[Certain pathogenetic mechanisms of development of atherosclerosis in dogs, induced without the use of cholesterol].

Chronic investigations (up to 3 years) conducted in dogs have evidenced that periodic disturbances of vascular permeability produced by using dicoumarin with a long-term blocking of the thyroid function greatly aggravate pathological manifestations induced by the action of 6-methylthiouracil. They result in the development in the animals of endogenous hypercholesterinemia, elevated content in the blood of beta-lipoproteins and in a periodic rise of the arterial pressure. These changes were particularly spectacular at the final stage of the experiment in animals with marked manifestations of atherosclerosis and were accompanied by reduced stand-by potentialities of the adrenal gland, disruption of metabolic processes in the myocardium along with the slowed down resorption of NaI131 from the heart muscle. The sensitivity of the heart to adrenalin was down too.

Comparative study of anti-endotoxic potency of dexamethasone based on its different ester types.

Difference in the anti-endotoxic potency in mice between dexamethasone 21-disodium phosphate (DM-P) and 21-sodium sulfate (DM-S) was investigated. Changes in the plasma levels of free 17-hydroxycorticoids (17-OHCS), further, were defined after intravenous administration of 2 mg/kg dexamethasone in elective surgical patients for elucidation of the metabolic basis of the different anti-endotoxic potencies of the two ester types of dexamethasone. Young male mice of DDN strain were pretreated with 0.2 mg DM-P or DM-S. Following the pretreatment, 0.5 mg of the endotoxin was administered at varying intervals of 0--24 hours, and survival rates were determined after 72 hours. In the DM-P group the mice exhibited a significant resistance to the endotoxin within two hours after the pretreatment, but in the DM-S group, the mice exhibited no anti-endotoxic potency. When different doses of dexamethasone (from 3.2 to 0.00625 mg) were administered with 0.5 mg of the endotoxin simultaneously, the mice in the DM-P group showed improvements in survival rates that tended to be dose related, but the mice in DM-S group showed no improvement in survival rates. Plasma levels of free 17-OHCS in elective surgical patients, further, were remarkably elevated after administration of DM-P, 2 mg/kg, but they showed no elevation after administration of DM-S, 2 mg/kg. Thus, it is clear that the in vivo metabolism of the two esters is different. DM-P easily liberates its free form, whereas DM-S does not. From this evidence, it was concluded that there is a close correlation between the anti-endotoxic potency of dexamethasone and its plasma level of free form, and that glucocorticoids can demonstrate their anti-endotoxic action when they are present at pharmacologically high levels in the bloodstream. There is considerable difference in biological action, moreover, between the two water-soluble esters of dexamethasone. More attention should be paid to this difference when investigating the treatment of shock with glucocorticoids.

Partial 11- and 21-hydroylase deficiencies in hirsute women.

Thirty-one women with idiopathic hirsutism were evaluated for partial 11- and 21-hydroxylase adrenocortical enzyme deficencies. Twenty-four hour urine collections for 17-ketosteroids, 17-hydroxycorticoids, tetrahydro compound S (THS), and -pregnanetriol were obtained basally and following a continuous 24 hours infusion of alpha 1-24 ACTH (cosyntropin). The results were compared to those in eight normal, nonhirsute women studied under identical conditions. Normal control subjects and 18 of 31 hirsute female patients (Group I) showed similar small increments in the excretion of THS and pregnanetriol following the infusion of cosyntropin. Thirteen hirsute women (Group II) showed cosyntropin-stimulated increments in either THS and/or pregnanetriol that were significantly greater than the mean response of the control group. The cosyntropin-stimulated increments in 17-ketosteroid excertion and basal sebum production rates were also significantly greater in Group II. The results are consistent with partial II - and/or 21-adrenocortical hydroxylase deficiencies in some hirsute women whose condition would have previously been designated as "idiopathic." Thus, prolonged ACTH stimulation testing may identify patients who might benefit from glucocorticoid suppression therapy.

[Glucocorticoid and androgenic functions of the adrenal cortex and the state of the sympathetic-adrenal system in thyrotoxicosis with primary organic disease of the central nervous system].

In patients with thyrotoxicosis combined with primary affection of the central nervous system (CNS)-encephalitis, diencephalitis-there was revealed, like in common thyrotoxicosis, an increased production by the adrenal cortex of hydrocortisone, its intensified transformation and inactivation, reduced production of 17-ketosteroid chiefly due to the fall of dehydroepiandrosterone content, an increased urinary excretion of adrenaline and a decreased-of noradrenaline. The use of functional test with the ACTH administration demonstrated organic affection of the CNS to sharply aggravate the weakening and even the exhaustion of the functional reserves of the glomerular and the reticular zones of the adrenal cortex developing during thyrotoxicosis, and also the reserve possibilities of the sympathico-adrenal system. Direction of the changes in the 24-hour excretion of 17-OCS and corticosteriods was the same in the majority of cases. The latter can point to the presence of functional association between the hypothalamus-hypophysis-adrenal cortex systems and adrenergic regulation in these patients.
