[Cicatrix phimosis and problems of circumcision in young children].

The article deals with the author's modified operation for circular excision of the prepuce with adaptation of the wound edges without sutures. The operation was performed on 200 patients under local infiltration anesthesia in the last four years. The advantages of the modified operative method are shown, schemes of the pathogenesis of cicatricial phimosis and its complications are compiled, a comparative evaluation of the existing types of operations for circular excision of the prepuce is made and their shortcomings are shown. The variant of the operation is suggested for introduction into pediatric surgery.

[Cutaneous metastases of melanoma localized on the cicatrix at the site of flap taking].

INTRODUCTION: We report one case of melanoma cutaneous metastases strictly localized on a skin graft donor site distant from the tumor site. CASE REPORT: A patient with a shoulder melanoma was treated by surgery. A split-thickness skin graft was taken from a thigh to cover the raw area. Six months later, nodular cutaneous metastases strictly localized on the skin graft donor site appeared. COMMENTS: Physiopathology of those lesions is unknown. Hyaluronic acid is increased in granulation tissue and could be chimiotactic for melanoma cells.

[Comparative study of collagen in hypertrophic and keloid cicatrix].

The relative percentage of type III to type I collagen, the ratio of alpha 1(I) to alpha 2(I) collagen chains and the ratio of dimers (beta-chains) to monomers (alpha-chains) in type I collagen have been measured in solubilized collagen fractions from keloids and hypertrophic scars aged about 1-7 years after burn. In tissue samples the content of crosslink with structure of pyridinoline were analyzed and expressed as mole of crosslink per mole of collagen. A comparison between hypertrophic and keloid scars has shown that the young (about 1 year) hypertrophic scars have higher ratios alpha 1(I)/alpha 2(I) and beta 11 + beta 12/alpha 1(I) + alpha 2(I). The increased proportion of beta-chains in the younger hypertrophic scars approximated to the same level as in normal skin within 7 years after lesion. There was no decrease in the ratio dimers/monomers of collagen type I with age of keloid scar tissue. The relative amount of collagen type III in young keloid scars was found decreased as compared to age-matched hypertrophic scars (13 +/- 3 and 17 +/- 7 respectively), but was similar in the older scars. The average pyridinoline crosslinks content per mole of collagen in keloids was 2 times as high as in hypertrophic scars. The data support the suggestion that general, as well as peculiar disturbances of collagen metabolism are involved in the development of keloids and hypertrophic scars.

[Keloid cicatrix of the face and neck. Apropos of 81 cases treated in Dakar].

The authors report their experience of the treatment of 81 cases of keloid scars in Africa. Many methods have been proposed and used but they do not consistently lead to good results. The fact that many operations have been indicates their back of efficacy. The natural tendency towards recurrence dictates the correct choice of operation. The best result is obtained by combining surgery with radiotherapy.

[Nodular sclerosing adenosis (adenosis tumor) of the breast. An immunohistologic study particularly in reference to the relationship with radial cicatrix and to the differential diagnosis with tubular carcinoma].

The clinical and pathologic findings of four cases of palpable sclerosing adenosis of the breast, called "adenosis tumor", are reported. Adenosis tumor is a rare lesion that clinically and sometimes histologically is misinterpreted as mammary carcinoma. In our study, adenosis tumor was detected in four women of 35-39 years (average 37 years). All cases were treated by local excision. None of the lesions had recurred at follow-up, 1-3 years later. Microscopically the most frequent growth pattern was classical sclerosing adenosis. Other findings were epitheliosis, collagenous spherulosis, microcysts, apocrine metaplasia and radial scars. Only in one case were detected foci of lobular carcinoma in situ. With immunoperoxidase staining, the proliferating cells stained positively for cytokeratin (AE1/AE3) and actin, revealing epithelial and myoepithelial differentiation. Coexpression of actin, S-100 protein and GFAP was detected in numerous stromal myofibroblast-like cells. In sclerosing adenosis and in radial scar the tubules were surrounded by a continuous intact basement membrane composed of type IV collagen, whereas in tubular carcinoma basement membranes are almost entirely absent.
