[Microbiological studies of the contamination of hospital waste and household refuse].

A bacteriological analysis of hospital waste and household refuse (garbage dump) showed a wider range of bacteria in hospital waste. However, quantitatively, household refuse was more contaminated, especially with fecal bacteria. Almost one third of all hospital waste showed no bacterial growth under the conditions chosen for the study. In blood-drenched waste and serum samples 2% of all samples examined were anti-HBc and anti-HBe positive.

The syndrome of abuse dwarfism (psychosocial dwarfism or reversible hyposomatotropism).

In abuse dwarfism the behavioral signs include some or all of the following: (1) a history of unusual eating and drinking behavior, reversible on change of domicile, such as eating from a garbage can and drinking from a toilet bowl, stealing food, alleged picky eating and rejecting food at the table, polydipsia and polyphagia, possibly alternating with vomiting and possibly also with self-starvation; (2) a history of such behavioral symptoms as enuresis, encopresis, social apathy or inertia, defiant aggressiveness, sudden tantrums, crying spasms, insomnia, eccentric sleeping and waking schedule, pain agnosia, and self-injury, all occurring only in the growth-retarding environment; (3) retarded motor development, with improvement on removal of the child from the domiclle of abuse; (4) retarded intellectual growht, reversible on change of domicile by as much as 30 to 50 IQ points; and (5) a history of pathologic family relationships, including unusual cruelty and neglect, either somatic or psychic or both.

Mortality among workers at municipal waste incinerators in Rome: a retrospective cohort study.

A retrospective mortality study of a cohort of 532 male subjects employed at two municipal plants for garbage recycling and incinerating has been conducted. All workers ever employed at the plants since 1962 were enrolled and followed up from January 1, 1965 to December 31, 1992. Standardized Mortality Ratios (SMRs) and 90% confidence intervals (90%CI) were calculated using regional population mortality rates. The study yielded a total of 8,585 person-years of observation. Mortality from all causes was significantly lower than expected (observed [obs] = 31; SMR = 0.71; 90%CI = 0.51-0.95). All cancer mortality was comparable with that of the general population (obs = 15; SMR = 0.95; 90%CI = 0.58-1.46). Mortality from lung cancer was reduced (obs = 3; SMR = 0.55; 90%CI = 0.15-1.42). Increased risk was found for gastric cancer (obs = 4; SMR = 2.79; 90%CI = 0.94-6.35). Analysis by latency indicated that the excess risk of gastric cancer was confined in the category with more than 10 years since first exposure. Our results for gastric and lung cancers indicate the need to further investigate the role on cancer of occupational exposure to organic dust and bacterial endotoxins in the waste management.

Life-style and environmental factors in the development of nasal NK/T-cell lymphoma: a case-control study in East Asia.

Cases of nasal NK/T-cell lymphoma (NKTCL) occur occasionally in Asian and Latin American countries but rarely in Western countries. The etiological role of life-style and environmental factors in nasal NKTCL was investigated. Five university hospitals in Japan and one each in Korea and China participated in this study; a total of 88 cases and 305 hospital controls were accrued during 2000-2005. The odds ratio (OR) of NKTCL obtained after adjustments of age, sex and country was 4.15 (95% confidence interval (CI), 1.74-9.87) for farmers, 2.81 (CI, 1.49-5.29) for producers of crops, 4.01 (CI, 1.99-8.09) for pesticide users, 11.65 (CI, 1.17-115.82) for residents near garbage burning plants, 2.95 (CI, 1.25-6.95) for former drinkers, and 0.49 (CI, 0.23-1.04) for current smokers. The ORs for crop producers, who minimized their exposure to pesticides by using gloves and glasses, and sprinkling downwind at the time of pesticide use, were 3.30 (95% CI, 1.28-8.54), 1.18 (95% CI, 0.11-12.13) and 2.20 (95% CI, 0.88-5.53), respectively, which were lower than those for producers who did not take these precautions. Exposure to pesticides and chemical solvents could be causative of NKTCL. Taken together, life-style and environmental factors might be risk factors for NKTCL.

Autophagy, organelles and ageing.

As a result of insufficient digestion of oxidatively damaged macromolecules and organelles by autophagy and other degradative systems, long-lived postmitotic cells, such as cardiac myocytes, neurons and retinal pigment epithelial cells, progressively accumulate biological 'garbage' ('waste' materials). The latter include lipofuscin (a non-degradable intralysosomal polymeric substance), defective mitochondria and other organelles, and aberrant proteins, often forming aggregates (aggresomes). An interaction between senescent lipofuscin-loaded lysosomes and mitochondria seems to play a pivotal role in the progress of cellular ageing. Lipofuscin deposition hampers autophagic mitochondrial turnover, promoting the accumulation of senescent mitochondria, which are deficient in ATP production but produce increased amounts of reactive oxygen species. Increased oxidative stress, in turn, further enhances damage to both mitochondria and lysosomes, thus diminishing adaptability, triggering mitochondrial and lysosomal pro-apoptotic pathways, and culminating in cell death.

Protein biomarkers in the plasma of workers occupationally exposed to polycyclic aromatic hydrocarbons.

Polycyclic aromatic hydrocarbons (PAHs) form a chemical family containing several hundred compounds, including benzo[a]pyrene and pyrene. They are usually produced by the incomplete burning of coal, oil, gas, garbage, or other organic substances like tobacco or charbroiled meat. Exposure to PAH causes tumors, primarily in the lung, the bladder, and the skin. To investigate the differentially expressed proteins resultant from PAH exposure, the protein expression in human plasma was analyzed using two-dimensional electrophoresis (2-DE). The plasma exposed to PAH was obtained from 48 waste gas pollution measurers working at an automobile emission inspection center. The 1-hydroxypryene (1-OHP) level, which is the urinary PAH metabolite used for evaluation of PAH exposure, was 0.28 micromol/mol creatinine in PAH exposure groups, and 0.078 micromol/mol creatinine in unexposed groups (control, n = 33). A protein upregulated by PAH (putative capacitative calcium entry channel) and five overexpressed proteins (two fibrinogen gamma-A chain precursors, a hemopexin precursor, an albumin precursor, and T-cell receptor beta chain C region) were identified with matrix assisted laser desorption/ionization-time of flight-mass spectrometry (MALDI-TOF-MS) and confirmed with tandem MS (MS/MS) and Western blotting. The putative capacitative calcium entry channel was partially validated with a laboratory made antibody of a representative peptide fragment in PAH-exposed human plasma samples.

[Hoarding and severe neglect. A case report].

Severe neglect of the living space due to excessive collection of unusable objects and refuse has been named "Vermuellung" (complete congestion with garbage) since 1985 in the German literature. The internationally used terms 'hoarding' or 'Diogenes syndrome' are in fact neither synonyms nor direct translations of the term. The condition is a problem mainly in social and geriatric psychiatry, yet it is seen also in young subjects without psychopathologic features. A frequent cause for the condition is obsessive-compulsive disorder, but several other disorders are associated with it. A 58-year-old female patient with severe Vermuellung due to obsessive-compulsive disorder is presented.

RESEARCH: Environmental Racism in the Sunbelt? A Cross-Cultural Analysis.

/ Sociologist Robert Bullard challenged the prevailing paradigm of environmentalism as a consensual issue in the United States by developing the concept of environmental racism. As he claims, ethnic minorities have been put "at greater environmental risk" than has the Caucasian majority in most areas of the country. This study of the Tucson metropolitan area examines this proposition by utilizing data from several sources: interviews with elected officials and other opinion leaders, GIS-generated socioeconomic data, articles in the press, and a literature review. We conclude that Bullard's concept has validity for this metropolitan area but that there also exist widely divergent differences of opinion on the subject. We explain why this is so. We further conclude that the allegation of "environmental racism" made by the Hispanic community in the 1980s and 1990s has had a transformative effect on local politics.KEY WORDS: Environment; Racism; Tucson; Garbage dumps; County government; Pollution; Equity; Justice

Health effects associated with indoor storage of organic waste.

OBJECTIVES: The storing of organic waste indoors for several days can be considered a risk factor for microbial contamination of homes. This has become relevant in the scope of modern waste management programmes. This study specifically addresses possible health aspects in this context. METHODS: In a cross-sectional study, doctors collected 384 questionnaires in three neighbourhoods without industrial sources for microbial contamination in 1997 in Hesse, Germany. Self-reported prevalence of airway, skin and general health complaints during the past year, lifetime diagnoses by a doctor, and home hygiene (storage of garbage, private composting, pet contact, indoor mould growth), were assessed. With logistic regression analysis, odds ratios (ORs) adjusted for age, gender, level of school education, and frequent contact with dog or cat were determined for indoor storage of waste for more than 2 days for the whole study group and also for a subgroup reporting atopic disposition. RESULTS: Longer indoor storage of organic waste was associated with "skin rash" (OR=2.05 [95% confidence interval (CI) 1.09, 3.83]), "itching skin rash (>2 months) in the past 12 months" (OR=3.58 [95% CI 1.61, 7.96]) and "itching skin rash (>2 months) ever" (OR=2.77 [95% CI 1.33, 5.75]), as well as lifetime diagnoses of skin disease by a doctor (OR=3.16 [95% CI 1.40, 7.10]) and allergy other than hay fever (OR=2.74 [95% CI 1.28, 5.85]). Associations remained when adjustment was made for atopic disposition. Atopic subjects were at higher risk for these skin-related complaints and diseases. CONCLUSIONS: These results point to a possible health relevance of the storing of organic waste in the home for several days, especially as far as skin irritation is concerned. These associations possibly result from indoor microbial contamination, e.g., endotoxins due to waste. Atopic disposition is a possible risk factor for this health impairment. Personal waste management should also be considered in studies on health and indoor environments.

Quantification of cumulated physical fatigue at the workplace.

Quantification of physical fatigue remains a challenge. We hypothesized that its effects on central autonomic nervous system activity could be explored for such a quantification. To test this relationship, we prospectively measured central autonomic nervous activity through nocturnal heart rate variability (HRV) in six French garbage collectors, aged 32.1+/-4.3 years, twice a week during 3 consecutive weeks of work, and during the following week of rest. Eight healthy sedentary males formed a control group. HRV indices were calculated by applying standard temporal domain and wavelet transform analyses to standard ECG recordings. During the 3 consecutive weeks of work, there was a significantly progressive decrease in HRV indices, particularly pNN50 (-34.2%, P<0.05), as well as the high (-33.3%, P<0.05) and low (-22.2%, P<0.01) frequency components of wavelet transform, while there was an increase, although non-significant, of the ratio of low to high frequencies (9.1%). During the resting period, there was a significant recovery of HRV indices, notably of its high (50.0%, P<0.05) and low (28.6%, P<0.05) frequency components. No such changes occurred in the control group. A central signature of cumulated physical fatigue can thus be detected and quantified through nocturnal autonomic nervous system activity. Its characteristics are those of a progressive parasympathetic withdrawal.

Natural Echinococcus multilocularis infection in a Norway rat, Rattus norvegicus, in southern Hokkaido, Japan.

Forty-two rats, Rattus norvegicus, captured at a garbage dump in southern Hokkaido, Japan, were examined, and one was found to be infected with Echinococcus multilocularis. The lesions were found in the liver, lung, mesenteric lymph nodes, greater omentum and also free in the abdominal cavity. No necrosis was observed in any of the lesions, and inflammatory reactions were mild. Protoscoleces were observed in the large liver cysts. A homogenate of these cysts, when transplanted into the abdominal cavity of three Mongolian gerbils and a rat, yielded numerous fully developed protoscoleces at 4-7 months post-inoculation. Judging from this, it is postulated that the rat could become a natural intermediate host for E. multilocularis in this area.

Volatile organic compounds from household waste.

Ninety volatile organic compounds were identified in the headspace of biodegradable and mixed household waste by GC/MS. Most of them could be attributed to biological waste, microbiological growth in biological waste or packaging materials. Although none of the compounds were present at concentrations exceeding their threshold limit values, it is possible that in closed garbage cans or containers organic sulfur compounds could contribute to the reported gastrointestinal problems of collection personnel.

Physical abuse of abandoned children in India.

Child abuse and neglect, though existing in India, has not come to light because of underlying social constraints like poverty, malnutrition and infection. With an intention to study the problem of physical abuse in neglected children, patients admitted to the pediatric wards of our institution, predominantly with some form of physical abuse were specifically looked for from January 1976 onwards. Over a period of 4 1/2 years, 18 such cases were encountered. Most of the babies were less than four weeks of age, were predominantly females, were quite often premature or malnourished, and rarely had a physical deformity. These babies were deserted and later were found by police or social workers in gutters, dustbins, garbage tins, railway platforms, etc. The spectrum of abuse and injuries included newborn babies with intact placenta, at times lacerated, strangling marks on the throat, multiple bruises, bleeding from the umbilicus, avulsion of the scalp with severe bleeding, shock as a result of environmental stress (i.e., hypothermia), evidence of infection, rat bite marks, and fractures. The morbidity was very high, ultimately resulting in 88.9% mortality over varying periods of time, maximum survival being 22 days. The motive behind this abuse always was an unwanted baby.

Mutagenicity of particulates from the laboratory combustion of plastics.

Carcinogenic polycyclic aromatic hydrocarbons (PAHs) and nitropolycyclic aromatic hydrocarbons (nitro-PAHs) have been identified in airborne particulate organic matter extracts. The pollutant sources were generally contributed by motor vehicles and industrial activity. Massive quantities of urban solid wastes, containing plastic materials such as PVC, PET, PS, and PE, burnt in the open air in local garbage dumps are frequently found in developing countries. In this study, the smog particulates from the combustion of these synthetic polymers were produced in a laboratory combustion chamber. The mutagenicity of acetone extracts from the smog particulates was evaluated with Salmonella typhimurium TA98 and TA100 in the presence and absence of S9 mix. Four samples in TA98 exhibited higher mutagenicity than those in TA100. The greatest mutagenicity was observed from the extracts of particulates from combustion of PVC followed by that of PS, PET, and PE. To determine the major mutagenic compounds in these samples, mutagens were partially purified through TLC and their mutagenicity was monitored with TA98. 1-NP and DNPs in the above samples were also determined by HPLC. The amounts of 1-NP and DNPs generally corresponded with their mutagenicity. Higher levels of 1-NP and DNPs were generated from the combustion of PVC, PET, and PS. The combustion of synthetic polymer wastes might be responsible for the presence of high levels of 1-NP and DNPs in Taiwan urban air.

Detection of a carcinogen, 2-amino-1-methyl-6-phenylimidazo [4,5-b]pyridine, in airborne particles and diesel-exhaust particles.

A carcinogen, 2-amino-1-methyl-6-phenylimidazo [4,5-b]-pyridine (PhIP), has been measured in airborne particles, diesel-exhaust particles, and incineration ash from garbage-burning plants. PhIP was found in all kinds of samples. In the light of the present results, together with the previous findings that PhIP was present in cooked foods and cigarette smoke, PhIP is likely to be an ubiquitous environmental pollutant. These observations also suggest that PhIP may be formed through the combustion process.

N-Methyl-beta-carboline-3-carboxamide (FG 7142), an anxiogenic agent in airborne particles and cigarette smoke-polluted indoor air.

beta-Carboline-3-carboxylic acid methylamide (FG 7142), an anxiogenic agent, has been measured in airborne particles, automobile-exhaust particles, incinerator ash, smoke condensate of tree leaves and cigarette-smoke-polluted indoor air by high-performance liquid chromatography. This compound has been detected in indoor as well as outdoor air. The source of this compound in indoor air was determined as cigarette smoke, identified from smoking machine studies. This anxiogenic agent was detected in smoke condensate of tree leaves and incinerator ash from garbage burning plants, but not in diesel-exhaust particles. Considering the present results, together with the previous finding that cigarette smoke contains this compound, FG 7142 is likely to be formed through combustion of plants. Our data also suggest that this compound may be widely distributed in the environment.

New insights into proteasome function: from archaebacteria to drug development.

The proteasome is not simply a 'garbage disposal unit' but also has functions in the control of the cell cycle and immune responses. The structure of an archaebacterial proteasome has recently been determined to high resolution, and provides insight into the unusual mechanism of proteolytic cleavage by the proteasome.
