Prospective study of posttraumatic stress, anxiety, and depressive reactions after earthquake and political violence.

OBJECTIVE: The authors sought to assess the severity and longitudinal course of posttraumatic stress, anxiety, and depressive reactions among two groups of adults differentially exposed to severe and mild earthquake trauma and a third group exposed to severe violence. They also examined interrelationships among these reactions and predictors of outcome and compared posttraumatic stress disorder (PTSD) symptom category profile and course between those exposed to earthquake and those exposed to violence. METHOD: Seventy-eight non-treatment-seeking subjects were assessed with self-report instruments approximately 1.5 and 4.5 years after the 1988 Spitak earthquake in Armenia and the 1988 pogroms against Armenians in Azerbaijan. RESULTS: The two groups that had been exposed to severe trauma (earthquake or violence) had high initial and follow-up PTSD scores that did not remit over the 3-year interval. Overall, depressive symptoms subsided. Posttraumatic stress, anxiety, and depressive reactions were highly intercorrelated within and across both time intervals. No significant differences in PTSD severity, profile, or course were seen between subjects exposed to severe earthquake trauma versus those exposed to severe violence. CONCLUSIONS: After exposure to severe trauma, either an earthquake or violence, adults are at high risk of developing severe and chronic posttraumatic stress reactions that are associated with chronic anxiety and depressive reactions. Clinical evaluation and therapeutic intervention should include specific attention to these reactions. Early mental health intervention is recommended to prevent their chronicity.

A prospective study of posttraumatic stress and depressive reactions among treated and untreated adolescents 5 years after a catastrophic disaster.

OBJECTIVE: This study evaluated 1) the natural course of posttraumatic stress and depressive reactions among untreated adolescents from two cities in an earthquake zone (Gumri and Spitak) and one at the periphery (Yerevan) who were differentially exposed to the 1988 Spitak earthquake in Armenia and 2) the effectiveness of brief trauma/grief-focused psychotherapy among adolescents from Gumri. METHOD: One hundred twenty-five adolescents were assessed with the Child Posttraumatic Stress Disorder Reaction Index (CPTSD-RI) and the Depression Self-Rating Scale (DSRS) at 1.5 and 5 years postearthquake. At 1.5 years, trauma/grief-focused group and individual psychotherapy was provided over 6 weeks to a group of students in Gumri. RESULTS: CPTSD-RI scores among untreated adolescents from Gumri and Spitak subsided significantly but mildly at follow-up, with scores from Spitak, the city at the epicenter, remaining above the cutoff for a diagnosis of PTSD. DSRS scores increased mildly in both earthquake cities but only significantly in Gumri. Among treated adolescents in Gumri, improvement in CPTSD-RI scores was three times that of the untreated Gumri comparison group. The treated group also tended to improve on their DSRS scores, whereas these scores worsened significantly among untreated subjects. CONCLUSIONS: Untreated adolescents exposed to severe trauma are at risk for chronic PTSD and depressive symptoms. Brief trauma/grief-focused psychotherapy is effective in reducing PTSD symptoms and halting the progression of depression. This study supports the implementation of mental health intervention programs in schools after disasters to reduce trauma-related psychopathology.

Post-traumatic stress reactions in children after the 1988 Armenian earthquake.

One and a half years after the devastating earthquake in Armenia in 1988, 231 children from three cities at increasing distances from the epicentre were randomly screened in their schools to determine the frequency and severity of post-traumatic stress reactions, using the Children's Post-traumatic Stress Disorder Reaction Index (CPTSD-RI). A systematic clinical assessment of PTSD based on DSM-III-R criteria was also conducted on approximately half of this sample. A high CPTSD-RI score was strongly correlated with a clinical diagnosis of PTSD. A strong positive correlation was found between proximity to the epicentre and overall severity of post-traumatic stress reaction, as well as severity of core component symptoms of PTSD. High rates of chronic, severe post-traumatic stress reactions were found among children in the two most damaged cities, Spitak and Gumri. Analyses controlling for exposure revealed that girls reported more persistent fears than boys. These findings indicate that after catastrophic natural disaster, post-traumatic reactions in children may reach epidemic proportions, remain high for a prolonged period, and jeopardise the well-being of the child population of a large region. Systematic screening of children for PTSD can provide critical information for a rational public mental health programme after such a disaster.

Acute renal failure in the Armenian earthquake.

A destructive earthquake devastated northwestern Armenia on December 7, 1988. The size of the affected area (radius of 50 miles), the time of the day when it occurred (11:41 a.m.), deficiencies in the design and construction of buildings, and inadequate initial rescue and relief capabilities resulted in one of the most lethal and traumatic natural disasters of the century. A large but unknown number (estimated at 225 to 385) of the extricated victims who had sustained crush injury developed myoglobinuric acute renal failure requiring dialytic support. The limited number (8-10 dialysis machines) of antiquated dialysis facilities available locally were overwhelmed. International dialysis relief efforts resulted in meeting the immediate acute needs and provided the motivation and elements of the more efficient system for the future delivery of maintenance dialysis.

[Imported malaria in the Kabardino-Balkarian Republic].

Forty-three cases of imported vivax malaria were notified in the Republic of Kabardino-Balkaria in 1981-1997. These included 40 military men serving in Afghanistan, a citizen from this country, a student of the Kabardino-Balkaria State University, a student of a Nal'chik technological college, and a newcomer from Armenia. Among the patients, urban and rural residents were 27.9 and 72.1%, respectively. 37.2% were detected in the season of effective mosquito infection; new cases of malaria were identified in 41.9%, relapses were found in 58.1% of patients. Analyzing the reasons of late diagnosis of imported malaria suggests that the patients visited health facilities too late due to their poor awareness of a risk for malaria. Out of 43 patients, 62.8% referred to in the first 3 days after the onset, 18.6, 9.3, 9.3, and 2.3% did on days 4-6, 7-15, 16, and after a month, respectively. Their physicians made diagnostic errors in 53% of cases. Acute respiratory disease, influenza, pneumonia, and hepatitis were most commonly diagnosed. They were found in 27.9, 11.6, 4.7, and 4.7%, respectively.

[Variation of mini- and microsatellite DNA markers in populations of parthenogenetic rock lizard Darevskia rostombekovi].

Variation and clonal diversity in populations of the parthenogenetic rock lizard Darevskia rostombekovi was examined by means of multilocus DNA fingerprinting using mini- and microsatellite DNA markers M13, (GATA)4, and (TCC)50). The animals examined were shown to exhibit a clonally inherited, species-specific pattern of DNA markers (fingerprint profile) that is different from the species-specific patterns of parthenogenetic species D. dahli, D. armeniaca, and D. unisexualis. The mean intraspecific similarity index S was 0.950 (0.003) for a sample of 19 animals from three isolated populations of North Armenia. This significantly differed from the estimate of this parameter for a sample of 21 animals including two individuals from mountainous, relict population from the vicinity of the Sevan Lake, which was equal to 0.875 (0.001). A comparison of DNA fingerprints showed differences between 21 individuals attaining 79 DNA fragments of 1801 mini- and microsatellite markers included in the analysis. The results obtained show that intraspecific variation in D. rostombekovi is higher than that in the previously studied parthenogenetic species D. dahli (S = 0.962) and D. unisexualis (S = 0.950) (P < 0.001). Taking into account that D. rostombekovi is considered monoclonal on the basis of allozyme data, the problem of clonal variability is discussed with regard to the evidence on nuclear DNA markers. It is suggested that the hybrid karyotype of D. rostombekovi, which is more unstable than that of D. dahli and D. unisexualis, generates a series of chromosomal rearrangements (mutations). This may lead to the appearance of a geographically isolated chromosomal race (clone) in the population inhabiting the southeastern coast of the Sevan Lake.

[Prolonged crushing of the head (the biomechanics, clinical picture, diagnosis and treatment)].

Analysis of the victims of the earthquake in Armenia made it possible to distinguish a form of trauma, unique in biomechanics and pathogenesis, which was not described earlier, namely, prolonged compression of the head (PCH). Its clinical picture is characterized by superimposition and mutual aggravation of general organismic, general brain, cerebral and extracerebral focal symptomatology. Variants of the relations of the syndrome of prolonged compression of the soft tissues of the head and the craniocerebral damages proper are described. A detailed CT characterization of PCH is given. The principles of stage treatment of victims and the indications for compelled nonoperative and surgical tactics in PCH are suggested for the first time.

[Antigenic structure of saprophytic leptospirae and their classification].

The authors present the results of study of the serological properties of 46 strains of saprophytic leptospirae of different origin. On the basis of the affinity of the antigenic structure detected in the cross microagglutination reaction (MAR) 38 strains under study were united into 8 serological groups; the rest 8 strains were serologically independent in this reaction. The fact that 9 strains of water leptospirae isolated in the Armenian SSR belonged to one serological group was proved in the cross MAR and the test of aglutinin adsorption. This serological group was new and was named L. armenica. Five individual serological types of saprophytic leptospirae were differentiated in its composition. Comparative study of the serological interrelations between the group of strains isolated in Armenia and the strains of some serological groups and serological types the closest serological connections were noted in the K-1030 (serological group Armenica) and Bovedo (serological group Andamana) strains. It is believed that the existing division of the saprophytic leptospirae into two serological groups (Semaranga and Andamana) required widening and supplement by new serological groups and serological types.

[Alkalitolerant yeasts from natural biotopes].

Using a solid nutrient medium containing alkaline buffer (pH 10) and an antibiotic, alkalitolerant yeasts were isolated from samples of soda-rich saline soils (solonchaks) of Armenia (Arazdayan) and the Transbaikal Region (the Kungur Steppe). The species diversity of the yeast populations of the tested soda-rich soils was relatively insignificant. They only contained alkalitolerant representatives of asporogenic capsulated yeasts belonging to the species Cryptococcus laurentii, C. albidus, Rhodotorula glutinis, R. mucilaginosa, and Sporobolomyces roseus. C. laurentii representatives clearly dominated the isolates obtained, their number exceeding that of the other species by 2-3 orders of magnitude. All of the isolates grew on acidic wort agar, suggesting that they did not include obligate alkaliphiles.

Telemedicine: military applications.

Communications technology has enjoyed enormous growth in recent years and should be fully exploited by the medical community. Its application as part of disaster response was well demonstrated in the aftermath of the tragic earthquake in Soviet Armenia in 1988. Besides disaster response, telemedicine also has application for patient care, diagnostic imaging, and training and education. This capability would be particularly beneficial to the armed forces of many nations. If the communications equipment were portable, it could be well employed during peacetime, yet easily deployed to the battlefield. Therefore, armed forces should fully exploit telecommunications technology for the practice of military medicine.

[A reproductive study of women living in areas of intensive pesticide use].

The article contains the results of research in the productive function of women in the Ararat region of Armenia. The research technique was based on a questionnaire which included 81 items. The abnormal pregnancy rate in the areas of extensive use of chemical weed-killers and pest-killers reached 31.1% as compared with 22.0% in the areas of a moderate application of insecticides and pesticides. It was established that the percentage of abnormal pregnancies increased with consecutive pregnancies' growth. A comparison was made between 1-5 and 10th pregnancy terminations, in the result of which it was established that: consecutive pregnancies growth correlated with live-births' decrease, which apparently was a consequence of a greater number of therapeutic abortions, whereas the perinatal infant death rate also decreased, high infant death rate before and after the first year of life was revealed in first pregnancies and diminished in consecutive pregnancies, spontaneous and early abortions rates were stable irrespective of the number of consecutive pregnancies.

[Effects of acute psychogenic stress on the state of several functions of the visual analyzer].

Accommodation and binocular functions were examined after the earthquake of 1988 in 762 residents of Armenia who had not complained of their organ of vision before. Pseudo-myopia was detected in 30 percent of the examinees, concomitant strabismus in 4, and near-sight heterophoria in 4 percent. This permits a conclusion on the direct effect of an acute psychogenic stress during calamities on accommodation and binocular functions, particularly so in children.
