Alcohol outlets, gonorrhea, and the Los Angeles civil unrest: a longitudinal analysis.

This study tests the effect of neighborhood changes on gonorrhea rates. Prior studies that indicate gonorrhea rates are associated with alcohol outlet density and neighborhood deterioration have been cross-sectional and cannot establish causality. After the 1992 Civil Unrest in Los Angeles, 270 alcohol outlets surrendered their licenses due to arson and vandalism thus providing a natural experiment. We geocoded all reported gonorrhea cases from 1988 to 1996 in LA County, all annually licensed alcohol outlets, and all properties damaged as a result of the civil unrest. We ran individual growth models to examine the independent effects of changes in alcohol outlets and damaged buildings on gonorrhea. The individual growth model explained over 90% of the residual variance in census tract gonorrhea rates. After the civil unrest, a unit decrease in the number of alcohol outlets per mile of roadway was associated with 21 fewer gonorrhea cases per 100,000 (p<.01) in tracts affected by the Unrest compared to those not affected. Neighborhood alcohol outlets appear to be significantly associated with changes in gonorrhea rates. The findings suggest that efforts to control sexually transmitted diseases, including gonorrhea and HIV, should address contextual factors that facilitate high-risk behaviors and disease transmission.

Injuries caused by plastic bullets compared with those caused by rubber bullets.

The injuries sustained by 99 people struck by plastic bullets, were compared with those sustained by 90 people struck by rubber bullets. Plastic bullets struck the head and chest less often than rubber bullets and caused fewer serious injuries to the face and chest, but tended to cause more serious skull and brain injuries.

Tactical emergency medical services: an emerging subspecialty of emergency medicine.

Law enforcement agencies are recognizing the need to have emergency medical care available at the scene of any incident involving tactical operations. The potentially volatile and dangerous atmosphere surrounding tactical operations is conducive to severe injury to officers, hostages, suspects, and bystanders. This mandates the immediate availability of basic and advanced life support services. However, a purely traditional approach to emergency medical services in the tactical environment may not be feasible and may expose prehospital personnel to greater danger. It also may disrupt the law enforcement mission. These factors mandate a different set of field assessment and treatment priorities. To meet these needs, selected prehospital personnel and emergency physicians train to work with and support special weapons and tactics teams.

Secondary contamination of ED personnel from hazardous materials events, 1995-2001.

Hazardous materials (hazmat) events pose a health threat not only for those individuals in the immediate vicinity of the release (ie, members of the general public, on-site first responders, employees), but also for ED personnel (ie, physicians and nurses) treating the chemically contaminated victims arriving at the hospital. Secondary contamination injuries to ED personnel result when exposed victims enter the ED without being properly decontaminated. Data from the Agency for Toxic Substances and Disease Registry's Hazardous Substances Emergency Events Surveillance System were used to conduct a retrospective analysis on hazmat events occurring in 16 states from 1995 through 2001 that involved secondary injury to ED personnel. Six events were identified in which 15 ED personnel were secondarily injured while treating contaminated victims. The predominant injuries sustained were respiratory and eye irritation. Proper victim decontamination procedures, good field-to-hospital communication, and appropriate personal protective equipment (PPE) use can help prevent ED personnel injuries and contamination of the ED.

[Levator ani syndrome, functional intestinal disorders and articular abnormalities of the pelvis, the place of osteopathic treatment].

OBJECTIVES: Various treatments of the Levator ani syndrome (LVAS) have been proposed. None of them alleviate symptoms in more than 20% of the cases. A new approach combining massages of the coccygeus muscle with physical treatment of frequently associated pelvic joint disorders was assessed. The outcome of the Irritable Bowel Syndrome (IBS) that is frequently associated was also studied. METHODS: One hundred and one patients (76 females and 25 males, mean age: 54 years) with a diagnosis of LVAS were studied prospectively over one year. Massages were given with a patient lying on the left side. Physical treatment of the pelvic joint disorders was given at the end of each massage session. RESULTS: Forty-seven patients (46.5%) suffered both from LVAS and IBS. A mean of less than 2 sessions of treatment was necessary. Sixty-nine per cent of the patients were LVAS-free at 6 months (p<0.0001) and 10% were improved. At 12 months, 62% were still free of symptoms and 10% improved (p=0.37). A comparable trend according to the Rome II criteria was found in the IBS-patient group (53% IBS free initially, 78% at 6 months [p=0.00001], 72% at 12 months [p=1]). There was a significant correlation between the favorable outcome of IBS and the LVAS at 6 and 12 months. All IBS-free patients were LVAS-free at 6 months. CONCLUSION: The LVAS may be cured or alleviated in 72% of the cases at 12 months with one to 2 sessions. This new comprehensive treatment suggests the role of a muscular and osteoligamentary etiology in the LVAS. Since most of IBS benefitted from such treament, it is logical to suspect a mutual pathogeny and to screen for LVAS in all such patients.

Achieving high detection sensitivity (14 zmol) of biomolecular ions in bioaerosol mass spectrometry.

Bioaerosol mass spectrometry (BAMS) performs single-cell analysis in real time. However, the specificity of BAMS mass signatures has been limited by low sensitivity at high masses. To increase the mass range and sensitivity of BAMS, a novel design was developed that utilizes a linear flight tube with delayed extraction and an electrostatic ion guide. This study quantifies the sensitivity limits of the novel BAMS design and evaluates the feasibility of BAMS to detect higher mass biomarkers from single cells. All experiments were carried out using MALDI aerosol particles that were nebulized from solution. Sensitivity was assessed by generating particles with decreasing amounts of analyte via serial dilutions. The amount of analyte contained within each particle was calculated based on particle size, density, and molarity of the analyte within solution. A variety of biomolecular ions were studied and signals obtained from particles containing 300 zmol of maltopentaose, 132 zmol of alpha-cyclodextrin, and 14 zmol (approximately 8400 molecules) of gramicidin S are reported. The detection of 14 zmol of gramicidin S is to the best of our knowledge a record in sensitivity for MALDI TOF-MS.

The violence of despair: consultation to a HeadStart program following the Los Angeles uprising of 1992.

This paper describes an acute trauma consultation to one Los Angeles HeadStart program following the severe civil disturbance in the spring of 1992. By self-report, 7% (3/42) of the teachers had severe post-traumatic stress symptoms, and 29% (21/42) had moderate symptoms one month after the event. Teachers, comparing behavior following the disturbance to behavior two weeks before, reported statistically significant increases in the students' aggression and noise level, and decreases in "getting along" with peers and academic progress. Difficulties encountered in this type of community psychiatric consultation are discussed.

Ambient temperature and the occurrence of collective violence: the "long, hot summer" revisited.

Archival data pertaining to 102 instances of serious collective violence were examined in order to study the relationship between ambient temperature and the occurrence of such events. Results indicated that the frequency of collective violence and ambient temperature were curvilinearly related. Specifically, ambient temperature was directly associated with the frequency of collective violence through the mid-80s (degrees Fahrenheit). Beyond this point, however, further increments in temperature were associated with a decreasing incidence of such events. Additional findings indicated that ambient temperature increased significantly during the 7 days preceding the outbreak of collective violence, remained stable during its occurrence, but then decreased significantly in the 3 days following its termination. Possible implications of these findings, as well as their relationship to the results of previous laboratory studies, are discussed.
