Soil arsenic availability and the transfer of soil arsenic to crops in suburban areas in Fujian Province, southeast China.

The bioavailability, soil-to-plant transfer and associated health risks of arsenic in soils collected from paddy rice fields and vegetable fields in suburban areas of some major cities of Fujian Province were investigated. The total soil concentrations of arsenic ranged from 1.29 to 25.28 mg kg(-)(1) with a mean of 6.09 mg kg(-)(1). Available (NaH(2)PO(4)-extractable) arsenic content accounted for 0.7-38.2% of total soil arsenic and was significantly correlated with total soil arsenic content. For the vegetable soils, the available fraction (ratio of available As to total As) of arsenic decreased with decreasing silt (particle size 0.02-0.002 mm) and free iron (DCB extractable) contents and with increasing soil pH and organic matter content. The available fraction of arsenic in the paddy rice soils increased with increasing free iron and organic matter contents and decreasing soil pH and silt content. The correlation of NaH(2)PO(4)-extractable arsenic with the arsenic concentration of the vegetables was much better than that of total As. The transfer factor based on the soil available arsenic (TF(avail)) was chosen to compare the accumulation ability of the various crops. The TF(avail) values of rice grains (air-dried weight basis) ranged between 0.068 and 0.44 and were higher than those of the vegetables, ranging from 0.001 to 0.12. The accumulation ability of the crops decreased in the order of rice>radish>water spinach>celery>onion>taro>leaf mustard>fragrant-flowered garlic>pakchoi>Chinese cabbage>lettuce>garlic>cowpea>cauliflower>bottle gourd>towel gourd>eggplant. Daily consumption of rice and other As-rich vegetables could result in an excessive intake of arsenic, based on the provisional tolerable intake for adults for arsenic recommended by WHO.

An evaluation of a suburban railway pedestrian crossing safety programme.

This study evaluated a programme of educational and environmental (access prevention) interventions designed to reduce the incidence of illegal and unsafe crossing of the rail corridor at a suburban station in Auckland, New Zealand. Immediately after the programme of interventions, the proportion of those crossing the rail corridor by walking across the tracks directly rather than using the nearby overbridge had decreased substantially. Three months later, the decrease was even greater. However, the educational and environmental interventions were introduced simultaneously so that the effects of each could not be separated; nor could other unmeasured factors be ruled out. Anonymous surveys administered immediately before and 3 months after the interventions indicated that while awareness of the illegality of walking across the tracks had increased slightly, perception of risk had not changed. This suggests that the educational interventions may have had less effect than the access prevention measures.

Assessment, re-design and evaluation of changes to the driver's cab in a suburban electric train.

A new type of electric train, called the Tangara, began replacing older trains on the Sydney city and suburban network in 1990. Shortly afterwards, some of the train drivers began reporting pain in the arms while driving the new train. The Ergonomics Unit of Worksafe Australia was then engaged to identify and assess ergonomic problems in the driver's cab. This process included direct observation of drivers at work, distribution of a self-administered questionnaire to all drivers, and analysis of anthropometric problems using a computer-aided design package. The analysis of 193 completed questionnaires and the problems shown by the design study were used in developing an improved design. A mock-up of the modified cab was made and tried out by 134 drivers whose comments led to further changes. The modified design was applied to new production, and existing cabs were modified during major maintenance. By 1996, 96% of the driver's cabs were to the new design. An evaluation of the new design was undertaken using written questionnaires, which were completed by 227 drivers. The results were strongly in favour of the new design, showing the effectiveness of the ergonomic modifications.

Cervicovaginal human papillomavirus infection in suburban adolescents and young adults.

To investigate the prevalence rates of and risk factors for genital human papillomavirus (HPV) infection in white, middle-class adolescents, we obtained specimens for HPV from 107 patients undergoing pelvic examination at a suburban adolescent health service. Specimens were obtained by means of cervicovaginal lavage and were analyzed for HPV DNA by Southern blot analysis. The subjects' mean age was 18.5 (+/- 2.0 SD) years; 47% of the subjects were seen for contraception, 36% for possible pregnancy, 12% for a gynecologic complaint, and 5% for a general examination. We detected HPV in 32% of patients; HPV types included 6/11 (6%), 16 (3%), 18 (15%), 31 (3%), 42 (6%), 45 (18%), and 56 (6%). The DNA type was classified as uncharacterized in 47%, and 3% had more than one type. Univariate analysis revealed that patients found to have HPV were more likely to have more than two sexual partners, to have been sexually active for more than 2 years, to have had menarche before 12 years of age, and to have a history of sexually transmitted disease; logistic regression multivariate analysis demonstrated that number of partners and age at menarche were independent risk factors for HPV. These data indicate that HPV is common among sexually active teenagers regardless of ethnicity or socioeconomic status. The association of cervicovaginal HPV infection with sexual activity further supports the notion of the sexual transmission of this agent.

Recent water quality trends in the Schuylkill River, Pennsylvania, USA: a preliminary assessment of the relative influences of climate, river discharge and suburban development.

Climate, flow rate and land use are all known drivers of water quality in river systems, but determining the relative influences of these factors remains a significant challenge for aquatic science and management. Long-term data from the Schuylkill River at Philadelphia is assessed here in an attempt to ascertain the separate and combined influence of these major drivers on water quality in a developed watershed. Water quality measures including nutrients, conservative solutes and bacteria all elicited distinct seasonal patterns driven primarily by river discharge. Mass transport rates of sodium and chloride have increased with time, and were elevated in winter, presumably as a function of road salt deposition. A steady increase in developed land area in the watershed has occurred in recent decades, which allowed the use of time as a surrogate parameter for regional development in the construction of multiple factor linear models predicting the relative influences of precipitation, river discharge and developed land area on river water quality. Linear models predicting annually averaged water quality measures showed the effects of precipitation, discharge and developed land area to be of nearly equal importance in regulating levels of conductivity, alkalinity, sodium, and chloride in the river. Models predicting water quality variables for discrete samples demonstrated that river flow was the major determinant of daily variability in alkalinity, conductivity, hardness and calcium levels, while still resolving the highly significant influence of watershed development on water quality. Increases in solute transport in the Schuylkill River in recent decades appear to be the direct result of modern suburban development in the watershed.

Comparison of particulate mass, chemical species for urban, suburban and rural areas in central Taiwan, Taichung.

Aerosol samples for PM2.5, PM(2.5-10) and TSP were collected from June to September 1998 and from February to March 1999 in central Taiwan. Ion chromatography was used to analyze the acidic anions: sulfate, nitrate and chloride in the Universal samples. The ratios of fine particle concentrations to coarse particle concentrations displayed that the fine particle concentrations are almost greater than that of coarse particle concentrations in Taichung area. The average concentrations of PM2.5, PM(2.5-10) and TSP in urban sites are higher than in suburban and rural sites at both daytime and night-time. Chloride dominated in the coarse mode in daytime and in fine mode in night-time. Nitrate can be found in both the coarse and fine modes. Sulfate dominated in fine mode in both daytime and night-time.

Characterization of visibility and atmospheric aerosols in urban, suburban, and remote areas.

Visibility data from over the past four decades accumulated from urban areas of central Taiwan indicated that air pollutants have significantly degraded visibility in recent years. Currently, the annual average visibility in urban areas of the same region is approximately 8-10 km, while the visibility in remote areas is approximately 25-30 km. To understand how aerosols affect the visibility in this region, here we selected three sites in central Taiwan to measure the soluble ionic and carbonaceous species of PM(2.5) and PM(2.5-10) during 1997-1998. A MOUDI cascade impactor was used to measure the size distributions of atmospheric sulfate, nitrate, and carbonaceous particles. The aerosol data were then analyzed together with meteorological and air quality data. Comparing the results obtained from urban, coastal suburban and remote sites revealed that sulfate, carbonaceous species and local wind speed significantly affected the visibility in the urban area. However, sulfate concentration and humidity influenced visibility in the coastal area of central Taiwan. The particulate concentration at the remote station was roughly one-fifth of that in the city. Regression analysis results indicated that humidity is a dominant factor affecting remote visibility.

Influence of anthropogenic sources on total gaseous mercury variability in Grenoble suburban air (France).

Total gaseous mercury (TGM) has been monitored at Champ sur Drac, a suburban site of Grenoble in southern east France. TGM measurements have been made over 4 periods of approximately 10 days throughout 1999-2000 using cold vapour atomic fluorescence absorption technique. The first monitoring campaign was initiated on November 4, 1999, followed by three other campaigns respectively on January 12, 2000, April 10, 2000 and July 17, 2000. Concurrent monitoring of O3, NO, NO2, SO2 and of meteorological parameters have also been performed. The mean TGM concentration was 3.4 ng m(-3) with maximum hourly mean concentration of 37.1 ng m(-3). Although mean TGM concentration was not greatly different from those previously measured in the troposphere, the greater TGM variability as well as the occurrence of high TGM concentration linked to particular wind conditions suggested the strong influence of anthropogenic sources. The chlor-alkali plant located nearby, the others chemical industries using fuel combustion and the municipal waste incinerator were thought to contribute to mercury pollution events.

Influence of country of origin on prevalence of hepatitis B markers among employees in a small suburban hospital.

Hepatitis risk in a small suburban hospital was evaluated by laboratory screening of all hospital employees for hepatitis immunologic markers. Excluding four U.S.-born subjects who had received hepatitis vaccination, the laboratory screening revealed 21% of those tested had some hepatitis B antigen or antibody. Positive markers were found in 46% of foreign-born compared to 10% of native-born (P less than 0.001). Foreign-born participants accounted for 68% of the positive marker cases while representing only 31% of the employee population. The country of origin appeared to have a statistically significant greater importance as a determinant of hepatitis B status than occupational area in this particular hospital setting.

Seroepidemiology of group A rotavirus in suburban São Paulo, Brazil.

Age-specific patterns of rotavirus infection were investigated using a randomly selected and representative sample of sera from a suburban community of São Paulo, Brazil screened for class-specific antibodies to group A rotavirus. Age-serology of anti-rotavirus IgG showed primary infection predominant in young infants with a median age of around 18 months consistent with IgM serology suggesting highest rates of recent infection between ages 4 and 48 months. Anti-rotavirus serum IgA prevalence increased gradually with age. Paired samples from infants, collected 1 month apart, indicated high exposure rates with seroconversion occurring in several infants during the reported low transmission season. Between 5 and 10% of adults had elevated IgM levels indicative of recent infection and, potentially, of an important contribution adults may play to rotavirus transmission. Further understanding of the dynamics of rotavirus transmission within populations, at group and serotype level, would benefit the design and monitoring of future immunization programmes.

An analysis of invasive airway management in a suburban emergency medical services system.

INTRODUCTION: Airway management is the most critical and potentially life-saving intervention performed by emergency medical service (EMS) providers. Invasive airway management often is required in non-cardiac-arrest patients who are combative or otherwise uncooperative. The success of prehospital invasive airway management in this patient population was evaluated. METHODS: A retrospective review was undertaken of the records of all such patients requiring endotracheal intubation over a three-year period (1987-1989). The study population included 278 patients enrolled by five advanced life support (ALS) units serving a suburban population of 425,000. Field trip sheets were reviewed for diagnosis, intubation method and success, number of intubation attempts, provider experience, reasons for unsuccessful intubations, and complications. RESULTS: A total of 394 invasive airway management attempts were performed on 278 patients. The overall successful intubation rate was 75% (41% orotracheal, 52% nasotracheal, 7% other or unknown). The most common diagnoses were COPD and pulmonary edema (30%) and trauma (24%). Experienced providers were successful on the first attempt in 57% of cases compared to 50% by inexperienced providers (p=.24). Multiple intubation attempts were required in 33% of the patients. There was no statistically significant difference in success rates between the orotracheal and nasotracheal methods (p=.51). The most common reason for unsuccessful intubation was altered level of consciousness. Complications occurred with 7% of successful attempts and in 18% of unsuccessful attempts (p less than .001). Forty-six percent of the patients who were not intubated successfully in the field and required intubation in the emergency department (ED) received a neuromuscular blocking agent prior to successful intubation. CONCLUSION: Prehospital providers can intubate a high but improvable proportion of non-cardiac-arrested patients by both the orotracheal and nasotracheal routes. The use of pharmacologic adjuncts to facilitate the prehospital intubation of selected, non-cardiac-arrested patients is a promising adjunct that needs further evaluation.

Smoke alarm ownership and installation: a comparison of a rural and a suburban community in Georgia.

As part of a smoke alarm giveaway and installation program (The Get-Alarmed Campaign), a total of 454 households were surveyed in two counties in Georgia, one metropolitan and one nonmetropolitan. The targeted communities in these counties had a high prevalence of low-income and minority populations and thus were at high risk of house fire-related morbidity and mortality. The objectives of the program were to determine the prevalence of and predictors for installed, functioning smoke alarms, and to install at least one smoke alarm and/or smoke alarm batteries in 100% of participating homes in need. Characteristics associated with smoke alarm ownership included home ownership, having a higher income, and having a central heating source, factors which should be considered in targeting future intervention strategies. At onset, 159/454 households (35.0%) had no smoke alarms installed and 56/275 households with existing smoke alarms (20.4%) had none that were functional. Regardless of ownership status, a free smoke alarm was installed in the household of 93.8% of participants and new batteries were installed in existing smoke alarms for 31.7% of participants. This project illustrates the usefulness of a door-to-door campaign in increasing smoke alarm ownership in both a rural and a suburban community with a high concentration of residents at high risk of house fire-related morbidity and mortality.

Orthodontic care in suburban Cuyahoga County, Ohio: who provides treatment and whom do they treat?

It has been reported that orthodontic services are being provided to a larger segment of the population by an increasing number of providers. The present study surveyed the dental and orthodontic experiences of 10th grade students attending 16 public and two parochial high schools in suburban Cuyahoga County, Ohio, as well as two schools from the city of Cleveland. Questionnaires were distributed in the classroom and data obtained for 2808 students. Approximately 50% of the sample were girls with an average age of 15.5 +/- 0.8 years. Results of the survey revealed that 84% (2371/2808) had seen a dentist within the past year, and 37% (1047/2808) of the students had received orthodontic treatment from 171 different providers. Of those treated, 87.2% (913/1047) were treated by a specialist in orthodontics, 10.8% (114/1047) by a general dentist, and 0.7% (7/1047) by a pediatric dentist, with 1.3% missing or unknown (13/1047). Patients who had seen a dentist within the past year were more likely to have had orthodontic treatment. Only 7% of the untreated students were told by a dental professional that they needed braces compared with 71% of the treated group. Therefore, we conclude that orthodontic specialists provide most of the orthodontic services in the suburbs of Cuyahoga County, and visiting a general dentist positively influences the utilization of orthodontic services.

Infection with group C rotavirus in a suburban community in Brazil.

Group C rotaviruses are associated with sporadic outbreaks of gastroenteritis worldwide. Age-specific seroprevalence of group C rotavirus antibodies was investigated in sera, randomly collected and representative of a suburban community in Brazil which had previously been screened for group A rotavirus antibodies. Antibody prevalence to group C rotavirus was low in children under 5 years and increased slowly with age to 36% seropositivity in adults, reflecting continuous exposure to primary infection in all age groups. This suggests a higher incidence of infection than disease might predict. Adult antibody prevalence was similar to that in other geographical settings. No obvious patterns of infection with group A and group C rotavirus were found within individuals, which suggests independent transmission. However, further epidemiological studies are required to understand group C rotavirus dynamics and possible interactions with group A rotavirus transmission and immunity.

Skin cancer awareness in suburban employees: a Hispanic perspective.

Skin cancer is the most common form of cancer in the United States and accounts for an estimated 1 million new cases per year. In the white population, the rate of new melanomas diagnosed yearly has more than doubled from 5/100,000 in 1973 to 12/100,000 in 1990. The current lifetime risks for white persons are estimated to be 28% to 33% for basal cell carcinoma and 7% to 11% for squamous cell carcinoma. Skin cancer is more common in sun-sensitive, fair-skinned individuals but can occur in any ethnic group, especially with exposure in tropical or sunny climates. Little is known about awareness of skin cancer, risk perception, and performance of skin self-examination (SSE) by people with skin that rarely burns. Our purpose was to evaluate skin cancer awareness, perceptions of skin cancer risk, and performance of SSE in a Hispanic versus non-Hispanic white population with similar access to health care and promotion. One hundred forty people employed by a suburban city voluntarily attended a free skin cancer screening and completed a self-administered survey. Hispanic individuals reported decreased skin sensitivity and tendency to burn. Non-Hispanic individuals believed they were at greater than average risk for skin cancer, and most Hispanics believed they were at average or below average risk. None of the 27 Hispanics reported ever being taught SSE. More non-Hispanics (32%) had performed SSE within the last year than Hispanics (15%). Regular SSE was associated with a history of skin cancer. Multimedia messages usually link skin cancer warning signs and early detection strategies with having sun-sensitive skin. People without sun sensitivity did not perceive themselves as being at risk, did not learn the warning signs of skin cancer, and did not perform SSE. Awareness of melanoma and non-melanoma skin cancer and perception of risk among Hispanics were less than among non-Hispanics, which may contribute to presentation for care at an advanced stage.

Air pollution impact of a suburban highway segment.

Design traffic volumes, vehicle emission factors and meterological data are used to assess the impact of a suburban limited-access highway segment. Mesoscale effects are determined by emission inventory techniques. Microscale effects are determined using a pollutant dispersion model developed by the California Division of Highways. The air pollution impact of the highway segment is small, and should not be considered a significant criteria for decisions concerning construction of the highway.
