[Study on the regulative rule of reinforcing shen principle on sexual precocity and senescence at the molecular level].

The imbalance of Shen-yin and Shen-yang is a common pathological basis of sexual precocity and senescence. Senescence can be regarded as physiological Shen-deficiency, which is mainly the deficiency of Shen-Yang, while the essence of sexual precocity is hyperactivity of Fire due to Yin-deficiency, and deficiency of Shen-Yin is the predominance in the imbalance. The process of growth, development, maturing and aging led by Shen-qi, is mainly involving the hypothalamus-pituitary-growth hormone axis and sexual gland axis. Early initiation of these two axes appears in sexual premature children, manifesting syndrome of Fire hyperactivity due to Yin-deficiency, while in senescence, the decline of the axes appears ahead of time and manifests syndrome of Yang qi consumption. It has been shown in experimental and clinical researches that significantly good effects could be obtained by applying Chinese recipe for warmly reinforcing Shen-Yang to postpone senescence and recipe for nourishing Yin and removing Fire to inhibit sexual precocity. It also been proved by determining syndrome and speculating the damaged site according to the effects of drug that the pathologic damage of sexual precocity and senescence is sited on the hypothalamus or higher center.

[Function of pancreatic islet beta cells and features of TCM symptoms and syndromes in the non-diabetic first-grade relatives of patients with type 2 diabetes mellitus].

OBJECTIVE: To study the function of pancreatic islet beta cells, insulin resistance (IR) and features of TCM symptoms and syndromes in the non-diabetic first-grade relatives (ND1GR) of patients with type 2 diabetes mellitus (DM2). METHODS: A total of 68 ND1GR of DM2 patients were enrolled in the observed group and 45 healthy subjects with matched sex, age and body mass index (BMI) but without family history of DM were selected into the control group. Levels of fasting blood glucose (FBG), 2 hrs postprandial glucose (2hPG), fasting insulin (FINS) and 2 hrs postprandial insulin (2h INS) in all the subjects were measured to calculate and compare the IR and beta-cell function of the homeostatic model analog (HOMA-IR and HOMA-beta), and the insulin sensitive index (ISI). Moreover, the symptoms manifested in the ND1GR were also observed to analyze the features in them. RESULTS: FBG and FINS were obviously higher in the observed group than those in the control group (P < 0.01), while no significant difference was found in 2hPG or 2h INS (P > 0.05). HOMA-IR and HOMA-beta were significantly higher (P < 0.05) and ISI were significantly lower (P < 0.01) in the observed group than those in the control group. Compared with the control group, the main symptoms such as dark purplish tongue, listlessness, thready and thin pulse, lassitude in loin and legs in the observed group were seen more frequently. In the observed group syndrome of deficiency of Qi and Yin accounted for 51.47%, syndrome of deficiency of Yin for 30.88%, subjects with syndrome of blood stasis as the main accompanying syndrome accounts for 61.76%. CONCLUSION: Higher beta cell secretion function and lower insulin sensitivity appear in ND1GR of DM2 patients, suggesting the existence of insulin resistance. The feature of TCM syndrome in them is characterized by deficiency of Qi and Yin with inner obstruction of blood stasis.

[A preliminary study on hemodynamic changes of the heart-asthenia syndrome].

In this paper, hemodynamic changes of 254 subjects were observed by using impedance cardiogram in a double-blind control method. The subjects were divided into 5 groups: heart-asthenia syndrome (98 cases), lung-asthenia syndrome (23 cases), spleen-asthenia syndrome (23 cases), kidney-asthenia syndrome (10 cases) and normal group (100 cases). The results showed that hemodynamic abnormalities occurred in the patients with heart-asthenia syndrome only, the changed regularities were that deficiency of both vital energy and Yin greater than deficiency of heart-Yang greater than deficiency of heart-energy greater than deficiency of heart-Yin. When stroke volume (SV), cardiac output (CO), cardiac index (CI), stroke volume index (SVI), heather index (HI), vascular compliance (C) and ejection fraction (EF) were decreased and total peripheral resistance (TPR) and systolic time interval (STI) were increased, the signs and heart-asthenia syndrome could be found in clinical practice. In conclusion, the above parameters had certain diagnostic value for heart-asthenia syndrome and could be subjective quantitative indices for evaluating effects of treating the heart-asthenia syndrome.

[Preliminary study on pathophysiological basis of liver-blood deficiency syndrome].

By ways of HPLC and RIA, multiple laboratory parameters were determined simultaneously in 84 patients of iron--deficiency anemia (IDA) or chronic aplastic anemia (CAA) with Liver-Blood Deficiency Syndrome (LBDS) diagnosed by differentiation of TCM. The results showed that LBDS had some pathophysiological characteristics such as decreased function of sympathicus and adrenal medulla axis, increasing of L-EK, lower energy metabolism of erythrocyte, lower rate of body metabolism, sodium and water imbalance, and abnormalities in hemorrheology and erythrocyte deformability. It is suggested that the patients with LBDS are in a state of protective compensation in blood volume regulation.

[Simultaneously analysis of 15 parameters in the patients with the liver-blood deficiency syndrome].

OBJECTIVE: Using HPLC-ED or RIA, we determined simultaneously 15 indexes in 27 patients with the liver-blood deficiency syndrome (LDBD). By means of multivarivate hierarchical cluster analysis and selections of typical variate, the results showed that 15 indexes were classified into 5 groups, and the typical variates of each groups were NE, T3, TXB2, ALD and cGMP. It suggests that LBDS has some pathopysiological characteristics such as decreased functions of sympathetic nerve activation, lower T3 syndrome, imbalance of the active substance regulating cardiovascular function and metabolism of salt and water, and abnormalities second signal substance in cellular membrane.

[Combined use of factor analysis and cluster analysis in classification of traditional Chinese medical syndromes in patients with posthepatitic cirrhosis].

OBJECTIVE: To explore the significance of the combination of factor analysis and systematic cluster analysis in classification of traditional Chinese medical syndromes in patients with posthepatitic cirrhosis, and to provide a scientific basis for the criterion of the classification. METHODS: We designed a clinical questionnaire according to the clinical characteristics and the demands of traditional Chinese medical information collection for patients with posthepatitic cirrhosis. By means of clinical epidemiological research, with the four diagnosis methods for clinical information collection of traditional Chinese medicine, symptoms, physical signs, tongue conditions and pulse conditions in 310 patients with posthepatitic cirrhosis were collected, and the characteristics of traditional Chinese medical syndromes in these patients were explored with statistical methods, such as factor analysis, varimax and systematic cluster analysis. RESULTS: Analyzed by factor analysis and systematic cluster analysis with SPSS 11.0, the traditional Chinese medical syndromes in 287 of the 310 cases (92.58%) of posthepatitic cirrhosis could be classified. The syndromes could be divided into 7 categories, which were internal accumulation of damp-heat (55 cases), insufficiency of the spleen with overabundance of dampness (74 cases), accumulation of blood stasis plus deficiency of liver-yin and kidney-yin (73 cases), accumulation of blood stasis plus deficiency of both blood and qi (40 cases), deficiency of both blood and qi (16 cases), deficiency of yin and blood heat (6 cases) and stagnation of the liver-qi and deficiency of the spleen (23 cases). The traditional Chinese medical syndromes in the other 23 cases could not be classified. CONCLUSIONS: The clinical information collected with the four diagnostic methods of traditional Chinese medicine can be classified into different categories with the factor analysis and systematic cluster analysis. The factor analysis and systematic cluster analysis can reveal the characteristics and regularity of traditional Chinese medical syndromes in patients with posthepatitic cirrhosis in a way, and have value in researching the syndromes of traditional Chinese medicine.

A study on plasma norepinephrine and epinephrine levels in TCM liver syndromes.

The levels of plasma norepinephrine (NE) and epinephrine (E) were determined by HPLC in 220 patients with various TCM liver syndromes, and in 96 healthy subjects as controls. The plasma NE and E contents were higher in patients with liver excess syndromes, including Ganqi Yujie (GQYJ [symbol: see text]) syndrome, Ganyang Shangkang (GYSK [symbol: see text]) syndrome, Ganhuo Shangyan (GHSY [symbol: see text]) syndrome, and Ganyang Huafeng (GYHF [symbol: see text]) syndrome; while they were lower in patients with liver deficiency syndromes, including Ganqixu (GQX [symbol: see text]) syndrome, Ganxuexu (GXX [symbol: see text]) syndrome, and Ganyinxu (GYX [symbol: see text]) syndrome. Futhermore, when Ganshen Yinxu (GSYX [symbol: see text]) syndrome was turned into GYSK and then into GYHF, the plasma NE and E contents increased in order of precedence; while when GQYJ was turned into Ganyu Pixu (GYPX [symbol: see text]) syndrome, both NE and E contents decreased. The results suggest that the plasma NE and E contents are reliable objective parameters for the study of pathophysiological basis of the liver excess and liver deficiency syndromes, and the liver-kidney and liver-spleen interrelated syndromes.

[Long-term changes in the different types of coronary diseases treated with traditional Chinese and Western medicine].

64 cases with TCM differential types of CHD treated with TCM-WM in long term (an average of 10.5 years) were divided into four types according to deficiency of Yang [including weakness of Q1], deficiency of Yin, blood stasis and disorder of Qi and phlegm. Compared with pre-treatment 42 cases (65.62%) with TCM differential types showed improvement or no change in group I, II (clinical total effective rate 92.86%, the significantly effective rate 40.98%). While 22 cases (34.38%) with TCM differential type became aggravated or worse (clinical total effective rate 54.54%, the significantly effective rate 13.63%). The report indicated that the results in comparing group I, II with group III, IV were statistically significant; age (P less than 0.01), course of disease (P less than 0.01), impairment of myocardium (P less than 0.05), complication (P less than 0.05), ratio of deficiency of Yang (P less than 0.01), blood stasis (P less than 0.01) and high coagulation of blood (P less than 0.01). Therefore the author suggests that it is necessary to protect Yang-Qi and prevent and treat the complication of the CHD patients.

[Relation between treatment with traditional Chinese medicine for recurrent aphthous ulcer and human zinc and copper].

The purpose of the study is to explore the relation between general condition of patients with recurrent aphthous ulcer (RAU) and human zinc and copper with TCM conception of the organism as a whole, and adjustment effect of human body zinc and copper by TCM treatment. Results showed that the average value of serum zinc of 75 cases of RAU was on lower level within normal range, serum copper was normal, the rate of copper to zinc was higher than normal value. Analysis using the TCM theory showed serum zinc of patients of deficiency symptom-complex was lower than excessiveness symptom-complex, the rate of copper to zinc of patients of deficiency symptom-complex was higher than normal range. The zinc content of serum and the rate of copper to zinc were different in patients of various symptom-complexes of RAU. The zinc and copper contents of serum were adjusted, the rate of copper to zinc was normalized and the immune function of T-cell increased distinctly by TCM treatment according to an overall differentiation of symptoms and signs. Thus the therapeutic effect of TCM was better than zinc preparation.

[Effect of sanhuang jiangtang recipe on insulin peripheral resistance in type II diabetics].

OBJECTIVE: To observe the effect of Sanhuang Jiangtang recipe (SHJT) on insulin peripheral resistance in type II diabetics. METHODS: Ninety-five patients with type II diabetics were randomly divided into two groups. Fifty-three cases of SHJT group were given orally decoction and tablets of SHJT for 4-6 months. The efficacy was compared with that of 42 cases treated with Glipizide as the control. Before and after treatment standard steamed bread meal test was performed to measure the insulin peripheral sensitivity, insulin release to glucose and insulin sensitivity index. RESULTS: (1) The total effective rates of improving insulin peripheral resistance and reducing blood sugar in SHJT group were 79.2% and 80.1%, which was equivalent to levels in the control group, but SHJT recipe was more effective in relieving symptoms of Qi deficiency and signs of blood stasis. (2) In SHJT group, the insulin peripheral sensitivity and insulin sensitivity index were significantly increased (P < 0.05 and P < 0.01), meanwhile the fasting blood sugar and blood sugar area were reduced (P < 0.05), but the change of insulin release to glucose was blunted. (3) The lowering of blood sugar in SHJT group was significantly negative correlated with the changing of degree of insulin peripheral sensitivity and index of insulin sensitivity (P < 0.01 and P < 0.05), but not with that of insulin area. CONCLUSIONS: It suggested that the treatment of SHJT recipe might decrease insulin peripheral resistance (partial reversal) by means of reducing hyperinsulinemia and improving insulin sensitivity.

A survey comparing TCM diagnosis, health status and medical diagnosis in women undergoing assisted reproduction.

INTRODUCTION: For many women, undergoing assisted reproductive technology can be a difficult experience, and can result in changes in physical and emotional health and wellbeing. Recent research has suggested that acupuncture may be helpful for women undergoing assisted reproductive technology. To date, there is no information describing the traditional Chinese medicine (TCM) syndromes seen in women undergoing assisted reproductive technology, or relationships between TCM and Western medicine diagnoses. OBJECTIVE: To examine the health status of women undergoing assisted reproductive technology from both Western and Chinese medicine perspectives. METHODS: One hundred and eighty women were included in the study. All underwent a TCM diagnosis, 177 (98.3%) completed the quality of life questionnaire SF36. Information about Western medicine diagnosis was collected from case notes and was available for 176 (97.7%) women. RESULTS: Women in the trial reported poorer health on several domains of the SF36 compared with the South Australian population. The most common TCM diagnosis was 'Kidney Yang deficiency', diagnosed for 53.9% of patients. A TCM diagnosis of Qi or 'Blood stagnation' was associated with poorer quality of life on the mental health, emotional role function and social function domains of the SF36. No associations were found between TCM diagnosis and physical or general health components of the SF36. CONCLUSIONS: Associations between TCM and reproductive health diagnoses were demonstrated. Emotional health and wellbeing is an important aspect of patient care that needs to be addressed in clinical practice and research studies, as the findings suggest that this aspect of their health is often poorer during assisted reproductive technology.

[Therapeutic effect of Chinese herbal medicines for nourishing blood and reinforcing shen in treating patients with anovulatory sterility of shen-deficiency type and its influence on the hemodynamics in ovarian and uterine arteries].

OBJECTIVE: To observe the clinical therapeutic effect of Nuzhen Yunyu Decoction (NYD, a Chinese recipe for nourishing blood and reinforcing Shen) and its influence on the hemodynamic parameters in ovarian and uterine arteries. METHODS: Sixty-eight patients were randomly divided into two groups at the ratio of 2:1, 45 in the TCM group treated with Chinese medicine and 23 in the control group treated with clomiphene citrate. Menstrual cycle were taken as the indexes for clinical observation; basal body temperature, cervical mucus, vaginal exfoliative cells, serum hormones and B ultrasonic examination were taken as the indexes for ovulation and pregnancy assessment. The hemodynamic parameters of uterus and ovary were determined by using color Doppler ultrasonography, and the growth of follicles and ovulation, thickness of uterine endometrium were also monitored continuously. RESULTS: Results of 3 cycles treatment and 1 year follow-up study showed that the ovulation promoting rate, pregnancy rate and abortion rate in the two groups were similar, but the treated group showed better effects than that of control group, in such aspects as regulating menstruation, promoting growth and development of follicle, strengthening endometrium, improving blood supply and circulation of uterine and ovary (P < 0.05, P < 0.01). CONCLUSION: NYD has the effects of ovulation promotion and holistic regulation of reproductive system.

Type 2 diabetes in China: partnerships in education and research to evaluate new antidiabetic treatments.

There are 40 million people with diabetes in China, and the projected increase in the rates of obesity and premature cardiovascular disease is alarming. Most patients prefer to combine traditional Chinese medicine with Western medicine, but there is little or no information about the risks and benefits of this approach. Traditional Chinese medicine identifies three patterns of 'depletion-thirst' syndrome and therapy is aimed at reversing the deficiency in yin and qi, using a combination of products tailored to the symptoms and clinical features of individual patients. In Western medicine a number of new oral and injectable antidiabetic therapies are likely to enter routine clinical practice over the next 5 years, for example long-acting GLP-1 analogues, DPP-IV inhibitors and dual PPAR-alpha, PPAR-gamma agonists. To make best use of these agents in China and to promote diabetes education and health service development, there is a need for improved communication and collaboration between universities and hospitals both inside and outside China; and Western pharmacologists and clinicians need a better understanding of traditional Chinese medicine. There are several examples of institutional cooperation that should further diabetes research in China, for example the Beijing Chaoyang Diabetes Hospital linked with Imperial College, London, and the University of Nottingham, which has a new campus in Ningbo, south of Shanghai.

Analgesic effect of a herbal medicine for treatment of primary dysmenorrhea--a double-blind study.

We evaluated the analgesic effect of Toki-shakuyaku-san (TSS) in women who had a combination of "deficiency," of "Yin," "cold," and "stagnated blood" syndromes, and were suffering from dysmenorrhea. A diagnostic scoring system was used for determination of these conditions. We treated patients with either TSS or placebo during 2 menstrual cycles with a double-blind technique, and we followed them for 2 additional cycles. A significant alleviation of dysmenorrhea was observed in patients treated with TSS as compared to those treated with placebo. Our results suggest that TSS is effective for treatment of dysmenorrhea in patients with the above-mentioned conditions.
