Chemical face peel.

Chemexfoliation is extremely useful as an adjunctive tool to the cosmetic surgeon, but it is essential that the procedure not be taken lightly. Anyone can swab the face with a solution, but injudicious patient selection and inattention to details can easily lead immediately to disaster. If the patient is well informed as to expectations and the surgeon understands all the ramifications of the procedure, continued confidence in this type of surgery will remain firm. The procedure is carried out by a team approach. The surgeon applies the solution and provides the patient with postoperative instructions, but the patient then must become a part of the team and follow these instructions explicitly in order to insure the best possible surgical result. If the procedure is properly done and precautions are addressed, the rewards of a refined facial appearance can be enjoyed by the patient, and the surgeon may experience a true sense of accomplishment.

Holmium:YAG laser thermokeratoplasty for hyperopia.

Holmium:YAG laser thermokeratoplasty (LTK), a procedure using a solid-state infrared laser to treat hyperopia, was performed on 10 patients in phase I and 16 patients in phase II--in a total of 29 eyes at the Hunkeler Eye Clinic. Phase II was redesigned after phase I results showed undercorrection and regression. The follow-up period ranged from 1 to 24 months (mean 10.9 months). A total of 79% of phase II patients were within +/- 1.00 D of intended correction at the 6-month visit. Looking at both phases together, no patients had J2 or better near vision preoperatively, but 75% had J2 or better at the 6-month visit. A total of 43% of eyes in phase II lost 1 line and 7% lost two lines of best spectacle corrected visual acuity due to induction of irregular astigmatism. The surgical challenges are to insure appropriate centration of the procedure about the optical axis. Concerns about regression and stability will be defined as these patients are followed through their 2-year visits.

[Changes in the surgical health care system and future aspect of surgery].

Since universal coverage of health insurance of Japan was introduced in 1961. Japan's health care system has become one of the few comprehensive systems from the perspective of national welfare. In 2002, as the gross national medical expenditure (GNME) increased to 30 trillion yen, the Japanese Government employed a policy of medical cost containment. In Japan, the ratio of GNME to the gross national product (GNP) is not so high in the world, and it ranks 6th among developed countries (OECD-Health Data, 2002). During the past 38 years from the scientific viewpoint the Joint Committee of Social Insurance established by the Multidisciplinary Group of Surgical Associations has evaluated the rationalization of surgical fee (include doctor's fee). If our system is employed, more fair surgical care may be provided in the future.

Insurance incentives for ambulatory surgery.

This study is an attempt to address both the extent to which surgical procedures on an outpatient basis substitute cost-effectively for inpatient procedures, and whether or not an insurance policy's financial incentives increase the volume of outpatient surgical procedures. In particular, given an insurance product of a given composition: What is the probability that the insured will have surgery? and if a surgery does take place, what is the probability that it will occur in an outpatient setting? Finally, the article assesses the implication of such products on the total cost of care by quantifying the insurance plans along two parameters, the relative user price for outpatient versus inpatient surgery and the absolute price for the inpatient surgery. The results indicate that insurance policies that offer relatively lower out-of-pocket payments for ambulatory surgery do not increase the probability that surgery will be done in the ambulatory setting. However, higher out-of-pocket payments for surgery, regardless of site, do reduce the surgery rate. There are other patient and market characteristics, especially the availability of freestanding surgery firms, that do influence the location of surgery.

Private health insurance and veterans use of Veterans Affairs care. RATE Project Committee. Rate Alternative Technical Evaluation.

OBJECTIVES: This study examined the effect of private health insurance on the use of medical, surgical, psychiatric, and addiction services for patients eligible for publicly supported care. METHODS: The authors assembled administrative databases describing 350,000 noninstitutionalized veterans who had been discharged from a Veterans Affairs (VA) inpatient medicine or surgery bed section during a 1-year period. Patient use of care was followed for 1 year after the index discharge. Patient insurance information came from Medical Care Cost Recovery Billing and Collection files obtained separately from each of 162 VA Medical Centers. Distances between VA and non-VA sources of care were estimated from the Health Care Financing Administration's Hospital Distance File. RESULTS: Insured patients were less likely to seek surgical care but were 12 times (65 years of age and older) and 73 times (63 years of age and younger) more likely to initiate outpatient medical visits than were their counterparts, adjusted for patient demographic, diagnostic, and index facility characteristics. Patients who had private health insurance also were 3.4 (> or = 65) and 2.6 (< or = 64) times less likely to use VA surgical care in response to changes in available surgical staff-to-patient ratios than were their uninsured counterparts. CONCLUSIONS: Private health insurance may substitute (reduce) or complement (increase) the continued use of publicly supported health care services, depending on patient age, care setting, and service type.

Obstructing pseudocyst of the duct of Santorini in pancreas divisum.

Pancreas divisum is a pancreatic duct anomaly that occurs due to failure of fusion of the dorsal and ventral ducts. While recognition of this anomaly is increasing due to more aggressive endoscopic retrograde cholangiopancreatography, its significance remains unclear. A patient with chronic pancreatitis and a history of alcohol abuse was noted to have pancreas divisum. At surgical exploration, intraoperative pancreatography revealed an obstructing pseudocyst of the duct of Santorini. Extended sphincteroplasty and cystduodenostomy as well as Roux-en-Y pancreatojejunostomy were necessary to insure adequate accessory duct drainage. Surgical therapy of pancreas divisum in chronic pancreatitis should be designed to correct existing pancreatic duct obstruction.

[Cost analysis of peritrochanteric fractures according to types of surgical treatment and progressive care from the viewpoint of financial providers].

AIM: The aim of this study to calculate the health insurance cost of treatment of patients with pertrochanter fracture of femur from the first hospital admission for 18 months follow up period according to different surgical methods and progressivity levels. DATA AND METHODS: Recruitment criteria were: 1) all patients with a hip fracture in 2000 defined by the International Classification of Disease (ICD) as "S7210"; 2) working age between 18-60 and 3) first admission to surgical unit, and had an operation. The cost analyses include the cost of acute and chronic in-patient care, outpatient care and sick pay. RESULTS: The total costs were the highest in case of those types of operations with lower cost of prothesis device and lower load stability (Ender 588.000 Ft, fix angled plate 534.000 Ft) because of the higher sick-pay costs. The total costs were the lowest in case of those types of operations with higher cost of prothesis device and higher load stability (Gamma 512.000 Ft, DHS 465.000 Ft) because of the lower sick-pay costs. The gain in recovery time with the 3-4 months shorter disability period can be seen in case of operations with higher load stability. The average length of stay decreases from 15-17 days to 9-11 along progressivity levels. CONCLUSION: With the application of load stable prothesis device the total health insurance costs were the lowest at the universities and national institute while these costs proved to be higher.

Frozen semen -- a poor form of fertility insurance.

Two previously fertile males have requested surgical vasectomy reversal after divorce and failure of their new younger wives to conceive by repeated inseminations with their stored frozen semen. The sperm had been preserved for six and five years, respectively, as "fertility insurance" prior to sterilization by bilateral vasectomy. It is clear that the storage of fertile semen does not guarantee future fertility.
