Cutaneous fascioliasis: a case report in Vietnam.

A 40 year-old woman living in Gialai, Kontum, Vietnam, developed a red solid mass in the epigastric region. From ultrasound investigation, liver abscess and myositis of the intercostal muscle was diagnosed. Two weeks after treatment with antibiotics, the mass disappeared, but a migratory track developed in the right upper quadrant of the abdomen. An aspiration of the vesicular end of the serpiginous track showed a light brown, living worm that was later identified as an immature Fasciola sp. This is the first case report of cutaneous fascioliasis in the form similar to creeping eruption.

[A focus study from a case of human fascioliasis in Neuquén].

An epidemiological focal study was performed in Loncopué, Neuquén, Argentina, in November 2002 to detect the origin of the infection in a human case of fascioliasis confirmed by an indirect-ELISA test, six months before the study. Thirty five individual fecal samples were taken from domestic livestock, and watercress plants and snails were collected from the irrigation ditches connected to a main canal in the surroundings of the patient's house. A new blood sample was taken from the already recovered patient. The patient was still seropositive to Fasciola hepatica antigens. No metacercariae were found in the 222 watercress leaves checked. All the snails collected (n=130) were identified as Lymnaea viatrix and two out of 101 (2%) were infected with F. hepatica larvae. Coprological analysis showed F. hepatica eggs in 100% of goats (10/10), 82% of sheep (9/11) and 86% of bovines (6/7). The number of eggs per gram shed by positive goats (median=20.7, Q1=6.2, Q3=34.5) and sheep (4, 1.8, 13) was significantly higher than in cows (0.3, 0.3, 1.7) (p < 0.01). Local veterinary control programs were apparently not effective in this case. Anthelmintics used and treatment schedule should be revised and small herds raised at households should also be included and treated.

[Fascioliasis in children. A study of 10 cases].

These are the observation found in ten children with fascioliasis diagnosis at the Servicio de Parasitología, of the Instituto Nacional de Pediatría, from 1979 to 1990; six children in scholar age; one prescholar and three teenagers; only one was female. Four of them lined at the State of Mexico; three at Morelos; one at Puebla, another one at Oaxaca and the other one at Mexico City. In nine of the watercress eaten was confirmed. The most important clinic manifestations were: fever, weight lose, paleness, hepatomegaly, hiporexia, right hypochondrium pain and diarrhoea. Diagnosis was established by: counterimmunoelectrophoresis, indirect haemagglutination and there were found eggs by simple sedimentation, Ritchie's method and microscopic study of duodenal sample. Leukocytes counts were between 11,000 and 34,000/mm3. Eosinophils were to 77% with 24,430 totals. Only in three of them haemoglobin was found under 9 g/dL. All of them had hypergammaglobulinemia. In nine patients the alkaline phosphatase was found in higher levels; only in three of them, transaminase, oxalacetic and glutamic piruvic were found in higher levels. The most effective drug in the treatment was dehidroemetine.

Farm animals' fascioliasis in Ezbet El-Bakly (Tamyia Center) Al-Fayoum Governorate.

To continue the study on fascioliasis in Tamyia Center, some farm animals were investigated for natural infection with Fasciola species by stool examination. The results showed 40% infection in sheep, 20% in buffalos, 6.7% in donkeys and zero% in horses. The overall percentage of infection was 25.5. The sheep (total dose 1800mg) and the donkey (total dose (4500 mg.) were successfully treated with Mirazid. On the other hand, one buffalo was successfully treated by a total dose 7500mg, the seconds one did not cured, but the eggs deposited per gm markedly decreased.

Fascioliasis among local and imported sheep in Saudi Arabia: parasitological and serological diagnosis.

Eight-hundred-twelve local and imported sheep, slaughtered at Riyadh abattoir, were subjected for parasitological diagnosis of fascioliasis by detection of eggs in the stool and worms in the liver and for serological diagnosis by detection of circulating anti-Fasciola IgG (CAFIgG) and circulating Fasciola antigens (CFAgs) using the indirect ELISA and the double antibody sandwich ELISA respectively. Detection of eggs revealed 13.5% infection rate compared with 21.9% by detection of worms (p<0.001). Infection rate was significantly higher (p<0.001) among the imported sheep (15.1%) than among the local ones (4.96%). Positivity for CAFIgG was 64.3% among all sheep while that of CFAgs was 30.2% (p<0.001). Sheep passing Fasciola eggs showed positivity of 99.1% for CAFIgG and 87.3% for CFAgs (p<0.05). Sheep free from any parasitic eggs showed positivity of 61.2% for CAFIgG and 21.1% for CFAgs. There was a highly significant association (p<0.001) between positivity of both CAFIgG and CFAgs and positivity of eggs in the stool.

[Chronic fascioliasis with destructive cholangitis].

A 56-year-old Greek working in an electroplating shop, in Germany since 1965, had experienced abdominal pain with weight loss for about 16 years. He had received treatment for a variety of diagnoses. With the aid of ERC and aspired bile, the diagnosis of chronic fascioliasis was finally established.

Electron microscopic study of the peritoneal macrophages of rats with chronic fascioliasis and the carcinogenic effect of diethylnitrosamine.

The ultrastructure of peritoneal macrophages of rats with chronic fascioliasis and the carcinogenic effect of diethylnitrosamine (DENA) were studied. The phagocytic activity of macrophages on Staphylococcus aureus were examined in its dynamics (in the 1st, 5th and 24th h). The ultrastructural changes of the macrophages were the most pronounced in animals injected eight times with DENA and the weakest in the group of animals infected twice with Fasciola hepatica. As to the phagocytic activity of the macrophages the following events were observed: attraction and adhesion of the bacterial cells to the surface of the macrophages, their inclusion in phagosomes; partial to full lysis of the bacteria; and formation of residual bodies. The phagocytosis was the most active in macrophages obtained from animals infected twice with Fasciola hepatica and the weakest in those from the DENA-treated animals.

Antihepatotoxic properties of uridine-diphosphoglucose in liver fluke infection. Experimental fascioliasis in the rat.

The antihepatotoxic properties of uridine-diphosphoglucose (UDPG, Toxepasi) have been evaluated in a well-established model of liver damage, the liver fluke infection (experimental fascioliasis in the rat), which causes a dramatic loss of the microsomal drug-metabolizing monooxygenase (MFO) and glucuronosyltransferase (GT) enzyme systems as a consequence of peroxidative damage to microsomal membrane lipids. Administration of 100 mg/kg UDPG i.p. to the infested rat for the entire course of the infection (40 days) positively affects the parameters reflecting the integrity of the liver cell (serum glutamate-pyruvate, GPT and glutamate-oxaloacetate, GOT, transaminases) and the detoxifying capacity of the liver (cytochrome P-450, cytochrome b5, cytochrome P-450-dependent p-nitroanisole O-demethylase and aniline hydroxylase activities, and the p-nitrophenol glucuronidation) and greatly reduces the lipid peroxidative phenomen in membranes from whole liver (tissue malonic dialdehyde content) and in membranes of the microsomal fraction (conjugated diene absorption). As a consequence of this, the total lipid and phospholipid contents of the liver are restored, there is minimal loss of latency of GT enzyme(s), cytochrome P-450 conversion to cytochrome P-420 is fairly negligible and total liver glutathione content is also restored. Therefore, UDPG restores liver function by protecting the endoplasmic reticulum membranes from the oxidative stress resulting from activation of the CN-insensitive respiratory burst of the phagocytic cells consequent to Fasciola hepatica invasion, migration and growth. It is very likely that UDPG acts as an effective antilipoperoxidative agent through both direct (as demonstrated by our in vitro experiments) and indirect mechanisms (stimulation of the glycolytic pathway, and hence of the reducing equivalents----glutathione----vitamin E supply).(ABSTRACT TRUNCATED AT 250 WORDS)

[A family outbreak of acute hepatic fascioliasis].

The diagnosis of human fascioliasis was established in an 11 year old boy with prolonged fever by complement fixation and ELISA tests. Good clinical response followed the treatment with emetine. Four other family members were also affected with the disease and their treatment was equally effective. The origin of the infection was traced to the ingestion of infected watercress at a site 15 km away from Santiago.

Human fascioliasis: T cell subsets in liver before and after bithionol treatment.

Liver biopsies from 5 patients with established fascioliasis, before and after bithionol treatment were studied by immunoalkaline phosphatase technique for relative distribution of T cells and their subpopulations. T cell and its subsets are defined for OKT3+ (pan T), OKT4+ (helper/inducer) and OKT8+ (suppressor/cytotoxic) cells by using mouse monoclonal antibodies. Before bithionol treatment, lymphocytic infiltration in all hepatic lesions were predominantly of OKT3+ (pan T) lymphocytes. The distribution of OKT8+ cells was moderate to severe in comparison to the few OKT4+ cells presentation. After bithionol a noticeable regression of the OKT3 lymphocytic in all liver sections. The majority of the lymphocytic infiltration was of the OKT8+ cells, in comparison to the absence of the OKT4+ ones. This may indicate that suppressor/cytotoxic lymphocytes may have a role in the immune regulation of the disease and the mode of action of bithionol is by the accentuation of this immunoregulatory effect.

[Fascioliasis of the liver].

Clinico-morphologic characteristics of a late-stage fascioliasis in a woman, 21, living in the South Urals. The disease is diagnosed after the operative material (the 4th and 5th liver segments resected together with the gallbladder) was investigated. The importance of both damage by migrating fasciolae and the reaction of hypersensitivity in pathogenesis of the parasitic hepatitis is shown.

[Experimental equine fascioliasis: evolution of serologic, enzymatic and parasitic parameters].

Three, four, and one horses were respectively infected with 100, 1,000, and 5,000 metacercariae of Fasciola hepatica. Six of them were reinfected 38 weeks later with 1,000 metacercariae each. Specific antibodies assayed by counter-electrophoresis, passive hemagglutination and ELISA tests appeared three to six weeks post-infection and peaked 10 to 17 weeks post-infection. Horses infected by 1,000 metacercariae and more showed 17.6% of positive samples by counter-electrophoresis, 49.2% by ELISA, and 75.6% by passive hemagglutination. Plasma glutamate dehydrogenase and gamma-glutamyltransferase levels increased significantly 3 to 5 months post-infection in the most infected animals. Eggs of Fasciola hepatica were only observed in 2 of the 8 horses, 14 and 15 weeks post-infection. This last observation indicates the limits of fecal examination in the diagnosis of fascioliasis in horses.

Uncommon complications of human fascioliasis in Alexandria.

Fascioliasis is becoming more frequently discovered among citizens of Alexandria. The main presenting clinical manifestations including hepatic pain, colics, fever, anorexia, discomfort with meals and hepatomegaly. We select here 4 cases with unusual presentation from patient's record of the last year: one case developed liver abscess due to F. hepatica infection. Two cases were encountered during cholecystectomy in patients suffering from cholecystitis with cholelithiasis; in one of them the gall bladder had ruptured and the patient developed an abscess in the liver related to the gall bladder bed. In both cases F. hepatica worms were found in the bile duct. The fourth case presented with acalcular cholecystitis with empyema of the gall bladder.

[Fascioliasis in moufflons].

Subacute fascioliasis was diagnosed by pathomorphological and parasitological investigations on 13 dead moufflons (Ovis ammon musimon) from a herd of 21 animals (mortality 62%) which had succumbed between January and April 1988. The flock had been kept on meadow in the so-called Leipziger Auenwald. The main findings like severe hepatitis traumatica fasciolosa, fibrinous and fibrous perihepatitis, chronic interstitial hepatitis (pseudocirrhosis), cholangitis fasciolosa (X 13), wasting (X 8), heart dilatation (X 10), lung oedema (X 12), anemia (X 5), ascites (X 3), gut oedema (X 3) and occasionally observed lesions are described in detail and discussed with regard to diagnosis and pathogenicity. Beside severe infection with Fasciola hepatica (juvenile and adult flukes) the parasitological investigation demonstrated, in some cases, various additional but unimportant infections with protostrongylids, gastro-intestinal nematodes, coccidia (X 2) and Moniezia expansa (X 1). The analysis of meteorological data (January 1987 till March 1988) established optimal conditions for F. hepatica development stages and Galba truncatula so that high multiplication and infection rates of the snails and long surviving of metacercariae must be assumed.

Percutaneous cholangiography in the diagnosis of hepatic fascioliasis.

A case of hepatic fascioliasis is reported in which the diagnosis was made by direct visualization of the parasite by means of percutaneous transhepatic cholangiography. The peculiar radiological image should be included in the differential diagnosis of biliary tract pathology.

Pharmacokinetics of sulphobromophthalein, lidocaine and indocyanine green in the course of subclinical fascioliasis in sheep.

The pharmacokinetics of three drugs proposed for the assessment of liver function: sulphobromophthalein (BSP), lidocaine and indocyanine green (ICG) were determined in sheep four, eight, 12, 16 and 24 weeks after their infestation by an oral administration of 150 metacercariae of Fasciola hepatica. The disposition of BSP was altered by a significant decrease in its total plasma clearance from eight weeks after infection onwards. This change could be related to the low values of the elimination rate constant consistent with a reduced liver cytosolic conjugation of the dye to glutathione. Lidocaine pharmacokinetics were unaffected by parasitism and the only effect on ICG was an increased volume of distribution consistent with the liver hypertrophy caused by the subclinical fascioliasis.

[Fascioliasis--a contribution to the differential diagnosis of focal liver processes].

The case of a 60 years old patient with a rare parasitosis, fascioliasis hepatica, is presented. The clinical, parasitologic, radiologic and histologic findings are discussed with reference to the literature. The diagnosis was established by examination of the faeces of the patient. The radiologic examinations document the extent of the organic involvement, in the presented case that of the liver. The finding of circumscribed hypodense liver lesions can be decisive for the further diagnostic proceedings only in the light of an appropriate history and typical laboratory findings.

Fascioliasis in cattle in Louisiana: development of a system to predict disease risk by climate, using the Thornthwaite water budget.

A system correlating climate with the annual risk of fascioliasis in cattle in central Louisiana was developed, using the Thornthwaite water budget and a 6-year data base that included records on herd prevalence rates, transmission to fluke-free sentinel calves, and snail population dynamics. The system developed was compared with modifications of the wet-day and Mt forecasting systems previously developed for use in the oceanic climate zone of western Europe. The wet-day system correlated poorly with transmission data, whereas the Mt and Thornthwaite water budget-derived systems correctly ranked 5 of the 6 years in terms of annual numbers of flukes transmitted to sentinel calves. Compared with the Mt system, the water budget-based system more closely correlated with transmission data, on the basis of statistical regression analysis. The water budget system was calculated by use of a microcomputer software program, and provided a means of measuring the effect of moisture stress and flooding in snail habitats that serve as foci of transmission of Fasciola hepatica to cattle on pastures.
