Regional effects of indomethacin, acetylsalicylic acid and SC-19220 on the contractility of rabbit renal pelvis (pacemaker regions and pelviureteric junction).

The effects of the cyclooxygenase inhibitors indomethacin and acetylsalicylic acid (ASA) and the prostaglandin (PG) antagonist 1-acetyl-2-[8-chloro-10,11-dihydrodibenz (b,f)(1,4) oxazepine-10-carbonyl]hydrazine (SC-19220) on smooth muscle strips from the rabbit renal pelvis were examined. Circularly oriented tissues were taken from within the renal pelvis and subdivided into the four adjoining regions, from fornix to pelviureteric junction (PUJ). They were set in a tissue bath and isometric tension changes were recorded. Spontaneous contractions were observed and electrical field stimulations evoked twitch-like contractions. The frequency of spontaneous contractions was dependent on the region within the renal pelvis from which they were dissected, gradually decreasing from fornix (3.92/min.) to PUJ (0.43/min.). Indomethacin (10(-5)M) or ASA (10(-4)M) significantly increased the frequencies of spontaneous contractions in all four regions. In the upper three regions (pacemaker regions), both agents significantly decreased the amplitudes of spontaneous and stimulation-induced contractions. In the PUJ region, both agents significantly increased the amplitudes of spontaneous and stimulation-induced contractions. SC-19220 (3 x 10(-5)M) significantly increased the frequencies and decreased amplitudes of spontaneous contractions in all four regions. These results suggest that the PUJ has different contractile characteristics from those of pacemaker regions and that decrease of amplitude by indomethacin or ASA is based on their cyclooxygenase inhibitor property.

Small cell carcinoma of the renal pelvis: a clinicopathological, morphological and immunohistochemical study of 2 cases.

Two women, 62 and 66 years old, with combined small cell carcinoma and transitional cell carcinoma of the renal pelvis are reported. The clinical picture was similar to that seen in conventional transitional cell carcinoma. A brief review of the literature indicates that in the urinary tract, small cell carcinoma occurs most commonly in the bladder and is exceedingly rare in the renal pelvis; only 2 cases have been reported previously. The morphological spectrum of the small cell carcinomatous component is similar to that seen in lung tumors. Neuroendocrine differentiation of the small cell carcinoma component was supported by a positive immunoreaction to neuron-specific enolase in both cases and to synaptophysin in 1. One patient died with metastases 8 months after diagnosis, and 1 was alive with clinical evidence of lymph node metastases and contralateral papillary transitional cell carcinoma of the renal pelvis 16 months after diagnosis.

Prognostic variables in patients with transitional cell carcinoma of the renal pelvis and proximal ureter.

Recent technological advances in urological endoscopic surgery of the renal pelvis and proximal ureter via ureteroscopy or percutaneous nephroscopy have made it possible to consider parenchymal-sparing procedures in patients with transitional cell carcinoma. To define the role of these procedures in the management of renal pelvic or proximal ureteral transitional cell carcinoma we analyzed retrospectively 31 patients who underwent nephroureterectomy for transitional cell carcinoma of the renal pelvis and/or proximal ureter. High grade upper urinary tract transitional cell carcinoma and a history of metachronous or synchronous bladder transitional cell carcinoma were independent adverse prognostic factors. However, patients with low grade upper urinary tract transitional cell carcinoma and no evidence of a urothelial field change had a 100 per cent 5-year survival rate. It would appear that parenchymal-sparing endoscopic techniques should be regarded with caution in patients with either high grade transitional cell carcinoma of the renal pelvis and proximal ureter or a history of bladder cancer.

The pressure-volume relationship of the pig renal pelvis.

In five pigs weighing approximately 85 kg. the pelvic pressure-volume relationship was investigated. Through a catheter introduced into the renal pelvis from the ureter with ligation at the ureteropelvic junction, a continuous filling of one ml./minute was established. After drainage of the pelvis filling was begun showing an exponential relationship between pressure and volume. Fillings in all of the units confined the equation y = a + bx + cx2. Emptying showed unanimously clockwise hysteresis although 1/3 of the fillings revealed active contractile forces with an inverted S-shaped emptying curve. The normal multicalyceal renal pelvis in pigs of the same size showed a wide range of pressure-volume relationships both between individuals and between the two investigations. This wide range might be of importance in the development of dilatations due to obstruction.

[Squamous cell carcinoma of the renal pelvis. A clinical case].

Squamous cell carcinoma of the renal pelvis is uncommon, accounting for approximately 10% of all renal pelvic tumours. It's often associated with chronic renal calculi or infection and it usually presents at an advanced stage with pain or a palpable mass. We report an incidental case of squamous cell carcinoma of the renal pelvis, associated with chronic renal calculi and infection, and weight loss. The prognosis of patients with advanced squamous cell carcinoma of genitourinary origin is poor. In patients with chronic stones or infection squamous cell carcinoma of renal pelvis must be suspected if survival is to be affected.

MRI of the female and male pelvis: current and future applications of contrast enhancement.

Magnetic resonance imaging (MRI) of the pelvis has proven to be a most valuable diagnostic tool to examine pathologies of various organ systems in the pelvis. For most conditions MRI was found to be superior to other imaging modalities, like ultrasound and computed tomography. Controversy still exists on the value of intraluminal and intravenous contrast enhancement. This article gives an overview on currently available intraluminal and intravenous contrast agents. The techniques and results for contrast-enhanced MRI in the female and male pelvis are discussed based on a review of the literature. New developments and upcoming techniques, such as lymph node specific contrast agents and breathhold volumetric MRI, are described and initial results are presented.

Ultrafast MR imaging of the pelvis.

MR gradient systems with higher slew rates and gradient amplitude enable certain forms of imaging that are not practical with older gradient systems. These newer pulse sequences include single shot half-Fourier T2-weighted images and echo planar imaging. More important in MR imaging of the pelvis, these gradient systems benefit more conventional imaging methods such as gadolinium-enhanced 3D MR angiography, dynamic gradient echo contrast-enhanced images, and T2-weighted fast spin echo images, by shortening echo times. For most MR imaging of the pelvis, spatial resolution is paramount, and therefore sequences such as half-Fourier acquisition Turbo spin echo (HASTE) and 3D gadolinium-enhanced dynamic imaging play a less important role than in the upper abdomen. The potential of these techniques for diffusion or perfusion studies in the pelvis has not been explored.

In vivo temporal EPR imaging for estimating the kinetics of a nitroxide radical in the renal parenchyma and pelvis in rats.

The kinetics of a nitroxide radical in the renal parenchyma and pelvis in rats were investigated by employing an in vivo EPR imaging system equipped with a surface-coil-type resonator (SCR). The exposed kidney of a living rat was inserted into the single-turn coil of the SCR, with the renal major axis aligned with the direction of alternative magnetic field (B(1)). After the injection of nitroxide radical via the tail vein, EPR measurements were repeated. From the temporal EPR images of the kidney on the 2-D projection to the plane which is perpendicular to the direction of B(1,) the decay rate of nitroxide radical in the renal parenchyma and pelvis was estimated. The parenchymal decay rate was found to be significantly shorter than that for the pelvis.

Surgical management of desmoid tumors of the female pelvis.

BACKGROUND: Desmoid tumors are neoplasms characterized by proliferation of fibroblasts without cytologic features of malignancy. Although desmoid tumors are not uncommon, pelvic desmoid tumors are rare and therapeutically challenging. We describe the surgical management of seven patients with histologically confirmed desmoid tumors arising in the pelvis. STUDY DESIGN: All patients, six with primary and one with recurrent desmoid tumors of the female pelvis, were treated at Mayo Clinic Rochester between 1976 and 1997. The mean age of one African-American and six Caucasian patients was 29.8 years (range 17 to 59 years). No history of previous relevant pelvic trauma was identified; six patients experienced pelvic or leg pain (or both). All patients presented with isolated masses; five of seven were located left of the midline, generally involving the obturator internis or levator muscles (or both). The surgical approach was transabdominal, with a vaginal component in one patient. In addition to this retrospective review of these seven patients, literature addressing the management of desmoid tumors of the female pelvis is reviewed. RESULTS: Extirpative surgery necessitated resection of musculoskeletal, vascular, or adjacent soft tissue structures. Mean tumor diameter was 13.9cm (range 5 to 27 cm). Although the median recorded blood loss was 1,425mL, one patient who refused blood products died secondary to surgical hemorrhage. The median followup for the remaining evaluable patients was 57 months (range 13 to 249 months). Recurrent disease developed in 3 of 6 patients at 3, 11, and 13 months after surgical resection and was successfully managed with subsequent reoperations, with or without adjuvant radiotherapy. CONCLUSIONS: Although recent reports demonstrate selective regression of desmoid tumors with antiestrogen therapy, management of symptomatic pelvic desmoid tumors in young women may necessitate an operation. In our series, all primary and recurrent tumors were managed with resection of involved bone and other adjacent soft tissues. The role of adjunctive radiotherapy or hormonal therapy remains to be defined.

Transitional cell carcinoma of the renal pelvis with extension into the inferior vena cava: a report of two cases.

Of all primary malignant renal tumors, 10% to 15% originate from the renal pelvis, and 90% to 92% of these tumors are transitional cell carcinomas. Nonetheless, renal pelvis transitional cell carcinoma extending into the inferior vena cava is very rare. We report one confirmed case and one highly suspicious case of renal pelvis transitional cell carcinoma with a tumor thrombus in the inferior vena cava. Both of our patients died within 6 months of initial diagnosis, indicating the poor prognosis and advanced stage of transitional cell carcinoma with an inferior vena cava thrombus. Transitional cell carcinoma should be considered in patients with obstruction of the renal vein and the inferior vena cava.

Apoptotic cell death and Smad4 expression in transitional cell carcinoma of the renal pelvis and ureter.

PURPOSE: To investigate the frequency of apoptosis and the expression of Smad4 protein as well as their roles in transitional cell carcinoma (TCC) of the renal pelvis and ureter. METHODS: Apoptosis was detected by using terminal deoxynucleotidyl transferase (TdT)-mediated dUTP-biotin nick end labeling (TUNEL) technique in 34 formalin-fixed and paraffin-embedded specimens of renal pelvic and ureteral TCC. The expression of Smad4 was immunohistochemically studied. RESULTS: The incidence of apoptosis ranged from 1.10 to 3.75% with a median of 2.50% in TCC of the renal pelvis and ureter. The incidence of apoptosis was noted to be closely related to histologic grade but not to pathologic stage of the cancer. The expression of Smad4 was detected in six of 34 cases (17.6%). Regarding subcellular distribution, Smad4 protein was localized both in cytoplasm and nucleus of the cancer cells. In comparing the incidence of apoptosis with the expression of Smad4, no significant associations were seen between them. The expression of Smad4 was not related to the tumor grade nor stage of the cancer. CONCLUSIONS: The present study demonstrated close association of the incidence of apoptosis with the tumor grade of TCC of the renal pelvis and ureter. Significance of Smad4 expression was not noted in the study. It suggests that apoptotic cell death may play an important role in the tumor progression of renal pelvic and ureteral TCC.

Solitary fibrous tumor of renal pelvis.

A 70-year-old Japanese man was referred because of a right renal mass of 2 years in duration. Imaging studies, including magnetic resonance imaging, revealed an ovoid mass, with relatively abundant vascularity, in the right renal pelvis. Right radical nephrectomy was done and a tumor measuring 6.0 x 4.5 x 4.0 cm was found in the renal pelvis. Solitary fibrous tumor (SFT) was highly suspected by histology. Immunohistochemical study using a monoclonal antibody directed against the human hematopoietic progenitor cell antigen (CD34) stain confirmed SFT. This is the first case of SFT of the renal pelvis. Although SFT is extremely rare in urogenital organs, this tumor must be included in the differential diagnosis when we encounter urogenital tumors consisting of mesenchymal elements.

The increase of renal pelvis dilatation in the fetus and its significance.

OBJECTIVE: To determine the effects of the changes in fetal renal pelvis dilatation on post-natal diagnosis and outcome. METHODS: Prenatal sonographic fetal renal anteroposterior diameters of > or = 4 mm in the second trimester, which persisted to > or = 7 mm in the third trimester, were the inclusion criteria. Fifty-six fetuses and 73 renal units with normal karotypes and a solitary sonographic finding of renal pyelectasis, which met the inclusion criteria, were followed, post-natally, until the age of 30 months. The neonates were categorized into three groups, according to their final diagnosis: No hydronephrosis, pelvi-ureteric junction obstruction and hydronephrosis from other causes. The neonates were also categorized according to their outcome: no treatment, follow-up only and surgical treatment. RESULTS: In 17 neonates (30.4%), and 19 renal units, the diagnosis of hydronephrosis was excluded post-natally. In 39 neonates (69.6%), and 54 renal units, an urinary tract pathology was confirmed. The dynamics of pyelectasis in the second and third trimesters of pregnancy differed significantly among the neonates when categorized according to the final diagnosis (P < 0.05), or according to outcome (P < 0.05). CONCLUSION: It is possible to predict, and distinguish between, long-term conservative and surgical treatments of renal pelvis dilatation in the third trimester of pregnancy.

[Reduction of radiation exposure by using storage phosphor radiography on pelvis and lumbar spine].

PURPOSE: To examine the reduction of radiation dose for radiography of the pelvis and lumbar spine (LS) by using storage phosphor radiography (SPR)? MATERIAL AND METHODS: Each pelvis and LS of 2 phantoms (3 M) and 2 corpses were X-raxed digitally in an anterior-posterior orientation using SPR (ADC compact, Agfa). The tube current time product (mAs) was diminished gradually while keeping the voltage constant. The surface entrance dose was measured by a Dosimax (Wellhöfer) sensor. Five investigators evaluated the images for characteristics and critical features and pathological findings. RESULTS: The images of the 4 various pelvises/LS could be evaluated sufficiently down to an average dose of 34 to 40 %/18 to 35 %. For the assessment of hip joint, region typical corticalis and calcifications (pelvis/LS) 9 to 18 %/8 to 23 % of the initial dose (initial dose is equivalent to a conventional screen film system with a speed class of 400) are enough, spongiosa typical for the region and the sacrum 30 to 56 %/19 to 36 % on average among the readers. CONCLUSION: For the sufficient exclusion of a fracture the dose can be reduced on pelvis/LS to at least 39 %/28 % ( 23 %/17 %), for the assessment of sacroiliitis/osteolyses/demineralisation to at least 78 %/44 % ( 34 to 36 %/22 to 26 %), to position check after fracture to at least 15 %/25 % ( 9 %/10 %). Question-related tube current time products of the pelvis and LS can be deduced for various objects.

[Radiation exposure of the head, midface and pelvis in multi-slice CT (MSCT): comparison with single-slice CT (SSCT)].

PURPOSE: Comparison of the radiation exposure of the head, midface and pelvis in multi-slice CT (MSCT) and single-slice CT (SSCT) for manufacturer-implemented standard protocols. MATERIALS AND METHODS: An anthropomorphic Alderson-Rando phantom equipped with LiF-thermoluminescent dosimeters in the head, neck and pelvic region was examined with different MSCT and SSCT protocols, and the measured organ doses were compared. RESULTS: In comparison with the SSCT protocols, the MSCT protocols delivered about twice the organ dose to the head and pelvis and about eight times the organ dose to the midface. The comparison indicates that the parameters (slice collimation, mAs, pitch) used for the MSCT protocols deliver increased radiation doses. CONCLUSIONS: The current standard MSCT protocols generally lead to higher organ doses to the head, midface and pelvis than the SSCT protocols, raising the question whether using the MSCT protocols recommended by the manufacturer can be justified. The dose parameters should be optimized to achieve an acceptable balance of dose reduction and adequate image quality.

Recipient vessels for microsurgical flaps to the groin and pelvis: a review.

Reconstruction of large defects of the pelvis and groin sometimes requires microsurgical flaps when the otherwise rich supply of local options fails to be sufficient in difficult and complicated cases. Recipient vessels for microsurgical flaps to the groin and pelvis are important elements in planning such procedures. Seven groups have reported pelvis and groin microsurgical flap cases, and their recipient vessels have included the superior and inferior gluteal vessels, the femoral vessels, the deep perforators of the femoral system, the inferior epigastric vessels, intra-abdominal vessels, and interpositional vein grafts to distant recipient vessels. This review summarizes their experience and describes the recipient vessel options in the pelvic and groin region.
