Self-mutilations in private-accident-insurance cases.

Self-inflicted injuries can be classified in groups. One group deals with the simulation of illness, another with the occurrence itself and the application of chemical, thermic or mechanical methods. One sector concerns self-mutilation, which, from a psychiatrist's point of view, is interesting. At this time we are more concerned with the problems of proving it. In wartime and even during military service in peace-time soldiers inflict mutilating injuries on themselves. They are motivated by the notion that they will gain benefit from their action. Economic gain plays a role in the case of people who have taken out private accident insurance: self mutilation to simulate the result of an accident. Our investigation into self-mutilation started with an analysis under the following aspects of 123 cases: age, sex, occupation, place of residence, place and time of deed, method employed (weapon used), localisation, single or multiple wound, direction of injury, position of fingers, nature of edges of wound. Whether or not an injury was suffered voluntarily or involuntarily can only be determined with the help of auxiliary facts. It must be clarified whether or not the information given by the injured person ties in with facts concerning the place where the injury was sustained, its position and its direction. The medico-legal expert should not interpret medical findings without relating them to the facts of the case. Indeed, he should start by examining the claimant's account of the accident. To some extent it almost requires the work of a general staff to compare the findings of a careful medical investigation with the injuries themselves. If the injury was inflicted by a certain tool information must be available regarding, for example, the "accident with the saw" together with an assessment of the wounds sustained (utilization of clinical material). Sometimes tests on corpses need to be carried out because these can provide information on mechanical and physical problems. When the direction of the wound is being clarified together with an appraisal of any traces found electron scanning and microscopic tests should also be incorporated into the examination in addition to medical and X-ray tests. At the slightest suspicion that a wound might have been self-inflicted appropriate tests should be carried out immediately. Conclusions should only be drawn by someone who has made an intensive study of this special field which is of such great forensic interest.

Manifest sadomasochism of males: results of an empirical study.

Two hundred forty-five manifestly sadomasochistic West German completed an anonymous questionnaires concerning their sexual behavior and psychosocial problems. They were reached as placers of sadomasochistic contact advertisements or as members of sadomasochistic clubs. Thirty percent were exclusively heterosexually oriented, 31% bisexually oriented, and 38% homosexually oriented. Results are described with respect to the invisibility of deviant behavior, seeking of partners, participation in the subculture, realization of the deviant desires, self-acceptance, preferences for sadomasochistic roles and practices, masturbation, and coming out. The possibilities for realization of the deviance are poorer for heterosexual sadomasochists than for the other groups. However, subcultural groups exist among heterosexual sadomasochists.

Two types of cross-gender identity.

A revision of the typology of male cross-gender identity was carried out by means of formalized, easily replicable methods. The results suggest (1) that there are two discrete types of cross-gender identity, one heterosexual, the other homosexual; (2) that transvestism, and closely related conditions of cross-gender identity, occur exclusively or almost exclusively in heterosexuals; (3) that of the two types of transsexualism distinguished in this study, type A is, in heterosexuals, very rare or completely nonexistent; (4) that (in the course of time) transvestites or borderline transsexuals (defined below) may develop sustained cross-gender identity, as observed by Stoller (1971); (5) that although, according to Hoenig and Kenna (1974), transsexualism by itself is not an anomalous erotic preference, it is (virtually) always either preceded by transvestism or accompanied by homosexuality or cross-gender fetishism.

The prevalence and some attributes of females in the sadomasochistic subculture: a second report.

Nonprostitute women in the sadomasochism (S/M) subculture have been believed to be rare. A sample of 45 women from the S/M subculture of whom 34 were determined to be nonprostitutes was obtained. This sample is compared with a similar sample obtained by Breslow et al. (1985). Despite methodological differences between the present investigation and that of Breslow et al., interstudy similarities permit conclusions about women in the S/M subculture in addition to the fact that they occur with sufficient frequency to study. The women become aware of their orientation as young adults and most are satisfied with it. They tend to be better educated and less often married than the general population. A majority designate themselves as heterosexual but a substantial minority are bisexual. They tend more often to prefer the submissive role but preference for the dominant role or no preference are found with considerable frequency. Oral sex and bondage are favored activities.

Deviance as history: the future of perversion.

An approach to the social construction of concepts of sexual deviance and sexual perversions is considered. Deviance is conceptualized as a problem of control, perversion a problem of desire. These are seen as related to the larger sexual and nonsexual discursive practices of society and given to change as these contextualizing practices change. Changing conceptions regarding masturbation, homosexuality, pedophilia, and sadomasochism are examined.

Varieties of autogynephilia and their relationship to gender dysphoria.

The relationship between gender dysphoria and autogynephilia (erotic arousal in men associated with the thought or image of themselves as women) was investigated. Subjects were 238 nonhomosexual male outpatients divided into three groups: 94 reporting they were most aroused sexually by images of themselves as nude women, 67 most aroused by images of themselves as women in underwear, and 77 most aroused by images of themselves as fully clothed women. These were compared on questionnaire measures of gender dysphoria and other psychosexual variables. The Nude group was significantly more gender dysphoric than the Clothed group, which, in turn, was significantly more gender dysphoric than the Underwear group. These results support the hypothesis that those nonhomosexual men most aroused sexually by the thought of having a woman's body are also those most interested in acquiring a woman's body through some permanent, physical transformation. A secondary finding was that the Nude group was the same age as the Underwear group and significantly younger than the Clothed group. This outcome makes it unlikely that erotic fantasies of having a woman's body are the end result of some progression that necessarily begins with erotic fantasies of wearing women's clothes.

Sexual bondage: a review and unobtrusive investigation.

To investigate unobtrusively how individuals experience sexual bondage, we analyzed all messages about sexual bondage (N = 514) mailed to an international computerized discussion group on sexual bondage in 1990. The most frequent individual experience reported (n = 60) was playful use of bondage to explore new areas of sexual pleasure. Other frequent experiences were the exchange of power, intensified sexual pleasure, and tactile stimulation and bodily sensations. Preference for the dominant-initiator role was expressed in 71% of the messages by male heterosexuals, 11% of the messages by heterosexual females, and 12% of the messages by homosexual males. Preference for the submissive-recipient role was expressed in 29% of the messages by heterosexual males, 89% by heterosexual females, and 88% by homosexual males. In 33% of messages subjects stated that sexual bondage occurred simultaneously with sadomasochism or was perceived as part of it.

Comparison of transvestism in Australia and America.

Information concerning gender identity, sexual orientation, cross-dressing behavior, fetishism, and bondage was obtained from a questionnaire which was posted to members of two transvestite clubs, one in the United States and one in Australia. This study reports the responses of 136 American and 86 Australian self-designated transvestites who reported a period of fetishism to women's clothes at some stage of development. Characteristics of transvestism of subjects in both countries were remarkably similar; all were male, almost half the subjects first cross-dressed in prepuberty, and in the large majority cross-dressing was well established by late adolescence; intense fetishism was usually experienced during adolescence but waned in later years; in almost a quarter of subjects fetishism ceased, although the desire to cross-dress continued; in many subjects transvestism was associated with fantasies of bondage, usually of the subjects bound while cross-dressed; sexual orientation was predominantly or exclusively heterosexual in more than three-quarters of the subject. Subjects were categorized into two groups. One group, termed nuclear transvestites, were satisfied with cross-dressing. The second group, termed marginal transvestites, desired feminization by hormone ingestion or by surgical intervention. Marginal compared to nuclear transvesites reported significantly stronger feminine gender identity and tended to report a stronger interest in the homosexual direction. The differences appeared to be present from childhood. No significant differences were found between the nuclear and marginal transvestites with regard to characteristics of fetishism, bondage, and cross-dressing except that in the American group marginal transvestites currently cross-dressed more frequently than did nuclear transvestites.

On the prevalence and roles of females in the sadomasochistic subculture: report of an empirical study.

The existence of females in the sadomasochistic subculture has generally been denied in the theoretical, clinical, and empirical literature. The assumption that females do not exist in the subculture, or exist in such small numbers as to make analysis impossible, was tested. Questionnaires were placed in two publications that cater to sadomasochists, and additional questionnaires were mailed to advertisers whose ads appeared in a sadomasochistic contact magazine. Of 182 individuals who responded, 130 were males, and 52 were females, indicating a meaningful female presence in the subculture. An analysis of the replies revealed similarities and differences between the male and female respondents.
