Lung function and long-term exposure to air pollutants in middle-aged American adults.

The authors examined the association of long-term exposure to ozone (03) and ambient particulate matter <10 microm in diameter (PM10) with pulmonary function by using cross-sectional data of 10,240 middle-aged adults who participated in the Atherosclerosis Risk in Communities study. Air-pollution data came from the US Environmental Protection Agency's Aerometric Information Retrieval System. After an adjustment for important covariates, the inverse associations with forced vital capacity or forced expiratory volume in 1 second were observed separately for 03 and PM10). These associations were stronger among smokers than among the nonsmokers; among users of respiratory medications than among nonusers; and among participants currently experiencing respiratory symptoms than among those without symptoms. However, an additional adjustment for the study center eliminated the PM10 association. Moreover, center-specific associations between PM10 and pulmonary function were not significant, possibly as a result of the homogeneity of within-center exposures.

Wegener granulomatosis: an analysis of 158 patients.

OBJECTIVE: To prospectively study the clinical features, pathophysiology, treatment and prognosis of Wegener granulomatosis. DESIGN: Of the 180 patients with Wegener granulomatosis referred to the National Institute of Allergy and Infectious Diseases during the past 24 years, 158 have been followed for 6 months to 24 years (a total of 1229 patient-years). MEASUREMENTS: Characteristics of clinical presentation, surgical pathology, course of illness, laboratory and radiographic findings, and the results of medical and surgical treatment have been recorded in a computer-based information retrieval system. SETTING: The Warren Magnuson Clinical Center of the National Institutes of Health. MAIN RESULTS: Men and women were equally represented; 97% of patients were white, and 85% were more than 19 years of age. The mean period of follow-up was 8 years. One hundred and thirty-three patients (84%) received "standard" therapy with daily low-dose cyclophosphamide and glucocorticoids. Eight (5.0%) received only low-dose cyclophosphamide. Six (4.0%) never received cyclophosphamide and were treated with other cytotoxic agents and glucocorticoids. Ten patients (6.0%) were treated with only glucocorticoids. Ninety-one percent of patients experienced marked improvement, and 75% achieved complete remission. Fifty percent of remissions were associated with one or more relapses. Of 99 patients followed for greater than 5 years, 44% had remissions of greater than 5 years duration. Thirteen percent of patients died of Wegener granulomatosis, treatment-related causes, or both. Almost all patients had serious morbidity from irreversible features of their disease (86%) or side effects of treatment (42%). CONCLUSIONS: The course of Wegener granulomatosis has been dramatically improved by daily treatment with cyclophosphamide and glucocorticoids. Nonetheless, disease- and treatment-related morbidity is often profound. Alternative forms of therapy have not yet achieved the high rates of remission induction and successful maintenance that have been reported with daily cyclophosphamide treatment. Despite continued therapeutic success with cyclophosphamide, our long-term follow-up of patients with Wegener granulomatosis has led to increasing concerns about toxicity resulting from prolonged cyclophosphamide therapy and has encouraged investigation of other therapeutic regimens.

Cyclophosphamide-induced cystitis and bladder cancer in patients with Wegener granulomatosis.

OBJECTIVE: To describe the incidence of, clinical manifestations of, and risk factors for cyclophosphamide-induced urinary bladder toxicity in patients treated for nonmalignant disease. DESIGN: Retrospective analysis of patients followed at the National Institutes of Allergy and Infectious Diseases from 1967 to 1993. SETTING: The Warren G. Magnuson Clinical Center of the National Institutes of Health (NIH). PATIENTS: 145 patients who received cyclophosphamide for the treatment of Wegener granulomatosis and were followed for 0.5 to 27 years (median, 8.5 years), for a total of 1333 patient-years. MEASUREMENTS: Clinical characteristics, cystoscopic findings, results of cytologic examination of urine, surgical pathology, and total dose and duration of cyclophosphamide therapy were recorded and analyzed using a computer-based information retrieval system. RESULTS: Nonglomerular hematuria occurred in 73 of 145 patients treated with cyclophosphamide (50%). Sixty of the 73 patients with nonglomerular hematuria (82%) had cystoscopy at the NIH. Forty-two of the 60 patients (70%) who had cystoscopy had macroscopic changes consistent with cyclophosphamide-induced bladder injury. Seven patients (5%) developed transitional-cell carcinoma of the urinary bladder. In 6 of these 7 patients, the total cumulative cyclophosphamide dose exceeded 100 g, and the cumulative duration of cyclophosphamide therapy exceeded 2.7 years. Before they were given a diagnosis of bladder cancer, all 7 patients had had one or more episodes of microscopic or gross nonglomerular hematuria. In contrast, none of the 72 patients who had never had nonglomerular hematuria developed bladder cancer. Cox proportional hazards regression analysis showed that only microscopic nonglomerular hematuria was a significant risk factor for the development of bladder cancer (P < 0.01). CONCLUSION: Long-term oral cyclophosphamide therapy is associated with substantial urotoxicity, including the development of transitional-cell carcinoma of the urinary bladder. In this cohort of patients, the estimated incidence of bladder cancer after the first exposure to cyclophosphamide was 5% at 10 years and 16% at 15 years. Nonglomerular hematuria was a frequent manifestation of cyclophosphamide-induced cystitis, and it identified a subgroup of patients at high risk for the development of bladder cancer.

Medicaid program; state plan requirements and other provisions relating to state third party liability programs--HCFA. Final rule.

This final rule implements certain Medicaid State plan requirements and other provisions relating to State third party liability (TPL) programs. Its provisions deal with: (1) The integration of a State's pursuit of third party claims with its Mechanized Claims Processing and Information Retrieval System; (2) exceptions to the cost avoidance method of claims payment in TPL situations; and (3) provider restrictions and penalties related to attempts at collection of cost sharing or portions of those amounts from Medicaid recipients when TPL has been established. These regulations implement portions of section 9503 of the Consolidated Omnibus Budget Reconciliation Act of 1985.

Behçet's disease in southern Chinese patients.

OBJECTIVE: To describe the clinical characteristics of Behçet's disease (BD) in a southern Chinese population and compare them with those reported in other ethnic groups. METHODS: Patients with BD from 4 large regional hospitals in Hong Kong were identified from the hospital information retrieval system. Their records were retrospectively reviewed and the diagnosis was verified by at least 2 rheumatologists using the International Study Group (ISG) criteria. Demographic data and various clinical features of our patients were collected and analyzed. RESULTS: Between 1978 and 2000, 37 adult patients with definite BD according to the ISG criteria were identified. All patients were ethnic southern Chinese. There were 19 women and 18 men (F:M = 1.1:1). The mean age at diagnosis was 36.2 +/- 10 (18-74) years. Presenting features at diagnosis, in decreasing order of frequency, were oral ulceration (100%), genital ulceration (81%), skin lesions (73%), arthritis/arthralgia (54%), and ocular lesions (35%). Among skin lesions, the commonest manifestations were erythema nodosum (74%), folliculitis (44%), and acneiform nodules (30%). Arthritis was usually polyarticular (75%) and symmetrical, and involved the upper limb joints. Anterior uveitis was the most common ocular complication (77%) and was often bilateral. Involvement of other systems such as the central nervous system, gastrointestinal (GI) tract, and cardiopulmonary system was uncommon. Vascular complications were rare. Only 2 (6%) patients had a positive pathergy test. Our patients were followed for a mean of 59.9 +/- 52 months. Apart from recurrent attacks of orogenital ulceration and uveitis, new systemic features were unusual. One patient died of bowel perforation as a result of active GI disease. Compared with other reported series, our patients had fewer ocular and systemic manifestations, and a significantly lower incidence of the pathergy reaction. CONCLUSIONS: BD is rare among southern Chinese in Hong Kong and tends to run a more benign course with less systemic involvement and fewer serious ocular complications. Whether the observed difference in clinical manifestations of the disease is related to a different HLA association or environmental factors warrants further studies.

[Prospects for providing information services in the task force scientific center].

The paper deals with the practice of the Information Service, Moscow Research Institute of Tuberculosis, RSFSR Ministry of Health, to provide information services with regard to the actual information needs of all specialists of research institutes and dispensaries that are as part of Phthisiopulmology Task Force Research Center. The results of sociological studies made it possible to define the types of information production and services which should be developed due to the change to the new economic relations. A new approach to the information--patent provision of scientific problems is proposed within the Task Force Research Center which allows the relationships to be regulated between the information retrieval systems and the usera of economic methods.

[Clinico-organizational aspects of prevention of mortality among urban and rural population].

The article entitled "Clinico-organizational aspects of preventing mortality among urban and rural population" provides results of the comprehensive socio-hygienic survey of the health status of urban and rural population residing in Leningrad region in the light of influence by modern urban environmental factors. With the help of information-retrieval system more than 44 thousand of survey items were processed. A comprehensive review of mortality rates is provided, serious shortcomings in the prevention of mortality are revealed.

[Biomathematical approaches to the problems of radiation biology and medicine].

A vast experience in the introduction of modern mathematical methods into research activities of the Central Research Roentgenoradiology Institute was accumulated in the Department of Biomathematics. The Information-retrieval System for Storage and Analysis of Data on the Activities of X-ray Service in the USSR, a package of applied programs for statistical analysis of radiation therapy effectiveness, a controlled data base on 6 cancer sites, 2 systems for simulation of radiation effects on heterogeneous cell systems were worked out. Some priority results in the fields of mathematical biology and statistical analysis of incomplete selective data were obtained. The Department is heading the work on the problem "Mathematics and computer technology in radiobiology and radiation biophysics".

Comparison of attitudes and cognitive achievement of nursing students in three instructional strategies.

This study is a comparison of the attitudes and the cognitive achievement of nursing students in three instructional strategies (the traditional lecture, black and white televised instruction, and independent color televised instruction via the Dial Access Information Retrieval System). Though only thirty-one percent (31%) of students indicated prior exposure to independent study methology, sixty percent (60%) identified a desire for active involvement in courses of study. The only significant finding regarding attitudes toward the three strategies was a preference for an greater interest in color videotapes than for black and white televised material. No significant differences in cognitive achievement were noted between any of the three strategies. Implications from the study for curriculum revision are discussed, stressing the probable value of maintaining traditional techniques concurrent with innovative methodological experimentation.

Checkup interval and cancers in automated multiphasic health testing and services.

The purpose of this study was to disclose which types of cancer and how many persons with cancer were detected among the AMHTS examinees of our AMHTS center by using the hospital information retrieval system, and to study the relationship between cancer and the number of examinees, checkup intervals, and frequency in AMHTS. The examinees who had checkups more than twice were divided into three groups based on their checkup intervals: within one year, one to two years, and over two years. The relationship between cancer ratios and checkup intervals was evaluated in each group of examinees. In those having checkups within one year and from one to two years the cancer rate was 2.9 patients per 1,000 persons. However, in those having checkups after a two-year period or longer, the cancer rate was 4.3, clearly greater than the rate of the other two groups.

A computerized system for retrieval of case information in a veterinary diagnostic laboratory.

Morbidity and mortality data are necessary bases for the decision-making processes relevant to allocation of public funds for animal disease diagnoses and research. A system for information storage and retrieval capable of handling diagnostic data such as results of microbiology, parasitology, necropsy, and histopathology as well as demographic data such as owner, species, sex, breed, or geographic origin of the animal is described. This information is available to veterinarians, epidemiologists, herdsmen, and others involved in disease prevention or control efforts. The system described utilizes natural language, thus overcoming difficulties encountered in systems with numerical intermediates. Used and revised for the last 10 years, the system described has proved useful for annual administrative quantitation of services performed. In fact, the Concordance Index serves as the annual report of the University of Missouri Veterinary Medical Diagnostic Laboratory. Having accurate detailed information on individual cases, as well as a variety of composite data, has been extremely helpful in the documentation necessary for attracting funding for study of specific disease states.

Terminological problems in information retrieval.

This is the first in the series of the papers on an application of the systems analytic approach to evaluation of information retrieval (IR). At the beginning, the importance of consistent terminology in information retrieval is discussed, and different definitions of "information retrieval" and "IR system" are presented. The importance of identifying boundaries of the IR system for clarifying terminological differences is discussed. The terminological problems in IR are summarized, and some of the existing approaches to their solution are identified. A historical overview of IR research is presented in order to explain the existing terminological differences and to create a common basis for a discussion of evaluation approaches to IR.

The development of a microcomputer system for poisons information retrieval.

The Poisons Information Centre at the Red Cross War Memorial Children's Hospital in Cape Town makes use of a microcomputer for information retrieval. An outline is given of the data base design and the manner in which data were collected and collated. User operation of the system is described and its effect on the efficiency of the service noted.
