Telescoping intramedullary rodding with Bailey-Dubow nails for recurrent pathologic fractures in children without osteogenesis imperfecta.

We reviewed four children with multiple, recurrent, pathologic fractures caused by bone disease, excluding osteogenesis imperfecta, and in whom multiple osteotomies with telescoping intramedullary stabilization had been performed. The diagnoses were myelomeningocele in two children and Rett syndrome and fibrous dysplasia in one child each. Five long bones were stabilized; four femora and one tibia. The average age at surgery was 6 years 6 months; the average number of fractures that had occurred in the involved bone before surgery was 3.0 per bone. At an average follow-up of 2 years 4 months, the average number of fractures that had occurred after surgery in the stabilized bones was 0.2 per bone. There were no infections, nonunions, or rod complications. We believe that in the carefully selected patient, multiple osteotomies with intramedullary telescoping rod stabilization can be quite successful in solving the problem of recurrent fractures in children with diseases other than osteogenesis imperfecta.

[Telescopic adhesive anastomosis of small blood vessel applied in formation of arteriovenous fistula for hemodialysis].

The formation of an arteriovenous fistual for dialysis by routine interrupted sutures anastomosing the vein and artery is difficult to perform and time-consuming. A new method, telescopic adhesive anastomosis was studied and applied in 10 hemodialysis patients, who were in need of an arteriovenous fistula. The external diameter of the vessels anastomosed was 2.40 +/- 0.20 mm (radial artery) or 2.40 +/- 0.35 mm (cephalic vein). After thorough debridement of the vascular ends, the arterial end was put in the venous lumen. In order to fix the telescopic vessels, two stitches were applied 180 degrees apart from each other and tied. Each stitch was inserted from vein (penetrating the whole wall) to artery (just through the adventitia and partial thickness of the media vasorum). The distance from the stitch to the edge of the vein was 0.5 mm, and that of the artery was approximated to the external diameter of the vessle. The medical adhesive was then applied for sealing the anastomotic adventitia. Ten seconds were given for the solidification of the adhesive. The patients were followed up for 8 months. The patency rate was 100%, and the rate of blood flow was more than 300 ml/min (measured by ultrasonography). It was shown that this method could be managed easily and quickly, and the so-formed fistula would fulfill the need of hemodialysis.

Advanced biomaterials used for a new telescopic retainer for removable dentures.

Telescopic retainers with conical ceramic abutment crowns and electroplated gold copings define a new retainer for removable dentures exhibiting favorable tribological properties that could offer clinical advantages. The objective of this study was to evaluate the clinical effects of this retainer. To do so, a novel treatment modality was developed based on intraoral bonding of the copings to the framework in order to be able to realize, in a clinical setting, the retainer function based on a precision fit. A total of 32 patients wearing 33 dentures (16 mandibular, 17 maxillary; period of risk [months]: minimum = 3; maximum = 58; mean = 27.6), supported by 147 abutments (83 natural teeth, 64 implants) with ceramic abutment crowns (IPS Empress 1, IPS Empress 2, InCeram, Procera, CerAdapt; CeraBase) were followed at 6-month intervals. The ceramic abutment crowns showed low plaque accumulation (mean PI = 17.3%). The gingival tissues around natural abutments were generally free of inflammation; so was the mucosa around the implants (mean SBI = 4.9%). Denture adhesion and occlusion did not change, and there were no rocking movements. The mobility (as determined by Periotest) of 29 teeth was reduced in a highly significant manner within 6 months (t test, p < 0.001). Osseointegration was preserved for all implants. One abutment loosened. Six Empress 1 ceramic copings failed (94.6% Kaplan-Meier survival rate), and one abutment tooth was lost (99.2% survival rate). All patients reported problem-free handling, no rocking movements, and constant adhesion. The adhesive strength of one denture was too low at insertion. 94% of the patients experienced no problems with oral hygiene. The retainer examined has relevant clinical advantages and meets geriatric requirements for removable dentures.

Extreme coronary guide catheter support: report of two cases of a novel telescopic guide catheter system.

Extra support of the guiding catheter is required in selected cases of percutaneous coronary intervention (PCI). We describe two successful cases of PCI of very calcified and tortuous right coronary arteries in which a novel telescopic guide system was devised. The system utilizes a long sheath that "armors" the guide catheter. The operator can adjust the support of the guiding system from soft up to extremely stiff.

Telescopic anastomosis of the common bile duct in the rat.

A telescopic technique for the end-to-end anastomosis of the common bile duct in the rat is proposed as a way of reducing the number of sutures without prejudicing the impermeability of the union. Serial histological examinations of the scar process after 10, 20, and 30 days showed that patency was maintained.

Long-term histologic results of vein grafting with the telescoping anastomotic technique.

We studied the long-term histologic results of a new method for autogenous vein grafting to examine whether stenosis at the anastomosis is maintained over time. Nineteen rat inferior epigastric veins were grafted into the femoral artery using a telescoping sleeve technique at both the proximal and the distal anastomoses. Specimens were studied macroscopically and histologically three months later. Stenosis at the anastomosis was located near the tip of the inserted vessel. The smallest inner diameters of the proximal and distal anastomoses were about 80% of the corresponding femoral artery diameter; no statistically significant difference was found between the two anastomoses. The grafts had a thickened wall due to intimal hypertrophy and fibrosis of the media. The inner diameter of the graft was, however, about twice that of the femoral artery, and these graft changes did not create any apparent constriction within the graft.

Single-stage cluster sampling with a telescopic respondent rule: a variation motivated by a survey of dementia in elderly residents of Shanghai.

In this report, we consider the situation in which one wishes to identify a cohort of a specified number of individuals within each of several domains for future follow-up studies based on a single-stage cluster sampling design. We develop sample size formulae relevant to this situation and introduce a variation of single-stage cluster sampling that seems more suitable in this situation than is ordinary single-stage cluster sampling. The basis for this variation is the concept that the definition of eligible respondents is not the same for all clusters. The use of this modified respondent rule (which we call telescopic) enables one to meet specified sample sizes in all domains of interest without the need to sample extra individuals in some domains. We used a version of this sampling design successfully in the field with a survey of elderly persons conducted in Shanghai, People's Republic of China.

Protected telescopic catheter also allows anaerobic bacteria isolation in patients with ventilatory-acquired pneumonia.

OBJECTIVE: To analyze the possibility of isolating anaerobic bacteria using protected telescopic catheter (PTC) in patients with ventilatory-associated pneumonia. DESIGN AND SETTING: A prospective epidemiological study in a 12-bed intensive care unit. PATIENTS: 104 patients with suspected ventilatory-associated pneumonia undergoing PTC for microbiological determination including specific methods for anaerobic bacteria identification (mean age 59.3, range 17-82; SAPS II 44.6+/-17.9). MEASUREMENTS AND RESULTS: We performed 156 PTC procedures. Of the 14 cases ventilatory-associated pneumonia occurred early in 6 and in 8 late. In 52 samples at least one bacteria was isolated (33.3%). Of the 156 samples 14 yielded with anaerobic bacteria, representing 26.9% of the positive samples and 9% of all the samples. CONCLUSION: Considering the potential pathogenic role of anaerobic bacteria in patients with ventilatory-associated pneumonia, the possibility of isolating these bacteria using PTC and taking into account its easiness and limited side effects should be underlined.

A modified Hohmann method for hallux valgus and telescoping osteotomy for lesser toe deformities in patients with rheumatoid arthritis.

To preserve the function of metatarsophalangeal joints and to ensure forefoot stability in patients with rheumatoid arthritis (RA), we performed a modified Hohmann method for hallux valgus (HV) and telescoping osteotomy of lesser toe deformities instead of fusion of HV or resection of all metatarsal heads. From October 1995 through March 2001, 47 RA patients (90 feet) with severe HV and forefoot deformities were examined. The indication for the procedure in all the patients was disabling foot pain secondary to intractable plantar callosities below the lesser metatarsal heads, painful HV deformities, and the severe deviation of the sesamoid complex diagnosed by the basis of X-ray images. The HV and intermetatarsal (M1M2 and M1M5) angles and sesamoid complex were measured on the preoperative and postoperative roentgenograms. According to the results of a questionnaire survey, the patients were divided into three groups using the visual analogue scale; group 1: satisfied, group 2: fair and or no pain, group 3: dissatisfied. HV and M1M2 angles significantly improved compared between pre- and postoperative or preoperative and the follow-up periods. Out of the 47 patients, 78.9% were satisfied with the results of the operation and 8.9% were dissatisfied. Of these patients, 12.2% reported fair results. There were several complications, such as painful callosity, which was recurrent in seven feet, and delayed wound healing was observed in two out of 90 feet. A modified Hohmann method and abductor hallucis correction are effective in relieving pain and ensuring the bony union of the great toe in spite of severe osteoporosis.

Novel steerable telescoping catheter system for implantation of left ventricular pacing leads.

Advances in left ventricular transvenous lead delivery systems for biventricular pacing are leading to more refined techniques, shorter procedure times and higher implant success rates. Despite these advances, the inability to successfully cannulate the coronary sinus and deliver a lead to a distal location are still major causes of prolonged procedures times and implant failures. The pathophysiologic process of heart failure results in dilatation of the right atrium as well as other morphological changes in cardiac anatomy. Additionally, cannulation can be further complicated by congenital anomalous cardiac anatomy. This report describes the implant of a biventricular pacing system using a novel, steerable 7 French catheter system developed to aid in the cannulation of the coronary sinus ostium and its venous branches. The steerable catheter is used in conjunction with a 9 French braided sheath and guide-wire to create a telescoping system. The use of new tools and methods as described provides insight into available options for left ventricular transvenous lead implantation and dealing with difficult anatomy.

Introduction of loop sutures in microsurgical telescoping anastomosis.

We have designed a loop suture technique for use in microsurgical telescoping (sleeve) anastomosis to prevent problems from the free floating inserted vessel stump. The patency rate of rat femoral arteries repaired with this technique was 95% and that of interpositional vein grafts, in which both the proximal and distal anastomoses were completed with the same sleeve technique, was 81.0%.

Tooth-supported telescope restorations.

Entirely tooth-supported removable dentures with telescope crowns may be considered a good alternative to a fixed restoration of the whole dental arch. They provide a rigid splinting and stable occlusion and offer a better safety factor owing to their removability and their lesser dependence on each single abutment. Also they are applicable when parallelism among the abutments does not permit making a fixed restoration. Furthermore they can be cemented in stages, decreasing the risk of washout later. Psychologically, they are well tolerated by patients.

Post soldering a telescopic pontic.

A technique has been presented that involves the use of a telescopic ceramometal pontic soldered to a type III alloy substructure (Figs. 4 and 5). This technique eliminates the need for manufactured facings or the use of processed acrylic resin, but it incorporates the advantages of porcelain esthetics and wear with the increased ability to burnish the retainer margins.

Changes in retention of various telescope crown assemblies over long-term use.

An investigation was done on the effect of crown height, angle (one-half taper) and number of removals on the retention of telescope crowns. Assemblies of inner and outer crowns were constructed on master dies. Crown heights of 4 mm, 5 mm, and 6 mm were used. Cone angles of 0, 2, 4, and 6 degrees were used for each of the three heights. The inner crowns were fixed to the dies with an adhesive agent. the outer crowns were set over them and placed in a clamp. Separation forces were tested with an Instron machine (Instron Corp., Canton, Mass.). They were measured initially and after numerous 100 insertion/separation cycles up to 1000 cycles and numerous 500 insertion/separation cycles up to 10,000 cycles. The results demonstrated that the cone angle of the inner crown was more important than its height for maximum retention. Retention is rapidly lost after the cone angle exceeds 2 degrees. It was also shown that an accessory device is necessary to maintain retention capabilities after long-term use.

Using the Very Large Array (VLA) and other radio telescopes to perform a parasitic Search for Extraterrestrial Intelligence (SETI).

This paper describes several attempts to utilize various radio telescopes in a manner that we term "parasitic," that is in a manner that does not interrupt or seriously impact the standard astronomical observing programs in progress at the radio observatories. In the extreme case, only recorded astronomical data are accessed off-line, after the fact, without any burden on the observatory at all.

Survey of Earth orbital telescopes and their potential for exobiology.

A series of Workshops on Exobiology in Earth Orbit held at NASA Ames Research Center has recently concluded. The draft of the final report from these Workshops contains a prioritized list of telescopic observations (possible only from above the Earth's atmosphere) that relate to the origin and evolution of the biogenic elements and compounds from their nucleosynthetic creation within stars to their inclusion in living systems. These orbital observations and the ground based laboratory and theoretical research necessary to support them have been termed Observational Exobiology. The details available on spacecraft, platforms and instrumentation most likely to be launched in the near future by the U.S. and Europe were considered in the Workshops. The purpose was to determine what observational programs would be tractible and what area of interest to exobiology required hardware and/or mission capabilities not yet envisioned. This paper summarizes the exciting opportunities that exist for Observational Exobiology.

A prospective multicenter clinical trial to evaluate the safety and effectiveness of the implantable miniature telescope.

PURPOSE: To evaluate the safety and preliminary efficacy of a novel visual prosthetic device, the Implantable Miniature Telescope, IMT (by Dr Isaac Lipshitz) (IMT), in a phase I trial in patients with significant bilateral central vision impairment from late-stage age-related macular degeneration (AMD). The IMT is designed to reduce the relative size of the scotoma by rendering enlarged (threefold) central visual field images over the central and peripheral retina. DESIGN: Prospective, multicenter, open-label clinical trial. METHODS: In this prospective, multicenter phase I trial, 14 patients aged 60 or older with bilateral geographic atrophy or disciform scar AMD, cataract, and best-corrected visual acuity (BCVA) between 20/80 and 20/400 had an IMT implanted in one eye. Distance and near BCVA, endothelial cell density, and quality of life, measured as activities of daily life (ADL), were evaluated preoperatively and postoperatively. RESULTS: At 12 months, 10 (77%) of 13 patients gained 2 more lines of either distance or near BCVA, and eight (62%) of 13 patients gained 3 or more lines in either distance or near BCVA. Mean endothelial cell density decreased by 13%. All adverse events resolved without sequelae. ADL scores improved in the majority of patients. CONCLUSION: The results of this phase I trial support further evaluation of the IMT in a larger study population with late-stage AMD. A phase II/III trial is in progress.
