Intrahepatic bile duct strictures after human orthotopic liver transplantation. Recurrence of primary sclerosing cholangitis or unusual presentation of allograft rejection?

One of 55 patients transplanted for sclerosing cholangitis during the cyclosporin-steroid era (March 1980-June 1986) developed intrahepatic biliary strictures in the absence of allograft rejection within the 1st year posttransplantation. Although many causes underlie biliary pathology in the postoperative period (i.e., arterial injury, ischemia, chronic rejection, cholangitis), recurrent disease remains a possibility.

[Relation between changes in the breathing of patients with aneurysmal and stenotic lesions of the major cerebral vessels and the side of the lesion].

In patients with aneurysmal and stenosing injuries to the main vessels of the brain, there are significant differences in the duration of respiration arrests, respiration rate, systole frequency, basic metabolism, thermoregulation reflexes depending on the lateralization of vascular injury. Asymmetry manifested most distinctly in the pathology of the anterior cerebral artery. In women, functional asymmetry was pronounced to a greater extent than in men. Lateralization of the neurological symptomatology is also of importance for the line of ventilatory changes.

[The value of hysterosalpingographic findings in sterility investigations (author's transl)].

In the material presented, at least 117 women examined with hysterosalpingography became pregnant. Most of the pregnancies (68%) occurred within one year after the radiological examination. Clinical data is failing in many of the other cases, but 66 women were still infertile more than three years after the examination. The hysterosalpingographic findings of a group who got pregnant without any surgery and delivered at full term (73 patients) have been compared to the group of women still infertile and with a healthy partner (50 patients) and with another group of women, whose pregnancies ended in abortion (19 patients). No radiological signs is seen only in one of the groups but pathological changes of the oviducts are more common in the infertility group. Extreme positions of the uterus, irregularities of the cervical canal etc show no difference between the groups. Such findings in the uterus and the cervical canal are of no prognostic value.

[Morpho-functional changes in the vascular bed in obstructed portal blood flow].

Clinical observations were performed in 104 patients with intrahepatic blockade and 32 patients with subhepatic blockade of portal bloodflow and experimental studies in 24 mongrel dogs with the model of dosed stenosis of the portal vein. It was found that microcirculation bed of hollow organs of the gastrointestinal tract is also involved into the circle of pathological disorders in portal hypertension. Alterations of the microcirculation bed consist in the appearance of destructive lesions of the microvessel walls, disturbance of their permeability and hemodynamics. Undoubtedly, genesis of functional disturbances is based on structural alterations.

[Acquired telangiectases of the lower extremities].

The aim of our study is to place the different forms of telangiectasia of the lower limbs in their clinical categories, before tackling, along with Dr OUVRY, the therapeutic problem with which the phlebologist is daily confronted. The idiopathic telangiectasias of the lower limbs are very common in women, and are considered as simple, but clearly form an entity within the framework of venous pathology : unfortunately we do not have a very precise pathophysiological explanation to provide a basis for prophylaxis, which is still doubtful, or for definitive treatments : nevertheless that is a field for the exercise of the phlebologist's skill. Other types of telangiectasia, which we have mentioned, should call for a search for a general pathology in terms of a complete physical examination from a dermatological viewpoint. More rarely, their precise dermatological category will be that of a dysembryoplasia, while others will be vascular naeval disorders with delayed manifestation. These will all require a specific therapeutic approach.

Accelerations in "intra-partum" cardiotocographic record. IV. Correlation with the presence of a funiculus pathology.

The Authors compare the presence of accelerations and variable decelerations in a group of cases showing funiculus anomalies and in a control group. The group with funiculus pathologies showed a lower average frequency of accelerations and a higher frequency of variable decelerations than the control group. Furthermore, it was confirmed that an average of more than 5 accelerations is a clear indicator of fetal well-being.

Membranous obstruction of the inferior vena cava (obliterative hepatocavopathy, Okuda).

Confusion prevails throughout the world regarding the definition and classification of the Budd-Chiari syndrome. The original patients (Budd and Chiari) described had hepatic vein thrombosis, but this syndrome now encompasses various hepatic venous outflow blocks, of which membranous obstruction of the inferior vena cava (IVC) is the most common. This author has been suggesting that the classical Budd-Chiari syndrome or hepatic vein thrombosis and membranous obstruction of IVC or primary thrombosis of IVC at its hepatic portion are epidemiologically, pathologically and clinically different, and that they should be treated as two clinical entities that are not to be mixed. The two diseases have a different onset, different clinical manifestations and a different natural history. Whereas hepatic vein thrombosis is a severe disease with an acute onset, IVC thrombosis presents mildly at onset, but it recurs and eventually turns into a fibrous occlusion of IVC of varying thickness or stenosis of a various degree. The fibrous IVC occlusion is found as a mysterious thin membrane, but is more often much thicker than a membrane, and therefore 'membrane' is a misnomer. Although the genesis is not established, formation of a thin membrane may be an outcome of recurrent thrombosis. The past congenital vascular malformation theory no longer holds, because the disease occurs mostly in adulthood, and transformation of thrombosis into a membrane has now been well documented pathologically as well as clinically. This author suggests that a term 'obliterative hepatocavopathy' replace membranous obstruction of IVC, the term 'Budd-Chiari syndrome' be abandoned, and that primary hepatic venous outflow block be divided into primary hepatic vein thrombosis and primary IVC thrombosis (obliterative hepatocavopathy).

Synthesis and evaluation of long-acting D-penicillamine derivatives.

This study extends the use of two lathyrogens, ss-aminopropionitrile (BAPN) and D-penicillamine (DPA) from daily systemic or local-topical administration to long-time acting agents. This was achieved by converting the hydrophilic drugs into lipophilic derivatives. The synthesis of functional derivatives of DPA consisted in esterification with methyl-, hexyl-, or benzyl alcohols in the presence of thionylchloride. The esters formed were hydrochlorides, acidic and soluble in water. During neutralization in vitro or in vivo by tissue fluid, an oily substance is formed that elutes from a hydrogel polymer at a much slower rate than hydroplilic DPA itself. The degree of lipophilicity, measured as a partition coefficient between octanol/water, was highest for hexyl ester and lowest for methyl ester DPA. A single injection of either DPA hexyl ester HCl or 3-hexyl(amino) propionitrile into the full thickness skin incision wound in rats significantly lowered the breaking strength of the wound 12 days after injection, indicating the interference with collagen cross-linking. Both agents injected into the breast adenocarcinoma in Fisher rats significantly inhibited tumor growth without any signs of local or systemic toxicity. We conclude that these lipophilic lathyrogens with prolonged effectiveness are suitable in the treatment of pathologies, consisting of excessively cross-linked or deposited collagen (fibrotic adhesions, strictures, stenosis, and scar contractures) and in the treatment of single, solitary tumors, malignant and benign.

[Stenotic endometriosis of the sigmoid. Apropos a case].

The authors report a rare case of stenotic sigmoid endometriosis which presented with constipation and dysmenorrhea in a 48-year-old woman without past gynecological history. There were no typical radiological and endoscopic findings and only pathological examination revealed endometriosis. The patient did well after sigmoid resection and bilateral salpingo-oophorectomy and has had no further complaints. The authors stress the rare location of the disease and review the literature about etiopathogenetic hypotheses and pathological features. The diagnostic value of clinical examination, barium enema and colonoscopy is low as endometriosis rarely involves the mucosa. This case led the authors to discuss about diagnostic difficulties, differential diagnosis and therapeutic options.

[The assessment of autonomic nervous system function in women with degenerative pathology of the hand].

An all-round study was made of the function of the vegetative tone, responsiveness and supply of the parts of the autonomic nervous system in women with degenerative hand pathology. As compared with the control group, serious disorders were detected in all the parts and properties of the system. The conclusion is made that these mechanisms are implicated in the development of the disease group under study.

Development of the human pancreas: variations and pathology. A tentative classification.

The pathology of the pancreas is sometimes related to the embryological development of the organ. The first part of this paper is a presentation of the embryology, morphogenesis and cytogenesis of the pancreas. A tentative classification is then proposed to group together the lesions directly related to anomalies of the position and differentiation of the pancreatic buds. Pathological processes affecting the pancreas can be divided into those related to the ducts and those related to the parenchyma of the gland. In each case anomalies arising at a given stage of development lead to different diseases of the pancreas.

An unusual variant of popliteal artery entrapment.

A 43-year-old woman presented with incapacitating exertional pain in the right foot, ankle, and lower calf of 1 years' duration following a minor ankle sprain. Evaluation by several physicians had been inconclusive. Physical examination identified normal pedal pulses at rest but obliteration of pulses with active plantar flexion. Segmental pressures were normal at rest and duplex scanning showed occlusion of the popliteal artery with active plantar flexion. The findings were confirmed by arteriography despite a normal course of the popliteal artery. Magnetic resonance imaging (MRI) showed no muscular abnormality. At exploration entrapment was noted to be the result of compression by branches of the sural nerve and vein as they coursed medially inserting into the medial head of the gastrocnemius muscle. Division of the neurovascular bundle resulted in complete resolution of symptoms and arterial compression on duplex examination postoperatively. This case was unusual because of the patient's age, sex, and the pathologic findings that had not been previously reported. In this case MRI was not useful in demonstrating a muscular or neurovascular bundle abnormality, supporting the use of duplex scanning as the noninvasive diagnostic modality of choice.

Clinical value of MRC in the follow-up of liver transplant patients with a choledochojejunostomy.

BACKGROUND: We investigated the clinical value of magnetic resonance cholangiography (MRC) in liver transplant patients receiving choledochojejunostomy (CDJ). METHODS: Twenty-five MRCs were performed in 23 initially asymptomatic patients 19 months (mean) after liver transplantation with biliary reconstruction via CDJ. The images were evaluated by consensus (two investigators) for bile duct strictures and dilatations. As a standard of reference, clinical follow-up (including laboratory analysis) was used in 20 cases and direct cholangiography or surgery in three cases. RESULTS: Fourteen pathologic findings were observed in 11 patients (anastomotic strictures in four, left or right bile duct strictures in three, and peripheral segmental dilatations with or without strictures in seven). Patients with pathologic MRC findings had significantly higher levels of alkaline phosphatase (p < 0.05) and more frequently had histories of cholangitis than did patients with normal MRC. Four of six patients with stenoses of the central bile ducts subsequently developed biliary complications requiring treatment (three confirmed by direct cholangiography). In patients with unremarkable bile ducts or only peripherally located changes on MRC, no bile duct complications or relevant changes in the cholestasis parameters occurred during follow-up (mean = 30 months). CONCLUSION: MRC can noninvasively detect pathologic biliary tract changes in liver transplant patients in the asymptomatic stage and provide information for planning invasive therapeutic procedures.

[New criteria for the classification of the popliteal artery entrapment syndrome. Our experience with 14 extremities].

Authors explain their experiences with eight patients (14 affected limbs) with a popliteal artery entrapment syndrome. Classification, diagnosis and treatment were reviewed. Six limbs, with any malformation and presenting as a unique sign an important hypertrophy of their intern gastrocnemius muscle, couldn't be classified. As a result, a new classification of this pathology is presented being based on the anatomical and arteriographic aspects as well as oh the surgical indication. The important correlation between anomaly, physical complexion typus athletic and sports is noted.

[Clinical aspects of the progression of nonspecific brachiocephalic aortoarteritis].

Based on the features of the clinical course of nonspecific aorto-arteritis, three types of the course were identified: slowly progressive, rapidly progressive, and recurrent. The progression of the disease and involvement of new arterial beds into the pathological process were found to primarily depend on persistent inflammation. It is suggested that the types of the clinical disease course are directly related to immunological responsiveness.

[Postoperative anus stricture and leukoplakia lesions].

We present 35 cases postoperative anal stenosis of patients operated at the Sanatorio y Maternidad del Sur, Bahía Blanca, Argentina, in the last 9 years (1978-1986). In 9 cases (25.7% the pathologist reported leukoplasic lesions from the anus and anal channel. We remark the value of the pathology in these cases and we suggest to send to the pathologist all the tissues from anal surgery, because we believe this is the way to make the best prophylaxis of anal cancer.
