An easy method for laparoscopic-assisted percutaneous anterior gastropexy.

Laparoscopic antireflux surgery is becoming a standard procedure in pediatric surgery. Anterior gastropexy is often performed in antireflux procedures, as well as in children with recurrent and intermittent volvulus of the stomach. We present a simple and secure technique for anterior laparoscopic-assisted gastropexy.

Ruptured hemidiaphragm: unusual late presentation.

Diaphragmatic injuries may be undiagnosed in the acute posttraumatic period and may remain unrecognized despite a variety of chronic symptoms, until eventual emergency department presentation with an intestinal obstruction. We present a case of a female who experienced chronic chest pain during coitus that eventuated in an acute admission with an intrathoracic mesentero-axial volvulus of the stomach, 8 months after a motor vehicle accident. The literature regarding delayed presentation of a ruptured hemidiaphragm is reviewed.

The wandering gallbladder.

An unusual case of a hypermobile gallbladder and liver is presented. The liver and gallbladder, supported only by a long midline falciform ligament, rotated freely from the right to the left upper quadrant. Because of a long gastrohepatic ligament and mesocolon, intermittent volvulus of the stomach and colon were associated findings.

Gastric wall necrosis from organo-axial volvulus as a late complication of laparoscopic gastric banding.

We present a case of massive gastric wall necrosis associated with volvulus of the stomach occurring 17 months after laparoscopic gastric banding. The 19- year-old female was admitted to our hospital with acute abdominal pain with rapid deterioration from massive necrosis of a distended stomach which perforated distal to the gastric band, accompanied by splenic infarction. Total gastrectomy and spenectomy were required. We present this very rare complication which should be recognized and treated early, so that a less drastic operation is possible.

[Distal gastric volvulus as a cause of acute occlusion in paraesophageal hernia. Considerations on a clinical case].

A case of distal volvulus of the stomach as a cause of acute intestinal obstruction in a paraesophageal hernia is presented. The patient, an old woman aged 82, had been suffering from abdominal pain and vomiting for about 48 hours. She successfully underwent emergency operation for the reduction of hernia and plasty of the hiatus anterior the esophagus. On the basis of personal experience and review of literature data, stress is laid on the high incidence of paraesophageal hernia complications and the importance of early diagnosis and surgical repair is underlined.

[Incarcerated Chilaiditi disease. A rare indication for surgery].

Surgery was performed on a rare case of incarcerated Chilaiditi's syndrome, a syndrome that has been known for 10 years. The patient was in the state of ileus with volvulus of the stomach and hepatodiaphragmatic interposition of the stomach, transverse colon, and omentum majus. The patient was at high risk--77 years old and in bad condition due to hyperemesis and abdominal pain. Preoperative intensive care was necessary. During the operation the intestinal structures were repositioned, the diaphragmatic hernia was resected, and the muscular parts were sewn tight. The right position of the liver was reconstructed by sewing the ligaments. Colon resection was inevitable because the colon loop could not be separated and because there was lack of intraabdominal space. For indication of preventive and elective surgery, we propose that the diagnostics be enhanced when Chilaiditi's syndrome is found by chance and is still asymptomatic. These techniques may include a complete radiological examination with contrast medium, intestinal endoscopy, and computer tomography of the thoracoabdominal region.
