Population growth-a menace to what?

Originally, many of the initiators of the World Population Conference, which took place in Bucharest in 1974, had hoped that the Conference would imply a final breakthrough for the view that family planning measures should be given top priority in all less-developed countries. In fact, however, the Plan of Action passed by the Conference contains very little relating to population and family planning. Instead, the document is dominated by wordy phrases about the necessity of attaining social and economic development in those countries. Will the insight that family planning programs work efficiently only if they are an integral part of programs for the social and economic development of a country lead to such programs being realized? There is every reason to doubt that the plan of Action will have any such effect. The reasons for the underdevelopment of Third World countries cannot be removed through such United Nations resolutions. In the People's Republic of China, family planning is widely accepted, especially in the towns, and now also among the rural population. Limiting the number of children is considered part of China's development effort. China is a less-developed country that is in the process of rapid social and economic development. The issue at stake in other Third World countries is how to achieve a similar development. As soon as this goal is achieved, family planning efforts are meaningful and have a chance of success. The experience of China demonstrates that even there it took time before the efforts succeeded. There are many Third World countries that could, without much difficulty, support a population considerably larger than the present one. But there are no doubt also a number of countries where the population is already so large that a continued population increase would be harmful. The need to achieve rapid development becomes increasingly urgent, not in the least to make it possible to attain a reduced population growth. The sad truth is that so little development takes place in those countries. Without social and economic development, the present rapid population increase will continue in those countries where there is already an overly dense population.

Providing cost efficient detoxification services to alcoholic patients.

The literature was reviewed to determine whether social model detoxification programs are safe and adequate for treating persons with alcohol withdrawal symptoms. The alcohol withdrawal syndrome has three stages. Each stage, more severe than the last, is reached by a smaller percentage of those withdrawing from alcohol. The literature showed that the majority of alcoholics can be detoxified safely in social model programs. These programs presented two main benefits, program cost efficiency and the patients' increased commitment to treatment compared with those treated at medical model programs. Medically operated detoxification programs appeared necessary for patients with a severe withdrawal condition at intake (abnormal blood pressure and pulse) and those with a history of severe withdrawal symptomatology. The results of the review reiterated the importance of screening clients at intake to ensure the safety of the patient and the appropriateness of the detoxification program.

Medicare program; suggestion program on methods to improve Medicare efficiency. Health Care Financing Administration (HCFA), HHS. Final rule with comment period.

This final rule with comment period establishes a program to encourage individuals to submit suggestions that could improve the efficiency of the Medicare program. The rule implements section 203(c) of the Health Insurance Portability and Accountability Act of 1996. The intent of this rule is to encourage suggestions and to award, if we deem appropriate, monetary payments to individuals for suggestions that improve efficiency and produce monetary savings to the Medicare program.

Cost-efficiency of a community dental delivery program for migrant children.

PURPOSE: The purpose of this study was to describe an economically efficient dental prevention and treatment program aimed at migrant children, using dental school students. This dental treatment program aimed to provide comprehensive dental care to migrant children and to provide a unique learning experience for dental students expanding their knowledge of community delivery systems. METHODS: Twenty-four dental students treated 330 migrant children through a 6-week outreach treatment program sponsored by the University of Michigan School of Dentistry, as a part of the Northwest Michigan Migrant Health Program. RESULTS: The program cost $39,907 and resulted in just over $16,000 of Medicaid reimbursements. Factoring in the Medicaid reimbursements, the program cost $71.58 to treat each child comprehensively. CONCLUSIONS: In light of the reported oral health disparities throughout the United States, this program provides an alternative mechanism for efficient and cost-effective oral care in a group that has traditionally had poor oral health.

Decreased peptidyltransferase activity correlates with increased programmed -1 ribosomal frameshifting and viral maintenance defects in the yeast Saccharomyces cerevisiae.

Increased efficiencies of programmed -1 ribosomal frameshifting in yeast cells expressing mutant forms of ribosomal protein L3 are unable to maintain the dsRNA "Killer" virus. Here we demonstrate that changes in frameshifting and virus maintenance in these mutants correlates with decreased peptidyltransferase activities. The mutants did not affect Ty1-directed programmed +1 ribosomal frameshifting or nonsense-mediated mRNA decay. Independent experiments demonstrate similar programmed -1 ribosomal frameshifting specific defects in cells lacking ribosomal protein L41, which has previously been shown to result in peptidyltransferase defects in yeast. These findings are consistent with the hypothesis that decreased peptidyltransferase activity should result in longer ribosome pause times after the accommodation step of the elongation cycle, allowing more time for ribosomal slippage at programmed -1 ribosomal frameshift signals.

[The efficiency of home and outpatient exercise program in patients with rheumatoid arthritis].

OBJECTIVE: To compare the efficiency of home and outpatient exercise program in patients with rheumatoid arthritis. METHODS: Patients with rheumatoid arthritis were randomly assigned to home exercise program group and outpatient exercise program group. Variables of muscle and hand strength, joint mobility, visual analog scale of pain were assessed before and after the 3 months exercise course. RESULTS: Out of 43 patients, 31 completed the study. Both groups improved in measures of muscle strength, joint mobility and pain. Differences between groups were statistically significant for measures of grip strength in the home exercise group (p=0.0001), and joint mobility--in the outpatient exercise group (p=0.047). CONCLUSIONS: Exercise program was more effective in improving grip strength in the home exercise program group and joint mobility improved more in the outpatient exercise group.

[The language and speech skills in 417 children with cleft formations].

Children with cleft palate often suffer from hearing, speech, and language articulation disorders. In order to design an efficient rehabilitation program for children thus affected, it is mandatory to acquire knowledge of the long term results achieved by the various therapeutical strategies including velopharyngoplasty and speech therapy. In this follow-up study 417 children with cleft palate (excluding isolated cleft lip) were examined in an interdisciplinary approach by maxillofacial surgeons, orthodontists, otolaryngologists, audiologists, and speech and language pathologists. The examinations determined that 93% of the children had speech or language disorders and 80% of these children suffered from mild to severe conductive hearing loss with or without clinical signs of otitis media with effusion. In 58 children (14%) with rhinolalia aperta, which had not been improved after one year of speech therapy, velopharyngoplasty with a cranial based pharyngeal flap was performed. The study showed that language skills do not correlate to the type of cleft palate, but rather to the frequency and degree of hearing loss. Using an interdisciplinary approach in early detection and the prompt clinical correction of cleft palate disorders resulted in only 49% of the affected children having to undergo speech and language therapy. In 51% of the affected children no speech therapy was necessary at all. The results presented in this study lead to the conclusion that our program for managing the rehabilitation of children with cleft palate is efficient.

Experience with detailed surveillance of nosocomial infection.

Infection rates have been determined in a 332 bed university hospital using an efficient surveillance program. This computerized program analyzed, summarized and returned infection reports in six days. The efficiency rate of this system was 80 per cent as determined by prevalence survey. The mean nosocomial infection rate for the 12 month study period was 10.8 per cent. Neurology, medicine, neurosurgery, surgery and urology ahd the highest attack rates. Urinary tract infections composed 44 per cent of the total and predominated on seven of the 11 services. Escherichia coli predominated in infections of the urinary tract, surgical wounds and blood and ranked third in respiratory infections. Escherichia coli, klebsiella, pseudomonas and proteus mirabilis accounted for 61 per cent of the isolates. There is a need for efficient reporting of hospital infections to establish effective preventives.

An experimental evaluation of an AIDS educational intervention for WIC mothers.

The purpose of this study is to determine which of two educational approaches have the greater effect on the AIDS/human immunodeficiency virus (HIV) knowledge and attitudes of women participating in the Women, Infants, and Children (WIC) Program. A modified version of the Centers for Disease Control's (CDC) 1989 Health Risk Survey was administered to 217 women, who were then randomly assigned to either a control group receiving the usual written material, a nurse-educated group, or a videotape-educated group. The questionnaires were repeated immediately after and 2 months after the intervention. Chi square, Kruskall-Wallis ANOVA, and a repeated measures ANOVA were used for data analysis. Ninety-five percent of the subjects were black and the mean age was 25.8 years (+/- 5.9). The control group had significantly lower (p < or = 0.003) AIDS knowledge scores at both posttests, with the lowest knowledge level at 2 months. The videotape group had a greater (p < or = 0.048) intent to reduce risky behaviors at the initial posttest. Tolerance towards AIDS patients was significantly (p < or = 0.025) greater in the videotape and nurse groups. Both videotape and nurse education programs increased knowledge and influenced attitudes and behavioral intent. The more efficient videotape program had similar effects as the nurse program, and may be more generalizable to other populations.

A comparison of muscle training methods in patients with knee osteoarthritis.

This study sought to compare the efficacy of isokinetic and progressive resistive exercise (PRE) programs in patients with knee osteoarthritis (OA). Forty-four patients with bilateral knee OA were included in the study. The patients in Group 1 ( n=21) performed isokinetic exercises and the patients in Group 2 ( n=18) performed a PRE program. Disease severity, pain, walking time, WOMAC, Lequesne index, AIMS2 and SF36 were compared before and after the treatments. All the patients were evaluated via a Cybex isokinetic device before and after treatment. Disease severity, pain, Lequesne, WOMAC and walking time improved with treatment in both groups. In SF36 and AIMS2 assessments, pain and social evaluation parameters in the PRE group showed better improvement. On isokinetic assessment flexor and extensor peak torque and peak torque body weight values improved significantly in both groups compared to pretreatment measurements. When the assessed parameters were taken into account no statistical significant difference was observed between the two groups. Our conclusions were that isokinetic and PRE programs are efficient in the treatment of knee OA; no statistically significant differences could be found between the two programs; and the PRE program, as it is cheaper, more easily performed and efficient, may be preferable for the treatment of knee OA.

A simple assessment of the risk of low birth weight to select women for nutritional intervention.

There is growing evidence that the high prevalence of low-birth-weight (LBW) babies, a characteristic of many poor societies, can be significantly reduced through nutritional intervention programs. In order to increase the efficiency of these programs, pregnant women at risk of delivering LBW babies must be identified. Several risk scales have been extensively used in urban populations of developed countries. However, these indicators are of little use in poor communities, since most of them require expensive laboratory techniques, a long interview, at least two visits to a health center, and a precise estimation of gestational age. The present paper proposes several risk indicators of LBW, appropriate for use in areas with inadequate health resources. In a four-year longitudinal study of pregnant women from poor rural villages in Guatemala, mothers of LBW babies were found to be typically small in stature and head circumference. In addition, their houses tended to be of relatively poor quality. The proportion of LBW babies found among women at risk, as defined by these indicators, was significantly lower among women with similar characteristics receiving adequate food supplementation during pregnancy. Categories of high risk were based on maternal height, head circumference, and house quality. It is concluded that use of these categories will increase the efficiency of these programs without decreasing significantly their effectiveness.

Automated probabilistic method for assigning backbone resonances of (13C,15N)-labeled proteins.

We present a computer algorithm for the automated assignment of polypeptide backbone and 13C beta resonances of a protein of known primary sequence. Input to the algorithm consists of cross peaks from several 3D NMR experiments: HNCA, HN(CA)CO, HN(CA)HA, HNCACB, COCAH, HCA(CO)N, HNCO, HN(CO)CA, HN(COCA)HA, and CBCA(CO)NH. Data from these experiments performed on glutamine-binding protein are analyzed statistically using Bayes' theorem to yield objective probability scoring functions for matching chemical shifts. Such scoring is used in the first state of the algorithm to combine cross peaks from the first five experiments to form intraresidue segments of chemical shifts (Ni,HiN,Ci alpha, Ci beta, Ci'), while the latter five are combined into interresidue segments (Ci alpha,Ci beta,Ci',Ni + 1,Hi + 1N). Given a tentative assignment of segments, the second stage of the procedure calculates probability scores based on the likelihood of matching the chemical shifts of each segment with (i) overlapping segments; and (ii) chemical shift distributions of the underlying amino acid type (and secondary structure, if known). This joint probability is maximized by rearranging segments using a simulated annealing program, optimized for efficiency. The automated assignment program was tested using CBCANH and CBCA(CO)NH cross peaks of the two previously assigned proteins, calmodulin and CheA. The agreement between the results of our method and the published assignments was excellent. Our algorithm was also applied to the observed cross peaks of glutamine-binding protein of Escherichia coli, yielding an assignment in excellent agreement with that obtained by time-consuming, manual methods. The chemical shift assignment procedure described here should be most useful for NMR studies of large proteins, which are now feasible with the use of pulsed-field gradients and random partial deuteration of samples.

SEQ-ED: an interactive computer program for editing, analysis and storage of long DNA sequences.

The rapidly growing body of sequenced DNA demands efficient computer programs for its analysis and storage. The program described in this paper, SEQ-ED, has been designed to handle a large number of DNA sequences up to 200 kilobases [kb] long stored in a sequence library. In order to minimize the required storage space, the sequences are stored in a compressed format using three binary digits per base. In the development of this program, special care has been given to make it easy to use for molecular biologists without any previous computer experience.

Efficiency of single-sire mating programs with beef bulls mated to estrus synchronized females.

Efficiency in reference to pregnancy rates of breeding beef bulls with estrus synchronized cows and heifers was tested. Most bulls (104 of 112) were given a breeding soundness examination and two 10-min libido/serving capacity tests. Females received either Syncro-Mate-B (SMB) or two injections of Prostaglandin F(2)alpha (PGF) to synchronize estrus. They were assigned to single-sire breeding groups with bull-to-female ratios ranging from 1:7 to 1:51. Control groups consisted of untreated females maintained in single-sire breeding pastures with ratios from 1:24 to 1:37. Continuous observations of sexual activity were made for 30 h (SMB) and 48 h (PGF). After the 120-h posttreatment breeding period, females were placed in breeding pastures. During the synchronized breeding period the percentage of pregnant cattle of total treated was 43.5 +/- 1.7% compared (P < 0.01) with 58.9 +/- 3.3% for the control group after 23 d of breeding. At end of 28-d (treated) and 46-d (control) period, the percentage of pregnant females was 75.0 +/- 2.4 and 79.6 +/- 4.7, respectively (P > 0.05). In SMB trials, the percentages of females exhibiting estrus, those serviced at estrus and those pregnant following service during the synchronized breeding period were 90.8 +/- 1.5, 73.3 +/- 4.5 and 56.4 +/- 5.6%, respectively. In PGF trials, the means for these same factors were 78.3 +/- 2.4, 70.4 +/- 5.9 and 56.1 +/- 6.5%, respectively.

The dissemination of cost-effective psychosocial programs for children in health care settings.

Psychosocial programs for children must be both efficient and efficacious. Efficiency is dictated by cost, case of implementation, and likelihood of program compliance. Unfortunately, in attempting to promote efficient programs, inefficacious programs have been disseminated. Also, there are instances of failing to disseminate efficacious programs because they are not efficient. This paper presents a three-stage model as a guide for the dissemination of psychosocial programs. Further, specific suggestions are given as to how we may move from the level of efficacious treatments that are currently used on a limited scale to widespread dissemination.

An "integrated model" of programmed ribosomal frameshifting.

Many viral mRNAs, including those of HIV-1, can make translating ribosomes change reading frame. Altering the efficiencies of programmed ribosomal frameshift (PRF) inhibits viral propagation. As a new target for potential antiviral agents, it is therefore important to understand how PRF is controlled. Incorporation of the current models describing PRF into the context of the translation elongation cycle leads us to propose an 'integrated model' of PRF both as a guide towards further characterization of PRF at the molecular and biochemical levels, and for the identification of new targets for antiviral therapeutics.

Training of somatosensory discrimination after stroke: facilitation of stimulus generalization.

OBJECTIVE: Task-specific learning typifies perceptual training but limits rehabilitation of sensory deficit after stroke. We therefore investigated spontaneous and procedurally facilitated transfer of training effects within the somatosensory domain after stroke. DESIGN: Ten single-case, multiple-baseline experiments were conducted with stroke participants who had impaired discrimination of touch or limb-position sense. Each experiment comprised three phases: baseline, stimulus-specific training of the primary discrimination stimulus, and either stimulus-specific training of the transfer stimulus or stimulus-generalization training. Both the trained and transfer stimuli were monitored throughout using quantitative, norm-referenced measures. Data were analyzed using individual time-series analysis and meta-analysis of intervention effects across case experiments. RESULTS: Stimulus-specific training was successful for trained texture and proprioceptive discriminations, but it failed to show spontaneous transfer to related untrained stimuli in the same modality in seven of eight experiments in which this was possible. In contrast, intramodality transfer was obtained with stimulus-generalization training in four of five experiments that investigated stimulus-generalization training of texture discrimination. Findings were confirmed by meta-analysis. CONCLUSIONS: Our findings demonstrate generalization of training within a somatosensory modality poststroke, provided that a program designed to enhance transfer is used. This has implications for the design of efficient rehabilitation programs.
