Resynchronization of blood pressure circadian rhythm after westward trans-7-meridian flight with and without melatonin treatment.

Blood pressure (BP) of a healthy 37-yr-old male traveling from Milan to Houston was monitored for 36 h before the flight and continued for 5 d after the arrival. The rhythmometric analysis of BP data was made to investigate the rate of adaptation to a rapid rest-activity cycle shift. Since two trips were evaluated, during the second one the subject took melatonin (3 mg) before the nocturnal rest. In the first trip the BP circadian rhythm synchronization occurred on the 5th day. In the second trip melatonin promoted an immediate but unstable adaptation to the new rest-activity cycle.

[New method of astigmatism correction: bitoric single-meridian kerato-ablation in photorefraction surgery].

The technique and results of an original method for correction of common myopic astigmatism and mixed small-sphere astigmatism by means of bitoric photokeratoablation are described. Forty-six operations were performed on patients with 2-7 diopter astigmatism. Residual 0.87 diopter astigmatism was observed in patients with the initial value of less than 3 diopters. At initial astigmatism higher than 3 diopters, residual value was 1.49 diopters 3 months postoperation. No changes in the spheric component of initial refraction occurred. The proposed technique includes estimation of bitoric keratoablation, order of steps, and values of ablation algorithm enhancement. The method helps decrease the depth of keratoablation, rules out hypercorrection, and creates optimal conditions for reoperations.

[The meridian flowing and ancient Chinese philosophy].

The theories of Yin - Yang and Five - Evolutive phase were extensively applied after pre - Qin and both Han dynasties. In the Song - Ming dynasties, the background of New Confucianism, the achievements of art of Yi(Changes) advocated by the "School of Configuration - Arithmatic" were applied, in which the method of "Adopting of Jia" was almost entirely derived from the above school, mixing the theories of heavenly stems - earthly branches, Five - evolutive phases and Yin - Yang as a whole. An ancient system of Chronological treatment was thus established creatively by integrating clinical experience together. The system is a tight system of grafting medicine and philosophy formed on the basis of successfully utilizing the philosophical framework, thinking proposed by ancient physicians with keen observation and deep ponderation. The system is, by then, an advanced one, which is also an inevitable one.

Gradual adjustment of circaseptan-circadian blood pressure and heart rate rhythms after a trans-9-meridian flight.

Circadian and circaseptan adjustment of blood pressure and heart rate after a transmeridian flight by a family of four was relatively fast, but it was not abrupt; it was not completed during the first week in the new setting. Gradual changes continued for the ensuing three weeks. The circadian and circaseptan rhythm characteristics and their adjustment in particular differed among variables in the husband: the circadian component of heart rate adjusted more rapidly than that of blood pressure. The adjustment also differed among two members of the family. A new phenomenon, namely a difference in circadian rhythm adjustment between husband and wife, is also described herein: the wife's circadian rhythm in blood pressure advanced by 9 hours within the 4-week span of recording following a 9-hour advance in living routine (after a 9-zone transmeridian flight); since both schedule and rhythm moved in the same direction (albeit not at the same rate), the behavior may be dubbed concursive. By contrast, the husband delayed to achieve the same adjustment: his behavior was anticursive. The phenomenon of a difference in direction of adjustment for different variables in the same subject has been described earlier as polarity, now qualified as intra-individual polarity, to separate it from the precedent of an intrafamilial or more broadly inter-individual polarity or partitioning, reported apparently for the first time and dubbed 'cursion'.
