[Anaphylactoid reactions following administration of plasma substitutes in man. Prevention of this side-effect of haemaccel by premedication with H1- and H2-receptor antagonists (author's transl)].

In a randomized controlled single blind trial in 50 volunteers the problem was investigated whether a combination of dimethpyrindene (0.1 mg/kg b.w.) and cimetidine (10 mg/kg b.w.) could prevent anaphylactoid or allergoid reactions following Haemaccel infusion. In the control group 6 anaphylactoid and 9 allergoid reactions were observed, in the H1 + H2-group, however, none of those reactions occured. No single wheal could be detected. It is considered appropriate to recommend the general use premedication of an H1 + H2-blocker before all anaesthetics and operations. This mainly depends on further clinical trials with the antihistaminic drugs concerning their side-effects under various conditions.

Washout of the pig liver with Haemaccel after hypothermic preservation.

In hepatic preservation by simple perfusion and hypothermic storage, a portal and hepatic washout before revascularization would avoid receptor hyperkaliema. In this report we study the effectiveness of this washout with Haemaccel at room temperature. Large-White pigs were used and eight livers were perfused "in situ" via the portal wein with Hartmann's solution containing 10,000 IU of heparin at 4 degrees C, and afterwards, via portal and arterial routes with C2 solution at 4 degrees C. After a cold ischemia time of less than 31/2 hours a liver washout via the portal vein and hepatic artery with Haemaccel before portal revascularization was done. The high concentrations of glucose, K+, GOT, GPT and LDH in the effluents obtained during the washout are attributed to Haemaccel hyperosmolarity. A portal and arterial hepatic washout associated with free drainage of the first 50-100 ml of portal venous blood after hepatic portal revascularization through the infrahepatic inferior vena cava (IH-IVC), prevents hyperkaliemia from occurring after a portal and arterial revascularization in the orthotopic liver transplant (OLT) in pigs.

Whole body plasma extravasation in saline and Haemaccel loaded rats: effects of endotoxemia.

Endotoxin causes only a limited increase in plasma leakage in rats. This may be due to a concomitant fall in venous pressure. We therefore studied effects of increases in this pressure on plasma loss. After a 60 min. endotoxin (20 mg/kg. hr E coli 0127-B8) or saline (control) infusion anesthetized rats were volume loaded (3 ml/10 min. 100 g for 10 min.) at t = 120 min. with saline (shock group n = 8; control group n = 8) or Haemaccel (shock group n = 6; control group n = 8). At t = 180 min. the experiments ended. We measured mean arterial and central venous pressure, heart rate, arterial lactate, blood volume (51Cr-labeled red cells), hematocrit (conductivity cell), macromolecular extravasation (125I-HSA) and fluid retention (calculated from change in plasma volume). Whole body transcapillary filtration coefficient (Kf) and interstitial compliance (Ci) were obtained from saline retention curves. During volume loading central venous pressure increased, then fell again. At the end of saline loading (t = 130) 50 and 60% of infused volume remained intravascularly in control and endotoxin treated rats, respectively; at the end of Haemaccel infusion retention was 110 and 120%, respectively. Fifty minutes later (t = 180) it was 25 and 30% after saline loading and 50 and 60% after Haemaccel loading in control and endotoxin treated rats, respectively. Between control and endotoxin group filtration coefficients (Kf; .094 vs .111 ml/min.mmHg.100 g, respectively) and compliances (Ci; 1.90 vs 1.58 ml/mmHg.100 g, respectively) were not significantly different. No increase in leakage of 125I-HSA was found in either group. Increased venous pressure thus did not reveal an increase in macromolecular permeability in endotoxin treated rats.

[Evaluation of the liberation of urinary necrosis enzymes (NAG-AAP) after administration of plasma expanders: study of dextran 40, hydroxyethyl starch and polymerized gelatin].

We studied the 24-hour urinary elimination of enzymatic markers of renal tubular necrosis (NAG-AAP) in 21 patients (mean age 34.6 years old) who were treated with plasma expanders before peridural anesthesia. The patients were divided into three groups of seven subjects each: - 14 ml/Kg -1 of dextran 40 was administered to group 1 - 14 ml/Kg -1 of gelatin was administered to group 2 - 14 ml/Kg -1 of hydroxyethyl starch was administered to group 3. Urinary elimination of N-acetylglucosaminidase and of alanine aminopeptidase was determined in the 24-hour urine the day before surgery (controls), the day of surgery (G1) and the day after surgery (G2). The values of the samples, taken after plasma expander administration, did differ significantly from the control values (G1, G2). Therefore the administration of 14 ml/Kg -1 of dextran, gelatin or hydroxyethyl starch does not affect the renal tubular epithelium.

[Proteinuria after infusion of gelatin. Comparison of Plasmion and Haemaccel].

Modified gelatin are said without deleterious effect on kidney, an important proteinuria as been seen however in surgical patients after gelatin perfusion. A study in 15 patients scheduled for abdominal surgery compared the renal effects of two modified gelatin: Plasmion (gr P) and Haemaccel (gr H) administered in a similar manner. In the two groups proteinuria appears as soon as perfusion begins with at the third hour a peak which may be as high as 6 g/l. In the same time low molecular weight proteinuria (less than 30 kdalton) appears. The beta 2 microglobulinuria (beta 2m) is significatively enhanced (p less than 0,001). Albuminuria is also enhanced but without statistic signification. Comparison between the two groups reveals that in gr P proteinuria is of the same importance, but delayed, with a significatively smaller elimination of beta 2m (1,8 mg/mmol creatininuria versus 8,6,p less than 0,001). Enzymuria increases in a variable fashion. Proteinuria is probably due to tubular reabsorption inhibition of filtered protein induced by gelatin, particularly by amino acids arginine and lysin which become free after gelatin hydrolysis. If this phenomenon is pathologic or not is unclear and gelatin cannot be said absolutely innocuous. However this phenomenon must be known when proteinuria specially beta 2m is to be interpreted.

Experimental allergic neuritis: effect of plasma infusions.

The effect of intravenous fresh frozen plasma (FFP) and artificial plasma infusions upon the clinical course of chronic experimental allergic neuritis (EAN) in the rabbit was investigated. A total of 12 animals allocated to treatment groups received rabbit FFP or a gelatin plasma expander Haemaccel (Hoechst) and were compared to 13 control non-treated animals. Animals receiving Haemaccel at a rate of 15 ml/kg/day for 7 days showed no significant clinical benefit at any stage. However, animals receiving 15 ml/kg/day FFP for 8 days showed significant clinical benefit during treatment initiated at the onset of definite neurological symptoms of EAN (Mann-Whitney U test, day 4 post-allocation P less than 0.05; day 6 post-allocation P less than 0.01; day 8 post-allocation P less than 0.05). Relapse was observed after cessation of treatment such that comparisons of clinical scores at day 14 and 22 post-allocation revealed no significant differences. Analysis of plasma anti-myelin IgG levels by ELISA showed that non-immunogenic plasma volume expansion decreased anti-myelin IgG concentrations immediately by an average of 34% but had no long-term effect. In contrast, anti-myelin IgG concentrations in FFP infused animals were significantly decreased, compared to controls, when measured 24 h after the last infusion (Student's t-test P less than 0.05). Identical percentage weight losses for both control and treatment groups post-allocation indicated that this decrease was immunologically mediated and not due to plasma dilution. Similar plasma cortisol concentrations measured in both groups showed no significant artifactual induction of endogenous steroid production. Infusions of FFP during early disease progression are able to mediate clinical remission in animals with chronic EAN.

Technetium-99m haemaccel: a new lymphoscintigraphic agent.

Haemaccel, a denatured gelatin polymer with average molecular weight of 35,000, was labeled with Tc-99m and evaluated in rats and rabbits. The labeling efficiency was consistently higher than 85%. Tc-99m haemaccel (200 microCi in 0.05 ml) was injected intradermally into the hind footpads of rats lightly anaesthetized with diethyl ether vapour. Using a gamma camera, static images of the anterior view were obtained at various time intervals after injection. The images obtained demonstrated good localization of Tc-99m haemaccel in regional lymph nodes. In rats, about 3% of the injected dose was trapped in the node, and 48% remained at the injection site at 4 h after injection. In rabbits, good-quality images of lymph nodes and lymphatic channels of the hind legs were obtained within 15 min after intradermal injection of Tc-99m haemaccel. Tc-99m haemaccel showed fast migration from the injection site, good accumulation in the lymph nodes, and good visualization of lymphatic channels. Hence, it has a potential application in lymphoscintigraphy.

Right-heart echocontrast in the anesthetized dog after i.v. administration of a new standardized sonographic contrast agent. 3rd communication: comparison of various contrast agents employed in contrast echocardiography.

Right-ventricular contrast provided by the new standardized sonographic contrast agent SH U 454, which consists of pure galactose microparticles, with 300 mg microparticles/ml and by 5 other currently employed ultrasonic contrast media was examined by 2 D echocardiography in 10 anesthetized female beagle dogs (6.8-12.7 kg) following i.v. administration. With a 5-min interval between injections each animal was given 3 injections of 2 ml of each formulation in random order using an injection speed of approximately 2 ml/s. The echocardiographic investigation was recorded on videotape and the contrast assessed blind using a visual score system (rating 0-5) by two investigators working independently and also by a videodensitometer. Only SH U 454 caused an intense, homogeneous and reproducible opacification of the right heart chambers. By comparison with other formulations for right-heart echocontrast, SH U 454 was significantly superior (p less than 0.05) in every parameter assessed.

[Effect of gelatin and dextran infusion on the reticuloendothelial system].

The purpose of this study was evaluation of the phagocytic ability of peripheral blood leucocytes and reticuloendothelial system of the lungs, liver and spleen after one infusion of plasma-substituting preparations: Fluigel and dextran 70 in volumes of 20 ml/kg of body weight. The experiments were done in rabbits. The response of the reticuloendothelial system was assessed on the basis of its ability of eliminating foreign erythrocytes labelled with 51Cr from the circulating blood and radioactivity accumulation in the liver and spleen. It was found that infusion of Fluigel and dextran produced transient changes in the function of the reticuloendothelial system with increased number of granulocytes and a fall of their radioactivity 3 and 6 hours after infusion, reduced ability of pulmonary 51Cr accumulation and increased storage in the spleen observed 24 hours after infusion.
