PrP) gene on chromosome 20 encodes a protein designated PrPC. An abnormal, protease-resistant isoform of PrPC, denoted PrPCJD or PrPSc, is present in the brains of patients with Creutzfeldt-Jakob disease (CJD). In Libyan Jews, CJD segregates with a point mutation at codon 200 of the PrP gene, resulting in the substitution of lysine for glutamate. In the present study, we examined the presence of PrP in fibroblasts and leukocytes derived from eight CJD patients with the codon 200 mutation. In cultured fibroblasts as well as in leukocytes, there was a significant increase in PrP as judged by immunocytochemistry in addition to immunoblotting. Most of the PrP in fibroblasts and leukocytes could be released from the external surface by phosphatidylinositol-specific phospholipase C, a property characteristic of PrPC. In leukocytes only, part of the protein was protease resistant, resembling PrPCJD. The concentration of PrP mRNA was similar in fibroblast lines derived from controls and CJD patients. These results suggest that in CJD patients carrying a mutation at codon 200 of the PrP gene, the metabolism of PrP, rather than PrP synthesis, is abnormal.

The spectrum of familial inclusion body myopathies in 13 families and a description of a quadriceps-sparing phenotype in non-Iranian Jews.

The frequency, patterns of inheritance and clinical phenotypes of inherited myopathies with histologic features of rimmed vacuoles, tubulofilamentous inclusions and absence of inflammation (familial and hereditary inclusion body myopathy [f-IBM]) are poorly defined. Quadriceps sparing is a characteristic of f-IBM seen in the Iranian Jewish population. Among 101 patients with the feature of a red-rimmed vacuolar myopathy, characterized as inclusion body myopathy, seen during the last 4 years, we identified 13 families with f-IBM (12.8% frequency when one member per family was considered). Five families had an autosomal dominant and eight had an autosomal recessive form of inheritance. Among the latter group, five patients with early-onset disease (two Caucasian Americans, an Asian Indian, and two unrelated Iranian Jews) had the distinct feature of quadriceps sparing, which was confirmed by MRI of the thighs. Their disease began with weakness and strophy of the foot extensors, forearm flexors, and first dorsal interossei muscles and progressed to the forearm flexors, girdle, and axial muscles, but spared the quadriceps. Serum CK was normal. Muscle biopsies showed rimmed vacuoles, small fibers in groups, amyloid deposition (in one patient), tubulofilaments, and no inflammation. Immunocytochemistry did not reveal abnormalities of various membrane or cytoskeletal proteins. Major histocompatibility complex (MHC) class I antigen was expressed only in a few degenerating fibers invaded by macrophages. T-cell infiltrates were not present. We conclude that in a large referral population, dominant and recessive hereditary and familial forms of IBM are not rare. Quadriceps-sparing myopathy appears to be a clinically distinct, autosomal recessive, nonimmune, distal vacuolar myopathy that is not limited to Iranian-Jewish ethnic groups.

No longer invisible: gay and lesbian Jews build a movement.

The organized movement of lesbian and gay Jews took root in the mid-1970s when groups of Jewish homosexuals in the United States, England, and Israel began gathering for religious, educational, and social purposes. After centuries of denial, the Jewish community was faced with the reality of this increasingly visible and vocal minority. By 1989, nearly 30 groups of Jewish gay men and women throughout the world were part of the World Congress of Gay and Lesbian Jewish Organizations, an international body devoted to community education about homophobia and support for both member and newly emerging gay Jewish groups.

Pathologizing dissent: identity politics, Zionism and the 'self-hating Jew'.

This article discusses problems with Kurt Lewin's notion of self-hatred among Jews (Lewin, 1941/1948), and illustrates the ways in which the concept is used in identity politics. It argues that the way the notion of self-hatred is often used makes it problematic as a psychological concept because it requires that we accept particular definitions of group identities and particular political positions as central to those identities. Often, however, such issues are disputed by group members. Examination of the literature illustrates that it is rarely a straightforward decision whether those behaviours or attitudes identified as manifestations of self-hatred are best explained in this way. The function of the self-hatred concept in current debate over Israeli policy is described as an example of how arguments over identity are part of political conflict. In the case of current Middle Eastern politics, the concept of self-hatred is used by rightwing Zionists to label those who criticize policies of the current Israeli government as disloyal and pathological.

Alcoholism among the Jews in Israel: a pilot study. I. Research rationale and a look at the ethnic factor.

There is a marked difference in the extent of alcoholism and related problems among Israeli Jews of the three major ethnic communities--Ashkenazi, Sephardi, and Oriental (in ascending order)--which may be explained sociologically in terms of historic differentiation of Jewish minorities from the drinking norms of surrounding majorities, liturgical differences prior to immigration, and differentials in the social stresses of post-immigration adaptation to life in modern Israel.

Origins of Falasha Jews studied by haplotypes of the Y chromosome.

DNA samples from Falasha Jews and Ethiopians were studied with the Y-chromosome-specific DNA probe p49a to screen for TaqI restriction polymorphisms and haplotypes. Two haplotypes (V and XI) are the most widespread in Falashas and Ethiopians, representing about 70% of the total number of haplotypes in Ethiopia. Because the Jewish haplotypes VII and VIII are not represented in the Falasha population, we conclude that the Falasha people descended from ancient inhabitants of Ethiopia who converted to Judaism.

Screening for genetic disorders among Jews: how should the Tay-Sachs screening program be continued?

The screening program in Israel for Tay-Sachs disease has proven very successful, giving Jewish couples a choice not to have affected children. The technology of carrier detection is now possible in several other severe genetic diseases that are relatively frequent among Jews. Due to the current confusion, a policy is needed to determine how the TSD screening program should be continued in the Israeli Jewish population. We propose that such a screening program include only mutations agreed by consensus as causing a disease severe enough to warrant the possibility of therapeutic abortion. We also propose that general screening include only mutations that are relatively frequent, taking into account the carrier frequencies in the Israeli Jewish population.

Apparently dominant transmission of a recessive disease: deficiency of factor VII in Iranian Jews.

In inherited disorders transmitted as autosomal recessive traits the children of affected individuals are usually asymptomatic and phenotypically normal because they are heterozygous for the defect. In an Iranian Jewish family with moderately severe deficiency of coagulation factor VII (an autosomal recessive bleeding disorder) the son of an affected woman was also affected. DNA analysis of the factor VII gene showed that this unusual situation was due to the fact that he inherited an abnormal allele with the Ala244Val missense mutation from both the homozygous mother and the heterozygous father. The parents, although not overtly consanguineous, belong to the same ethnic group of Iranian Jews, among whom this factor VII gene mutation reaches high frequencies (between 2 and 3%) in the heterozygous state.

The equivalence of United States census data for persons of Russian stock or descent with American Jews: an evaluation.

While the U.S. Bureau of the Census has had a long-standing policy of abstaining from enumerating the religious beliefs or backgrounds of the American people, at least two-thirds of the Jewish population of the United States has been enumerated in decennial censuses and sample surveys in the guise of persons of Russian stock or origin. This has come about through the migration policy of the old Russian Empire and the statistical categories utilized by American immigration authorities and by the U. S. Bureau of the Census for immigrants and their children. Comparisons between the returns from an ethnic survey and a survey on the religious composition demonstrate the close congruence between persons of Russian stock or descent and American Jews on the national level.
