Factors influencing utilization of the curative health services among children aged 6-60 months in Bahrain (Part II).

Two hundred twenty one out of 2587 children aged 6-60 months were selected at random from 8 villages in Western region of Bahrain. The data were collected by interviewing the mothers and checking out the health records. The study revealed that family size, mother's education, socioeconomic status and mother's attitude toward the curative services were significant factors for the use of the curative care. However, no significant relationships were found between the utilization of the curative care and age and sex of child, family structure and the situational factors.

[Lymph node involvement: the only prognostic factor after curative resection of cancer of the stomach. Results of a multivariate analysis].

The aim of this study was to evaluate the independent influence of clinical and pathological variables on survival of patients with gastric carcinoma using the Cox regression proportional hazard model. Of 156 patients operated on for gastric adenocarcinoma, 46 (29.5%) underwent palliative operation, 24 (15.5%) had a palliative resection, and 86 (55%) had a curative resection. The overall 5-year survival rate was 25 +/- 4%. After curative resection, the 5-year survival rate was 44 +/- 6%. Univariate analysis applied to these patients showed that poor survival was related (p less than 0.01) to: age (over 80 years), absence of epigastric pain, vomiting and dysphagia, total gastrectomy, tumor size (more than 4 cm), lymph node involvement (LNI), invasion through the muscularis propria, absence of intestinal metaplasia near the tumor, and linitis plastica. In multivariate analysis, lymph node involvement was found to be the only independent prognostic factor. The 5-year survival rate was 75.5 +/- 8% without LNI, 28 +/- 10% with proximal LNI and 7 +/- 6% with distal LNI. Our results suggest that classification into 3 LNI groups is the best staging system for curative resection in gastric carcinoma.

[Intraarterial and intraportal infusion chemotherapy to prevent hepatic metastasis after curative resection of pancreatic cancer].

In order to prevent postoperative hepatic recurrence after curative resection of pancreatic cancer, we developed a new method of intra arterial and intraportal infusion chemotherapy. This method involves placing a catheter in the portal vein and another catheter in the hepatic artery, before closing the abdomen. Postoperatively, 5-FU (125 mg/day) was infused continuously into these two catheters, for more than 28 days. Twelve patients received this therapy, without liver dysfunction or myelosuppression. During this therapy, the concentration of 5-FU was more than 0.14 micrograms/ml in the portal blood and 0.02 micrograms/ml or less in the systemic blood. The cumulative survival rate was 77% at 18 months and cumulative rate of hepatic metastasis was 25% at 18 months. These data were better than those in the control group (45 curative resections without chemotherapy). Therefore, this method seems beneficial for patients who have received curative resection for pancreatic cancer, since lowers the incidence of hepatic recurrence.

[Curative effects of modified xiao-jin pills and chemotherapy after surgery in patients with mid and late phase gastric cancer].

From 1986, the authors mainly used Modified Xiao-Jin Pills (MXJP) added with chemical treatment to cure patients in mid and late phase of gastric cancer after operation. 72 patients were verified as gastric cancer by pathological section and phases were divided according to TNM classification which was revised by the national cooperative group of gastric cancer in 1978. The patients were randomly divided into curative and control groups. The two groups were basically similar in sex, age, clinic pathological phase and operation. One month after operation, the two groups received the same treatment except that the control group did not take MXJP. The indexes of observation consisted of living rate, substance of tongue and 5 indexes of blood coagulation (AT-III, Fn, FA, VIII R:Ag and beta TG). The curative effects showed that MXJP could enhance gastric cancer patients living rate for 1, 1.5 and 2 years among which it was obvious for phase IV's gastric cancer patients to enhance 1 year's living rate (P less than 0.05), and lower the negative rate of substance of tongue (P less than 0.001). The indexes of AT-III, Fn FA and VIII R:Ag and beta TG of gastric cancer patients had changed for the better (P less than 0.05). The results showed that MXJP can enhance the capacity of body to antagonize coagulation fibrinolysis, remove blood stasis and further enhance the living rate of patients. MXJP has better curative effects of short term.

Factors influencing prognosis of gastric cancer after curative resection.

Four hundred eighty eight patients with gastric cancer were admitted to the Department of III Clinica Chirurgica of the University of Bologna-Italy; 451 patients underwent surgery: there were 17 deaths with an operative mortality rate of 3.8%. Only 286 curative resections were performed 252 sub-total gastrectomies and 34 total gastrectomies. The purpose of this paper is to define the prognostic role of some clinical and pathological variables in patients with carcinoma of the stomach who underwent curative resection. The hospital records of 229 patients submitted to curative resection between 1969 and 1982 were retrospectively reviewed. Fifty two patients were lost to follow-up study and 4 died for post-operative complications. The variables analyzed included: age (greater than 65 years, less than 65 years), sex, time of onset of clinical symptoms (greater than 6 months, less than 6 months), site of cancer (lower, two thirds, upper), size (greater than 3 cm, less than 3 cm), gross appearance (ulcerating, non ulcerating), histologic type (intestinal, diffuse), invasion of the gastric wall (T1-T2 vs. T3-T4), nodal status (negative, positive), stage (S1-S2 vs. S3-S4). The statistical significance of the results was calculated using chi 2 test and comparing survival at 1, 3 and 5 years. Of the variables selected only six (age greater than 65 years, size greater than 3 cm, diffuse histologic type, degree of the gastric wall invasion, positive nodal involvement, stage 3-4) were validated by statistic evaluation, whereas the others lost their prognostic relevance.(ABSTRACT TRUNCATED AT 250 WORDS)

Importance of Lyt 2+ T-cells in the curative effectiveness of a low dose of melphalan for mice bearing a large MOPC-315 tumor.

We have previously demonstrated that the curative effectiveness of a low dose (2.5 mg/kg) of melphalan (L-phenylalanine mustard; L-PAM) for mice bearing a large s.c. (approximately 20 mm in diameter) MOPC-315 tumor and extensive metastases requires the participation of T-cell-dependent antitumor immunity in tumor eradication (S. Ben-Efraim et al., Cancer Immunol. Immunother., 15: 101-107, 1983). Here we show that the Lyt 2+ T-cells, and not the L3T4+ T-cells, participate in the cure of such tumor-bearing mice by a low dose of L-PAM. Specifically, depletion of Lyt 2+ T-cells from mice bearing a large MOPC-315 tumor by treatment with monoclonal anti-Lyt 2.2 antibody abolished the curative effectiveness of the low dose of drug. In contrast, depletion of L3T4+ T-cells from mice bearing a large MOPC-315 tumor by treatment with monoclonal anti-L3T4 antibody did not reduce significantly the curative effectiveness of the low dose of drug. Histological examination of tumor nodules on various days following low-dose L-PAM therapy revealed widespread lymphocytic infiltration by Day 5 following the chemotherapy, and this infiltration was drastically reduced when the L-PAM-treated tumor bearers were treated with either anti-Thy 1.2 or anti-Lyt 2.2 antibody but not with anti-L3T4 antibody. The antitumor immunity exhibited by Lyt 2+ T-cells derived from mice which were in the process of eradicating a large MOPC-315 tumor following low-dose L-PAM therapy was exploited successfully to confer systemic antitumor immunity to mice bearing a barely palpable tumor. Specifically, the adoptively transferred Lyt 2+ splenic T-cells, in conjunction with a subcurative dose of L-PAM, brought about the cure of most mice. The Lyt 2+ splenic T-cells from L-PAM-treated MOPC-315 tumor bearers were also found to be capable of exerting a direct potent lytic effect against MOPC-315 tumor cells in an antigen-specific manner. Thus, it is conceivable that the direct cytotoxic activity of Lyt 2+ T-cells for MOPC-315 tumor cells is responsible, at least in part, for the ability of the Lyt 2+ T-cells from L-PAM-treated MOPC-315 tumor bearers to bring about the eradication of the tumor burden not eradicated through the direct antitumor effects of the low dose of drug.

[The relation of the patient's condition and outcome of drug maintainance therapy in schizophrenia (analysis of the curative effect in 324 cases)].

Cf, the methodology and diagnostic standard of 12 collaborative units about "Epidemiological investigation" of 1982, we traced to investigate the relation between the patients' condition outcome and drug maintain therapy of 324 cases with schizophrenia in community. The investigative result showed the cure rate of insisting on taking medicine group was 25.21%, the effective rate was 97.48%, the cure rate of irregular taking medicine groups was 6.63%, the effective rate was 68.37%, there was remarkable difference between the cure rate and the effective rate in two groups. Otherwise we also compared the patients, condition of insisting on taking drug groups with during investigation. We found there was no remarkable change that showed insisting a drug maintain therapy out the hospital to the curative effect of the disease to possess on important meaning. The pattern also compared the curative effect of a time onset of disease group and many times. There was no remarkable difference about the statistical analysis of the curative effect among each group. It showed me never lose confidence to the patients. We should treat actively them.

Curative cancer chemotherapy.

Cancer chemotherapy provides variably effective treatment for the majority of forms of human cancer and curative treatment for some 12 categories of cancer. Curative treatment is defined as the proportion of patients who survive beyond the time after which the risk of treatment failure approaches zero, i.e., the disease-free survival plateau. This progress has resulted from a closely integrated scientific effort, including drug development, pharmacology, preclinical modeling, experimental design with respect to clinical trials, quantitative criteria for response, and a series of clinical trials (initially in children with acute lymphocytic leukemia) in which the importance of complete remission, of dose and schedule, of sequencing chemotherapeutic agents, of pharmacological sanctuaries, and particularly of combination chemotherapy was studied. The principles derived from these studies, particularly those relating to combination chemotherapy, resulted in curative treatment for disseminated Hodgkin's disease, non-Hodgkin's lymphoma, pediatric solid tumors, testicular cancer, and limited small cell lung cancer. Many patients with certain stages of solid tumors, such as breast cancer and osteogenic sarcoma, are at high risk of having disseminated microscopic disease. Experimental studies indicate that treatment which is only partially effective against macroscopic disease is much more effective against microscopic tumors. Therefore chemotherapy is administered immediately following control of the primary tumor in patients at high risk of having disseminated microscopic disease, a treatment known as adjuvant chemotherapy. This program has been highly successful in increasing the cure rate in patients with pediatric solid tumors and in prolonging disease-free survival in patients with premenopausal breast cancer. Given dissemination of the technology, it is estimated that 15,000-30,000 patients per year are potentially curable in the United States. Curability of cancer by chemotherapy generally is inversely related to age, i.e., the above tumors are most common in children and young adults. There are new and promising treatment strategies, such as neoadjuvant chemotherapy and autologous bone marrow transplantation. The revolution in molecular and cellular biology is providing an increase in targets, rationale, and opportunity for more effective and novel chemotherapeutic approaches.

Cancer of the stomach: are curative resections of any importance with respect to postoperative survival in patients with locoregional disease?

Ninety-seven patients operated on for cancer of the stomach with locoregional spread, i.e. invasion of adjacent tissue and/or lymph node metastases, have been studied in order to evaluate whether the surgeon's intraoperative assessment of "curative resection" was of any significance with respect to postoperative survival. Survival was not significantly different in patients who had undergone curative resection as compared with those who underwent palliative resection. Patients with unresectable disease had a significantly reduced survival rate compared with the other two groups. We conclude that resection should be performed if at all possible in patients with locoregional disease as resection seems to be effective in prolonging survival. On the other hand, whether the resection is though to be curative or palliative seems to be of less importance with respect to postoperative survival.

Second laparotomy following 'curative' resection for colorectal cancer.

Second laparotomy was undertaken in 63 patients with suspected local recurrence following 'curative' resection for colorectal cancer. Nine patients proved to have benign disease and 4 a new cancer. Local tumour recurrence was confirmed in 50 patients. Of these, 19 underwent further resection of the tumour, 19 had palliative colostomy or bypass, and laparotomy only was performed in 12. Five of 15 patients undergoing secondary 'curative' resection survived 1 year and 3 survived 18 months. In contrast only 6 of the remaining 35 patients survived 1 year and none survived 18 months. Second laparotomy may be of benefit in a small number of patients with local recurrence following apparently curative resection for colorectal cancer.

Prostate cancer. Mortality and morbidity after non-curative treatment with aspects on diagnosis and treatment.

AIMS OF THE STUDY: To investigate the mortality, need for hospital care and palliative treatments in patients with prostate cancer (PC) treated with non-curative intention (i.e. deferred or hormonal treatment). To evaluate acceptance by patients and complications of a new diagnostic procedure for PC -- transrectal ultrasound (TRUS) and core biopsies. To investigate if knowledge of prostate volume enhances the accuracy of prostate specific antigen (PSA) to indicate non-palpable PC. Finally, to investigate how neo-adjuvant hormonal treatment before radical prostatectomy affected PSA and tumour volume. METHODS: In a retrospective analysis of all 536 patients with a known diagnosis of PC who died in the city of Göteborg during the years 1988-90, age at diagnosis, survival time, need for hospital care and cause of death were registered (I and II). A questionnaire was sent to 511 patients who underwent TRUS with or without prostatic biopsies (III). In 120 consecutive patients admitted for TURP due to presumed benign prostatic hyperplasia, a comparison was made between PSA and prostate-volume-adjusted (measured via TRUS) PSA (PSADensity) to indicate the presence of non-palpable PC (IV). Of 56 patients who underwent radical prostatectomy, 28 received 3 months' pretreatment with a GnRH-agonist. The effects on tumour volumes (assessed by the planimetric method on whole mount slides) and PSA were studied (V). RESULTS: Overall, 62% of patients with a known diagnosis of PC died of the disease when all patients were followed from diagnosis until death (up to 25 years). Of patients in stage M0 at diagnosis, 50% died of PC. However, in patients who survived for more than 10 years the mortality reached 63% (I). The average PC patient needed 27 days of hospital stay (geriatric wards excluded) and 185 patients needed at least one palliative TURP, 103 patients palliative radiation therapy and 55 patients procedures due to upper urinary tract obstruction. The lion's share of these resources was consumed by patients who later succumbed to PC (II). Ninety-five per cent of patients reported none or minor discomfort after TRUS of the prostate and 92% if TRUS was combined with transrectal core biopsies of the prostate. Haematuria for > 2 days occurred in 13%, haematospermia > 2 days in 9% and blood in stool > 2 days in 3% among patients who underwent core biopsies but none of these patients needed active treatment. Overall, 4.1% of biopsied patients experienced urinary tract infection (III). The use of PSADensity with a cut-off value of 0.10 ng/ml/cc rendered both higher sensitivity (75 vs 50%) and positive predictive value (0.33 vs 0.15) for indicating non-palpable PC in symptomatic patients with benign findings on digital rectal examination (IV). Pretreatment with a GnRH-agonist resulted in a significant PSA decrease not explained by changes in tumour volume. Tumour volume reduction was found in 36% of the patients. CONCLUSIONS: According to these studies, PC is a progressive disease with considerable mortality and morbidity when managed by non-curative intention. Since new diagnostic and therapeutic methods described in this thesis are well accepted by patients and may increase the chance of radical surgery, it is reasonable to offer younger patients with long life expectancy the chance of early detection and treatment with curative intention.

Clinicopathological features of patients who died with second primary cancer after curative resection for gastric cancer.

The appearance of a second cancer in patients who had undergone curative operation for the first gastric cancer is one of the crucial problems for the clinician. We analysed data on 910 patients with gastric cancer treated with curative resection, with respect to the risk factors for second primary cancer and the prognosis. Of 910 patients, 69 (7.6%) died with a second primary cancer. In patients with a second primary cancer, there were more men and age was more advanced, compared to the survivors. The gastric tumor was larger, the serosal invasion was more prominent and lymphatic involvement was more frequent. The postoperative 5-year survival for patients with a second primary cancer was 60.9%, the 10-year rate was 31.9% and the 15-year was 19.6%. Multivariate analysis revealed that risk factors for a second primary cancer was advanced age, male sex and a larger tumor. Our findings suggest that during the follow-up of patients with gastric cancer treated by curative resection and risk factors, a second primary cancer may occur in other organs, in addition to a recurrence of the first cancer.

Importance of TNF production for the curative effectiveness of low dose melphalan therapy for mice bearing a large MOPC-315 tumor.

We have previously shown the importance of the acquisition of CD8+ T cell-dependent tumor-eradicating immunity for the curative effectiveness of low dose melphalan (L-PAM, L-phenylalanine mustard) for mice bearing a large MOPC-315 tumor. Here we show the importance of TNF production for the curative effectiveness of low dose L-PAM for such tumor-bearing mice. Studies regarding the mechanism(s) through which TNF exerts its antitumor effects in L-PAM-treated MOPC-315 tumor-bearing mice (L-PAM TuB mice) revealed that MOPC-315 tumor cells are not sensitive to the cytotoxic effects of TNF either before or after the chemotherapy. However, TNF is essential for the in vitro generation of potent CTL activity by CD8+ T cells from L-PAM TuB mice. Moreover, addition of exogenous TNF to in vitro stimulation cultures of spleen cells from untreated mice bearing a large MOPC-315 tumor resulted in the generation of a greatly enhanced CTL activity against MOPC-315-associated Ags. Finally, we have previously shown that TCR V beta 8+/CD8+ T cells are involved in the curative effectiveness of low dose L-PAM for mice bearing a large MOPC-315 tumor. Here we show that TNF is important for the ability of MOPC-315-specific V beta 8+/CD8+ T cell lines from L-PAM TuB mice to bring about tumor eradication upon adoptive transfer into tumor-bearing mice. Taken together, these studies illustrate that low dose L-PAM mediates its therapeutic effectiveness for mice bearing a large MOPC-315 tumor, at least in part, via TNF production (e.g., by V beta 8+/CD8+ T cells), which in turn promotes the generation of anti-MOPC-315 CTL activity.

[The influence of changing the Ca2+ concentration of the point quze (PC3) on the curative effect of puncturing neiguan (PC6) in experimental arrhythmic rabbits].

The experimental arrhythmic model was made with Aconitin in rabbits. The Neiguan (PC6) point was punctured to observe the influence on recovering time of arrhythmia. After the curative effect of puncturing Neiguan(P6) was confirmed, Ca2+ in the Quze (PC3) point of the Pericardium is chelated by EDTA solution to observe the influence on curative effect of arrhythmia by puncturing Neiguan. The result shows that the curative effect doesn't exist after the Ca2+ in Quze is chelated. It is probable that Ca2+ is one of the most important factors producing acupuncture effect. As a result, Ca2+ probably involves in the activities of channels and collaterals.

Characteristics and sequence of the recurrent patterns after curative esophagectomy for squamous cell carcinoma.

BACKGROUND: A large number of patients with esophageal cancer experience recurrence, even when a curative operation is performed. The purpose of this study was to clarify the characteristics of recurrence after a curative esophagectomy. METHODS: The patterns and the time of recurrence after curative esophagectomy were examined in 187 cases of thoracic esophageal cancer. Ninety-five of these patients died of evident recurrence. The recurrence patterns were classified into lymphatic, hematogenic, mixed type (lymphatic and concomitant hematogenic), and intramural recurrences. RESULTS: The number of cases in each recurrence group were 46, 23, 22, and 4, respectively. The disease free intervals were fairly closely correlated with pTNM stage in the lymphatic and mixed type groups whereas they were within 2 years in all cases of the hematogenic group. Among the 22 cases of the mixed type, hematogenic recurrence preceded lymphatic recurrence in only one case. Furthermore, 17 of 20 cases with either cervical or mediastinal lymph node recurrence of the mixed type were accompanied with lung recurrence, whereas the remaining two cases with abdominal lymph node recurrence had liver recurrence. CONCLUSIONS: These findings suggest that most mixed type recurrences and most lymphatic ones are primarily caused by lymph node metastases; thus the importance of a radical lymphadenectomy was supported. On the other hand, a careful examination for hematogenic recurrence is essential soon after operation.

The value of follow-up after curative surgery of colorectal carcinoma.

Between 1978 and 1989, 1045 of 1399 patients (580 male and 474 female) had undergone curative surgery for colorectal carcinoma. Of these patients, 350 (33%) had recurrences, another 16 (1.5%) developed a metachronous colorectal cancer, and 23 (2%) had cancers of other organs. An isolated locoregional recurrence was found in 75 of 350 (21%). The remaining 275 of 350 (79%) of the patients showed systemic dissemination of the carcinoma. Reoperations with curative intent were performed on 56 of 350 (16%) of the patients. Only 21 of the 56 resected patients (38%), i.e., 21 of 350 (6%), are without recurrence at the end of the follow-up period on December 31, 1990. Despite a curative reoperation, 62% of the patients again developed recurrent growths. There is an imbalance of the efforts invested in tumor follow-up and the benefits gained. Further follow-up programs should be investigated in a controlled, prospective fashion.

[Late damage of curative oncologic therapy. Results of a patient sample with Hodgkin's disease and testicular tumors at the Hannover Medical School].

BACKGROUND: The present analysis deals with 2 main problems among therapy-associated late toxicities in patients after curative treatment for malignant tumors, such as Hodgkin's disease and testicular cancer: gonadal late toxicities in young patients after treatment for lymphomas and the risk of secondary neoplasia after curative treatment for malignant male germ cell tumors. PATIENTS/RESULTS: In 66 patients with Hodgkin's disease and 24 patients with non-Hodgkin's lymphomas ( < 45 years), who were in complete remission for more than 2 years after chemo- and/or radiotherapy, gonadal damages were analysed based on hormone levels (FSH, LH, testosterone, estrogen), clinical examination and patients' histories. 50% of female patients with Hodgkin's disease suffered from premature ovarian failure and 65% of men were either sub- or infertile. In contrast, only 18% of patients treated for non-Hodgkin's lymphoma showed signs of gonadal toxicity. The risk for gonadal toxicity in patients with Hodgkin's disease was related to the use of procarbacine as a part of the COPP-regimen and to infradiaphragmatic radiotherapy. Sperm conservation should be offered to male patients prior to therapy. In female patients with Hodgkin's disease hormone levels should be evaluated after therapy and substitution should be initiated early in order to avoid osteoporosis, an increased cardiovascular risk and psychological problems. In patients with testicular cancer it could be demonstrated that current therapeutic strategies are associated with a small but identifiable risk for secondary neoplasia: radiotherapy causes a 2- to 3-fold increased risk for solid cancers and chemotherapy, particularly the use of etoposide-based regimens, is associated with secondary leukemias. However, a risk-benefit analysis shows that cure rates above 80% for patients with metastatic disease will outweigh the risk of secondary neoplasia. CONCLUSION: The investigation of therapy-associated late toxicities will remain an important issue even with the use of new treatment approaches such as high-dose therapy. Prospective evaluation of late toxicities needs to be incorporated into new studies for the curative treatment of malignant tumors.

Missed opportunities for immunisation in curative and preventive services in a community health centre. A follow-up survey.

OBJECTIVES: To compare prevalences of missed opportunities for immunisation between preventative and curative clinic patients and to evaluate changes 13 months later. DESIGN: Repeat cross-sectional survey of patients' Road to Health cards. SETTING: A large community health centre in Khayelitsha, South Africa. PARTICIPANTS: Patients under 6 years of age. MAIN OUTCOME MEASURES: Proportion of children needing immunisation who were not vaccinated during the visit. RESULTS: At baseline, prevalences (95% confidence intervals) of missed opportunities or immunisation were 15.7% (9.2 - 22.2%) among preventative clinic patients and 92.0% (85.9 - 98.1%) among curative clinic patients. Thirteen months later the respective prevalences were 12.0% (5.0 - 19.0%) and 90.5% (84.6 - 96.4%). CONCLUSIONS: Separate provision of curative and preventive paediatric care results in many missed opportunities for immunisation in the former service. Fewer opportunities are missed if immunisation services are available all day every day, rather than for limited periods of the week.
