[CHRONIC DIARRHEA OF INFECTIOUS ETIOLOGY]

Chronic Diarrhea syndrome is due to several causes. In LatinAmericen countries, infectious and parasitic etiology is frequent. Withinthese infections, the following has been determined: parasitic and enterobacterial agents, overpopulation of bacteria syndrome in the upper small bowel (SOBIA, abbreviation for Síndrome de Sobrepoblación Bacteriana del Intestino Delgado Alto), intestinal TBC, and AIDS. In these processes, the precipitating factor may be unique or multifactor; or there could also exist a tendency towards pathologies for these infections. There has been evidence of SOBIA cases without associated factor in Latin American countries. A study made in Peru shows SOBIA cases in 30 percent of chronic diarrhea.In chronic diarrhea cases, and even more, in those diarrheas of infectiousetiology, a complete study should be made, considering the several factorsthat generate diarrhea in a same one case, such as the mechanisms ofimmunodeficiency, neurological, endocrinemetabolic, and others associated with intestinal infections. This approach will be helpful to make a complete diagnosis and apply timely treatment.

UNICEF supports birth control despite Holy See.

The Vatican has failed in its attempt to prevent the UN Children's Fund (UNICEF) from spreading the message of family planning in developing countries. Over the strong objections of the Holy See, the 41-member Executive Board of UNICEF has asked the agency to cooperate with the UN Population Fund (UNFPA) and the WHO "to support family planning in the context of sustainable national healthcare systems." A member of the Executive Board said the Vatican held the view that UNICEF had no legitimate right to be involved in family planning. "But the Holy See was in a minority of one," he said, adding that "the whole problem arises from the fact that the Vatican continues to equate birth control and abortion with family planning." A recently concluded board meeting unanimously adopted a resolution requesting UNICEF to "contribute substantively" to the UNFPA-sponsored International Conference on Population and Development (CPF) set to take place in 1994 in Cairo. The board also asked UNICEF Executive Director James Grant to submit a policy paper "on the involvement of UNICEF in family planning, taking into account the health of the child and the mother." The paper is to be presented for the consideration of the board at its regular session next year, with a view to approving it prior to the ICPD. Prior to the adoption of the resolution, J. Klink, a spokesman for the Holy See, told the Executive Board that there were "concerns" over calls for UNICEF to involve itself in family planning activities. He said UNICEF should not be concerned with family planning because there were other agencies entrusted with that mandate. Responding to the support given by Nordic countries to the UNICEF resolution, Klink said that wealthy nations must not impose dictates as to the "appropriate" size of poor families. "The Holy See would not propose that UNICEF halt its current balanced, informational approach to the spacing of births," he said. "Families must be free to decide their size." UNICEF, he pointed out, had clearly stated that it was not involved in abortion or sterilization and did not provide contraceptives. "UNICEF must continue to respect cultural and religious diversity," he said. At a meeting of the Preparatory Committee of the UN Conference on Environment and Development (UNCED) in April this year, the Vatican succeeded in striking a reference to family planning from a blueprint for development known as Agenda 21. Paragraph 99 of Agenda 21 had originally referred to the need for "integrated healthcare, including universal access to family planning services and the provision of safe contraceptives." But this paragraph was deleted in the final document adopted at UNCED in Rio de Janeiro in June. Given that family planning is the single most effective means with which to contain population growth, the UNFPA considered the omission a retrograde step in handling the dangers of overpopulation and environmental degradation. In Rio de Janeiro, Werner Fornos, president of the Washington-based Population Institute, criticized the Holy See "for interfering in the lives of people throughout the world because of religious dogmas." Grant told the board meeting last month that UNICEF would work with the UNFPA to spread knowledge and support program activities for successful family planning which he said was integral to safe motherhood. Birte Poulsen of Denmark told the meeting that reduced population growth was a prerequisite for sustainable development, and the UNICEF "must accord family planning issues top priority." And Inger Ann Ravum of Norway said the integration of family planning activities into UNICEF's program was welcome.

Agrarian change and labour migration in the Sierra of Ecuador.

"Among the effects produced in the Sierra of Ecuador by the programme of land reforms launched in 1964 was a reduction in the incomes from small-scale farming. At the same time, the growth of productive employment in urban areas was insufficient to provide round-the-year work to the fast growing army of jobseekers. Analysis of the agrarian structure and migration patterns before and after 1964 suggests that it was primarily this combination of circumstances that produced a pronounced shift towards short-term rather than permanent migration, thereby providing industry and services with the labour they needed while avoiding the disadvantages of severe urban overpopulation."

Population and resources in Mauritius.

"The island of Mauritius was facing a crisis by the 1950s as the relationship between its population and resources became unbalanced....A two-pronged strategy was set in place to change the relationship between population and resources. Firstly, an aggressive family-planning policy was established, reducing population growth. Secondly, the economy was diversified with tourism, financial services and, especially, manufacturing in the Mauritius Export Processing Zone, creating extra finance and resources. The changes have not been cost-free but Mauritius ends the century, not as a classic case of overpopulation, but more [as] a model micro-state that has overcome many population and resource problems, largely through its own efforts."

An integration programme of poverty alleviation and development with family planning.

The State Council (the central government) recently issued a Circular for Speeding Up the Integration of Poverty Alleviation and Development with the Family Planning Programme during the Ninth Five-year Plan (1996-2000). The Circular was jointly submitted by the State Family Planning Commission and the Leading Group for Poverty Alleviation and Development. The document sets the two major tasks as solving the basic needs for food and clothing of the rural destitute and the control of over-rapid growth of China's population. Practice indicates that a close Integration Programme is the best way for impoverished farmers to alleviate poverty and become better-off. Overpopulation and low educational attainments and poor health quality of population in backward areas are the major factors retarding socioeconomic development. Therefore, it is inevitable to integrate poverty alleviation with family planning. It is a path with Chinese characteristics for a balanced population and sustainable socioeconomic development. The targets of the Integration Programme are as follows: The first is that preferential policies should be worked out to guarantee family planning acceptors, especially households with an only daughter or two daughters, are the first to be helped to eradicate poverty and become well-off. They should become good examples for other rural poor in practicing fewer but healthier births, and generating family income. The second target is that the population plans for the poor counties identified by the central government and provincial governments must be fulfilled. This should contribute to breaking the vicious circle of poverty leading to more children, in turn generating more poverty. The circular demands that more efforts should focus on the training of cadres for the Integrated Programme and on services for poor family planning acceptors.

[Fluctuations in numbers in box populations of Drosophila and selective genetic mechanism of their regulation].

Box populations of Drosophila melanogaster are characterized by two types of periodical fluctuations of numbers: with low and high frequency. High frequency fluctuations are determined by existence of preimago and imago stages and subsequent delay in density-dependent limitation of imago reproduction, duration of which is determined by time of preimago stage. The period of these fluctuations should be limited within two generation, that is confirmed by experimental data. Low frequency fluctuations with the period of 13-15 generations are the result of ecological density-dependent effect. In this case during pick density one can observe continuous degradation of population (i.e. decrease in fecundity and life time of imago) and following decrease in numbers. Temporary changes in fecundity of females and their offspring of the second generation are positively correlated with low frequency fluctuations in numbers. Such relationships show the possibility of density-dependent, cyclic, genetic changes in fecundity connected with fluctuations in numbers. It means that at the phase of growth in numbers when the density is still low, the selection is directed to the individuals with high fecundity sensible to overpopulation. The phase of decline in numbers is connected with high density and selection directed to the individuals with low fecundity in low density populations. The changes in genetic structure of fluctuating population lead to the weakening of this fluctuations and to the maintaining of population under such conditions.

South African prisoner wins payout after prison HIV infection.

An ex-prisoner who sued South Africa's prison authorities after contracting HIV has obtained a landmark payout--the first time the South African government has been held accountable for the spread of HIV in overpopulated prisons, Reuters reported on 12 February 2003.

Seropositivity of Epstein-Barr virus in Eastern Anatolian Region of Turkey.

In the present study, we have investigated the seropositivity of Epstein-Barr virus (EBV) and the epidemiological factors affecting this seropositivity. A total of 540 subjects living in central Elazig Province and its surroundings were enrolled in the study. IgG antibodies against capsid antigen of EBV (anti-VCA IgG) were determined by ELISA. Seropositivity was found to be 99.4%. There was a significant relationship between increased anti-VCA IgG levels and age, living in crowded families, exposure in public places and low income (p < 0.05). However, no significant alterations in serum anti-VCA IgG antibody were observed in terms of gender, blood transfusion and educational status (p > 0.05). In conclusion, we have demonstrated the commonly presence of EBV Infections in the public and contamination with the infection at early stages of life in the Elazig region. An overpopulated family environment, low income status and living in shared accommodation facilities play important roles in acquiring EBV infection.

A disposable vaginal speculum for the chimpanzee (Pan troglodytes).

The unanticipated underutilization of chimpanzees (Pan troglodytes) by the National Institutes of Health (NIH) for current NIH-supported research has prompted the application of a variety of contraceptive forms. Among the many methods attempted, the intrauterine device (IUD) has been described as having an efficacy similar to that described for humans. In addition, the device is both a financially sound and practical solution to prevention of overpopulation, while minimizing disruption of normal cyclic hormonal fluctuations. However, variations in the size and shape of the sexual skin of the female chimpanzee and the depth of the vaginal vault have posed physical constraints on the use of instrumentation developed for humans for assisting with insertion of the device. In addition the literature is lacking in specific methodology for pelvic examination in chimpanzees. Previously, the University of Louisiana at Lafayette New Iberia Research Center used a 3-ml plastic syringe case as a vaginal speculum to accommodate requirements of an approved research protocol in African green monkeys (Chlorocebus aethiops). Similarly, a simple and effective disposable vaginal speculum was developed for the chimpanzee. The closed tip of a plastic syringe case of appropriate size (20 to 60 ml) was removed to provide an open tube; momentary heating smoothed the cut edge. This simple speculum allowed for sufficient visualization of the cervix for assessment and assistance in the insertion of the IUD. Variously sized speculums were prepared to accommodate differences in animal size. This simple and effective speculum was clean, disposable, and inexpensive.

Chinese teenagers' concerns about the future: a cross-national comparison.

Teenagers in the U.S., U.S.S.R., Eastern Europe, Western Europe, and New Zealand have consistently rated death of a parent and nuclear war as their greatest concerns about the future. In the present study, however, Chinese teenagers rated overpopulation and environmental pollution as their greatest concerns; these were usually rated quite low by teenagers in other countries. While still of concern to Chinese teenagers, nuclear war seemed more remote to them than it did to U.S. and U.S.S.R. teenagers and therefore more survivable. Speculation is offered as to how teenagers' concerns reflect those of a country's general population.

[Environmental contamination and children].

Environmental contamination is a growing problem and a major worldwide concern. Industrialization and overpopulation underlie it's first origins. Contamination has severe outcomes for the whole planet, and very particularly for mankind. But these consequences are specially ominous for childhood. Childs are more extensively exposed to contaminants than adults, for a longer period of time, and the effects are more deleterious as they act during the period of growth and development Concerning the risks associated to contaminants there is still a large paucity of data, particularly as referred to low grade exposure.

[Mating behavior in mutant strains of Drosophila melanogaster at different population densities].

The effects of mutations and genetic background on the mating activity of male and receptivity of female Drosophila melanogaster have been studied at different population densities. Population density, as well as its combinations with other factors, significantly affects mating behavior of D. melanogaster. There are two distinct trends in the effect of this factor on mating behavior: the maximum larval overpopulation may cause either a significant suppression of the behaviors studied or an increase in their expressivity. The mating behaviors of wa and cn mutants against a certain genetic background changed similarly in response to varying population density.

Mycetes and urban areas.

Mycetes are ubiquitous organisms that can cause mycoses in human and animals. The role of animals in the epidemiology of human mycoses in urban areas is multiform, but here will be discuss only two features: A) animals as vectors of mycoses and B) animal substrates as growth factor of pathogenic fungi. A) Animals as vectors of mycoses: this role is important as zoofilic dermatophytes are very important agents of zoonosis; the urban dermatophytozoonoses are prevalent caused by Microsporum canis which is prevalent in cats and dogs. Cats are often asymptomatic carriers. The pattern of human dermatomycoses has changed in Italy during the past century: at the beginning of the century anthropophilic fungi were prevalent while at present the zoophilic fungi are the most important causes. B) Animal substrata as growth factor of pathogenic fungi: soil "animalization" (i.e., the addition of such debris as hair, skin scales, dropping and other organic matters) creates an optimal substratum for the growth and the multiplication of geophilic or saprophyitic fungi, such as Microsporum gypseum and Cryptococcus neoformans. The present human lifestyle, which favours a an overpopulation of birds, wild animals, domestic mammals and sinanthropic together with man in crowded areas seems to favour the formation of environments adapted to the abundant growth of some pathogenic fungi with consequent infection for man and animals. Finally, an environment heavily populated by fungi can cause allergic pulmonary reactions as well as reactions in other organs and tissues. The control of human and animal fungi, and the efficient use of a monitoring system require ample knowledge of mycological problems both in human and veterinary medicine and of efficient laboratories capable of resolving the needs of both disciplines. Close collaboration between veterinarians, doctors and mycologists is necessary in order to resolve health problems linked to mycosis.

[Contractile function, extracardiac regulation and morphological changes in the hearts of rats in the dynamics of a long-term continuously acting stressor situation].

24 hrs of the overpopulation stress effect induced a set of shifts typical for an acute stress syndrome in rats. Within 5 days the organism mobilizes the cholinergic stress-limiting system functioning for 15 days, limiting and normalizing the contractile function (CF) of the heart, and eliminating the contracture damages of the myocardium on the 30th day. Starting from the 30th day of the stress effect, adrenergic features of the extracardiac regulation (ER) re pear, the CF parameters increase, cardiac resistance to isometric loads decreases insignificantly, acute alterations appear in the myocardium (the 60th day). The ER shift towards sympatheticotonia is substituted by increased vagal effects on the heart (the 120th day). The wave-like oscillations of the tonus of the vegetative nervous system and CF of the heart occur against the background of appearing and accumulating small foci of cardiosclerosis.

Diarrhoeal disease--epidemiology and intervention.

The causes of the complex triad of gastro-enteritis, other related infections and malnutrition have at their roots poverty, lack of political will, overpopulation and destruction of the environment. Intervention involves multiple sectors of the economy and not only the health services of a country, and should take place as early as possible during the natural history of the disease.

[Lymphatic filariasis in Haiti: historical sequel or future public health problem in this geographic region?].

Wuchereria bancrofti persists in Haiti in localized foci, discontinuously distributed. The parasite, transmitted by Culex quinquefasciatus, develops in mesoendemic or hyperendemic fashion in urban and suburban areas, on the coastal plains sheltered from the prevailing winds, predominantly in the Nord department and around the gulf of La Gonave. Lymphatic filariasis is a major public health problem. Its impact is more evident in males on account of the genital repercussion of the disease. The acute lesions appear from early infancy and are related to microfilarial density. The chronic lesions begin with adolescence, and occur with high frequency in adults. Lymphatic filariasis has retreated in the countries where socioeconomic or sanitary conditions have improved: Puerto Rico, French West Indies, Cuba. In countries confronted by underdevelopment, overpopulation and uncontrolled urbanization, the disease is prevalent: Haiti, Dominican Republic, Trinidad. It is essential to carry out epidemiological investigations in order to identify the extend of the lymphatic filariasis problem in the Caribbean, particularly as population migration in both directions is becoming intensified: the native people looking for work in the industrialized countries, and tourists seeking the attractions of the tropics.

[The activity of Bacillus thuringiensis var. israelensis against black fly larvae under field conditions in South Bohemia].

The effectiveness of Bacillus thuringiensis var. israelensis spore suspension against black fly larvae was tested in two regulated brooks in the Ceské Budĕjovice district, South Bohemia. The sites under study were small, regulated, paved brooks of trapezoidal profile, with vegetation of different density, and with stream flow rates ranging from 40 to 90 cm/sec. Lyophilized spores (produced by Roger Belon) at a final concentration of 1 X 10(5) spores per ml were used. The spore efficacy was determined in both natural and artificial substrates in the field and under laboratory conditions, on the larvae collected at the control sites. A 90-100% mortality of the larvae was recorded in all parts of the brooks examined both in the natural samples studied in laboratory and in those kept on artificial substrates. In the area of fading spore suspension action, a higher percentage of the last-instar larvae was found in larval population which may be ascribed, in agreement with other authors, to an increased susceptibility of younger larvae to the preparation. The results of the tests demonstrated that Bacillus thuringiensis var. israelensis is an effective agent to control the black fly species occurring in Central Europe and may be of value mainly in the control of some species of veterinary importance overpopulated in regulated brooks.
