An epidemiological study of urinary tract infections in Benghazi, Libya.

Two groups of patients comprised 100 males and 100 females were investigated. All patients were more than 15 years of age and presented to the medical outpatient department with urinary tract symptoms. Fifty six male and female patients had no significant bacteriuria despite symptoms similar to those observed in 144 bacteriuric patients. Pyuria was observed in both bacteriuric and non-bacteriuric patients. Escherichia coli was the most common pathogen isolated from clinical specimens and was followed by Proteus sp. and Staph epidermidis. Escherichia coli was the main cause of acute urinary tract infection in adult females, while Proteus sp. caused a significantly greater proportion of symptomatic infections in males. Streptococcus faecalis was isolated only from males, and irrespective of chemotherapy. Infections with Staph epidermidis resembled the coliform ones, especially in the nature and severity of symptoms and in the degree of pyuria. Although E. coli and Proteus sp. were more predominant in our patients a wider range of resistant bacteria was involved. This development of resistance to antibacterial agents may be due to the frequent prescription of these agents by the general practitioners in public health clinics, dispensing of antibiotics by private pharmacies to the general public without medical prescriptions, and the misusage of these antibiotics by the general population.

The incidence of water-related diseases in the Brak area, Libya from 1977 to 1979, before and after the installation of water treatment plants.

The incidence of nine water-related diseases in the Brak oases of the Sahara desert, before and after the installation of water treatment plants, are reported. Immediately following installation of the plants there was a drop in the incidence of most of the water related diseases. There then followed a gradual deterioration in the treatment plants and within a year the incidence of four of the diseases was again rising. Furthermore neither malaria or giardiases showed any drop in incidence over the study period. Bacillary dysentary, infectious hepatitis and bilharzia did however drop significantly over the three years. A correlation between bacillary dysentary and the mean noon-time temperature for two of the three years was noted.

Pasteurella haemolytica A2 infection in two sheep flocks in Tripoli area (Libya).

330 sheep from two different flocks were examined for respiratory manifestations including serous to mucopurulent nasal discharge dyspnoea, cough and light depression. 55 animals were sick (16.6%) and 13 died (3.9%). Samples were taken from healthy, diseases and dead cases as well. Pasteurella haemolytica A2 was identified from 18 isolates, and serotyped by rapid plate agglutination (RPA). Drug susceptibility was tested and treatment applied in line with the results.

Field-oriented trial of the Chinese Brucella suis strain 2 vaccine on sheep and goats in Libya.

The Chinese Brucella suis S2 vaccine was studied in a flock of sheep and goats in field conditions. The locally-produced vaccine was orally administered in the form of a drench to 446 ewes, 50 lambs and 20 adult goats. After vaccination, abortion and excretion of the vaccine strain in vaginal discharges or in milk did not occur. Serological tests became positive in 92% of animals at 4 wk post-vaccination and declined to near nil after 1 yr. The protection conferred by the vaccine against a double virulent B melitensis conjunctival challenge which infected 10/10 control ewes was on average 53% in ewes (32/60) and 71% in goats (5/7). Abortion rates were respectively 100% (7/7) in control ewes versus 44% (25/57) and 28% (2/7) in vaccinated ewes and goats. The vaccine was thus found to be safe, antigenic and reasonably protective against the challenge dose used.

[Symptomatology of schizophrenia in Libya].

The most frequent symptoms of schizophrenia in Lybia are described and compared with symptoms of schizophrenia in Great Britain. The phenomenon of pseudoneologisms is explained. It was found that symptoms of primary importance after K. Schneider are applicable only to a small part of schizophrenias and then almost only to schizophrenics of the Chado-Hamitic Negroid tribes.

Haemolytic activity of Aeromonas species isolated in Libya.

Twenty seven Aeromonas strains (5A. hydrophila, 8A. sobria and 14A. caviae) isolated from children with diarrhoea and 34 Aeromonas strains (9A. hydrophila, 7A. sobria an 18A. caviae) isolated from children without diarrhoea were tested from haemolysin production. The results obtained showed that haemolysin production using human, horse or sheep erythrocytes was significantly associated with A.hydrophila and A sobria but not with A.caviae, regardless of whether these strains were isolated from children with or without diarrhoea. Human or horse rather than sheep erythrocytes are recommended for use in the haemolysin assay.

[Chest injuries treated in the town hospital of Sabhā, Libya].

This is a report on 113 patients with chest injuries subjected to treatment over the period 1991 through 1993. Of them 72.5 per cent are with ages ranging from 11 to 30 years, and 94.7 per cent are men. The series includes cases presenting incised penetrating wounds (49.6 per cent), involvement in motor-vehicle road accidents (26.5 per cent) and gunshot wounds (15.9 per cent). Of the total number 23 per cent are polytraumatized. Most of the patients (56.7 per cent) have hemopneumothorax with pleural drainage being the only treatment applied. In 15.9 per cent thoracotomy, laparotomy or thoracotomy plus laparotomy are used. Complications are recorded in 7.1 per cent, with deaths amounting to 2.7 per cent.

Immunoblot evaluation of the 100 and 130 kDa antigens in camel hydatid cyst fluid for the serodiagnosis of human cystic echinococcosis in Libya.

Two high molecular weight antigens with molecular masses approximately 100 kDa and 130 kDa were identified by immunoblotting camel hydatid cyst fluid, with 94% sensitivity in sera from surgically confirmed Libyan cystic echinococcosis cases. 40% of sera from surgically confirmed alveolar echinococcosis cases cross-reacted with the 100 and 130 kDa antigens, as did 5.3% of sera from human Taenia solium cysticercosis patients. No cross-reaction occurred with sera from human schistosomiasis mansoni or onchocerciasis patients. In addition, all sera from patients with non-hydatid space-occupying lesions (i.e. simple liver cysts, kidney cysts, lung tuberculosis, pulmonary carcinoma, pulmonary empyema, and lung abscess) were seronegative against the same antigens, as were control serum samples from healthy individuals. The 100 and 130 kDa antigens were strongly recognized by sera from cystic echinococcosis patients when camel or horse hydatid cyst fluid was used in immunoblotting but were only weakly recognized if sheep or human hydatid cyst fluid was used. Camel hydatid cyst fluid could be an important source of diagnostic antigens for human cystic echinococcosis in the Middle East endemic region.

Fertility of the cysts of Echinococcus granulosus in domestic herbivores from Benghazi, Libya, and the reactivity of antigens produced from them.

Unilocular cysts produced by Echinococcus granulosus were recovered from 110 domestic herbivores (60 sheep, 25 cattle, 20 goats and five camels) slaughtered in Benghazi. The proportion of the cysts from the sheep found to be fertile (75%) was higher than that of the cysts from the goats (55%), camels (40%), or cattle (0%). When tested in indirect haemagglutination assays (IHA) with eight sera from human cases of cystic echinococcosis, the fluid from the cattle cysts never gave a positive reaction. Antigens in the fluids collected from sheep or goat cysts did react with the sera, with antigens from each of the two sources giving similar titres with each serum. However, crude somatic antigens (prepared from protoscolices and brood capsules collected from sheep cysts) appeared to be more sensitive for the immunodiagnosis of human cystic echinococcosis than the cyst-fluid antigens.

Salmonella enteritis in children in Libya: role of antibiotic therapy.

In vivo sensitivity of salmonella has been studied in 38 children with acute gastroenteritis. They were treated for 7 days with gentamicin, to which the organisms were sensitive in vitro. Stool culture remained positive in 30 of the 38 cases. The children improved on supportive and symptomatic therapy. This study shows that antibiotics have no role to play in the management of acute salmonella gastro-enteritis in children.
