[Gynecomastia in children].

The experience with treatment of 34 children with gynecomastia is summarized. The examination included roentgenography of the Turkish saddle, definition of bone age, investigation of buccal sex chromatin, excretion of 17-ketosteroids, excretory urography against the background of retropneumoperitoneum, ultrasound investigation of the mammary glands. In juvenile gynecomastia, the mammary glands were removed, the expectant tactic being considered unwarranted. The long-term results of the surgical treatment are good.

[Pheochromocytoma in black Africa. Report of 9 cases].

Pheochromocytoma exists in black African countries with the same frequency observed in other places. The characteristics of our series of nine patients are the followings: an average age of 19, 6 years old, a sex-ratio of 1, 25, a paroxysmal high blood pressure for six patients, no MEN neither malignant tumors, the measures of V.M.A. done positively eight times, a retropneumoperitoneum scan, seven arteriographies, three echographies which were consistently positive, one death post surgery by collapsus consecutive to tumor excision, only one contralateral recurrence and positive followings in all other cases.

[Magnetic resonance tomography of the adrenals in the diagnosis of total and partial hypercorticism].

The paper is devoted to the use of a new method of nuclear magnetic resonance tomography (MR tomography) in the diagnosis of diseases of the adrenal glands developing with the syndrome of total and partial hypercorticism. A MR tomograph with resistive magnet (0.234 T) was employed. Altogether 25 patients were examined (21 with Itsenko-Cushing disease of various degrees of severity, 2 patients with Itsenko-Cushing syndrome and 2 patients with aldosteronoma). MR tomography was shown to possess a high informative value permitting the assessment of a type of adrenal changes without irradiation of a patient. In combination with routine methods of examination (hormonal investigation, retropneumoperitoneum, x-ray computerized tomography) MR tomography makes it possible to improve considerably the diagnosis of Itsenko-Cushing disease and to specify tumor sizes and tumor growth type.

[Evaluation of diagnostic methods in kidney cancer].

Different methods for the renal cancer diagnosis: roentgenological, vasographic, radioisotopic, cytological, immunological and others are assessed comparatively. Among the routine roentgenological methods excretory and infusion urography, retropneumoperitoneum associated with tomography and excretory urography, retrograde pyelography proved to be effective. Kidney scanning with neohydrine is a simple and safe technic of examination. Selective arteriography is the most effective method for recognizing renal cancer, however in 7.2% of patients the response was negative. Cytological and immunological methods are of a secondary value. The scheme of examining patients suspected of renal cancer is recommended.

[Malignant adrenocortical tumor. A propos of a case].

The authors report a case of malignant cortico-adrenaloma in a young woman of 27. This malignant tumour represents 10% of the causes of Cushing's syndrome. It is noted for the rapid onset of the syndrome and a marked hyperandrogenism. The diagnosis depends on an I.V.P. with retropneumoperitoneum completed by arteriography and ultrasound or by more recent investigations: the isotope scan and the C.T. scan. Its prognosis is very poor and the treatment consists of an extended adrenalectomy.

[Complications of laparoscopic cholecystectomy].

The laparoscopy cholecistectomy is a surgical procedure described in 1987, and it has had an important apogee and it's had substitute to open procedure. Between its advantages is notable, the postoperatory evolution without pain, paralytic ileus and the short time of hospitalization besides the patient can go away the same day in some cases. In Venezuela, the procedure has been received with enthusiasm by the surgeons with prefer this procedure however, there are complications derived in part for the lack of experience in the first cases, but occur non related of the experience. The quantization of the complications is not simple to realize for different reasons. We present twelve complications, two belong to patients intervened in the Hospital Universitario de Caracas and ten belong to patient transferred from other center. The complications were: pseudoaneurism of hepatic artery, hematoma of the wall, cutting of common bile duct, section the right bile duct, abscess of vesicular bed in two cases, retropneumoperitoneum, ascites by biliary fistula and four biloma. The treatment varied in every case and the utilization of radiologic technique with percutaneous drainage were useful in five cases. We conclude: first, this technique is not free of complications. Second, the percutaneous drainage (abscess, biloma), endoscopic procedure (biliary prosthesis) in fistulaes and arteriographics (arterial embolization), are useful in some cases, and could prevent the surgical reintervention.

[Endoscopic, extraperitoneal adrenalectomy].

Between March and June 1994 three adrenalectomies were performed via an endoscopic extraperitoneal approach. After creating a carbon dioxide retropneumoperitoneum three 10 mm trocars were introduced in the area of conventional lumbar incision. Via these ports adrenalectomy was performed. Time of operation amount to 215, 290 and 330 min. No postoperative complications were observed. Time of postoperative hospitalisation amount to 3, 4 and 7 days. The advantage of the extraperitoneal approach is that it decreases postoperative pain and allows more rapid recovery.

Retroperitoneal laparoscopic nephrectomy in children: preliminary report of six cases.

Laparoscopic nephrectomy is a new procedure that must be evaluated in adults and children. This technique allows a reduction in complications and sequelae. The majority of indications, such as renal dysplasia and destroyed kidneys due to obstructive or refluxing uropathy, are suitable for laparoscopic nephrectomy. Contraindications are Wilms' tumor and trauma, which represent only 20 percent of nephrectomies in our experience. As in open surgery, to perform nephroureterectomy for benign disease, a retroperitoneal approach seems more logical than transperitoneal approach, which is the usual approach for laparoscopic surgeons. We have attempted six retroperitoneal laparoscopic nephrectomies in children from 3 months to 14 years old. The patient is positioned in a lateral position after creation of a retropneumoperitoneum under visual control; three or four ports are needed and renal vessels are dissected then clipped, or coagulated if small. Destroyed kidneys are generally of small size, so they can be extracted via a 10- or 12-mm cannula site without morcellation. Operative time ranges from 35 to 210 mm (median 120 mm). We have had no complications or conversions. Retroperitoneal laparoscopic nephrectomy in children is a feasible and safe procedure in well-trained hands.

[A retroperitoneal abscess after endoscopic sphincterotomy: a severe and infrequent complication].

We report the case of a 55-year-old female patient, previously cholecystectomized, who was admitted to the Hospital because of recurrent biliary abdominal pain, cholestasis and common bile duct enlargement by ultrasonography. 48 hours after an endoscopic sphincterotomy the patient developed abdominal pain and fever. CT scan demonstrated air and an infected liquid collection in the retroperitoneum. Although a percutaneous drainage was attempted, the persistence of the symptoms and the low drainage outflow of the infected material made surgical treatment necessary. Duodenal perforation in endoscopic sphincterotomy is an unusual but severe complication that leads to a high mortality rate if surgical treatment is not applied promptly. Sometimes retropneumoperitoneum can be satisfactory solved just with a conservative medical therapy, but if there is a suspected or confirmed infected collection surgical treatment must be applied promptly.

[Laparoscopic replacement of the abdominal aorta. Experimental study in the pig].

BACKGROUND: Progress in abdominal laparoscopy led us to study end-to-end anastomoses performed laparoscopically. METHOD: An experimental protocol in 10 castrated male pigs weighing 74-95 kg was approved by the ethics committee. After conventional anesthesia, each animal was positioned in lateral decubitus and a retropneumoperitoneum was created. CO2 inflation was maintained at 14 mmHg for insertion of 3 trocars, 5 to 10 mm width. The entire infrarenal aorta was dissected and resected with insertion of a 6 mm dacron prosthesis. Postoperative arteriography was performed in all cases. The animal was sacrificed for direct examination. RESULTS: One animal died during anesthesia induction and the entire protocol was conducted in 9 animals. Mean operative time was 397 min (305-535 min) including a mean 123 min (65-150) for aortic dissection, 82 min (30-155) for proximal anastomosis and 70 min (45-105) for distal anastomosis. Total blood loss varied from 100 to 450 cc (mean 252 cc). Mean difference between pre- and postoperative hematocrits was 4% (0-6%). Among the 18 aortic anastomoses performed, arteriography showed one with moderate leakage and one anastomotic thrombus. Stenosis > 50% was found in 4 cases and < 50% in 4 cases. Analysis of the different operative parameters showed a learning curve with decreasing operative time and improved quality of the anastomoses. CONCLUSION: This study demonstrates the feasibility of aortic reconstruction via retroperitoneal laparoscopy in the animal. This procedure could be introduced in man.
