A multisite phase III study of the safety and efficacy of a new manganese chloride-based gastrointestinal contrast agent for MRI of the abdomen and pelvis.

The purpose of this study was to evaluate the safety and efficacy of a manganese chloride-based oral magnetic resonance (MR) contrast agent during a Phase III multisite clinical trial. Two hundred seventeen patients were enrolled who were already scheduled for MRI of the abdomen and/or pelvis. In this group of patients, it was postulated that the use of an oral agent would better allow discrimination of pathology from bowel. Patients with known gastrointestinal pathology including peptic ulcer disease, inflammatory bowel disease, obstruction, or perforation were excluded to minimize confounding variables that could affect the safety assessment. Of these 217 patients, 18 received up to 900 mL of placebo, and 199 patients were given up to 900 mL of a manganese chloride-based oral contrast agent, LumenHance (Bracco Diagnostics, Inc.). Safety was determined by comparing pre- and post-dose physical examinations, vital signs, and laboratory examinations and by documenting adverse events. Efficacy was assessed by unblinded site investigators and two blinded reviewers who compared pre- and post-dose T1- and T2-weighted MRI scans of the abdomen and/or pelvis. In 111 (57%) of the 195 cases evaluated for efficacy by site investigators (unblinded readers), MRI after LumenHance provided additional diagnostic information. Increased information was found by two blinded readers in 52% and 51% of patients, respectively. In 44/195 cases (23%) unblinded readers felt the additional information would have changed patient diagnosis and in 50 patients (26%), it would have changed management and/or therapy. Potential changes in patient diagnosis or management/therapy were seen by the two blinded readers in 8-20% of patients. No clinically significant post-dose laboratory changes were seen. Forty-eight patients (24%) receiving LumenHance and four patients (22%) receiving placebo experienced one or more adverse events. Gastrointestinal tract side effects were most common, seen in 29 (15%) of LumenHance patients and in 3 (17%) of the placebo patients. LumenHance is a safe and efficacious oral gastrointestinal contrast agent for MRI of the abdomen and pelvis.

Risk of second malignancy after cancers of the renal parenchyma, renal pelvis, and ureter.

Risk of second primary malignancy was assessed in a population-based survey of persons who developed cancers of the renal parenchyma, renal pelvis, or ureter in Connecticut during the period 1935-1982. Among 4176 patients with a first primary tumor of the renal parenchyma, a second cancer was reported in 219 (5%), yielding a small but significantly elevated relative risk (RR) of 1.2, which reflects excesses for cancers of the bladder, kidney, and lymphatic-hematopoietic system. Among 939 patients with a first primary tumor of the renal pelvis or ureter, a second cancer was reported in 155 (17%), associated with a significantly elevated RR of 2.7. This resulted mainly from a 21-fold increase in risk for bladder cancer, although significant excesses were also found for lung and prostate cancers, and metachronous cancers of the renal pelvis and ureter. These associations seem to reflect the multicentric behavior of tumors arising in the urinary tract, the role of cigarette smoking, and host factors yet to be defined, and some degree of heightened medical surveillance and detection of tumors, especially in the same organ system.

Sectional anatomy of the pelvis in the male rat with ultrasound correlations.

Ten-week-old male Wistar rats were used for sectional anatomy and ultrasonic diagnosis of the pelvis. The prostate and the urinary bladder were identified easily on longitudinal section by ultrasound examination. The prostate (ventral, lateral, and dorsal lobes) was located in the midline of the pelvis cavity on transverse sections and the caudal side of the urinary bladder on longitudinal sections. The urinary bladder was positioned at the cranial side of the prostate on longitudinal sections and in the midline of the pelvis cavity on transverse sections. The seminal vesicles were located at right and left positions of the urinary bladder. Ultrasonic diagnosis was estimated to be useful for experimental studies and we hope that ultrasound techniques may reduce the number of rats used for the treatment of prostate and urinary bladder cancers.

Exclusion of small bowel from pelvis for postoperative radiotherapy for rectal cancer.

The pelvis is the preferential site of disease when recurrence occurs after curative resection of rectal cancer. Postoperative adjuvant radiotherapy decreases local recurrence rates for Dukes stages B and C; however, after abdominoperineal or anterior resections, the small bowel descends into the lower pelvis, and, due to its low tolerance to radiation, optimal doses cannot be safely delivered unless the small bowel can be excluded from the pelvis. This can be achieved by cystopexy and/or by creating a pelvic sling with a knitted polypropylene mesh. Three cases are presented in which radiotherapy was satisfactorily delivered with these techniques.

Diuresis urography in equivocal pelvi-ureteric obstruction.

In 43 patients with equivocal pelvi-ureteric obstruction, conventional urography was supplemented with diuresis urography. After peroral hydration with 0.5 liters fluid, a large volume of urographic contrast medium was administered and increased diuresis provoked by intravenous injection of furosemide. A relative obstruction of flow was then accentuated by increased dilatation of the renal pelvis. The technique deserves a wider use in the radiologic investigation of patients with equivocal obstruction at the pelvi-ureteric junction.

[Surgical treatment of osseous metastases of the pelvis].

Metastases are the most frequent malignant tumors of the skeleton. Breast cancer is the most frequent primary tumor with osseous metastases followed by renal cell carcinomas, bronchial carcinomas, and thyroid tumors. The predominant localization of bone metastases is the spine, followed by the pelvis, femur, ribs, skull, sternum, and humerus. The overall incidence of bone metastases has increased due to improved diagnosis, prolonged survival by adjuvant therapy, and improved follow-up. Osseous metastases of the pelvis are usually diagnosed only late in the course of the disease because of their considerable delayed onset of clinical symptoms. Therefore, treatment is usually focused on a palliative approach to reduce pain and to improve the quality of life. We introduce three patients with bronchial, renal cell, and cholangiocellular carcinoma in an incurable tumor stage with osseous metastases in the pelvis. The metastatic osteolyses caused pain and symptoms of instability. Despite high doses of opiate analgesics, the patients were neither free of pain nor could they be mobilized. The palliative treatment included preoperative tumor embolization followed by tumor resection and implantation of a hip prosthesis. The preoperative embolization reduced intraoperative bleeding and made tumor resection, osteosynthesis, and implantation of the hip prosthesis possible. Mobilization with moderate pain could be achieved in these three patients for the rest of their lives.

Variations of caudal, central, and cranial acetabular anteversion according to the tilt of the pelvis.

Variations of acetabular anteversion measured at the level of the diameter of the acetabulum according to the tilt of the pelvis are known. But the values of this anteversion at other levels of the acetabulum, near the roof or near the obturator foramen, are unknown. Attracted by the very changeable morphology of the acetabular cover, we studied the caudal, central, and cranial acetabular anteversion of 12 pelvises in four positions of dorsal-ventral inclination. The caudal, central, and cranial acetabular anteversion diminished with the ventral tilt of the pelvis. The anteversions also diminished from the caudal part of the acetabulum to the cranial part, whatever the inclination of the pelvis. These notions represent a reference that, once completed with clinical studies of healthy volunteers and patients presenting an anomaly of the acetabulum cover, will enable researchers to define threshold values of normal and pathological conditions.

Fluctuating fetal or neonatal renal pelvis: marker of high-grade vesicoureteral reflux.

Fetal renal pelvic dilatation is a predictor of vesicoureteral reflux (VUR), but has low specificity. The aim of this study was to determine the clinical significance of fluctuating size of the renal pelvis on sonography, particularly in relationship to VUR. We prospectively recorded fetal renal pelvic diameter >/=4 mm in 1,092 fetuses (692 boys), from May 1989 to December 1995, with a minimum follow-up of 7 years. Fluctuation, defined as size of renal pelvis changing by more than 4 mm during the course of obstetric (23), postnatal (128), and both pre- and postnatal (7) renal sonograms, was recorded prospectively in 159 (117 boys). Of the 1,092, 849 (593 boys) had a voiding cystourethrogram at a mean age of 7 weeks (range 3-20 weeks). Of the 31 (16 girls) with high-grade VUR, fluctuation was observed in 20 (9 girls). Fluctuation was strongly associated with high-grade VUR (odds ratio 11.1, P=0.0000003) and with renal damage (sensitivity 61%, positive predictive value 31%). Primary high-grade VUR was persistent, seen equally in boys and girls, and required surgery in the majority. Fluctuation was associated with renal duplication anomalies ( P=0.00009) and megaureter ( P<0.00000001). Fluctuation of the renal pelvis on sonography is a marker for persistent high-grade VUR and renal damage in girls and boys.

Sagittal balance of the pelvis-spine complex and lumbar degenerative diseases. A comparative study about 85 cases.

Retrospective analysis of the spino-pelvic alignment in a population of 85 patients with a lumbar degenerative disease. Several previous publications reported the analysis of spino-pelvic alignment in the normal and low back pain population. Data suggested that patients with lumbar diseases have variations of sagittal alignment such as less distal lordosis, more proximal lumbar lordosis and a more vertical sacrum. Nevertheless most of these variations have been reported without reference to the pelvis shape which is well-known to strongly influence spino-pelvic alignment. The objective of this study was to analyse spino-pelvic parameters, including pelvis shape, in a population of 85 patients with a lumbar degenerative disease and compare these patients with a control group of normal volunteers. We analysed three different lumbar degenerative diseases: disc herniation (DH), n = 25; degenerative disc disease (DDD), n = 32; degenerative spondylolisthesis (DSPL), n = 28. Spino-pelvic alignment was analysed pre-operatively on full spine radiographs. Spino-pelvic parameters were measured as following: pelvic incidence, sacral slope, pelvic tilt, lumbar lordosis, thoracic kyphosis, spino-sacral angle and positioning of C7 plumb line. For each group of patients the sagittal profile was compared with a control population of 154 asymptomatic adults that was the subject of a previous study. In order to understand variations of spino-pelvic parameters in the patients' population a stratification (matching) according to the pelvic incidence was done between the control group and each group of patients. Concerning first the pelvis shape, patients with DH and those with DDD demonstrated to have a mean pelvic incidence equal to 49.8 degrees and 51.6 degrees, respectively, versus 52 degrees for the control group (no significant difference). Only young patients, less than 45 years old, with a disc disease (DH or DDD) demonstrated to have a pelvic incidence significantly lower (48.3 degrees) than the control group, P < 0.05. On the contrary, in the DSPL group the pelvic incidence was significantly greater (60 degrees) than the control group (52 degrees), P < 0.0005. Secondly the three groups of patients were characterized by significant variations in spino-pelvic alignment: anterior translation of the C7 plumb line (P < 0.005 for DH, P < 0.05 for DDD and P < 0.05 for DSPL); loss of lumbar lordosis after matching according to pelvic incidence (P < 0.0005 for DH, DDD and DSPL); decrease of sacral slope after matching according to pelvic incidence (P = 0.001 for DH, P < 0.0005 for DDD and P < 0.0005 for DSPL). Measurement of the pelvic incidence and matching according to this parameter between each group of patients and the control group permitted to understand variations of spino-pelvic parameters in a population of patients.

Vaginal metastasis from transitional cell carcinoma of renal pelvis in horseshoe kidney.

We describe a case of transitional cell carcinoma of the renal pelvis in a horseshoe kidney with vaginal tumor as the first manifestation of remote metastasis. While the occurrence of adenocarcinoma in a horseshoe kidney appears to be no higher than in nonfused kidneys, the incidence of transitional cell carcinoma among neoplasms in horseshoe kidneys is higher. Vaginal metastasis from transitional cell carcinoma of the renal pelvis is considered extremely rare. To our knowledge, this is the first case reported of transitional cell carcinoma of the renal pelvis in a horseshoe kidney with metastasis to the vagina. The patient was successfully treated with surgery, irradiation, and chemotherapy.

Duplex Doppler scanning in the pelvis and abdomen.

New Doppler applications of Doppler ultrasound are becoming apparent in the abdomen and pelvis. Interpretation of the Doppler signals may be qualitative, semiqualitative or quantitative. Normal time-velocity spectra in major vessels of the abdomen and pelvis are described. In the diagnosis in pathology, Doppler can be used to document the presence and direction of flow in the portal venous system. In can aid in the identification of an abnormal structure. It provides much of the perfusion data required before and subsequent to liver transplantation. In normotopic renal vessels, it can exclude thrombosis and stenoses in many patients. In renal transplants, it is invaluable as a rapid means to assess patency, renal artery stenosis and vascular rejection, thereby extending the role of conventional imaging. Early studies in the pelvis have demonstrated changes in the ovarian signals associated with ovulation. Doppler can be used to assess bowel viability, impotence due to vascular insufficiency and to exclude testicular torsion. Finally, blood flow in malignant tumors has been demonstrated to give rise to Doppler signals with characteristic qualities, and this technique awaits thorough investigation in the abdomen.

Adjuvant radiotherapy in high stage transitional cell carcinoma of the renal pelvis and ureter.

This review was undertaken to assess the influence of adjuvant radiation therapy on failure patterns and survival in high stage transitional cell carcinoma of the renal pelvis or ureter. Ninety-four patients with transitional cell carcinoma of the renal pelvis or ureter were retrospectively reviewed. Twenty-six had American Joint Commission stage T3 or T4 N0/+, M0 disease and underwent curative resections (median follow-up 13.5 months, range 3-311). Local failure was defined as recurrence in the tumor bed, regional nodes, or ureteral stump. Time to recurrence and survival were calculated from the time of pathologic diagnosis. Variables associated with local failure, distant metastasis, and survival were analyzed using univariate and multivariate analysis. Seventeen received surgery only, nine received adjuvant radiation therapy (median dose 50 Gy). Local failure occurred in 9 of 17 without and 1 of 9 with adjuvant radiation therapy (p = 0.07). Actuarial 5-year local control was 34% without and 88% with adjuvant radiation therapy. Cox step-wise regression confirmed adjuvant radiation therapy (p = 0.006) and grade (p = 0.006) as significantly associated with local failure. No patients with low grade lesions suffered local failure either with or without adjuvant radiation therapy. High grade lesions had an local failure rate of 15% with and 71% without adjuvant radiation therapy. Metastatic disease occurred in 4 of 9 and 8 of 17 with and without radiation therapy. No significant factors influencing distant failure were identified. Five-year actuarial survival was 44% with and 24% without adjuvant radiation therapy. The survival differences were not statistically significant on univariate or multivariate analysis. High staged transitional cell carcinoma of the renal pelvis or ureter has a substantial local failure risk after surgery alone. Adjuvant radiation therapy markedly reduces this risk but has no impact on distant disease which occurs in approximately 50%. Effective adjuvant therapy will require effective systemic therapy in addition to adjuvant radiation therapy.

Arcus tendineus fascia pelvis: a further understanding.

OBJECTIVE: The study was undertaken to further define the anatomy of the arcus tendineus fascia pelvis (ATFP). STUDY DESIGN: Thirty cadavers were dissected to find the average length, SD, and range of the ATFP. Comparisons were made to height and pelvis type. The average distance between the ischial spine and the attachment of the fascia of the rectovaginal septum (RVF) to the ATFP was measured. RESULTS: The average length, SD, and range in centimeters for the ATFP are 9.0, 0.70, and 7 to 10.5, respectively. The length of the ATFP increased with height. No associations could be made regarding pelvis type. The average distance between the ischial spine and the attachment of the RVF to the ATFP is 2.15 cm with a SD and range of 0.21 and 1.75 to 2.5, respectively. CONCLUSION: In this study, an average length for the ATFP is established and the distance between the ischial spine and the attachment of the RVF to the ATFP is redefined.

Stroke affects the coordination and stabilization of head, thorax and pelvis during voluntary horizontal head motions performed in walking.

OBJECTIVE: This study was conducted to investigate and compare the coordination and stabilization of axial segments during walking with and without horizontal voluntary head turns, in healthy (n=5) and hemiparetic (n=10) subjects. METHODS: Subjects were instructed to turn the head as fast and as soon as possible in the direction indicated by an illuminated arrow signal (right, left or none) that was triggered at initial contact of the right (healthy) or paretic (hemiparetic) foot. Head, thorax, and pelvis motions were obtained from a 9-segment model using retro-reflective markers and a Vicon-512 system with 6 high-resolution cameras. Coordination of axial segments in the horizontal plane was characterized using cyclographs and cross-correlation analyses. Stabilization of the segments was quantified using root mean square (RMS) values of the segment's normalized acceleration profile. RESULTS: The healthy subjects showed a direction-dependent modulation of axial segment coordination, with head turns toward and away from the stance limb favoring and hindering, respectively, the contra-rotational pattern of the thorax with respect to the pelvis during locomotion. Meanwhile, pelvis motions remained unaltered. This direction-specific modulation pattern was disrupted in the hemiparetic subjects, both in the spatial and temporal domains. Moreover, larger RMS values for head and thorax segments were observed in the hemiparetic groups, both with and without the superimposition of voluntary head motions. CONCLUSIONS: The findings suggest that: (1) head rotations during walking modify axial segment coordination in a direction-specific manner, (2) the pelvic rotations associated with locomotion remained unaffected by head rotations and (3) stroke alters this coordination behavior, which may contribute to balance dysfunctions during locomotion.

Vibration testing of a fresh-frozen human pelvis: the role of the pelvic ligaments.

A biodynamic model of the human pelvis is being developed in the frame of a research project on low back pain. In order to validate such model, the dynamic behaviour of the human pelvis needs to be investigated. In this study, a human fresh-frozen specimen comprising the three bones of the pelvic girdle and its ligamentous system has been used to perform vibration testing. In such test the response of the system to vibrations is measured at various points on the structure for frequencies between 10 and 340 Hz. The vibration testing is performed a first time on the specimen with intact ligamentous system. The measurements are taken two more times after subsequent bilateral resection of both the sacrotuberous and the sacrospinous ligaments first, and the iliolumbar ligaments afterwards. A comparison between the system response obtained in the three configurations provides information on the role of the resected ligaments in the dynamics of the system, thus on their relevance in the model. Results indicate that the sacrospinous, the sacrotuberous and the iliolumbar ligaments do not play a role in the pelvis dynamics as measured in this study, and will therefore not be represented in the biodynamic model.

Finite element modelling of the pelvis: inclusion of muscular and ligamentous boundary conditions.

Previous finite element studies of the pelvis, including subject-specific studies have made extensive simplifications with regards to the boundary conditions used during analysis. Fixed boundary conditions are generally utilised at the pubis and superior part of the ilium. While it can be demonstrated that these models provide a close match for certain in vitro experiments that use similar boundary conditions, the resulting stress-strain fields in the cortex in particular are unlikely to be those found in vivo. This study presents a finite element analysis in which the pelvis is supported by muscular and ligamentous boundary conditions, applied using spring elements distributed over realistic attachment sites. The analysis is compared to an analysis in which the pelvis is restrained by fixed boundary conditions applied at the sacro-iliac joints. Striking differences in the stress-strain fields observed in cortical bone in particular, are found between the two analyses. The inclusion of muscular and ligamentous boundary conditions is found to lower the occurrence of stress concentrations within the cortex.

Steps to reduce operative time in laparoscopic dismembered pyeloplasty for moderate to large renal pelvis.

OBJECTIVES: To describe the steps to reduce the operative time for laparoscopic dismembered pyeloplasty in ureteropelvic junction obstruction with moderate to large hydronephrosis. METHODS: Using a transperitoneal approach and three ports, pyelotomy was done proximal to the ureteropelvic junction, and the ureter was spatulated laterally before dismembering it. Using the three suture line technique, the first suture was taken at the distal end of the spatulated ureter outside-in to the most dependent part of the pelvis inside-out and left as such. The second suture was taken at the proximal end of the ureter outside-in to the corresponding site at the posterior wall of the pelvis and then continued distally to complete the posterior suture line. A double-J stent was placed in an antegrade way. The anterior wall was completed with the first suture, taking continuous bites. The third suture line was used to close the pelvis. The operative time was recorded from placing the patient in the lateral position to the closure of the last port. RESULTS: From January 2002 to January 2004, 24 patients underwent pyeloplasty performed by a single surgeon. The mean pelvic volume was 80.4 cm3 (range 70 to 110). The mean patient age was 24.5 years (range 5 to 57). The body mass index was 22.28 kg/m2 (range 20 to 25). The mean operative time was 120.4 minutes (range 80 to 160). Minor complications were noted in 3 patients. All patients showed improvement in symptoms and drainage after a mean follow-up of 17 months (range 10 to 28). CONCLUSIONS: These steps helped to reduce the operative time to close to that of open surgery, making laparoscopic pyeloplasty a more desirable alternative.

Botryoid Wilms' tumor of the renal pelvis extending into the bladder.

We report a case of botryoid Wilms' tumor that occupied the renal pelvis and extended into the bladder. A 3-year-old boy was referred to us with a chief complaint of gross hematuria and micturition pain. Computed tomography showed tumor occupying the right renal pelvis and ureter and extending into the bladder. Right radical nephroureterectomy was performed. The resected specimen showed a botryoid sarcoma-like appearance, occupied the right renal pelvis and ureter, and protruded into the bladder. Histologic findings showed typical triphasic Wilms' tumor. Botryoid Wilms' tumor has been reported in only 16 cases in the literature and in only 3 cases extended into the bladder.
