Experimental suture studies in strabismus surgery. II. Comparison of tensile strength of plain catgut with polyglycolic acid (Dexon) sutures after extraocular muscle surgery.

From our experimental studies it appears Dexon satisfies more of the criteria for an ideal suture for strabismus surgery than plain catgut for the following reasons. Its handling, tying, and knot-holding (if tighthened firmly) qualities are excellent. Tissue reaction is minimal. It seems to be non-antigenic. Of paramount importance. Dexon's tensile strength is high initially and it retains its strength through the critical period of muscle-scleral wound healing, then dissolves in a rapid uniform manner.

Transscleral suture fixation of posterior chamber intraocular lenses through the ciliary sulcus: endoscopic comparison of different suture techniques.

Two technical difficulties exist in transscleral suture fixation of posterior chamber intraocular lenses (PCL) in the ciliary sulcus: first, exact needle penetration of the sulcus and second, exact positioning of the PCL haptics in the sulcus. We used, for the first time, intraocular endoscopy to visualize the site of needle penetration and the final location of the haptics in patients. It turned out that with our previously described standard techniques, precision was far less than anticipated. Thus, new technical ways had to be sought to improve the precision of positioning. In secondary implantation, we achieved best results when the needle was passed from the outside before opening the eyeball, taking advantage of a precisely prepared blue-white line. Passing the needle in an already hypotonic eyeball gives much less precise results. In combination with perforating keratoplasty, passing the needle from the inside by feeling one's way into the sulcus with the needle tip gives the best results. Correct positioning of the PCL haptics is at least as difficult as is needle penetration, a fact which up to now has mostly been ignored. More suitable lens designs may offer a solution for this problem. Their clinical value has to be established by intraocular endoscopy.

[Suture of the Achilles tendon with a reinforcing-plasty--experiences with 30 primary sutures of Achilles tendon ruptures].

We analysed 32 patients with fresh Achilles tendon rupture. 30 operations with a special primary suture and plasty of the fascia triceps muscle. Most of the patients were content with the operative treatment and the functional result. We saw one wound infection and one wound rand necrosis. The objective controls of OSG foot function and the muscle activity is good.

Use of the Ethicon 1713 suture for McCannel suturing.

The double-arm, straight-needle Ethicon 1713 suture with its STC6 micropoint spatula needle offers advantages over previously available needles for fixing intraocular lenses. Under certain circumstances, such as securing posterior chamber haptics to the iris, the advantage is significant. However, I recommend a shorter needle than the one presently available. Intracameral injection of sodium hyaluronate (Healon) increases the safety of this procedure.

The iris cerclage suture for permanent mydriasis: a running suture technique.

Permanent mydriasis can be a disabling condition. The author describes an iris cerclage running suture technique for the treatment of permanent mydriasis caused by diffuse iris sphincter damage. A long, curved transchamber needle on 10-0 polypropylene is passed in and out of the anterior chamber through limbal paracentesis openings while weaving the needle through the iris near the pupillary margin to form the cerclage. The technique minimizes stretching and distortion of the iris and creates a pupil that is precisely sized and rather round.
