[Electronmicroscopic findings in a malignant hepatoma after oral contraceptives (author's transl)].

In this paper we report the electronmicroscopic findings in a hepatocellular carcinoma of a 24 years old woman after taking oral contraceptives. We saw alterations of the nucleus (finger-like or finger-formed nuclei, nuclear invaginations and intranuclear vesicular membranbodies), changes of the mitochondries (number, polymorphy and osmiophil bodies) as well as changes of lysosomes (myelinshaped residualbodies). The observed and described changes are not specific for malignant tumors of the liver.

Quantitative evaluation of the spermicidal potencies of vaginal contraceptives with a transmembrane migration method.

The spermicidal potencies of four vaginal contraceptives, namely Delfen cream, Samtol tablet, Lorophyn suppository and Ortho-Gynol jelly were compared using a transmembrane migration method which measured the proportion of sperm that moved from a semen-drug mixture across a Nuclepore membrane with 5 micron pores into phosphate buffer. The concentration-response curves of all preparations were parallel. The maximal dilutions that decreased sperm motility to 50% of control for Delfen, Samtol, Lorophyn and Ortho-Gynol were 240, 160, 75 and 35 respectively. The trans-membrane migration method is a simple, reproducible and quantitative test for evaluating the spermicidal potencies of vaginal contraceptives.

Carbon dioxide laser treatment of vaginal adenosis in DES-exposed offspring: a prospective study.

Seventy-nine patients with history and physical findings characteristic of antenatal DES exposure were randomly divided into two groups. Fourty-four DES-exposed offspring had their vaginal adenosis treated with the carbon dioxide laser (group I), and the remaining 35 DES-exposed offspring (group II) did not receive any specific treatment for this condition. Additionally, the 79 DES-exposed offspring were compared to an age-matched control population (group III). Treatment of vaginal adenosis with the carbon dioxide laser did not significantly reduce the incidence of development of new dysplasia in the DES-exposed offspring. This study also showed no statistical difference (p less than or equal to 0.05) in the incidence of dysplasia in DES-exposed offspring as compared to a control population.

Malignant hypertension in women aged 15 to 44 years and its relation to cigarette smoking and oral contraceptives.

To study the factors associated with malignant hypertension in young women, we reviewed the medical records of all women aged 15 to 44 years who were hospitalized in any of 15 affiliated hospitals from 1971 to 1980 with malignant hypertension. There were 15 new patients in this period, an incidence of 0.5 per 100,000 women of these ages per year. Four women had underlying renal disease. All of the remaining 11 used oral contraceptives, smoked cigarettes, or both. Nine were white, and 6 women had normal blood pressure in the 24 months before hospitalization. These findings support a relation of oral contraceptive use and cigarette smoking to an increased risk of malignant hypertension in young women.

Pharmacokinetics of norethisterone in humans.

The pharmacokinetics of a dose of 1mg norethisterone administered with 50 micrograms ethynyloestradiol was studied in 83 subjects. The dose was rapidly absorbed and there were wide variations in the serum NET concentrations at any particular time after dosing; the concentrations at 24 h varied from 100 to 1700 pg/ml. There was a significant negative correlation between the serum NET concentration and the time after dosing in all women. There were large inter-subject variations in the pharmacokinetic parameters, the elimination half-life and bioavailability showing 3- and 5- fold variability, respectively. Mean values for the parameters were t1/2, 7.6 h; bioavailability, 53.6 ng/ml/h; C max, 4.63 ng/ml; clearance, 22.6 l/h; and Vd 2361. There were a number of statistically significant correlations between the pharmacokinetic parameters and analysis of the correlations suggested that clearance was an important determinant of the bioavailability and of C max whereas the elimination half-life was the determinant of the NET concentration at 24 h. The pharmacokinetics of NET are compared with those of ethynyloestradiol. The wide variation in pharmacokinetics is likely to be important in determining inter-subject variations in efficacy and, particularly, side-effects of oral contraceptives especially now that low-dose formulations are widely used.

Long-acting contraceptive agents: analysis and purification of steroid esters.

More than 200 samples of esters of norethisterone (17 alpha-ethynyl-17 beta-hydroxyestr-4-en-3-one) and levonorgestrel (13 beta-ethyl-17 alpha-ethynyl-17 beta-hydroxygon-4-en-3 -one) have been analysed by a combination of techniques, including high performance liquid chromatography (HPLC). Compounds having a purity below the required limit (99.5%) were purified, mainly by preparative HPLC, prior to formulation and biological evaluation as long-acting progestogens.
