[Analysis of the effectiveness of treating children with different forms of intellectual deficiency with piriditol].

The clinical efficacy of piriditol treatment in children with intellectual insufficiency was studied by means of special techniques, determining the velocity of visual information processing (VVIP) and short-time visual memory (SVM), which reflects the functional state of the brain. Piriditol was administered to 50 children aged from 7 to 12 years. The clinical state was characterized as mental retardation, due to mild organic brain lesions or oligophrenia in the form of mild debility. The use of special techniques (VVIP and SVM) for the study of intellectual activity permitted to analyze more differentially the stimulating and sedative effect of piriditol, to detect the new property of the drug, i.e. to remove the processes of fatigue in the brain tissues. A positive effect of the treatment was most marked in children with mental retardation rather than in oligophrenic patients.

Use of encephabol in anaesthesia and post-anaesthesia resuscitation.

On the basis of good results in a pilot study of 40 patients who had undergone anaesthesia for various surgical operations, infusions of pyritinol was tested under controlled double-blind conditions on 60 further patients. Pyritinol 600 mg was infused immediately after anaesthesia. The aim of this study was to verify the favourable effect on the post-anaesthesia phase. It was shown that pyritinol significantly shortens the wakening time and, furthermore, positively influences the subjective feeling of the patients after anaesthesia.

[Neuropharmacologic and neurotoxic studies on the Renshaw mechanism].

Recurrent axon collaterals of alpha-motoneurons formed with Renshaw cells cholinergic synapses. The effects of ethanol, pyritinol-HCl and type A botulinum toxin on this central nervous cholinergic synapse were studied. Ethanol caused a marked increase in Renshaw cell activity. The excitatory influence effects above all the Renshaw cell synapses directly. Pyritinol-HCl decreased the activity of Renshaw cells. The increase of Renshaw cell activity, caused by ethanol, changed into a decrease after pyritinol-HCl application so that pyritinol-HCl has a protective effect on this cholinergic system. The injection of the neurotoxin botulinum type A into the muscle or even into the ventral root was without effect on the Renshaw cell activity during the test period. Only the direct application of botulinum toxin type A into the spinal cord led to a decrease of the early and the late response of Renshaw cells. The results are discussed and the clinical relevance is considered. The cholinergic Renshaw cell synapse is not only considered as a special synapse but also as a model for cholinergic transmission which is assumed in several parts of the central nervous system.

Effect of pyritinol-GCl on blood flow and oxidative metabolism of the brain in patients with dementia.

In order to test whether and in what way pyritinon-HCl affects cerebral blood flow and oxidative metabolism in patients with organic brain disorders, the following parameters were measured in a group of 87 patients: cerebral blood flow using the Kety and Schmidt method, cerebral consumption of oxygen and glucose, and also CO2 and lactate ouptut. 45 out of the 87 patients were given pyritinol in a dose of 900 or 1000 mg/day, 42 out of 87 patients formed a control group and were given 500 mgof 5% laevulose i.v. daily for the average duration of the study of approximately 3 weeks. The results can be summarised as follows: 1. Cerebral blood flow and oxidative metabolism are changed in patients with organic brain disorders in different ways voth as regards quality and quanitity. Findings from earlier investigations could thus be confirmed. 2. When laevulose only was used, the parameters measured did not alter on average. In addition to deteriorations in the findings, spontaneous improvements or normalisationof previously disturbed cerebral blood flow and cerebral metabolism values were also abserved. 3. Pyritinol-HCl usually improved previously disturbed cerebral glucose metabolism significantly. An effect on disturbed cerebral blood flow or pathologically changed cerebral oxygen consumption was not found.

Vasotropic drugs--a survey based on a unifying concept of their mechanism of action.

A group of vasotropic drugs was surveyed using a new assay method based on the inhibition of endothelaemia increases after a standard vascular lesion in rats. The new test is considerably more sensitive than all previous ones. It provides a common link between all tested vasotropic drugs and points also to their common mechanism of action. All of them have a protective effect on endotherlium based, in some of them at least, on the preservation of the consistency of endothelial cement. The effect is probably mediated by their influence on calcium availability.

Teratogenic effect of cyclophosphamide and the preventive effects of pyrithinoldehydrochloride monohydrate.

Teratogenic effects were induced in outbred Swiss mice by treatment of 8-week-old pregnant females with one of five different doses (5, 10, 20, 30, 40 mg/kg b.w.) of cyclophosphamide. Because the 11.5th day of pregnancy was the most effective day to induce malformations in mice, animals treated on this day were used for prophylactic studies. A preliminary study had shown that 30 mg/kg b.w. cyclophosphamide was the ideal dose to produce 100% malformations on the 11.5th day of pregnancy. From day 7.5 to 9.5 all fetuses were resorbed with this dosage. Most malformations were induced with 20, 30 or 40 mg/kg b.w., whereas 5 and 10 mg/kg b.w. caused no alterations of the fetuses. The frequency distributions of the various induced malformations and combined malformations of a positive control group (30 mg/kg b.w. of cyclophosphamide on the 11.5th day of pregnancy) were compared to those found in groups receiving different doses of the prophylactic. This substance reduced significantly the rate of malformations; 1/160 LD50 pyrithinoldehydrochloride monohydrate administered 4 times prior to cyclophosphamide treatment was the most effective. However, a complete prevention of teratogenic effects could not be accomplished.

[Skin manifestations observed during treatment with penicillamine and its derivatives].

Many forms of toxidermia are observed during D-penicillamine therapy. The exact type seems to depend on the pathology for which the drug was prescribed; pemphigus during rheumatoid arthritis, perforating serpiginous elastoma in Wilson's disease, cystinuria... The two physiopathogenic mechanisms discussed, an autoimmune reaction and a direct action of D-penicillamine on the skin, would seem to be complementary.
