In sharp contrast to the excellent short-term results of total hip arthroplasty, the complex problems presented by severe, non-septic failure secondary to loosening and by major deficiencies in bone stock are formidable. Frozen femoral head allogenic bone provides the massive bone grafts needed in such cases when the autogenous femoral head bone is missing. Frozen femoral head allografts were used in 13 hips (12 patients). The femoral head allogenic bone made it possible to reconstruct a joint that otherwise would not be available to total joint cup arthroplasty. The long-term results of the allografts are unknown. A dual surgical approach for rigid fixation of the allogeneic bone to the pelvis using bolts makes it possible to fix the nut on the bolt under direct vision. Follow-up ranged from six to 34 months (mean, 17.3).

The bone response to two hydroxyapatite (HA) products (Osteogen and Alveograf) and a porous methacrylate resin material (HTR) in rats was compared. Fifty rats had four holes drilled in each iliac crest. The three alloplasts were placed in separate cavities and the fourth cavity was left empty as an absolute control. The survival times varied. The results were evaluated by subjective and histomorphometric analysis. Data were analyzed with ANOVA. There was a statistical difference in bone apposition between HTR and the HA products. The HTR was less osteoconductive, while HA consistently showed dense bone formation. The HA products are generally associated with greater bone formation, suggesting that HA is a better material for bone replacement than HTR.
Histomorphometric studies were performed on biopsies of the spongiosa from the iliac crest of patients with fracture who had been taking medication for a psychiatric condition. The biopsy had been performed in 30 individuals, 10 of which were males and 20 females, ages ranging from 23 to 63 years, and from 21 to 75 respectively. Twenty healthy individuals were use as control. All anti-psychiatric drugs in this study have been calculated in terms of chlorpromazine (CPZ). The purpose of the study and intraoperative biopsy had been explained to the patients and their agreements were obtained before the surgery. The undecalcified specimen made from the iliac crest were studied using a semiautomatic image analyzer. In the result of this study, there was decrease in the bone volume (BV/TV), the osteoid surface (OS/BS), and the eroded surface (ES/BS) in the affected subjects compared to the controlled group. However, there was increase in the relative osteoid volume (OV/BV) among the subjects who had had a psychiatric condition for 21 years or more and taking more than 500 x 10(4) mg of CPZ. There was no significant difference in the mineral apposition rate (MAR) between the dosage of CPZ and periods of intake; however the MAR was elevated in the affected group as compared to the controlled group. In conclusion, prolonged administration of CPZ may increase the volume of osteoid and disturb the calcification.
