The representation of discourse in the two hemispheres: an individual differences investigation.

Two experiments were conducted to investigate discourse representation in the two cerebral hemispheres as a function of reading skill. We used a lateralized visual-field procedure to compare left hemisphere (LH) and right hemisphere (RH) sensitivity to different discourse relations in readers with varying skill levels. In Experiment 1, we investigated two levels of discourse representation in memory: (a) the propositional representation and (b) the discourse model. We found that all readers were sensitive to propositional relations in the LH. In contrast, sensitivity to propositional relations in the RH increased as a function of reading skill. In addition, reading skill was positively related to topic relations in the LH, whereas it was negatively in the RH. In Experiment 2, we investigated propositional relations of different distances and again found that all readers were sensitive to propositional relations in the LH, whereas sensitivity to propositional relations in the RH was negatively related to reading skill. In general, reading skill appears to be associated with left-lateralized discourse representations.

Fetal-maternal bleeding associated with genetic amniocentesis.

Maternal serum alpha-fetoprotein (AFP) elevation following midtrimester genetic amniocentesis (post-AFP) was studied as an indicator of fetal-to-maternal bleeding (FMB). A post-AFP elevation was observed in 28 of 333 (8.4%) consecutive patients who before the procedure had preliminary placental localization by ultrasound and a normal serum AFP concentration (pre-AFP) for the gestational age. The absolute elevations of post-AFP and serial AFPs and the correlation with placental localization all suggest that this is related to FMB. The frequency of spontaneous abortion in patients with an elevation of post-AFP (4 of 28 cases, 14.29%) as opposed to the frequency of those patients who did not demonstrate this phenomenon (3 of 305, 0.98%) was significantly different. The higher frequency of anterior placental implantations in the former group suggests that preliminary placental localization by ultrasound may be important when counseling the patient concerning the risk of amniocentesis. The implications of these findings regarding Rh sensitization and the need for Rh immunoglobulin prophylaxis following this procedure are discussed.

Fetal blood group determination in first-trimester pregnancy for the management of severe immunization.

The presence or absence of Rho (D) and Kell antigens on fetal red blood cells was determined in the first trimester of pregnancy on erythrocytes obtained by chorionic villi sampling with the use of mixed agglutination and solid phase microimmunofluorescence techniques. Pregnancies in one Kell-sensitized woman and seven severely RH-sensitized women with a poor obstetric history and a partner heterozygous for the offending antigen were examined. A conclusive diagnosis could be made in seven of the eight cases studied.
