[The chologon-sorbitol functional test under ultrasonic control in pathology of the gallbladder and biliary tract].

The degree of the filling up and the dilation of the gall bladder, its functional state as well as the passibility of d. cysticus are evaluated by ultrasound examination and computer determination of the surface and dimensions of the gall bladder. 546 patients, 20-78 years of age (484 women and 62 men) were examined by a pharmacodynamic functional test with chologon (acidum dehydrocholicum) in a dose of 10 mg/kg body mass and sorbitol (in 116 patients) in a dose of 20 g in 100 ml of water. The functional test is easily performed in the course of the ultrasound examination and side effects were registered. The test allows the assessment of the functional state of the gall bladder, the passibility of d. cysticus, the degree and mechanism of the filling up of the gall bladder and the bile ducts with bile. The better filling up of the gall bladder with bile after application of chologon (by passable d. cysticus) ensures better conditions for detecting microgallstones in the gall bladder as well as of diseases linked with its wall--cholesterosis, cancer and polypi of the bladder, etc.

[A case contribution to the Gardner syndrome].

As to its course the Gardner syndrome is similar to a chronic disease and has the following symptoms: 1. Fibro-epithelial tumours of the skin with first symptoms in early childhood. 2. Osteomes, osteofibromas, desmoids mainly located in the cranial region. 3. Disturbances of the dental development in the form of tooth retentions, persistence of milk teeth and supernumerary tooth germs. 4. Intra- and retroperitoneal or intramesenteric fibromatoses which scarcely become clinically manifest. 5. Intestinal polyposis occurring in most cases only in the third decade of life. The tumours of the skin, of the connective tissue and of the bone are always benign, whereas the intestinal polypi show a considerable tendency toward malignant degeneration. For this reason, the prognosis of the Gardner syndrome must be considered unfavourable. On the basis of a case report, the clinical symptoms and findings in the maxillofacial region are outlined.

Is hysteroscopy of value in the investigation of female infertility?

A prospective multicentre study was designed to ascertain the value of hysteroscopy in the evaluation of female infertility. The study comprises 619 CO2 hysteroscopies in women complaining of infertility. Four hundred and ninety nine examinations (80%) were performed without anaesthesia or cervical dilatation. Twenty eight cases of malformation were diagnosed. Adhesions were found in 68 patients (11%), polypi in 56 (9.1%) and submucous fibroids in 51 (8.2%). Endometrial hyperplasia was diagnosed by hysteroscopy in 67 patients, but there was only 42% histological correlation. Examination of the tubal ostia by hysteroscopy revealed 26 patients (4%) with unilateral tubocornual polyps and one with bilateral polyps. The correlation with hysterosalpingography (HSG) in 185 patients was poor. In 20% where the HSG was reported as normal there were hysteroscopically demonstrable lesions whereas there were false positive findings at HSG in 35% of cases. Hysteroscopy is the only technique which provides an accurate clinical evaluation of the uterine cavity although it does not provide significant information about the cervical canal or fallopian tubes.

Benign endobronchial tumours treated by neodymium-YAG laser.

We present two patients with benign endobronchial tumours: polypi and a chondroma. Both were treated by Neodymium-YAG laser photoresection. There have been no signs of recurrence in the four years since treatment. Endobronchial resection by Neodymium-YAG laser is an effective treatment for benign endobronchial tumours.

[Results of endonasal ethmoid bone operations in diffuse hyperplastic chronic paranasal sinusitis].

Endonasal endoscopic surgery of the paranasal sinuses for chronic sinusitis has been accepted as the treatment of choice by many authors, but detailed evaluation of the results is often lacking. Of a consecutive series of 220 patients with diffuse chronic paranasal polyposis who had undergone endoscopic pan-sinus operations, 90 were re-examined endoscopically after 1-7 years. Normal respiratory mucosa was found in 57% of cases and minor circumscribed mucosal hyperplasia in a further 25%. Unsatisfactory healing or recurrence of polypi was found in 18%. A questionnaire completed by all 220 patients showed that 49% were completely relieved, 33% reported a marked improvement, 12% remained unchanged, and 6% claimed a deterioration. Of those patients with polyposis and bronchial asthma, 57% achieved marked relief. The very low incidence of surgical complications after complete endoscopic ethmoidectomy should be emphasized.

[Penetration behavior of mezlocillin in chronic polypous inflamed mucosa of the paranasal sinuses].

Recent investigations have shown that conservative treatment of chronic sinusitis, especially with polypi in the nasal cavity and in the paranasal sinuses, is not as successful as in acute inflammation. Surgical intervention to remove the diseased mucosa is necessary. The newly developed more conservative endonasal techniques may nevertheless allow spread of the pathogenic bacteria during surgery. Mezlocillin has a broad spectrum against both aerobic and anaerobic micro-organisms: its penetration into the hyperplastic mucosa of the chronically diseased maxillary sinus was therefore investigated. In 15 patients with chronic sinusitis a 5 g bolus injection of mezlocillin was given half an hour before the operation. Two more doses of the antibiotic were given 6 and 12 h afterwards. The specimens investigated were blood taken before the injection of mezlocillin, and blood and tissue taken by the endoscopic endonasal techniques innovated by Wigand (1977) between 30 and 120 min after the start of the operation. The concentration of mezlocillin was determined by bioassay using Bacillus subtilis ATCC 6633 as indicator organisms. The results were compared with the minimal inhibitory concentration of bacteria usually found in chronic maxillary sinusitis. The results showed that mezlocillin may be of value in chronic paranasal infections in the conservative treatment of complications of sinus infections, especially if surgery is contraindicated because the patient is a high anaesthetic risk.

[Juvenile polyposis. A report on two cases (author's transl)].

The discovery of multiple polypi in a small child does not necessarily indicate that this is a case of familial adenomatosis. Two cases of juvenile polyposis were confirmed by histological examinations, and their clinical and, more particularly, their radiological characteristics were studied. The only way to explore the full extent of the lesions in a satisfactory manner is by double-contrast examination of the whole colon. The authors stress the value of radiological examinations, though the frequency with which they should be carried out is open to discussion. Review of the published literature reveals that the association of true familial polyposis or adenocarcinomatosis exists in the same family or even in the same patient. Diagnostic difficulties are usually due to the fortuitons co-existence of a lymphoid hyperplasia, the significance of which is still not known.

[Bladder bilharzioma and carcinoma in a bilharzial bladder].

Over 7 years, 592 patients infected by Schistosoma haematobium were detected. Among these, 102 had a cystoscopy which found carcinoma in 3 patients, polypi in 1 and vesical "bilharzioma" (so-called pseudo-polyps) in 14. From the results of the biopsies and data from the medical literature the relationship between schistosomiasis and carcinoma in the schistosomial bladder is discussed, as well as the meaning of various lesions, such as malpighian metaplasia, papillomatous hyperplasia and glandular cystitis, seen in biopsy specimens from "bilharziomas". The cystoscopic diagnosis of one (or several) tumours resembling "bilharzioma" requires biopsy, resection and close monitoring.

[Benign tumors of the esophagus and stomach].

From the tumors of the oesophagus studied, 30% were found to be benign. They occurred in a ratio of 2:1 predominantly in men with an average age of 37,1 years compared to 52,5 years in women. The symptoms were unspecific and the prognosis was quo ad sanationem very good. Only in one case did a neurofibroma relapse explosively with in 4 months and was already inoperable at the time, when the new diagnosis was made. The percent of benign stomach tumors amounted to 2,6%. Tumors of the mucous tissue behaved differently than the neurogenic and mesenchymal ones. The former became noticeable mainly due to epigastric complaints, the latter due to haemorrhages. Epithelial tumors occurred, with a ratio of 2:1, more often in women, but without any difference in age. Mesenchymal and neurogenic tumors occurred in almost the same sex ratio, but at an average age, which was 18 years higher in women. Solitary stomach polypi can mostly be removed endoscopically, whilst all other benign oesophagus and stomach tumors must be removed by operation due to the symptoms and the dignity of the tumor, which is seldom clear.

[Polypectomy during gastric endoscopy].

The endoscopic polypectomy with electrocoagulation is a method of nonsurgical elimination of accessible polypi of the upper segments of gastrointestinal tract. Results are reported of 113 endoscopic polypectomies in 84 patients being of therapeutic and diagnostic value. The method of endoscopic polypectomy is described as well as the preparation of the patients and their postoperation follow up. The polypectomy was successful in 83 of the patients and in 15 patients with multiple polyposis successive polypectomies were realized. The endoscopic polypectomy was performed in cases with tumor process localized in the distal part of esophagus, stomach and duodenum as well as in case of resected stomach. One case of recidivation is described after polypectomy and one case with newly formed polyps necessitating a repeated polypectomy. Complications after endoscopic polypectomy are reported in three patients--hemorrhage, conservatively coped and in 2 patients--perforation of gastric wall, forcing an emergency operation.

[Gastric carcinoma; pathology (author's transl)].

Frequency, macro- and microanatomy, and propagation routes of gastric cancer are outlined. Early carcinoma is discussed and the relation of gastric mucosal polypi to carcinoma of the stomach. Finally the significance of biopsies and the possibility of its misinterpretation are stressed.

[Polyposis coli et recti familiaris (author's transl)].

From 1970 to 1977 we observed 35 patients with polypi of the colon and rectum, in 18 cases on the basis of heredity. We recommend total proctocolectomy and ileostomy making use of the reservoir technique after Kock. The operation should be performed not until the beginning of the third decade of life.

[Cryosurgery in carcinomas of the rectum and anus (author's transl)].

3 years of clinical experience in cryosurgical treatment of carcinomas of the rectum and anus and of malignant polypi is reported. Nearly all patients were in so a poor condition not to underwent conventional operations. In these cases cryosurgery offers an alternative to the usual local procedure of tumour resection.

Significance of tumour markers in colorectal carcinoma associated with Schistosomiasis.

Schistosomiasis as well as Colorectal Carcinoma are equally prevalent in Egypt. However, the role of Schistosomiasis as a risk factor for Colorectal Carcinoma is not well established. Three tumour markers have been assessed in 69 patients with large bowel disease. They were classified into five groups. Group 1 (A) included 21 cases with Schistosomal hepatic fibrosis. Group 2 (B) included 6 cases of ulcerative colitis. Group 3 (C) included 10 cases of adenomatous polypi of which 12 cases had Schistosomiasis. Group 4 (D) consisted of 21 cases of colorectal carcinoma, 12 cases had schistosomiasis in association with colorectal carcinoma constituting group 5 (E). Elevated CEA was observed in benign tumours but showed non significant difference in G4 and G5. Significantly increased AFP levels were evident in G1, G4, and G5. Significant increase of B-HCG was observed only in G4 and G5 indicating its significance as diagnostic index in case of malignancy. It has been observed that Schistosomal hepatic fibrosis induced increased levels of some of the tumour markers. Therefore, the factor of Schistosomal hepatic fibrosis should be considered during the assessment of tumour markers in colorectal carcinoma cases.

[Degenerative colorectal polyps].

In 23 patients with malignant colorectal polyps we reviewed the frequency of recurrence. In the group without subsequent local resection, 8 of 11 patients were without recurrent tumour after 2.7 (0.8-4.1) years, in the group with subsequent local or conventional colon resection, 10 of 12 patients after 5.3 (0.8-9.0) years. None of the patients showed any evidence of residual tumour or lymph node metastasis. All recurrent tumours (2 x carcinoma, 3 x benign polyps) could be resected curatively. There was no difference in recurrence in both groups. For safe histopathological examination of malignant colorectal polypi in respect of depth of invasion, free resection margins and lymphatic vessel invasion, intraoperative fixation and marking must be performed, since local recurrence was seen after more than 5 years, long-term follow up is necessary.

[Allergic rhinitis in children (author's transl)].

Some of the problems within the research field of allergology are discussed. The main topic is the new treatment of allergic nasal diseases with a local glucocorticosteroid. Beclomethasone dipropionate (Beconase) can be given intranasally without any risk of systemic side-effects. The drug is found very effective in the treatment of hay fever, vasomotor rhinitis and nasal polypi in adults. In a double-blind crossover trial with 19 hay fever children, aged 8-16 years, 18 preferred the active drug, none preferred the placebo and one had no preference. It is concluded that 400 mug beclomethasone dipropionate a day is a safe and very effective treatment of nasal hay fever symptoms in children.

Rynacrom (sodium cromoglycate) in intractable perennial rhinitis.

A double-blind trial of Rynacrom (sodium cromoglycate) was conducted on 39 patients with perennial rhinitis. Improvement in nasal blocking was achieved at a statistical significance while sneezing and rhinorrhoea only reached the 0.1 level. During long term follow up, four patients who had previously required polypectomy for recurrent nasal polypi, had their condition resolved and subsequently controlled by Rynacrom therapy of one year's duration. Sixty percent of the original trial population are also being successfully maintained at minimal dosage.

[Laparoscopic cholecystectomy for benign gallbladder diseases].

From Sept. 1991 through Sept. 1992, 700 patients underwent laparoscopic cholecystectomy (LC), among them 171 were male and 529 were female with age ranging from 18 to 77, and 238 of them were overweighed. There were 35 cases of gallbladder polypi, 657, chronic calculous cholecystitis, 8 acute cholecystitis, with 13 cases having had previous upper abdominal surgeries. Intraoperative transcystic duct cholangiography were carried out in 10 cases and choledocholithiasis was confirmed in 5 of them. Exploring the CBD laparoscopically was done in 2.14 cases were converted to open surgery (2.6%). Twenty-five patients had postoperative complications including injuries to the extrahepatic bile duct and bleeding requiring laparotomic hemostasis in 3 and 2, respectively, with 1 patient dying of cardiac arrhythmia.
