Dual photon absorptiometry of the proximal tibia.

Bone mineral content (BMC) and bone mineral density (BMD) of the proximal tibia were determined by dual photon absorptiometry on 44 women, aged 23-87 years. The area of the tibia measured was a 2.01 cm region immediately distal to the medial and lateral tuberosities. Values of BMC ranged between 5.09 and 14.57 g and BMD between 0.380 and 1.180 g/cm2. Both tibial BMC and BMD declined with age and tibial BMD was significantly correlated with lumbar spine (r = 0.70), femoral neck (r = 0.73), and femoral trochanter (r = 0.74). However, the large standard errors of estimate (SEE) (0.08-0.14 g/cm2) do not allow for reliable prediction in an individual of other skeletal sites by the tibia. Repeated measurements demonstrated that dual photon absorptiometry of the proximal tibia is a reliable measurement and may be a useful tool in the monitoring of therapeutic or intervention modalities in those individuals with skeletal diseases in whom measurement of the lumbar spine or proximal femur may not be possible.

Assessment of the tibia using ultrasonic guided waves in pubertal girls.

The purpose of this study was to compare low frequency ultrasonic guided wave measurements with established ultrasound and bone density measurements in terms of their ability to characterize the tibia in pubertal girls. Subjects were 12-14-year-old girls ( n=106) who were participating in a calcium and vitamin D intervention study. A prototype low frequency pulse transmission device consisting of a uniaxial scanning mechanism and low frequency transducers orientated perpendicularly to the limb was used to measure two ultrasound velocities in the tibia. The first velocity, V1, was that of the first arriving signal, similar to that measured by existing commercial tibial ultrasound devices. The second velocity, V2, was that of a slower wave propagating at 1,500-2,000 m/s, which has been shown elsewhere to be consistent with the lowest order antisymmetric guided mode in the bone. In addition, commercial ultrasound devices (Omnisense, Sunlight Ltd.; QUS-2, Quidel Corp.) were used to measure the speed of sound (SOS) in the tibia and the radius and attenuation (BUA) in the calcaneus. Cortical bone cross-sectional area (CSA), mineral density (BMD) and cortical thickness (cTh) of the tibia were measured using pQCT, site-matched to the ultrasound measurements. Both V1 and V2 correlated significantly with cortical BMD and with cTh and CSA. On the other hand, tibial SOS correlated with BMD, but not with cTh and CSA. These results indicate that the prototype device using guided waves captures aspects of tibial cortical bone geometry in addition to bone density, thereby potentially offering increased diagnostic information compared to existing tibial ultrasound devices.

Synchronous Paget's sarcoma of tibiae in which Paget's disease was limited to these bones.

A 51-year-old native of Rio de Janeiro presented with bilateral synchronous Paget's sarcomas in the tibiae, which developed in the upper right tibia and in the distal third of the left tibia. There were no other areas of Paget's disease. The largest tumor spread to the right inguinal nodes and also soft tissue. The tumor in the left tibia spread dramatically in the soft tissues up the leg and only involved the medullary cavity in its inferior portion. The patient died, but there was no autopsy. Comments are made about the prevalence of Paget's disease in Rio de Janeiro.

Callus formation in the humerus compared with the femur and tibia during limb lengthening.

We investigated whether the callus formation in the humerus during the distraction period of limb lengthening proceeds at a higher rate than that in the femur and tibia. Ten achondroplastic patients underwent 3 bilateral humerus, 3 bilateral femur and 4 bilateral tibia lengthenings. To reduce the confounding effect of bone size, we used bone mineral apparent density (BMAD) to compare the three groups; this is a volumetric bone mineral density measurement. BMAD in the distracted callus space was evaluated at 8 weeks after the start of distraction using dual-energy X-ray absorptiometry (mean +/- SD; g/cm3): in the humerus (0.24 +/- 0.08) it was significantly higher than in the tibia (0.10 +/- 0.02), while there was no difference between the humerus and femur (0.35 +/- 0.11). We conclude that the callus formation in the humerus during the distraction period of limb lengthening proceeded at a significantly faster rate than in the tibia, but there was no significant difference between the humerus and femur.

Pressure-induced pain on the tibia: an indicator of low bone mineral density?

Previous literature investigating bone pain in osteoporosis has prominently focused on painful conditions following osteoporotic fractures. "Is osteoporosis really a silent disease without bone pain and tenderness unless a fracture occurs?" Our aim in this study was to answer the question by assessing the questionable tenderness on tibia bones of fracture-free patients with low bone density and to compare the findings with a normal population. One-hundred-thirty-three consecutive postmenopausal female patients with the mean age of 56 years admitted to our clinic for bone mass measurement were included in the study. Bone mineral density (BMD) values of lumbar spine (L2-L4) and right proximal femur (neck, trochanter, Ward's triangle) were measured by dual-energy X-ray absorptiometry (DXA). Patients with T scores lower than -1 formed the osteopenic-osteoporotic group of patients (low BMD group) whereas those with T scores higher than -1 constituted the normal BMD group according to the osteoporosis definition regarding T score for DXA. Mechanical pressure was applied by a hand algometer on the middle points of three equally divided sections on the anterior part of tibia, and the pressure levels starting the pain sensation (POPL) were recorded. Although the patients in the normal BMD group reported consistently high POPL at all regions of tibia for all BMD measurement sites, this difference reached to a statistical significance level only for the femur neck region. Only mean POPL for the whole tibia had independent association with only femur neck BMD by multiple linear regression analysis. These results are encouraging for assessing the significance of pressure-induced tibial pain as an indicator of low BMD in the future.

Modelling human tibia structural vibrations.

Mode shapes and natural frequencies of human long bones play an important role in the interpretation, prediction and control of their dynamic response to external mechanical loads. This paper describes an experimental and theoretical study of free vibrations in an excised human tibia. Experimentally, seven tibial natural frequencies in the range 0-3 kHz were identified through measured structural transfer functions. Theoretically, a beam type Finite Element model of a human tibia is suggested. Unknown parameters in this model are determined by a Bayesian parameter estimation approach, by which very fine model/observation-accordance was achieved with realistic parameter estimates. A sensitivity analysis of the model confirms that the human tibia in a vibrational sense is more uniform than its complicated geometry would immediately suggest. Accordingly, two simple tibia models are identified, based on uniform beam theory with inclusion of shear deformations.

Permeability of cortical bone of canine tibiae.

Movement of interstitial fluid in cortical bone is considered one of the important mechanisms affecting bone remodeling and fracture healing. To better understand such interstitial fluid movement, the hydrostatic permeability of cortical bone of the canine tibia was measured in this study. It was found that age has significant influence on permeability. The cortical bone permeability of puppy tibiae is six times higher than that of adult tibiae. Adult tibial cortex is impermeable unless the superficial layer of the periosteal cortex is removed. The permeability differs between regions and locations of the tibia. The pathways for such extravascular interstitial fluid movement were also identified to be primarily the haversian and Volkmann's canals and secondarily the canalicular and lacunar spaces. The high permeability of their cortical bone may explain the increase in periosteal new bone formation seen in puppies when a venous tourniquet is applied.

Evaluation of hollow and full stems implanted in the rabbit tibia: preliminary results.

Full and hollow stems were implanted in the right and left tibiae of five rabbits to study the effects of the implant shape on revascularization and remodelling of the bone after 6 months. No significant difference of periosteal and endosteal circumference was observed when the left and right tibiae of each group were compared. Medullary area was, not surprisingly, significantly smaller in the full stem implanted tibiae. The cortical bone area result was significantly higher in the hollow stem group. In hollow stems, vessels were present both inside and outside the implant with connections through the holes of the implant. The observed differences of cortical bone area can be explained in terms of increased vascular spaces inside the cortical bone of full stem implanted tibiae. This supports the hypothesis that hollow implants provide more space for medullary revascularization and are consequently capable of inducing less endosteal remodelling. Cementless prostheses rely for mechanical fixation on a large interface of the implant with bone. Two factors, i.e. revascularization of the medullary canal and stiffness of the stem, can have a relevant effect on the quality of interface. This non-weight-bearing model suggests the relevance of vascular factors.

Morphology-mechanical property relations in trabecular bone of the osteoarthritic proximal tibia.

Alteration of morphologic and mechanical properties of trabecular bone in the osteoarthritic proximal tibia may be a contributing factor in tibial component loosening. To explore this issue, the authors performed tissue property measurements, morphologic analysis, and mechanical testing of subchondral, epiphyseal, and metaphyseal trabecular bone specimens retrieved from six human proximal tibias exhibiting a range of medial unicondylar osteoarthritic degeneration. Apparent density in the proximal tibia was altered according to varus misalignment and medial subluxation associated with medial osteoarthritis of the knee. In subchondral bone, a decrease in tissue mineralization contributed to a significant reduction in axial mechanical properties with degenerative disease (P < .0005). In epiphyseal and metaphyseal bone, trabecular thickness and the number of trabeculae increased linearly with volume fraction, providing a power law relationship between axial elastic modulus and apparent density (R2 = .84). Average elastic properties of the tibial epiphysis and metaphysis were not reduced by degenerative disease (P < .05). The results suggest that absolute minimization of tibial resection might not be an optimal strategy for tibial component fixation and that mechanical properties of the tibial resection surface are more homogeneous in planes parallel to the joint surface than in a plane normal to the longitudinal axis of the tibia.

In vivo measurements of the phase constants of transverse mechanical waves in a human tibia from 100 to 1000 Hz.

This paper contains the details of a new technique for measuring the mechanical phase constants of transverse waves in human tibia, in vivo. The measurements are made in the frequency range from 100 to 1000 Hz. The importance of these measurements is that healthy tibia has quite small phase delays along the bone at these frequencies. On the other hand, a fractured tibia has a large phase delay at the fracture site. This paper establishes a set of typical phase delays for a range of tibia so that subsequent measurements on fractured patients can be interpreted. It contains a description of the method of measuring the phase delay and a detailed analysis of errors. In particular, it includes an account of the phase delay through the soft tissues as well as the bone and how the former can be eliminated. It is not an exhaustive account of this topic but contains sufficient results to show that the measurements can be made relatively easily. Finally, there is a theoretical model which can be used to predict the phase shift. However, in order to use the model various other measurements are necessary and a direct measurement of phase is easier than using a model.

Terminal differentiation of osteogenic cells in the embryonic chick tibia is revealed by a monoclonal antibody against osteocytes.

Monoclonal antibodies against the surface of embryonic osteogenic cells have been used to characterize the sequence of transitions involved in the osteoblastic cell lineage. These previous data identified distinct cell stages within the osteogenic lineage, but were incomplete. To further refine and extend these observations, additional monoclonal antibodies were generated against the surface of osteogenic cells by immunizing mice with a heterogeneous population of chick embryonic bone cells. Supernatants from growing hybridoma colonies were immunohistochemically screened against frozen sections of stage 35 (day 9.5) chick tibiae. One cell line, SB-5, which secretes an antibody against the surface of osteogenic cells was successfully cloned, stabilized, and immortalized. Studies on the developmental progression of osteogenesis in the embryonic chick tibia reveal that cells within the lineage stages from Pre-Osteoblast to Secretory Osteoblast were never observed to react with antibody SB-5 at any time. By contrast, strong cell surface immunoreactivity was present on mature osteoblastic cells as they became Osteocytes. Furthermore, in cultures of osteogenic cells derived from embryonic calvaria or tibiae, cells possessing the SB-5 antigen on their surface displayed a morphology remarkably similar to that of Osteocytes found in situ. Double immunofluorescent staining of developing chick tibiae with SB-5 and SB-2, a monoclonal antibody directed against the surface of Secretory Osteoblasts, indicates that these cells proceed through an intermediate lineage step before becoming terminally differentiated Osteocytes. This transitory cell state is characterized by the simultaneous cell surface binding of antibodies SB-2 and SB-5, and is referred to as the Osteocytic Osteoblast stage.(ABSTRACT TRUNCATED AT 250 WORDS)

Bone marrow capacity for bone cells and trabecular bone turnover in immobilized tibia after sciatic neurectomy in mice.

Trabecular bone turnover and bone marrow capacity for the development of bone cells in the tibia were assessed after sciatic neurectomy (NX) in mice. The right hindlimbs of 6-week-old DDY mice were neurectomized and left hindlimbs were sham-operated and served as NX controls. Histomorphometrical analyses of the trabecular bone of the proximal tibia demonstrated the initial decrease in bone formation rate for the first 14 days and the subsequent increase in osteoclast surface for the next 14 days. The number of adherent stromal cells per tibia obtained for the NX limbs was reduced on days 7 and 10 postsurgically, and then recovered on day 12. However, the alkaline phosphatase activity of the cells was persistently depressed. The formation of osteoclast-like multinucleated cells in the marrow cultures obtained from NX limbs at days 10, 12, and 14 showed a significant increase in the medium containing parathyroid hormone (PTH). The number of colonies cultured for colony forming units-fibroblastic (CFU-f) that developed from the marrow cells did not differ in the NX and the contralateral limbs at any time during the period. On the other hand, the number of colonies cultured of colony forming units for granulocytes and macrophages (CFU-GM) was markedly increased for both the NX and the contralateral tibiae at days 12 and 14. This study clearly demonstrates that there are two stages in the development of osteopenia after NX. During the first 14 days, trabecular bone formation and number of marrow stromal cells are reduced. In the second 14 day period, the trabecular osteoclast number is increased and osteoclast formation from the bone marrow cells is enhanced in the presence of PTH. However, neither the CFU-f nor the CFU-GM assay could identify the changes in osteogenic or osteoclastogenic potential of the bone marrow. These in vitro assays provide limited information on the shifts in bone marrow cell lineages and the local environment producing osteopenia in the immobilized limb in vivo.

Retrograde cartilage transplantation on the proximal and distal tibia.

The authors, with experience with more than 400 osteochondral autograft transplantation (OATS) cases since 1996, report a new technique of a retrograde osteochondral autograft transplantation for the treatment of isolated osteochondral lesions of the proximal and the distal tibia started in 1999. We treated 5 patients, 3 who presented with painful traumatic chondral defects in the central weight-bearing portion of the tibial plateau (1 in the medial and 2 in the lateral compartment), and 2 who presented with painful chondral lesions on the distal tibia. An anterior cruciate ligament (ACL) drill guide positioned in the center of the defect was used to accurately prepare the cartilage surface, in one case arthroscopically and in 4 cases through an open incision. A guide-wire was introduced and drilled through the tibia, and a cannulated reamer equal to the diameter of the defect was advanced. An osteochondral cylinder was harvested from the non-weight-bearing zone of the femoral trochlea at the angle that corresponded to the angle on the ACL drill guide. The autograft was inserted in a retrograde fashion from the cortical window into the tibial tunnel to be flush with the articular surface in press-fit technique. The remaining tunnel defect between the cortical window on the tibia and the distal aspect of the autograft was filled with a cancellous bony cylinder and secured with a diagonal bioabsorbable screw. A concomitant varus deformity with the lesion on the medial tibial plateau was corrected in the same surgery using a high tibial osteotomy to relieve stress on the graft. Patients were followed up for 6 to 35 months. A complete healing of the grafts was seen in control magnetic resonance images (MRIs). All patients were satisfied with the surgery. Control arthroscopies showed the osteochondral cylinders well integrated and flush with the articular surface.

Ornithine alpha-ketoglutarate increases mineralization and mechanical properties of tibia in turkeys.

Skeletal disorders in rapidly growing poultry are commonplace. This study was performed to investigate the effect of ornithine alpha-ketoglutarate (OKG) administration during the last 7 weeks of life on structural properties, mineralization, and mechanical endurance of skeleton in turkeys at slaughter. Healthy HB Medium Bronze female turkeys were randomly assigned to two weight-matched groups at the age of 12 weeks. OKG was administered orally to the experimental group (N=17) at the dose of 0.4 g/kg body weight per day, while the control group (N=16) received an equal dose of the vehicle. The turkeys were slaughtered at the age of 19 weeks and the tibiae were isolated for analysis. The effect of OKG on skeletal system development in turkeys was evaluated in relation to both geometrical and mechanical properties as well as quantitative computed tomography (QCT). Free amino acids concentrations were assessed with the use of ion-exchange chromatography. Significantly increased bone mineral density of the trabecular and the cortical bone of tibia in the turkeys given OKG for the last 7 weeks of production cycle were observed (P<0.05). OKG administration improved mechanical endurance of the tibia estimated by the three-point bending test (P<0.01). Plasma amino acid analyses showed increased level of aspartate, proline, alanine, valine, isoleucine, leucine, and ornithine (all P<0.05) after OKG treatment, whereas cystathionine concentration was decreased (P=0.03). Obtained results indicate that oral OKG administration has beneficial effects on skeletal development in fast growing turkeys and this effect is connected with increased amino acid synthesis. These observations may serve to improve skeletal properties in birds, especially when considering that skeletal disorders often affect the tibia and the proper function of the skeletal system plays an essential role in animal welfare and poultry production.

Radiation-induced impairment of osseous healing with vascularized bone transfer: experimental model using a pedicled tibia flap in rat.

Aim of this study was to establish an appropriate animal model for investigating the healing of vascularized osseous transplants to irradiated recipient sites applying metabolic, vascular and immunologic experimental studies. In 20 Wistar rats (male, weight 300-500 g), a pedicled osseous tibia flap was raised and transferred to a subcutaneous pocket in the ipsilateral groin. The remaining tibia was stabilized with a monocortical titanium miniplate. To create a pre-irradiated transplant bed, the donor-area including the adjacent bone of the tibia was irradiated with a total dose of 50Gy (5 x 10 Gy) in 10 animals. The interval between irradiation and retransfer of the non-irradiated pedicled tibia flap was 4 weeks. Ten animals received no radiation. Evaluation of osseous healing and the success of the transferred flap were based on a clinical and quantitative histomorphometric assessment. Testing for significant differences was performed using the non-parametric Mann-Whitney U-test. The rate of complete osseous healing in the non-irradiated animals was 90%. In contrast there was no significant bone union observed in the group of the pedicled flaps grafted to the pre-irradiated (50Gy) recipient site (P = 0.001). Similarly bone formation in the transitional zone between bone graft and recipient bone was significantly lower in the preirradiated group (P < 0.001) (16.9 +/- 3%) in contrast to the non-irradiated transplant bed (47.9 +/- 6%).

Low amplitude, high frequency strains imposed by electrically stimulated skeletal muscle retards the development of osteopenia in the tibiae of hindlimb suspended rats.

The purpose of this study was to determine the extent to which high frequency, low amplitude skeletal muscle contractions, induced using electrical stimulation, could prevent or retard disuse osteopenia. Ten minutes of 30 Hz electrical stimulation was delivered, 5 days a week, during a 4 week rat-hindlimb suspension protocol. Each pulse generated a peak compressive dynamic strain on the tibia of approximately 200 microepsilon. We hypothesized that the electrical stimulation protocol would significantly reduce the loss of tibial bone mineral density compared to the contralateral control tibia that did not receive electrical stimulation. Compared to the contralateral control limb, the tibia of the stimulated limb had significantly higher bone mineral density and enhanced newly formed bone in the tibial diaphysis. The diaphysis, specifically the posterior bone cortex, of the tibia of the limb receiving the stimulation also demonstrated substantially larger mineral-binding fluorochrome biomarker within the osteocyte lacunae and canalicular volumes. Although the protocol did not prevent disuse osteopenia the evidence suggests that it was effective at reducing the extent of the osteopenia. One possibility for this outcome may be the insensitivity of bone to static, compared to dynamic compressive loads. In the present study there was a considerable static component to the compressive loads that accounted for a large component of the peak load generated by the stimulated skeletal muscle. Nevertheless, the results provide impetus for further development of the methods by which muscle contraction-induced loading of bone can be clinically exploited.

Less Invasive Stabilization System (LISS) for fractures of the proximal tibia: indications, surgical technique and preliminary results of the UMC Clinical Trial.

The surgical treatment of proximal tibia fractures, with or without intraarticular involvement, is associated with well-described patterns of failure and significant complication rates. Recent surgical advances allow for a minimally invasive approach to such injuries, which may improve healing times, increase union rates, and decrease complication rates. The Less Invasive Stabilization System (LISS) for proximal tibia fractures employs a laterally based fixed angled implant, allowing for the placement of locking screws proximal and distal to the fracture, which may be placed percutaneously. This manuscript presents the surgical technique and indications of the Tibia LISS. A preview of the early clinical results of the treatment of bicondylar tibial plateau fractures and proximal tibia diaphysis will also be presented.

Tibialis anterior turnover flap coverage of exposed tibia in a severely burned patient.

A case report of a longitudinally split tibialis anterior turnover flap reconstruction of an exposed tibia in a burn patient is presented here. The patient had sustained deep partial- and full-thickness burns to 70 per cent of his total body surface area (TBSA), resulting in an exposed left patella and upper two-thirds of the left tibia. Although full thickness loss of skin occurred on the left lower leg, no muscle trauma was sustained. Reconstruction was therefore deemed possible using local muscle tissue to provide transposed flap coverage. A gastrocnemius muscle flap was used to cover the exposed patella and superior aspect of the tibia. A portion of the tibialis anterior muscle was split longitudinally and turned over medially to cover the remaining exposed tibia. The advantages offered by this infrequently used flap include technical simplicity, reliability, minimal donor site dysfunction and the allowance of future use of the soleus flap. The tibialis anterior turnover flap may therefore have wide applicability for reconstruction of the severely burned lower extremity.
